OECD 국가를 중심으로 한 의료개혁 동향과 교훈 (Health Care Reform in OECD and It's Lessons)

  • Health policies in many countries have come under critical scrutiny in recent years. This is because of increasing national health expenditures. Also many persons in health sector have been the perception that resources allocated to health services are not always deployed in an optimal fashion. And they believe that the scope of resources in health services is limited, there is need to search for ways of using existing resources more efficiently. A further concern has been the desire to ensure access to healthcare of various groups on an equitable basis. In some European countries this has been linked to a wish to enhance patient choice and to make service providers more responsive to consumers, while Korea integrated health insurance funds into single fund in 2000. Many European countries are under considerable pressure to review and restructure their health care systems. There are several reasons of pressure to reform. There are demographic changes, pattern of disease change, advances in medical sciences will also give rise to new demands within the health services, public expectations of health services are rising as those who use services demand higher standards of care. These circumstances require the change of health care delivery system based on hierarchical regionalism, which was basis of health care delivery since 1920s. Korea is also under similarly pressure to restructure our own health care systems. We will have good learning from OECD experiences. In this paper we reviewed and compared among OECD countries' various experiences.

노인 건강증진 프로그램의 효과 분석 (The Effectiveness of Health Promotion Program for the Elderly)

  • Purpose: This study was conducted in order to identify the effect of a community based health promotion program for elders in Korea. Methods: The PICO-SD (Participants, Intervention, Comparison, Outcome, and Study Design) strategy was established. A search of the electronic bibliographic database of NDSL, RISS, KMbase, and KoreaMed etc. was performed and 28 articles met inclusion criteria. Results: Both exercise and health education were in most programs and were more effective than one of the two. The health education included stress management, mental health, health promotion lifestyle, chronic disease, and medication. Various exercises such as walking, stretching, gymnastics, rhythmic activity, muscle strength, and joint exercise were performed. The programs for elders showed an association with significantly improved muscular strength, flexibility, depression, quality of life, subjective health status, satisfaction with life, self-efficacy, and health promotion behavior. Conclusion: Intervention programs including both exercise and health education are effective in improving health promotion behavior and physical and psychological health status in elders. Therefore, these results could provide useful guidelines for development of effective health intervention programs for the elderly.

정신약물 복용과 관련된 국내 간호연구의 고찰 (Review of Nursing Research on Psychotropic Drugs in Korea)

  • Purpose: This study critically reviewed nursing research psychotropic drugs that has been published in Korean journals. Another aim of this study was to identify trends in nursing research on psychotropic drugs and make suggestions for further study in Korea. Methods: Data were collected from degree theses and original articles on psychotropic drugs published in Korean journals from 1992 to 2013. Thirty-four articles were analyzed of which at least one nursing author participated in the study. Search keywords were "psychotropic drug" and "mentally ill patient & medication". Results: For the research design, quasi-experimental study was 58.8%, descriptive study was 17.7%, descriptive correlational study was 8.8%, qualitative study was 8.8% and model development research was 5.9%. Variables measured were knowledge of medication & symptom management, knowledge of disease, side effects, drug attitude, medication pattern, diet & activity, quality of life, and self-care. Conclusion: Despite recent increased interest in psychiatric medication, research on psychotropic drugs remains very limited, particularly regarding findings from a nurse's perspective. More research project should be designed to develop programs for the treatment of side effects from a nursing view-point.

Inhibitory effects of Tinospora cordifolia and Rubia cordifolia Linn. on egg albumin-induced experimental allergic conjunctivitis in rats

  • Allergic conjunctivitis is the most common allergic disease. These diseases are severe & frequent which requires search of new treatments. The aim of the study was to investigate the effects of Tinospora cordifolia (TC), Rubia cordifolia Linn. (RC) on experimentally induced allergic conjunctivitis in rats. In this study, dried water soluble extracts of TC and RC. (250 and 500 mg/kg, p.o. for 7 days) were evaluated for their antiallergic activity in Wistar rats. They were tested for inhibition of egg albumin-induced vascular permeability, inhibition of histamine release from the rat conjunctiva as well as in histamine content in tears. TC and RC showed significant (P < 0.05) inhibition in vascular permeability, inhibition in histamine release from the rat conjunctiva which is reflected by reduced level of histamine content in tears. The activities were found to be comparable to azelastine hydrochloride. These results suggest that the inhibitory effect on egg albumin-induced experimental allergic conjunctivitis in rat may be due to the antihistaminic activity of TC and RC. Our studies provide evidence that TC and RC may be beneficial in the treatment of allergic conjunctivitis.

