  • A diffuse alveolar hemorrhage (DAH) is a distinct form of pulmonary hemorrhage that originates from the pulmonary microcirculation. Disseminated intravascular coagulation (DIC) is one cause of DAH. Although HELLP syndrome associated with DIC can cause DAH, there are no published case reports that the authors are aware of. We report the case of a pregnant woman with HELLP syndrome who developed DAH. Because pregnant women with HELLP syndrome can develop DAH as a form of ARDS, a bronchoalveolar lavage may be used to make a differential diagnosis of this lung manifestation.

Localized and disseminated histiocytic sarcoma in two dogs (개에서 국소형과 범발성형 조직구육종 2예)

  • A male mixed breed dog with unknown age (case 1) and 5-year-old female golden retriever (case 2) were admitted to the Department of Veterinary Pathology at College of Veterinary Medicine, Seoul National University after sudden death. At necropsy, stifle region of case 1 was replaced by coalescing firm tan nodules approximately 1-3 cm in diameter. Neoplastic nodules were also presented in the lung. In case 2, the spleen was enlarged 3-5 times its normal size and numerous firm tan nodules were noted in the kidney, pericardium, diaphragm and lymph nodes. Histopathologically, the neoplastic masses of case 1 consisted of plump spindle cells having abundant cytoplasm and indistinct cell borders that formed interlacing bundle. In case 2, the masses were consisted of round to polygonal cells with abundant eosinophilic cytoplasm and eccentric nuclei. Numerous multinucleated giant cells containing nuclei of various sizes were noted. Immunohistochemically, neoplastic cells of all two cases stained intensely with vimentin and lysozyme. Based on the gross and light microscopic findings together with immunohistochemistry, both cases were diagnosed as localized histiocytic sarcoma with pulmonary metastasis and disseminated histiocytic sarcoma, respectively.

  • Oligodendrogiomas account for about 4 per cent of intracranial gliomas and surgery is known to be an essential first step to establish an accurate diagnosis and when oligodendrogliomas recur with or without anaplastic features after initial resection, radiation and chemotherapy consisting of the administration of procarbazine, lomustine, and vincristine are usually indicated. We report our experience of an excellent result with intraventricular methotrexate chemotherapy for a patient with disseminated anaplastic oligodendroglioma. A 29-year-old male patient presented with diplopia and headache for two months. MRI showed a irregular, faintly enhanced mass in the posterior fossa. The hisotological diagnosis was an anaplaplastic oligodendroglioma and he was treated with chemotherapy of PCV regimen and radiotherapy followed by surgery. CSF dissemination was revealed by a follow-up MRI during the period. Intraventricular methotrexate(0.175mg/kg) was given twice a week for 4 weeks through ommaya reservoir and the size of the multiple tumors was decreased significantly on follow-up MRI. This case report suggests that an aggressive treatment involving intravent-ricular chemotherapy may be helpful even when anaplastic oligodendrogliomas disseminates to leptomeninges.

Partial Interferon-γ Receptor Deficiency in Patients with Disseminated Tuberculosis (파종성 결핵 환자에서 interferon-γ 수용체의 부분결핍에 관한 연구)

  • Background : Interferon-gamma ($IFN-{\gamma}$) is essential in the immune response to mycobacterial infections, and a complete or partial deficiency in the $IFN-{\gamma}$ receptor 1 ($IFN{\gamma}R1$) or the $IFN-{\gamma}$ receptor 2 ($IFN{\gamma}R2$) have been reported to confer susceptibility to a disseminated infection with nontuberculous mycobacteria. However, similar mutations in the $IFN-{\gamma}$ receptor have not been specifically examined in the patients with clinical tuberculosis. Methods : This study searched for mutations in the $IFN-{\gamma}$ receptor gene that resulted in a partial $IFN-{\gamma}$ receptor deficiency in six patients with disseminated tuberculosis. The previously identified $IFN{\gamma}R1$ and $IFN{\gamma}R2$ coding regions were sequenced after amplification. Results : There was no partial $IFN{\gamma}R1$ deficiency including a homozygous recessive missense mutation causing an amino-acid substitution in the extracellular domain of the receptor (I87T) and a hotspot for small deletions (818delT, 818del4, 818insA) found in any of the patients. In addition, a partial $IFN{\gamma}R2$ deficiency of the homozygous missense mutation (R114C) was not found in any of the patients. Conclusion : Genetic defects causing a partial $IFN-{\gamma}$ receptor deficiency were not identified in our patients with disseminated tuberculosis.

