A Case of Pneumonia Caused by Pneumocystis jirovecii Resistant to Trimethoprim-Sulfamethoxazole

  • A 50-year-old male visited the outpatient clinic and complained of fever, poor oral intake, and weight loss. A chest X-ray demonstrated streaky and fibrotic lesions in both lungs, and chest CT revealed multifocal peribronchial patchy ground-glass opacities with septated cystic lesions in both lungs. Cell counts in the bronchoalveolar lavage fluid revealed lymphocyte-dominant leukocytosis, and further analysis of lymphocyte subsets showed a predominance of cytotoxic T cells and few T helper cells. Video-assisted wedge resection of the left upper lobe was performed, and the histologic examination was indicative of a Pneumocystis jirovecii infection. Trimethoprim-sulfamethoxazole (TMP-SMX) was orally administered for 3 weeks; however, the patient complained of cough, and the pneumonia was aggravated in the follow-up chest X-ray and chest CT. Molecular studies demonstrated mutations at codons 55 and 57 of the dihydropteroate synthase (DHPS) gene, which is associated with the resistance to TMP-SMX. Clindamycin-primaquine was subsequently administered for 3 weeks replacing the TMP-SMX. A follow-up chest X-ray showed that the pneumonia was resolving, and the cough was also alleviated. A positive result of HIV immunoassay and elevated titer of HCV RNA indicated HIV infection as an underlying condition. This case highlights the importance of careful monitoring of patients with P. jirovecii pneumonia (PCP) during the course of treatment, and the molecular study of DHPS mutations. Additionally, altering the anti-PCP drug utilized as treatment must be considered when infection with drug-resistant P. jirovecii is suspected. To the best of our knowledge, this is the first case of TMP-SMX-resistant PCP described in Korea.

  • We have investigated the lasing characteristics of the InGaAs quantum dot laser diode (QD-LD) and InGaAs quantum well laser diode (QW-LD) operated at the 980 nm wavelength range. The 980-nm lasers are used as a pumping source for a erbium-doped fiber amplifier (EDFA) and it shows high efficiency in long-haul optical fiber network. We have compared the threshold current density, the characteristic temperature, the optical power and the internal efficiency of QD-LD and QW-LD under a pulsed current condition. The QD-LD shows superior performances to the QW-LD. Further optimization of a LD structure is expected to the superior performances of a QD-LD.

Efficient Index Reconstruction Methods using a Partial Index in a Spatial Data Warehouse (공간 데이터 웨어하우스에서 부분 색인을 이용한 효율적인 색인 재구축 기법)

  • A spatial data warehouse is a system that stores geographical information as a subject oriented, integrated, time-variant, non-volatile collection for efficiently supporting decision. This system consists of a builder and a spatial data warehouse server. A spatial data warehouse server suspends user services, stores transferred data in the data repository and constructs index using stored data for short response time. Existing methods that construct index are bulk-insertion and index transfer methods. The Bulk-insertion method has high clustering cost for constructing index and searching cost. The Index transfer method has improper for the index reconstruction method of a spatial data warehouse where periodic source data are inserted. In this paper, the efficient index reconstruction method using a partial index in a spatial data warehouse is proposed. This method is an efficient reconstruction method that transfers a partial index and stores a partial index with expecting physical location. This method clusters a spatial data making it suitable to construct index and change treated clusters to a partial index and transfers pages that store a partial index. A spatial data warehouse server reserves sequent physical space of a disk and stores a partial index in the reserved space. Through inserting a partial index into constructed index in a spatial data warehouse server, searching, splitting, remodifing costs are reduced to the minimum.

Pre-Filtering based Post-Load Shedding Method for Improving Spatial Queries Accuracy in GeoSensor Environment (GeoSensor 환경에서 공간 질의 정확도 향상을 위한 선-필터링을 이용한 후-부하제한 기법)

