A Case of Combined Korean-Western Medicine Treatment on Chorea Hyperglycemia Basal Ganglia Syndrome Induced by Nonketotic Diabetes Mellitus (비케톤성 당뇨병으로 유발된 고혈당성 무도증의 한·양방 복합치료 증례보고)

  • Objectives: Chorea Hyperglycemia Basal Ganglia Syndrome(C-H-BG) is a syndrome with chorea-ballismus induced by hyperglycemia. This case study reports the clinical effect of Korean medicine treatment on C-H-BG. Methods: A 73-year-old male patient with left side chorea-ballismus visited Daejeon Korean Medicine Hospital. He had no diabetes mellitus(DM) history but the laboratory test result suggested nonketotic DM. We diagnosed him as C-H-BG and treated with herbal medicine, acupuncture and hyperglycemic medication. To evaluate chorea-ballismus, we checked Abnormal Involuntary Movement Scale(AIMS) score everyday. Results: After 14 days of treatment, the chorea-ballismus improved and AIMS score decreased from 38 to 4. Gait disturbance caused by the symptom disappeared. Conclusions: We suggest that Korean medicine treatment can improve the symptom of C-G-BG.

Cardiopulmonary Symptoms, Quality of Sleep, and Depression in the Hospitalized Patient with Pneumoconiosis (요양병원 진폐 환자의 심폐증상, 수면의 질 및 우울에 관한 연구)

  • Purpose: This study was to examine the cardiopulmonary symptoms, quality of sleep, and depression, and to identify the influencing factors in the hospitalized coal worker's pneumoconiosis patient. Methods: 137 hospitalized patients who received pneumoconiosis treatment were conveniently selected. A descriptive correlational study was conducted. Data were collected using structured questionnaires and were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Results: The mean score of cardiopulmonary symptoms was relatively high ($3.97{\pm}0.56$, out of 5). The 5 and over score of quality of sleep, sleep disturbance was 94.2%, and the reported depression was 95.6%. The levels of depression depended on the oxygen therapy and family/acquaintance visiting. The depression was significantly correlated with the cardiopulmonary symptoms and quality of sleep. The influential factors affecting depression were cardiopulmonary symptoms and quality of sleep, which explained about 34.1% of the variance. Conclusion: The results of this study indicate that nursing interventions are needed to reduce depression, and to improve cardiopulmonary symptoms and quality of sleep in the hospitalized pneumoconiosis patient. These results can provide for nursing intervention to facilitate reduction of depression.

Case Report : 3 Cases of CPM Combined with Korean Medical Treatment for Knee Joint Rehabilitation (한방 치료를 병행한 슬관절 재활 CPM 증례보고 3례)

  • Objectives : The aim of this study was to report the cases of knee injury that are treated with both CPM and Korean medicine. Methods : We treated 3 patients who had knee joint pain and gait disturbance by knee injuries with Korean medicine and CPM treatment. We evaluated the outcomes by checking NRS, ROM, Lysholm knee score, walking state and other symptoms. Results : The NRS, ROM, Lysholm knee score, and other symptoms of 3 patients were improved before they. These results show possibilities of simultaneous treatment of Korean medicine and CPM. But there is a limit that the number of cases is not sufficient, and there was no long-term follow-up after discharge. Conclusions : Simultaneous treatment of Korean medicine and CPM has an effect on knee joint rehabilitation. Further studies are needed to prove the effectiveness of Korean medicine combined with CPM.

A Case Report of Miller-Fisher Syndrome with Ophthalmoplegia (밀러-피셔 증후군 환자의 안근 마비 치험 1례)

  • Objectives : The aim of this study is to report the effect of Korean Medicine Treatment for patient with ophthalmoplegia in Miller-Fisher Syndrome. Methods : A patient with ophthalmoplegia treated with herbal medicine, cupping therapy and acupuncture(including pharmacopuncture, electroacupuncture). We used photographs of eye movement to evaluate the changes in symptoms, Scott and Kraft score of left eye, visual field test for measuring peripheral visual field, subjective symptom, and VAS for headache, subjective symptom. Results : After treatment, Scott and Kraft score increase from -4 to -2. Visual field of left side improved from 45° to 80°. Also, other symptoms such as diploma, dizziness, headache, gait disturbance was also improved. There are no adverse effects and no relapse of ophthalmoplegia after 20 days of discharge. Conclusions : This case report suggests that Korean Medicine Treatment can be effective for patient with ophthalmoplegia in Miller-Fisher Syndrome.

