Population Pharmacokinetics for Gentamicin in American and Korean-American Appendicitis Patients Using Nonparametric Expected Maximum(NPEM) Algorithm (비모수적 기대최대치(NPEM)연산방법에 의한 미국인과 재미동포 충수돌기염 환자에게 겐타마이신의 모집단 약물동태학)

  • Population pharmacokinetics for gentamicin were compared with 24 American patients (16 male and 8 female) and 16 Korean-American appendicitis patients(12 male and 4 female). Two to six blood specimens were collected from all patients at the following times: just before a regularly scheduled infusion and at 1/2 hour after the end of a 1/2 hour infusion. Nonparametric expected maximum(NPEM) algorithm for population modeling was used. The estimated parameters were the elimination rate constant(K), the slope of the relationship between K versus creatinine clearance(KS), the apparent volume of distribution(V), the slope of the relationship between V versus weight(VS), gentamicin clearance(CL) and the slope of the relationship between CL versus creatinine clearance and the VS(CS). The output includes a 3-dimensional plot of the joint probability density function(PDF), two marginal PDF, means, medians, modes, variance, skewness, kurtosis, and CV%. The mean K(KS) were 0.424$\pm$0.139(0.00429$\pm$0.00164) and 0.411$\pm$0.135 hr$^{-1}$ (0.00475$\pm$0.00180[hr.mL/min/1.73m$^{2}]^{-1}$) for American and Korean-American populations, respectively. The mean V(VS) were not different at 15.6$\pm$4.77(0.233$\pm$0.0526) and 15.1$\pm$3.84L(0.239$\pm$0.0492 L/kg) for American and Korean-American populations, respectively (P>0.2). The mean CL (CS) were 6.28$\pm$1.85(0.0634$\pm$0.0191) and 5.70$\pm$1.77 L/hr(0.0701$\pm$0.0215 L/kg[hr.mL/min/1.73m$^{2}$)] for American and Korean-American populations, respectively. There are no differences in gentamicin pharmacokinetics between American and Korean-American Appendicitis patients.

Growth and Characterization of Epitaxial YIG Films for Microwave Devices (마이크로파 소자용 에피틱시 YIG막의 성장과 특성)

  • YIG $(Y_3Fe_5O_{12})$ films with 4~80 ${\mu}{\textrm}{m}$ thickness were epitaxially grown on GGG $(Gd_3Ga_5O_{12})$ substrates by LPE (liquid phase epitaxy) techniques. Using various melts having different chemical composition the growth temperature was varied as a parameter. Growth rate, surface morphology, chemical composition, lattice constant, saturation magnetization, and magnetic resonance of the films were investigated. Lattice mismatch between the substrate and film Δa, saturation magnetization, and magnetic resonance line width ΔH increased, decreased, and increased, respectively, as undercooling temperature ΔT increased. The films grown by using the melt with larger R$_1$and smaller R$_3$had smaller ΔH. The major origin of the increase of ΔH was the increase of Δa. It is considered that the magnetic field in the film became locally inhomogeneous with the increase of Δa due to the increase of inhomogenity in stress distribution to the film depth direction. Therefore, in order to grow YIG films with small microwave loss it is necessary to grow films at small ΔT using the melt with large R$_1$and small R$_3$resulting in a small Δa.

Poly(ε-caprolactone) Microcapsule with Encapsulated Nifedipine Prepared by Magnetic Stirrer