  • In u-GIS environment, GeoSensor environment requires that dynamic data captured from various sensors and static information in terms of features in 2D or 3D are fused together. GeoSensors, the core of this environment, are distributed over a wide area sporadically, and are collected in any size constantly. As a result, storage space could be exceeded because of restricted memory in DSMS. To solve this kind of problems, a lot of related studies are being researched actively. There are typically 3 different methods - Random Load Shedding, Semantic Load Shedding, and Sampling. Random Load Shedding chooses and deletes data in random. Semantic Load Shedding prioritizes data, then deletes it first which has lower priority. Sampling uses statistical operation, computes sampling rate, and sheds load. However, they are not high accuracy because traditional ones do not consider spatial characteristics. In this paper 'Pre-Filtering based Post Load Shedding' are suggested to improve the accuracy of spatial query and to restrict load shedding in DSMS. This method, at first, limits unnecessarily increased loads in stream queue with 'Pre-Filtering'. And then, it processes 'Post-Load Shedding', considering data and spatial status to guarantee the accuracy of result. The suggested method effectively reduces the number of the performance of load shedding, and improves the accuracy of spatial query.

Product Characteristics as Factors of Process Parameters in Starch Phosphates Preparation by Twin-screw Extruder (이축압출성형기로 인산전분 제조시 Process Parameters에 따른 제품의 특성)

  • Starch phosphates were prepared from the corn starch mixed with 2% sodium tripolyphosphate by twin-screw extruder with a feed rate of 20 kg/hr and an extrusion temperature of $130^{\circ}C$, and the effects of extrusion variables on the physicochemical properties (target parameters) of starch phosphates were investigated. Interrelations of system parameters (specific mechanical energy and extrudate moisture) and rheological properities of starch was analyzed by using the response surface analysis. Degree of substitution (DS) was increased with increasing the feed moisture, and showed the maximum value at the screw of near 250 rpm, Degree of gelatinization was proportionally increased with increasing the screw speed and decreasing the feed moisture. Apparent viscosity of the paste was increased with increasing the feed moisture, but it was not significantly affected by the screw speed. It was found by scanning electron microscopy that the starch microgranules were much more degradaded, and as consequent result, the intrinsic viscosity was decreased, whereas, water solubility index was increased. The rate of retrogradation of the gels was retarded with increasing DS and decreasing viscosity.

Analysis of Flavor Pattern from Different Categories of Cheeses using Electronic Nose (전자코를 이용한 다양한 유형의 치즈 제품 풍미성분 분석)

  • The objective of this study was to analyze the flavor pattern of different varieties of cheeses. Four of the each following cheese varieties such as shred type pizza cheese, Cheddar cheese, Mozzarella block cheese, and white mold-ripened cheeses, stored at $4^{\circ}C$ during 2 wks were examined before and after cooking at $70^{\circ}C$ and $160^{\circ}C$. Flavor patterns of these cheeses were analyzed using an electronic nose system based on mass spectrometer. All data were treated by multivariate data processing based on discriminant function analysis (DFA). The results showed the discriminant model by DFA method. Data revealed that flavor patterns of pizza cheeses were well separated as storage prolonged and obviously discriminated as the higher the cooking temperature. The result of pattern recognition analysis based on discriminant function analysis showed that new brand of pizza cheese produced by Imsil Cheese Cooperative was located at middle between the flavors of the imported brands of pizza cheese and those of domestic brand of pizza cheeses. Imsil cheese has a unique flavor pattern among other variety of cheeses. Application of pattern recognition analysis by electronic nose might be useful and advanced technology for characterizing in flavor pattern of cheese products from different origins and different categories of cheeses.

Properties of Anti-S. mutans IgY Separated from Egg Yolk (난황으로 부터 항충치 항체의 분리 및 그 특성)

  • Chick antibodies (IgY) raised against Streptococcus mutans (serotype c) were separated from egg yolk and their properties were investigated. The purity of IgY extracts prepared by the method of ${\lambda}-carrageenan$, $gammaYolk^{TM}$, and $EGGstract^{TM}$ was 20%, 46%, and 48%, respectively, and the yields of IgY extracts from a gram yolk were 11. 3 mg, 1.7 mg, and 1.8mg, respectively. Quantitative immunoprecipitation test showed that specific IgY content of crude IgY prepared by ${\lambda}-carrageenan$ method was 12.2%, which means that 0.85 g of crude IgY from an egg yolk (15 g) contains about 100 mg of specific IgY. When the reactivity of the specific IgY towards 3 caries-inducing strains (serotype: b, c, f) was examined, the strains cultured in sucrose-added medium showed higher reactivity (the orders were c(+), f(+), b(+)) than those cultured in sucrose-free medium. Heat and pH stability of specific IgY was good, for crude IgY contained 50% of antibody activity after heat treatment at $70^{\circ}C$ for 5 min and they were stable at pH $4{\sim}8$.

