Delayed Presentation of a Post-traumatic Mesenteric Arteriovenous Fistula: A Case Report (외상 후 지연성으로 발생한 장간막 동정맥루: 증례보고)

  • Introduction: A post-traumatic mesenteric arteriovenous fistula (AVF) is extremely rare. Case Report: A previously healthy 26-year-old male was injured with an abdominal stab wound. Computed tomography (CT) showed liver injury, pancreas injury and a retropancreatic hematoma. We performed the hemostasis of the bleeding due to the liver injury, a distal pancreatectomy with splenectomy and evacuation of the retropancreatic hematoma. On the 5th postoperative day, an abdominal bruit and thrill was detected. CT and angiography showed an AVF between the superior mesenteric artery (SMA) and the inferior mesenteric vein with early enhancement of the portal vein (PV). The point of the AVF was about 4 cm from the SMA's orifice. After an emergent laparotomy and inframesocolic approach, the isolation of the SMA was performed by dissection and ligation of adjacent mesenteric tissues which was about 6 cm length from the nearby SMA orifice, preserving the major side branches of the SMA, because the exact point of the AVF could not be identified despite the shunt flow in the PV being audible during an intraoperative hand-held Doppler-shift measurement. After that, the shunt flow could not be detected by using an intraoperative hand-held Doppler-shift measuring device. CT two and a half months later showed no AVF. There were no major complications during a 19-month follow-up period. Conclusion: Early management of a post-traumatic mesenteric AVF is essential to avoid complications such as hemorrhage, congestive heart failure and portal hypertension.

  • Coronal discoloration is a common sequela to traumatic injuries. In subluxation cases, although the injury is not strong enough to rupture the apical vessels, discoloration may appear by tearing thin walls or occluding small capillaries. In absence of infection pulpal regeneration can occur, and as a result discoloration may completely or partially subside. But judging pulpal status by coronal discoloration can be dangerous and it may lead to unnecessary treatment. This case presents coronal discoloration and recovery following traumatic injury of maxillary anterior teeth. In diagnosing traumatized teeth routine cold tests or electric pulp tests are known to be unreliable, but with the aid of ultrasound doppler imaging, assessing pulp vitality of traumatized teeth can be more accurate.

  • It has been known that the color of skin reflects the blood flow within. In lips, the capillaries close to the skin surface are numerous; hence lips are redder than the rest of face. However, dermatological research on lips is not as advanced as research on facial or body skin, and little was known about the relationship between relatively dull-colored lips and skin blood flow. The physiological differences between colorful and dull-colored lips were studied by a two-dimensional laser Doppler blood flow analyzer, a spectrometer for the measurement of the degree of oxygen saturation, and a confocal microscope for observing inside lips non-invasively. Dull-colored lips and the corner of lips (dull-colored compared to the center) showed relatively poor blood flow and lower oxygenated hemoglobin. It was found that colorful lips (generally the young) had a blood flow that tended to run straight in parallel with the skin surface. This unique blood-capillary structure can express clear red blood. Those with dull-colored lips had lost this unique structure. Their blood ran perpendicularly from the deep of the skin and down back again into the deep part as like the blood circulation patterns of facial skin. Therefore, the lips of the latter group had fewer blood capillaries near skin surface in the lips than that of the colorful-lips group. A lipstick containing a-glucosyl-hesperidin, which is derived from certain citrus fruits and can enhance blood circulation, was applied for evaluating its effects. Blood flow was increased 30 min after the application. After two weeks of daily application, the lips' condition became noticeably less dull. These findings suggest that the decrease of blood flow in dull-colored lips is caused by the loss of the unique capillary structure and the use of the lipstick to increase blood flow can give a vivid color to lips.

  • This study focuses on the implementation of C-band radio navigation in the 5.03 ~ 5.15 GHz terrestrial band to cooperate with GNSS navigation mainly used in existing UAMs. This is one of the navigation technologies that can fully satisfy the requirements of Title 14 of CFR-135.165. According to the FAA, the use of two or more independent navigation sources for aircraft is proposed for aircraft. This study proceeded with the link budget derivation through radio wave propagation path loss analysis, and antenna shape design for miniaturized Doppler VOR, and DME design with enhanced positional distance resolution compared to conventional aircraft. The ground navigation system which is the result of this study, consists of a VOR/DME ground station and a terminal that can be mounted on UAM. Significant performance was confirmed through the production and testing of each prototype.

  • Military Autonomous Underwater Vehicle (AUV) is utilized to search a mine under the sea. This paper presents design and performance verification of real-time hybrid navigation system for AUV. The navigation system uses Doppler Velocity Log (DVL) integration method to correct INS error in underwater. When the AUV is floated on the water, the accumulated error of navigation algorithm is corrected using position/velocity of GPS. The navigation algorithm is verified using 6 Degree Of Freedom (DOF) simulation, Program In the Loop Simulation (PILS). Finally, the experiments are performed in real sea environment to prove the reliability of real-time hybrid navigation algorithm.

  • Intramuscular hemangiomas of the masseter muscle are uncommon tumors and therefore can be difficult to accurately diagnose preoperatively, due to the unfamiliar presentation and deep location in the lateral face. A case of intramuscular hemangioma of the masseter muscle in a 66-year-old woman is presented. Doppler ultrasonography showed a 34×15 mm hypoechoic and hypervascular soft tissue mass in the left masseter muscle, suggesting hemangioma. The mass was excised via a lateral cervical incision near the posterior border of the mandibular ramus. The surgical wound healed well without complications.