Identifying the Pattern of Furniture Possession of Bedrooms in 3 Bed-reem Apartment of Korea (한국 3침실형 아파트의 침실공간 가구보유패턴 연구)

  • During last 4 decades of industrial development, apartment has been rapidly disseminated throughout the nation. Its main progress, however, only appeared in quantity, not in quality. To promote housing industry in more way and to enhance peoples' life quality through the better housing environment, housing plan should be based on living items including furniture, appliances and etc. Not enough research has been done to find out living items for better house plan development and furniture design. Among living items, furniture reflects both directly and indirectly people's lifestyle and way of living. This study attempted to find out the pattern of furniture possession which people own in their bedrooms. An in depth questionnaire survey of 600 households was carried during the year 2000. A variety of patterns were found which indicated diverse living conditions within a very similar uniform multifamily apartment complex. The result of this study will be helpful to understand the living conditions in a more systematic way and to generate new concept for both residential unit plan and furniture design.

Cerebral Toxoplasmosis Combined with Disseminated Tuberculosis

  • A 24-year-old man presented with mental change, fever, abdominal pain, tenderness and palpable mass on the lower abdomen. He was a non-Korean engineer and did not accompany a legal guardian, so medical history taking was difficult due to his mental status. Brain magnetic resonance imaging showed multiple rim-enhanced lesions of the brain, and abdominal computed tomography showed huge paraspinal abscess. Chest X-ray and computed tomography showed poorly defined nodular opacities. We initially thought that this patient was infected with toxoplasmosis with typical cerebral image finding and immunoglobulin laboratory finding of cerebrospinal fluid and serum study. The abdominal abscess was confirmed as tuberculosis through the pathologic finding of caseous necrosis. We used anti-tuberculosis medication and anti-toxoplasmosis medication for almost 4 months, and then his clinical state and radiological findings were considerably improved.

Efficient Distributed Storage for Space Information Network Based on Fountain Codes and Probabilistic Broadcasting

  • This article investigates the distributed data storage problem in the space information network (SIN) using distributed fountain codes. Since space nodes in the SIN are resource-limited, in order to reduce energy consumption while improving the storage reliability, an efficient distributed storage based on fountain codes and probabilistic broadcasting (DSFPB) strategy is proposed. In the proposed strategy, source packets are disseminated among the entire network according to probabilistic broadcasting (PBcast), and the final degree distribution is close to the desired robust soliton distribution (RSD), this is benefited from the appropriate packets encoding procedure of the proposed strategy. As presented by the analysis and simulations, the total cost of data dissemination is greatly reduced compared with existing representative strategies, while improving the decoding performance.

  • Mucormycosis is a rare but fatal fungal infection with low survival rate in immune-compromised patients. It is caused by a fungus belonging to the Mucoraceae family of the Zygomycetes class. Mucormycosis is classified as rhino-orbital-cerebral, pulmonary, cutaneous, gastrointestinal, disseminated, and miscellaneous types according to its clinical manifestations. Early diagnosis and treatment along with correction of the underlying medical condition is important for favorable results. This case presentation describes mucormycosis involving the anterior maxillary region in a leukemic patient with prolonged neutropenia. The patient benefited from a timely biopsy and immediate treatment with amphotericin B, and was successfully managed with an interdisciplinary team approach consisting of dental and several medical specialists.

  • Ethernet ring protection is a new technology introduced in ITU-T Recommendation G.8032, which utilizes the generic Ethernet MAC functions. We introduce an alternative enhanced protection switching scheme to suppress penalty in the switching transient, in which the Ethernet MAC filtering database (FDB) is actively and directly modified by information disseminated from the nodes adjacent to failure. The modified FDB at all nodes are guaranteed to be consistent to form a complete new ring network topology immediately. This scheme can reduce the capacity requirement of the G.8032 by several times. This proposed scheme can be also applied in IP protection rings.