  • Both Gitelman syndrome and Bartter syndrome are autosomal recessively inherited renal tubular disorders characterized by hypokalemic metabolic alkalosis, salt wasting and normal to low blood pressure. Gitelman syndrome is caused by mutations in the thiazide-sensitive Na-Cl cotransporter (NCCT) and distinguished from Bartter syndrome, which is associated with mutations of several genes, by the presence of hypomagnesemia and hypocalciuria. In most of the patients with Gitelman syndrome, the disease manifests with transient episodes of muscular weakness and tetany in the adult period, but, often, is asymptomatic. We report here an 11 years-old female with Gitelman syndrome who presented with aggravation of epileptic seizure. The diagnostic work-up showed typical clinical features of metabolic alkalosis, hypokalemia, hypomagnesemia and hypocalciuria. We also identified a heterozygote mutation($^{642}$CGC(Arg)>TGC(Cys)) and an abnormal splicing in the SLC12A3 gene encoding NCCT.

Survey of Major Disease Incidence of Rice in Korea during 1999-2008 (1999-2008년도 벼 주요 병해 발생 개황)

  • During the period from 1999 to 2008, we surveyed the major diseases incidence of rice in Korea. Leaf blast showed 0.85% of the diseased leaf area in 2000 and then consistently decreased to 0.02% by 2008. However, panicle blast irregularly appeared by the years depending on weather conditions for disease development from heading to milk stage of rice. Diseased lesion height of sheath blight had the lowest (10.1%) incidence in 2001 and the highest (21.4%) incidence in 2007. A negative corelation ($r=-0.88^*$) was measured between diseased lesion height of sheat blight and sunshine hours from mid- to late August. In Chunbuk, Gyeongbuk, and Gyeongnam provinces, brown spot occurrence declined since 2001, however, in Chungnam, Jeonbuk, and Jeonnam provinces, it increased since 2005. False smut drastically decreased after showing the highest incidence of 7.59% in panicles in 2000. The correlation coefficient between diseased panicles of false smut and rainfall during the late August was calculated as $0.94^{**}$. Bacterial leaf blight severely increased in the chronically infested fields in Jeonnam, Jeonbuk, Gyeongnam, and Chungnam province since 2003.

Comparison of Biomechanical Characteristics of Rowing Performance between Elite and Non-Elite Scull Rowers: A Pilot Study

  • Objective: This study aimed to examine the characteristics of joint kinematics and synchronicity of rowing motion between elite and non-elite rowers. Methods: Two elite and two non-elite rowers performed rowing strokes (3 trials, 20 strokes in each trial) at three different stroke rates (20, 30, 40 stroke/min) on two stationary rowing ergometers. The rowing motions of the rowers were captured using a 3-dimensional motion analysis system (8-infrared camera VICON system, Oxford, UK). The range of motion (RoM) of the knee, hip, and elbow joints on the sagittal plane, the lead time ($T_{Lead}$) and the drive time $T_{Drive}$) for each joint, and the elapsed time for the knee joint to maintain a fully extended position ($T_{Knee}$) during the stroke were analyzed and compared between elite and non-elite rowers. Synchronicity of the rowing motion within and between groups was examined using coefficients of variation (CV) of the $T_{Drive}$ for each joint. Results: Regardless of the stroke rate, the RoM of all joints were greater for the elite than for non-elite rowers, except for the RoMs of the knee joint at 30 stroke/min and the elbow joint at 40 stroke/min (p < .05). Although the $T_{Lead}$ at all stroke rates were the same between the groups, the $T_{Drive}$ for each joint was shorter for the elite than for the non-elite rowers. During the drive phase, elite rowers kept the fully extended knee joint angle longer than the non-elite rowers (p < .05). The CV values of the TDrive within each group were smaller for the elite compared with non-elite rowers, except for the CV values of the hip at all stroke/min and elbow at 40 stroke/min. Conclusion: The elite, compared with non-elite, rowers seem to be able to perform more powerful and efficient rowing strokes with large RoM and a short $T_{Drive}$ with the same $T_{Lead}$.
