Total intrusion and distalization of the maxillary arch to improve smile esthetics

  • This case report illustrates the successful treatment of a patient with skeletal Class II malocclusion and an unesthetic smile involving excessive gingival display and large buccal corridors. By applying dual buccal interradicular miniscrews, total intrusion of the maxillary dentition along with distalization was induced to improve both the occlusion and smile esthetics. In addition to the conventional cephalometric superimposition, three-dimensional superimposition was performed and evaluated to validate the treatment outcome.

  • Objective: To compare the transverse dental changes induced by the palatally applied Frog appliance and buccally applied Karad's integrated distalizing system (KIDS). Methods: We evaluated the pre- and post distalization orthodontic models of 39 patients, including 19 treated using the Frog appliance, which is palatally positioned (Frog group), and 20 treated using KIDS, which is buccally positioned (KIDS group). Changes in intermolar and interpremolar distances and the amount of maxillary premolar and molar rotation were evaluated on model photocopies. Wilcoxon and Mann-Whitney U tests were used for statistical evaluations. A p-value of < 0.05 was considered statistically significant. Results: Significant distopalatal rotation of premolars and distobuccal rotation of molars were observed in Frog group (p < 0.05), while significant distopalatal rotation of molars (p < 0.05), with no significant changes in premolars, was observed in KIDS group. The amount of second premolar and first molar rotation was significantly different between the two groups (p < 0.05 and p < 0.001, respectively). Furthermore, expansion in the region of the first molars and second premolars was significantly greater in KIDS group than in Frog group (p < 0.001 for both). Conclusions: Our results suggest that the type and amount of first molar rotation and expansion vary with the design of the distalization appliance used.

  • II급 부정교합 비발치 치료 시, 특히 유구치의 조기탈락으로 인해 소구치 공간이 상실된 경우 상악 대구치의 원심이동이 필요하게 된다. Jones jig 장치는 환자의 협조가 필요치 않은 구내장치 중 하나로 효과적인 구치 원심이동을 이룰 수 있다. 그러나 고정원의 상실로 인한 부작용이 나타날 수 있으므로 장치 조정과 환자의 선택에 신중을 기해야 한다. 본 증례에서는 Jones jig 장치로 효과적인 구치 원심이동과 더불어 elastic thread를 사용하여 구개측으로 맹출한 소구치의 치열궁 내 배열을 가능하게 하는 부가적인 장점을 경험하였기에 보고하는 바이다.

Three-dimensional analysis of the distal movement of maxillary 1st molars in patients fitted with mini-implant-aided trans-palatal arches

  • Objective: The aim of this study was to investigate three-dimensional molar displacement after distalization via miniscrews and a horizontal modification of the trans-palatal-arch (TPA). Methods: The subjects in this clinical trial were 26 Class II patients. After the preparation of a complete set of diagnostic records, miniscrews were inserted between the maxillary 2nd premolar and 1st molar on the palatal side. Elastic modules connected to the TPA exerting an average force of 150-200 g/side parallel to the occlusal plane were applied. Cone-beam computed tomography was utilized to evaluate the position of the miniscrews relative to the adjacent teeth and maxillary sinus, and the direction of force relative to molar furcation. The distances from the central point of the incisive papilla to the mesiopalatal cusps of the 1st maxillary molars and the distances between the mesiopalatal cusps of the left and right molars were measured to evaluate displacement of the maxillary molars on the horizontal plane. Interocclusal space was used to evaluate vertical changes. Results: Mean maxillary 1st molar distalization was $2.3{\pm}1.1mm$, at a rate of $0.4{\pm}0.2mm/month$, and rotation was not significant. Intermolar width increased by $2.9{\pm}1.8mm$. Molars were intruded relative to the neighboring teeth, from 0.1 to 0.8 mm. Conclusions: Distalization of molars was possible without extrusion, using the appliance investigated. The intrusive component of force reduced the rate of distal movement.

Comparison of treatment effects between the modified C-palatal plate and cervical pull headgear for total arch distalization in adults

  • Objective: The purpose of this study was to evaluate the dental and skeletal effects of the modified C-palatal plate (MCPP) for total arch distalization in adult patients with Class II malocclusion and compare the findings with those of cervical pull headgear. Methods: The study sample consisted of the lateral cephalograms of 44 adult patients with Class II Division 1 malocclusion, including 22 who received treatment with MCPP (age, $24.7{\pm}7.7years$) and 22 who received treatment with cervical pull headgear (age, $23.0{\pm}7.7years$). Pre- (T1) and post-treatment (T2) cephalograms were analyzed for 24 linear and angular measurements. Multivariate analysis of variance was performed to evaluate the changes after treatment in each group and differences in treatment effects between the two groups. Results: The mean amount of distalization at the crown and root levels of the maxillary first molar and the amount of distal tipping was 4.2 mm, 3.5 mm, and $3.9^{\circ}$ in the MCPP group, and 2.3 mm, 0.6 mm, and $8.6^{\circ}$ in the headgear group, respectively. In addition, intrusion by 2.5 mm was observed in the MCPP group. In both groups, the distal movement of the upper lip and the increase in the nasolabial angle were statistically significant (p < 0.001). However, none of the skeletal and soft tissue variables exhibited significant differences between the two groups. Conclusions: The results of this study suggest that MCPP is an effective treatment modality for total arch distalization in adults.