  • Purpose : The objectives of this study were to assess ventricular function by tissue Doppler imaging in children who were receiving chemotherapy or who had received chemotherapy, and to apply repeated tissue Doppler imaging to make an early assessment in cardiac toxicity studies. Methods : This study was conducted on 23 oncology patients on-treatment or off-treatment from April 2005 to July 2005 at Dongsan Medical Center, Keimyung University. All patients(group 1) were divided into two groups, fractional shortening(FS) over 29 percent(group 2) and FS under 28 percent (group 3) in the first category. These same patients were also divided into the following groups : group treated with anthracyclin(group 4) and group treated without anthracyclin(group 5). Deceleration time(DT), isovolumic relaxation time(IVRT), FS, peak early diastolic(E), and peak late diastolic (A) velocity of transmitral flow were measured by M-mode and pulsed wave Doppler. Systolic(Sm), peak early diastolic(Em), and peak late diastolic(Am) velocity in apical 4-chamber and 2-chamber views were measured by tissue Doppler imaging. The author calculated a modified Tei index, E/A, E/Em ratio by using measured values. Results : Twenty three patients were enrolled : 12 boys and 11 girls. The average age of patients was 8 years and 4 months. Thirteen out of 23 patients were in the group treated with anthracyclin (group 4) and 6 had FS under 28 percent(group 3). E/Em ratio showed a significant difference between group 1 and control group($6.46{\pm}1.85$ vs $7.06{\pm}1.64$, P<0.05). Other parameters had no difference statistically. Conclusion : This study showed that the change of cardiac function developed earlier in diastolic function than in systolic function, as E/Em ratio reflecting the mean LV diastolic pressure showed a significant difference between the control group and chemotherapy groups. Echocardiography using tissue Doppler imaging is a non-invasive, comfortable and reliable method for post-chemotherapy follow up.

  • The objective of this study is to examine the three-dimensional turbulent flows occurring in the meandering channel with presence of a groyne. A series of laboratory experiments are carried out in a meandering channel with trapezoidal cross sections. The channel is a 24.4 m long, 1.5 m wide, and the bottom slope in the longitudinal direction is 0.02. Two cases with and without the groyne are considered in the experiment. Three-dimensional velocity fields are measured using an acoustic Doppler velocimetry (ADV) at approximately sixty locations in each cross section. The measured velocity fields are averaged in time, and the time-averaged flow revealed that the mean velocity magnitude along the outer bank of the channel was reduced significantly and the direction of the primary flow was directed toward the center of the channel due to the presence of the groyne.

SPECTROSCOPIC OBSERVATION OF THE SOLAR ACTIVE REGIONS IN HeI 10830 ${\AA}$ LINE

  • Here we report the results from spectroscopic observations of soloar active regions in the HeI 10830 ${\AA}$ line at the German Vacuum Tower Telescope(VTT) in Tenerife during the August 199:3 International EFR(Emerging Flux Region) Campaign. Four active regions in various stages of their evolution, i.e., NOAA7558, 7560, 7561, and 7562, were ovserved on 10 August 1993. From the observed HeI 10830 ${\AA}$ spectra in these active regions, spectroscopic quantities such as equivalent width(EW), doppler shift, doppler width, etc., were derived(see Figure l(a)) and the correlation between them were studied(see Figure l(b)). Our main results are as follows: (I)In NOAA7562, which is a young and evolving EFR, the EW is large, while it is small around a simple and roundish spot of NOAA7558. (2)In these active regions, redshift in the 10830 line is dominant when the EW is larger. (3)As the doppler width increases, the line tends to shift redward. (4)When the EW is smaller, it seems to exist another component which have dynamic characteristics different from the redshifting component. In NOAA7560 and NOAA7561, regions which have several small spots, the values of the EW are intermediate. Results (2) and (3) may suggest the possible existence of downflow above active regions, if the HeI 10830 ${\AA}$line is formed in the upper chromopshere, and it is consistent with the earlyer result from the SMM extreme-ultraviolet observation by Klimchuk(1987, Astrophys. Comparison of 99mTc-DMSA Renal Scan and Power Doppler Ultrasonography for the Detection of Acute Pyelonephritis and Vesicoureteral Reflux

  • Purpose: Urinary tract infection (UTI) is one of the common infectious diseases in children. Several imaging modalities can be used to confirm the presence of acute pyelonephritis (APN). Among them the 99mTcdimercaptosuccinic acid renal scan (DMSA scan) is used as a gold standard for diagnosis. Ultrasonography technology is evolving. Therefore, in this study, we investigated the sensitivity and specificity of Power Doppler ultrasonography (PDU) compared to the results from the previous study. Methods: There were 260 patients included in this study, aged between 1 and 12 months old. The patients were admitted to the Yeungnam University Medical Center between January 2008 and December 2015. All patients underwent both DMSA scan and PDU within 5days of admission. Voiding cystourethrography (VCUG) was performed in 195 patients with abnormal DMSA scan or PDU. Results: The diagnostic sensitivity of APN using PDU was 45.5% and specificity was 85.5% in 260 patients following detection of a defect on DMSA scan that was defined as APN. The diagnostic sensitivity and specificity of PDU for VUR were 65.5 % and 60.1%, respectively. The diagnostic sensitivity and specificity of DMSA scan for VUR were 95.7% and 14.1%, respectively. Conclusion: PDU has a high specificity but low sensitivity, so there are limitations in using it to replace a DMSA scan for the diagnosis of APN in children. DMSA scan and PDU have different sensitivity and specificity in diagnosis of VUR, respectively. Therefore, we suggest that the sensitivity and specificity of each test can be helpful in diagnosing APN and VUR when used in conjunction.