Effect and Satisfaction Measurement of Korean Medicine Treatment on Adolescent Dysmenorrhea (중고등학교 여학생의 월경통 한방 치료 효과 및 만족도 분석)

  • Objectives: The aim of this study is to evaluate the effect and satisfaction measurement of the Korean medicine treatment on the adolescent dysmenorrhea. Methods: The subjects of this study were 24 volunteers, suffering from dysmenorrhea, who were middle and high school students in Jeonbuk province. They were treated by Korean medicine, those were acupuncture, cupping, moxibustion and herbal medicine, for two months from July to December 2015. The results were investigated by the visual analogue scale (VAS) of overall pain, most severe pain and amount of the analgesic taken for menstrual period. The effects of treatment were assessed three times, those were before treatment, after treatment for 1 month and after treatment for 2 months, for two months. Results: The VAS score on dysmenorrhea was decreased significantly. Also, the VAS score on the disturbance in school activity was decreased significantly. The amount of analgesic taken for menstrual period was decreased, but not significantly. Conclusions: This study suggests that the Korean medicine treatment on the adolescent dysmenorrhea is effective.

Case Study of Soyangin Patient with Delayed Encephalopathy after Carbon Monoxide Intoxication (일산화탄소 중독 지연성 뇌손상 소양인 환자 치험 1례)

  • Objective This case report is to show the effectiveness of Sasang Constitutional Medicine treatment on the patient with delayed encephalopathy after acute carbon monoxide intoxication. Methods A 40-year-old female patient diagnosed with delayed encephalopathy after carbon monoxide poisoning 2 weeks ago before visiting our hospital. The patient suffered gait disturbance and cognitive disorder. She was identified as Soyangin and treated with Sasang constitutional Herbal Medicine(Yangkyuksanwha-tang and Hyungbangjihwang-tang). Improvement of symptoms was evaluated through Korean Version of Mini Mental Status Examination score(MMSE-K), Modified Barthel Index(MBI), Manual Muscle Testing(MMT), Range of Motion(ROM) and Hand Function Test. Results After 7 weeks of treatment, total score of MMSE-K increased from 0 to 26, the patient could walk by herself and had no trouble carrying out her daily life. Conclusions This case showed the effectiveness of Sasang constitutional medicine treatment for delayed encephalopathy after acute carbon monoxide intoxication.

A Case of Prurigo Nodularis with Skin Rash that Occurred after COVID-19 Vaccination (한방치료를 통해 호전된 COVID-19 백신 접종 이후 발생한 피부 발진을 동반한 결절성 양진 치험 1례)

  • Objectives: This study is to report a case of prurigo nodularis with skin rash that occurred after COVID-19 vaccination improved by Korean medicine treatment. Methods: The patient visited our hospital complaining of prurigo nodularis and skin rashes which occurred in various parts of the body (Total score 17). The patient complained of sleep disturbance (NRS 9) due to nighttime pruritus (NRS 7). Based on the patient's symptoms, herbal medicine Soshiho-tang and acupuncture on the left Triple Energizer Hangyeok was prescribed. Results: Most of the skin symptoms improved significantly leaving only scars, minor scratches and pigmentation (Total score 17 → 6). The patient's sleep condition improved (NRS 7 → 1) as the pruritus decreased from NRS 9 to 2. Conclusions: This study is significant in that the patient with prurigo nodularis accompanied by skin rash effectively improved by the Korean Medicine treatment.

Prediction of Sleep Disturbances in Korean Rural Elderly through Longitudinal Follow Up (추적 관찰을 통한 한국 농촌 노인의 수면 장애 예측)

