Preventing the Transmission of Tuberculosis in Health Care Settings: Administrative Control

  • It is well established that health care workers (HCWs) have a considerably higher risk of occupationally acquired tuberculosis (TB). To reduce the transmission of TB to HCWs and patients, TB infection control programs should be implemented in health care settings. The first and most important level of all protection and control programs is administrative control. Its goals are to prevent HCWs, other staff, and patients from being exposed to TB, and to reduce the transmission of infection by ensuring rapid diagnosis and treatment of affected individuals. Administrative control measures recommended by the United States Centers for Disease Control and Prevention and the World Health Organization include prompt identification of people with TB symptoms, isolation of infectious patients, control of the spread of the pathogen, and minimization of time spent in health care facilities. Another key component of measures undertaken is the baseline and serial screening for latent TB infection in HCWs who are at risk of exposure to TB. Although the interferon-gamma release assay has some advantages over the tuberculin skin test, the former has serious limitations, mostly due to its high conversion rate.

  • Tuberculosis, one of the oldest diseases known to affect humans, is caused by bacteria belonging to the Mycobacterium luberculosis complex. The diseases usually affects the lungs, although in up to one-third of cases other organs are affected. If properly treated, tuberculosis caused by drug-susceptible strains is curable in virtually all cases. If untreated, the disease may be fatal within 5 years in more than half of cases. Transmission usually takes place through the airborne spread of droplets of nuclei produced by patients with infectious pulmonary tuberculosis. Two patients with Pulmonary Tuberculosis and one with Tuberculous Meningitis were recently examined. The changes in these patient's symptoms through both western medical treatment and oriental medical treatment are reported.

  • Recently, domestic animal disease caused tremendous damage to farmhouses and the damage stretched in nationwide with the spread of epidemic disease. To prevent animal diseases from happening again, the system development to easily measure the temperature of sick animals and identify of them is needed, thereby quickly treat them, reducing losses of farmhouses. However, a lack of related equipment and human resource hampered its effort to minimize its losses. This study tries to develop diagnosis system as part of measures to curb these domestic animal diseases. This paper present the 센서 node based on IEEE 802.15.4 which can be attached to the animal body for real-time temperature measurement. We design and implement tiny chip-type that can be attached to the body of animals. Then, we use available power only when measuring temperatures in a long term-basis. In this paper, the 센서 node was applied to horse's neck. We measure the horse's body temperature between $32.2^{\circ}C{\sim}33.7^{\circ}C$ and analyze phenomenon data for 4 months.

Commercial Sex Workers' Condom Use Behavior in Korea

  • Purpose. With the continuous spread of the AIDS virus and other sexually transmitted diseases (STDs) throughout Asia, it has become imperative for HIV/AIDS prevention outreach programs to stress the need for using prophylactics as one form of protection. This is particularly true for commercial sex workers. This pilot study was conducted to investigate the rate of condom use among the commercial sex workers in Korea and reasons why they frequently do not wear condoms during sexual intercourse. Methods. Fifty female commercial sex workers who worked in the Yong San area were recruited while outreach was performed and forty eight responses were analyzed due to two incomplete questionnaires. The data were collected on June 28, 2004 as part of an outreach program for condom use promotion. Results. The subjects ranged in age from 21 to 45 years. Nearly half of the subjects ($47.9\%$) did not consistently use condoms. The major reason for not using protection was clients' reluctances for using condoms. When condoms were used, the subjects preferred lubricant-rich and less painful condom during intercourse. Conclusion. Due to the inconsistent use of condoms, it is clear that many commercial sex workers and clients are susceptible to sexually transmitted diseases. Therefore, it is important to not only educate commercial sex workers, but also to educate their pimps and clientele about the importance of condom use. In general, it is recommended that practical strategies and a policy for condom use promotion be developed.

  • Bronchopulmonary endometriosis is a rare cause of hemoptysis. We describe a woman with recurrent hemoptysis during menstruation. The site of origin of hemoptysis was localized on chest CT images. CT appears to be the method of choice for the correct intrapulmonary localization of this disease. The hematogenous spread from the pelvis to the lungs has been proposed for the mechanism for the development of parenchymal pulmonary endometriosis. Thoracic endometriosis is reviewed and new therapeutic modalities are discussed.