TADs와 R-jig를 이용한 상악 구치 원심 이동에 관한 3차원 유한요소 분석 (3-dimensional finite element analysis of maxillary molar distalization using R-jig with TADs)

  • 목적: 고정식 교정장치에서 removable sliding jig (R-jig)와 temporary anchorage devices (TADs)를 이용하여 상악 구치의 원심 이동 시 힘의 크기와 와이어의 단면 크기에 따른 치아 이동과 치조골에 미치는 효과의 차이를 유한요소방법을 통하여 분석하고자 한다. 연구 재료 및 방법: 정상적인 치아 크기와 악궁 형태를 가진 상악 치아와 상악 치조골을 3차원 형상 모델링하고, 브라켓, 교정용 호선 및 R-jig를 포함한 유한요소 모델을 제작하였다. 골격성 고정원인 mini-implant를 제2소구치와 제1대구치 사이 협측으로 주호선 기준 8 mm 상방 위치시키고, mini-implant를 직접 고정원으로 사용하여 R-jig에 후방력을 부여하였다. 주호선을 $0.019{\times}0.025$ inch SS로 설정하고, R-jig의 와이어 단면 크기를 $0.019{\times}0.025$ inch SS, 후방력의 크기를 200 gm, 결찰을 하지 않는 조건을 기준 조건으로 설정하였다. 그리고 R-jig의 와이어 단면 크기, 후방력의 크기를 다르게 한 조건들을 설정하여 비교하였다. 결과: R-jig의 와이어 단면 크기가 증가함에 따라 후방력에 의한 R-jig의 변형이 적게 관찰되었지만, 제2대구치의 변위량은 아주 미미한 차이를 보였다. 제2대구치에 가해지는 후방력의 크기가 증가할수록 변위량도 같이 증가하였고, 원심경사 경향과 수직 변위량도 더 커졌다. 결론: R-jig는 임상적으로 큰 부작용 없이 치아의 원심이동을 가능하게 한다.

Distalization pattern of whole maxillary dentition according to force application points

  • Objective: The purpose of this study was to observe stress distribution and displacement patterns of the entire maxillary arch with regard to distalizing force vectors applied from interdental miniscrews. Methods: A standard three-dimensional finite element model was constructed to simulate the maxillary teeth, periodontal ligament, and alveolar process. The displacement of each tooth was calculated on x, y, and z axes, and the von Mises stress distribution was visualized using color-coded scales. Results: A single distalizing force at the archwire level induced lingual inclination of the anterior segment, and slight intrusive distal tipping of the posterior segment. In contrast, force at the high level of the retraction hook resulted in lingual root movement of the anterior segment, and extrusive distal translation of the posterior segment. As the force application point was located posteriorly along the archwire, the likelihood of extrusive lingual inclination of the anterior segment increased, and the vertical component of the force led to intrusion and buccal tipping of the posterior segment. Rotation of the occlusal plane was dependent on the relationship between the line of force and the possible center of resistance of the entire arch. Conclusions: Displacement of the entire arch may be dictated by a direct relationship between the center of resistance of the whole arch and the line of action generated between the miniscrews and force application points at the archwire, which makes the total arch movement highly predictable.

Mandibular skeletal posterior anatomic limit for molar distalization in patients with Class III malocclusion with different vertical facial patterns

  • Objective: The aim of this study was to compare the differences in mandibular posterior anatomic limit (MPAL) distances stratified by vertical patterns in patients with skeletal Class III malocclusion by using cone-beam computed tomography (CBCT). Methods: CBCT images of 48 patients with skeletal Class III malocclusion (mean age, 22.8 ± 3.1 years) categorized according to the vertical patterns (hypodivergent, normodivergent, and hyperdivergent; n = 16 per group) were analyzed. While parallel to the posterior occlusal line, the shortest linear distances from the distal root of the mandibular second molar to the inner cortex of the mandibular body were measured at depths of 4, 6, and 8 mm from the cementoenamel junction. MPAL distances were compared between the three groups, and their correlations were analyzed. Results: The mean ages, sex distribution, asymmetry, and crowding in the three groups showed no significant differences. MPAL distance was significantly longer in male (3.8 ± 2.6 mm) than in female (1.8 ± 1.2 mm) at the 8-mm root level. At all root levels, MPAL distances were significantly different in the hypodivergent and hyperdivergent groups (p < 0.001) and between the normodivergent and hyperdivergent groups (p < 0.01). MPAL distances were the shortest in the hyperdivergent group. The mandibular plane angle highly correlated with MPAL distances at all root levels (p < 0.01). Conclusions: MPAL distances were the shortest in patients with hyperdivergent patterns and showed a decreasing tendency as the mandibular plane angle increased. MPAL distances were significantly shorter (~3.16 mm) at the 8-mm root level.

Do platelet concentrates accelerate orthodontic tooth movement?: a systematic review

  • Purpose: Surgical techniques in orthodontics have received widespread attention in recent years. Meanwhile, biomaterials with high molecular content have been introduced, such as platelet concentrates (PCs), which may accelerate orthodontic tooth movement (OTM) and reduce periodontal damage. The present systematic review aimed to answer the following PICO question: "In patients in whom orthodontic surgical techniques are performed (P), what is the effectiveness of using PCs over the surgical site (I) when compared to not placing PCs (C) to achieve faster tooth movement (O)?" Methods: A search was performed in 6 databases. The criteria employed were those described in the Preferred Reporting Items for Systematic Reviews and Meta-Analyses declaration. The present review included studies with a control group that provided information about the influence of PCs on the rate of OTM. Results: The electronic search identified 10 studies that met the established criteria. Conclusions: The included studies were very diverse, making it difficult to draw convincing conclusions. However, a tendency was observed for OTM to be accelerated when PCs were used as an adjuvant for canine distalization after premolar extraction when distalization was started in the same session. Likewise, studies seem to indicate an association between PC injection and the amount of canine retraction. However, it is not possible to affirm that the use of PCs in corticotomy shortens the overall treatment time, as this question has not been studied adequately.