  • Objectives: Sleep disturbance is a very rapidly growing disease with aging. The purpose of this study was to investigate the prevalence of sleep disturbances and its predictive factors in a three-year cohort study of people aged 60 years and over in Korea. Methods: In 2012 and 2014, we obtained data from a survey of the Korean Social Life, Health, and Aging Project. We asked participants if they had been diagnosed with stroke, myocardial infarction, angina pectoris, arthritis, pulmonary tuberculosis, asthma, cataract, glaucoma, hepatitis B, urinary incontinence, prostate hypertrophy, cancer, osteoporosis, hypertension, diabetes, hyperlipidemia, or metabolic syndrome. Cognitive function was assessed using the Mini-Mental State Examination for dementia screening in 2012, and depression was assessed using the Center for Epidemiologic Studies Depression Scale in 2012 and 2014. In 2015, a structured clinical interview for Axis I psychiatric disorders was administered to 235 people, and sleep disturbance was assessed using the Pittsburgh Sleep Quality Index. The perceived stress scale and the State-trait Anger Expression Inventory were also administered. Logistic regression analysis was used to predict sleep disturbance by gender, age, education, depression score, number of coexisting diseases in 2012 and 2014, current anger score, and perceived stress score. Results: Twenty-seven percent of the participants had sleep disturbances. Logistic regression analysis showed that the number of medical diseases three years ago, the depression score one year ago, and the current perceived stress significantly predicted sleep disturbances. Conclusion: Comorbid medical disease three years previous and depressive symptoms evaluated one year previous were predictive of current sleep disturbances. Further studies are needed to determine whether treatment of medical disease and depressive symptoms can improve sleep disturbances.

The Verification of Korean Version Swallowing Disturbance Questionnaire (K-SDQ) (한국판 삼킴 곤란 척도(K-SDQ)의 번안본 검증)

  • Swallowing disorders that can affect nutrient intakes and quality of life are commonly shown among the elderly as well as patients with neurogenic disorder. This study verifies the reliability and validity of the Swallowing Disturbance Questionnaire (SDQ), a subjective swallowing disability assessment tool, modified for Koreans' eating habit and cultural sentiment, against 105 stroke patients, in order to help identify early swallowing problems of the elderly. Reliability of internal consistency in the Korean version of SDQ is .601, test-retest reliability is .97, and concurrent validity is .956. Based on 8 points of cut-off score, 46.8% of sensitivity and 81.6% of specificity. Comparing the results of video fluoroscopic study (VFSS), an objective swallowing disorder test with those of Korean version of SDQ, negative predictive value (NPV) and positive predictive value (PPV) was shown as 81% and 53%. The Korean version of SDQ is expected to be a useful testing tool to discriminate swallowing disorders in stroke patients. It has great clinical significance in that swallowing difficulties shown by subjects can be sorted out to request a diagnostic assessment before clinical evaluation by a rehabilitation therapist or ruling out unnecessary exposure to additional tests by accurately identifying stroke patients without swallowing problems.

The Characteristics of Depression in Cancer Patients on Chemotherapy (항암제 투여 환자의 우울 양상)

  • Objectives : The purpose of this study was to identify characteristics of depression in cancer patients treated with chemotherapy. Methods : Subjects were 37 cancer inpatients of oncology in Yeungnam university hospital. BDI and ZDS were done and HDS was performed through semistructured interview. Results: 1) There were no significant differences of depression scale score according to sex, education, religion, marital status. 2) The mean scores of BDI, ZDS and HDS in 37 cancer patients were 18.27${\pm}$7.73, 36.51${\pm}$10.82, 15.14${\pm}$6.60, respectively. 3) HDS, ZDS scores were significantly higher in other cancer group receiving high dose chemotherapy. 4) Item score for depressed mood, diurnal variation(p<0.001), dissatisfaction(p<0.01), physical anxiety, decreased libido, sleep disturbance(p<0.05) were significantly higher in other cancer group than others. 5) Eight patients were diagnosed as having major depressive disorder, 11 as adjustment disorder, and 18 patients had no axis I diagnosis. 6) In major depressive disorder group, the score of the depressed mood item in ZDS was high (p<0.05). HDS item score for depressed mood, work difficulty, anxiey(p<0.001), psychomotor retardation(p<0.01) were significantly higher in major depressive disorder group. 7) In patients with adjustment disorder, ZDS item score of constipation(p<0.001), fatigue, anorexia (p<0.01), emptiness, sleep disturbance, dissatisfaction, weight loss(p<0.01) were high. HDS item score of hypochondriasis(p<0.01), agitation(p<0.01), fatigue, decreased libido(p<0.05) were significantly higher in adjustment disorder group. Conclusion : Some psychiatric disorders, such as adjustment disorder and major depressive disorder were common in the cancer patients in chemotherapy. Psychiatric intervention will increase compliance of cancer treatment and improve the quality of life. This study suggests that it would be important to consider the nature of somatic symptoms in diagnosing depression in cancer patients.