Genomic Insights into Nematicidal Activity of a Bacterial Endophyte, Raoultella ornithinolytica MG against Pine Wilt Nematode

  • Pine wilt disease, caused by the nematode Bursaphelenchus xylophilus, is one of the most devastating conifer diseases decimating several species of pine trees on a global scale. Here, we report the draft genome of Raoultella ornithinolytica MG, which is isolated from mountain-cultivated ginseng plant as an bacterial endophyte and shows nematicidal activity against B. xylophilus. Our analysis of R. ornithinolytica MG genome showed that it possesses many genes encoding potential nematicidal factors in addition to some secondary metabolite biosynthetic gene clusters that may contribute to the observed nematicidal activity of the strain. Furthermore, the genome was lacking key components of avermectin gene cluster, suggesting that nematicidal activity of the bacterium is not likely due to the famous anthelmintic agent of wide-spread use, avermectin. This genomic information of R. ornithinolytica will provide basis for identification and engineering of genes and their products toward control of pine wilt disease.

  • Primary lung cancer can metastasize by a direct local extension, hematogenous dessemination, and lymphatic spread. However, it less commonly metastasizes via a transbronchial invasion. Approximately half of all patients with lung cancer have metastases at the initial presentation. Autopsy data showed that there are an average of 4.8 metastatic sites. The most common sites for metastases include the lymph nodes, liver, adrenal gland, bone, and brain. However clinically significant metastases isolated in the small bowel seldom occur. Here we report a case of lung cancer with a small bowel metastasis.

  • Objectives: In this paper, we aimed to investigate the evolving debate over border closure in Korea during the coronavirus disease 2019 (COVID-19) pandemic, to address the main themes associated with border closure, and to discuss the factors that need to be considered when making such decisions. Methods: We collated and reviewed previously conducted review studies on border closures during infectious disease outbreaks to derive relevant themes and factors. Results: According to our systematic review on border closures and travel restrictions, the effects of such containment efforts are limited. We suggest considering the following factors when determining whether to impose border closure measures: (1) disease characteristics, (2) timeliness of implementation, (3) transmission delay and the basic reproduction number, (4) globalization and pandemics, and (5) social and economic costs. Conclusions: Our assessment indicates that the effects of border closures are at best temporary and limited. Alternative measures must be contemplated and implemented to suppress the spread of COVID-19 in particular and infectious diseases more broadly.

  • During the last two decades, the rapid increase in aquaculture world wide, particularly in finfish and shellfish culture, is the result of both area expansion and production intensification. Under these conditions, the prevalence and spread of infectious diseases have increased as a result of higher infection pressure and decreased resistance of the aquatic animals. Accordingly, the effective control of infectious diseases has become more and more important to development of aquaculture. Recently the Pathology Team of National Fisheries Research and Development(NFRDI), has played pivotal roles in establishing the national management and control system of aquatic animal health and disease by new legislative works and technical guidelines. Under the national aquatic animal disease control framework, the Pathology Team of NFRDI and MOMAF(Ministry of Maritime Affairs and Fisheries) should also establish effective management and regulations, with the support of enforcement and monitoring policies. In conjunction with effective aquatic disease national management, financial assistance or alternative compensation options for aquaculture should also be available in the event of production losses or eradication system.

  • 현재 지구촌은 신종 감염병인 '코로나바이러스 감염증-19(COVID-19)로 인해 혼란을 겪고 있다. 우리나라의 경우 1만 명 이상 감염자가 발생하였으며, 사망자 역시 200여 명이 넘었다. 하지만 위와 같은 감염병을 치료하기 위한 백신은 계속 개발중에 있으며, 현재는 감염 예방 수칙으로 감염의 위험성에서 벗어나고 있는 실정이다. 또, 지구촌에 큰 혼란을 주었던 사스, 신종플루, 메르스와 같이 주로 호흡기를 통해 타인에게 전파되는 신종 감염병 출현 빈도 역시 계속 증가하고 있다. 이외에는 황사, 미세먼지에 관한 문제들로 인해 일상속에서 이뤄지는 생활 방역의 중요성이 대두되고 있는 상황이다. 본 논문에서는 생활 방역의 대표적 수칙인 마스크 착용에서 상황별 올바른 마스크 선택의 중요성을 알려주어 시민의식을 높일 수 있는 VR 연동 'Wearing a Mask' 구현에 대해 제안하고자 한다.