  • The microencapsulation of nifedipine (NF) with 4 wt% of poly(${\varepsilon}-caprolactone$) (PCL)/polyvinylpyrollidone (PVP) or PCL/polyethylene glycol (PEG) was carried out by solvent evaporation method in oil in water emulsion system to investigate the effect of PVP and PEG addition on drug release behavior of the microcapsules. The PVA (emulsifier) concentration of 1.0 wt% was chosen for the formation of PCL capsule having an average size of $154{\pm}25{\mu}m$ due to nearly spherical shape with a narrow size distribution. As PCL/PVP and PCL/PEG ratios were raised from 10/0 to 6/4, the capsule size increased gradually from $154{\pm}25{\mu}m$ to $236{\pm}32{\mu}m$ and $248{\pm}56{\mu}m$, respectively. The drug release rate of PCL/PVP and PCL/PEG capsules increased dramatically from 0 to 4 h at the beginning and then reached the plateau region from 20 h. As the concentration of PVP or PEG increased, the amount of drug release increased, suggesting that the larger capsule size was attributed to the higher drug content. However, the drug release behavior remained almost constant. The PCL capsules exhibited no evidence of causing cell lysis or toxicity regardless of NF loading, implying that the microcapsules are clinically suitable for use as drug delivery systems.

  • To control hot spot in a plug flow reactor (PFR) is important for the yield and purity of products and safety. In this paper, coolant temperature is set as a state variable, and radial distributions of heat and mass are considered to model the PFR more realistic than without considering radial distributions. The model consists of three state variables, reactant concentration, reactant temperature, and the coolant temperature. The flow rate of the isothermal coolant is a manipulated variable. This paper shows that the controller considering the radial distributions of heat and mass is more effective than the controller without them. Assuming that u3,0 is 0.7, the suggested control equation was robust when St is bigger than 1.3, and Ac/A is smaller than 2.0. Under this condition, the hot spot temperature changed within the relative error of one percent when the temperature of input altered within the range of five percent.

  • The infrastructures such as offshore, bridges, power plant, oil and gas piping and aircraft operate in a harsh environment during their service life. Structural integrity of engineering components used in these industries is paramount for the reliability and economics of operation. Two regression models based on the concept of Gaussian process regression (GPR) and Minimax probability machine regression (MPMR) were developed to predict stress intensity factor range (𝚫K). Both GPR and MPMR are in the frame work of probability distribution. Models were developed by using the fatigue crack growth data in MATLAB by appropriately modifying the tools. Fatigue crack growth experiments were carried out on Eccentrically-loaded Single Edge notch Tension (ESE(T)) specimens made of API 5L X65 Grade steel in inert and corrosive environments (2.0% and 3.5% NaCl). The experiments were carried out under constant amplitude cyclic loading with a stress ratio of 0.1 and 5.0 Hz frequency (inert environment), 0.5 Hz frequency (corrosive environment). Crack growth rate (da/dN) and stress intensity factor range (𝚫K) values were evaluated at incremental values of loading cycle and crack length. About 70 to 75% of the data has been used for training and the remaining for validation of the models. It is observed that the predicted SIF range is in good agreement with the corresponding experimental observations. Further, the performance of the models was assessed with several statistical parameters, namely, Root Mean Square Error (RMSE), Mean Absolute Error (MAE), Coefficient of Efficiency (E), Root Mean Square Error to Observation's Standard Deviation Ratio (RSR), Normalized Mean Bias Error (NMBE), Performance Index (ρ) and Variance Account Factor (VAF).

Quality characteristics of fresh cut apples during storage according to processing steps and browning inhibitor treatment

  • We examined the changes in the physicochemical quality characteristics and antioxidant activity of fresh-cut Fuji apples (Malus pumila) during processing and storage after treatment with browning inhibitors. The primary aim was to elucidate processing suitability and storability. We observed that in the processing stage of slicing fresh Fuji apples, there were no significant differences in 1,1-diphenyl-2-picrylhydrazyl (DPPH) and 2,2'-azinobis (3-ethylbenzothiazoline-6-sulfonic acid (ABTS) radical scavenging ability and ferric-reducing antioxidant power (FRAP). However, after NaClO treatment, total flavonoid and total polyphenol contents decreased. When freshcut Fuji apples were treated with browning inhibitors and stored at 4℃, the L value and hardness decreased; however, the weight loss rate increased based on the storage period of all fruit groups treated with Citrus unshiu Markovich (CuM), calcium ascorbate (CA), and ascorbic acid (AA). The pH increased after 2 days of storage in 1% CuM and after 6 days of storage in 1% CA; however, no changes in pH were observed during the storage period in 1% AA. The DPPH radical scavenging activity was generally good under storage conditions of 1% AA at 4℃ and 1% CA at 20℃. Furthermore, FRAP remained relatively constant under storage conditions of 1% CA. The quality characteristics and antioxidant activity of fresh-cut fruits during processing and storage can be used as basic data for industries. Furthermore, we can gain confidence in quality improvements by improving the production and distribution environment of fresh-cut agricultural products.

Arthroscopic Full-Thickness Rotator Cuff Repair in Elderly Patients (고령 환자의 관절경적 회전근 개 봉합술의 결과)

  • Purpose: To examine the clinical and structural outcomes of an at least two-year follow-up of arthroscopic full-thickness rotator cuff repairs with a single-row or suture-bridge technique in patients more than 65 years of age. Materials and Methods: Patients diagnosed with a full-thickness rotator cuff tear who were more than 65 years of age, underwent arthroscopic rotator cuff repair after at least six months of conservative treatment, agreed to take a follow-up magnetic resonance imaging (MRI) six months postoperatively, and visited outpatient for at least two years were enrolled in this study. Clinical evaluations were done using The University of California Los Angeles score, Constant Shoulder Score, and visual analogue scale evaluated two years after the surgery. The structural integrity was analyzed using follow-up MRI. During surgery, a suture-bridge technique was used if the rotator cuff tendon could cover half of the footprint under constant tension. Otherwise, single-row repair was performed. Results: The samples were 158 cases, consisting of 93 single-repairs and 65 suture-bridge repairs. A preoperative comparison of the age distribution, fatty degeneration of supraspinatus and infraspinatus muscle, medial retraction of torn cuff tendon, and tear size between the two groups were not significant. The clinical scores were improved significantly in all cases. The distribution of the structural integrity by Sugaya classification were 49 cases in type 1 (31.0%), 62 cases in type 2 (39.2%), 30 cases in type 3 (19.0%), 11 cases in type 4 (7.0%), and six cases in type 5 (3.8%). The re-tear rate of the single-row group was 9.7% (nine out of 93 cases) and 12.3% (eight out of 65 cases) for the suture-bridge group. Conclusion: Satisfactory clinical and radiological outcomes were achieved after arthroscopic full-thickness rotator cuff repair in patients more than 65 years of age. Both single-row and suture-bridge techniques would be beneficial for the elderly.

Optimization of Analytical Conditions for the Quantification of Explosive Compounds in Soil using HPLC (HPLC에 의한 토양내 화약물질 정량분석조건 최적화)

  • A series of experiments was performed to develop an optimized analytical procedure for the analysis of explosives in soil by HPLC with soil samples collected at two live-fire military shooting ranges. The minimum amount of soil to be collected, Wmin, for the analysis of explosive compounds was 125g, based on the segregation and homogeneity constants that account for soil heterogeneity and non-homogeneous distribution of target explosive compounds. The optimization of extraction and HPLC analytical conditions were also studied based on analytes CV values. The most effective soil/ extractant ratio was estimated to be 10g-pretreated soil/20 mL acetonitrile as extractant. The optimized HPLC elution conditions for the separation of US EPA designated 14 explosive compounds, were column temperature 30${\circ}C$, eluents ratio of isopropanol: acetonitrile: water = 18 : 12: 70, and flow rate of 0.8 mUmin at 230 nm. However, UV wavelength 254 nm was better for the analysis of NB, 2,4-DNT, 2NT, 4NT, and 3NT.

Usefulness of serum cystatin C to determine the dose of vancomycin in neonate

  • Purpose: The vancomycin dosage regimen is regularly modified according to the patient's glomerular filtration rate (GFR). In the present study, we aimed to assess the usefulness of serum cystatin C (Cys-C) concentration, compared with serum creatinine (SCr) concentration, for predicting vancomycin clearance (CLvcm) in neonates. Methods: We retrospectively analyzed the laboratory data of 50 term neonates who were admitted to the neonatal intensive care unit and received intravenous vancomycin, and assessed the pharmacokinetic profiles. Creatinine clearance (CLcr) and GFR based on Cys-C (GFRcys-c) were estimated using the Schwartz and Larsson formulas, respectively. Results: The mean CLvcm (${\pm}$standard deviation) was $74.52{\pm}31.17L/hr$, the volume of distribution of vancomycin was $0.67{\pm}0.14L$, and vancomycin half-life was $9.16{\pm}17.42hours$. The SCr was $0.46{\pm}0.25mg/dL$ and serum Cys-C was $1.43{\pm}0.34mg/L$. The peak and trough concentrations of vancomycin were $24.65{\pm}14.84$ and $8.10{\pm}5.35mcg/mL$, respectively. The calculated GFR based on serum creatinine concentration (GFR-Cr) and GFRcys-c were $70.2{\pm}9.45$ and $63.6{\pm}30.18mL/min$, respectively. The correlation constant for CLvcm and the reciprocal of Cys-C (0.479, P=0.001) was significantly higher than that for CLvcm and the reciprocal of SCr (0.286, P=0.044). GFRcys-c was strongly correlated with CLvcm (P=0.001), and the correlation constant was significantly higher than that for CLvcm and CLcr (0.496, P=0.001). Linear regression analysis showed that only GFRcys-c was independently and positively correlated with CLvcm (F=41.9, P<0.001). Conclusion: The use of serum Cys-C as a marker of CLvcm could be beneficial for more reliable predictions of serum vancomycin concentrations, particularly in neonates.

We have derived the age-specific rates foorm economically active population survey, whoich were conducted by the Bureau of Statistics, Economic Planning Borard of the Korean government. Working life tables are constructed for men wtable these materi- als in the year of 1970, 1980 and 1988 by a modified Wolfbein-Wool's method. Some of the findings may be summerized as follow : 1) A central part of constructing working life table is calculation of stationary' economic active population, which represents the number of men in the stationtary population extoected to be in the labor force at each age group in the life span. The stationary economic active population by age have generally a universal pattern, where they rise sharply in the early twenties, approach its' peak in the thirties decline thereafter. at first graolually and then more rapidly at an advanced age. Korean men show the same general pa ttern of age distribution of stationary eco-- nomic active population with sharp increase hegining from the age interval of 20 to 24, reaching to maximum level at older age. The population. however, presumably, increased substantially due to increaseing school atte endance rates. Another difference exiSts in the youngest age groups, that is the activity rate in the year of 1988 is lower than that of Japan. The table shows an analysis of changes in the age distrihution of labor force between 1970, 1980 and 1988. 2) It was shown an analysis of changes in the age distribution and cause of separation from labor force. The entrance rate to labor force has increased from 18~\5 persons to 299 persons per 1000 head of stationary population between that of 1980 than that of 1988 for Korean men in 20~24 age group. The entrace rate to labor force shows a rapid entrance appearance concentrated on the 15~24 age group. The separation rate from labor force by retirment in Korea in the year of 1988 shows a great difference of the about four times as much as that of Japan. 3) The functions of table illustrate the patterns of working life of males in Korea in 1970, 1980 and 1988. The average remaining number of economically active years, e at age 15 in 1988 is 46.39 which is 2.12 years of increase compared with that of at age 15 in 1970,1980 and 1988 are 43.90,44.27 and 46.39 respectively, showing steadily increase dur- ing the past double decade the increase in the length of economically active life various age may be considered to have come both from extention of general life expectancy and from increasing entrance rate to economic activity in high age that of working is far greater in 1988 than that of 1980. The gaps between expectation of life and average remaining years of economically active widened due to rapid improvement of mortality level in Ko- rea. This observation together with the population pressure by the appearance of a group of younger population implies that constant increase of economically inactive population among older age group.