신증후군 환아 어머니의 불안에 따른 대응양상 (An Inquiry on the Coping about Anxiety in Mothers of Children with Nephrotic Syndrome)

  • Purpose: This study was to identify and to search the related disposition of the pattern of anxiety and coping in mothers of children with nephrotic syndrome through the use of Q-methodology. Method: 34 Q-samples were finally selected in the concept of anxiety and coping. A P-sample of 35 was selected the mothers of children with nephortic syndrome. The result of the Q-sorting was coded and analyzed using QUANL PC program. Result: There were 3 types of special opinion. The first type is called ' Pursuit of hope type.' Members of this type were cope with the anxiety by spiritual behavior like a pray, positive thinking. The second type is called 'Worry about reality type.' Members of this type were to be filled with apprehension like an indigestion, insomnia. The third type is called ' Solving problem type.' Members of this type were cope with the humanity effort by conversation. Conclusion: The mothers of children with nephrotic syndrome were used various coping patterns to cope with the anxiety conditions that their child were result from admission to hospital and treatment of the disease. Therefore, nursing assessment and nursing intervention skills have to develop in consideration of the subjectivity of coping about anxiety in each individual.

Recent Advances in Toxoplasma gondii Immunotherapeutics

  • Toxoplasmosis is an opportunistic infection caused by the protozoan parasite Toxoplasma gondii. T. gondii is widespread globally and causes severe diseases in individuals with impaired immune defences as well as congenitally infected infants. The high prevalence rate in some parts of the world such as South America and Africa, coupled with the current drug treatments that trigger hypersensitivity reactions, makes the development of immunotherapeutics intervention a highly important research priority. Immunotherapeutics strategies could either be a vaccine which would confer a pre-emptive immunity to infection, or passive immunization in cases of disease recrudescence or recurrent clinical diseases. As the severity of clinical manifestations is often greater in developing nations, the development of well-tolerated and safe immunotherapeutics becomes not only a scientific pursuit, but a humanitarian enterprise. In the last few years, much progress has been made in vaccine research with new antigens, novel adjuvants, and innovative vaccine delivery such as nanoparticles and antigen encapsulations. A literature search over the past 5 years showed that most experimental studies were focused on DNA vaccination at 52%, followed by protein vaccination which formed 36% of the studies, live attenuated vaccinations at 9%, and heterologous vaccination at 3%; while there were few on passive immunization. Recent progress in studies on vaccination, passive immunization, as well as insights gained from these immunotherapeutics is highlighted in this review.

Antioxidant Effects of Plant Extracts on Free Radicals and Lipid Peroxidation

  • Reactive oxygen species damage biomolecules such as lipids, proteins, sugars and DNA, which can not only lead to various disease but also oxidative damage resulting aging. In order to search for antioxidants from plants, the antioxidant effects of the MeOH extracts from 182 plants were evaluated. The results showed that thirteen plant extracts exhibited antioxidant activity (>80%) in DPPH radical assay, seven plant extracts demonstrated antioxidant activity (>40%) in the hydroxyl radical assay and eighteen plant extracts were active (>80%) in the lipid peroxidation assay. In particular, the extracts of Distylium racemosum (Hamamelidaceae), Astilbe koreana (Saxifragaceae), Astilbe chinensis and Euphorbia supina (Euphorbiaceae) were identified as potent principles of antioxidant activity in all the assay systems.

치매노인의 낙상위험요인에 관한 메타분석 (A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.

Hypoxia-inducible Factor 1 Alpha (HIF-1α) as a Prognostic Indicator in Patients with Gastric Tumors: A Meta-analysis

  • Background and Objective: Though researched for years, the prognostic role of hypoxia-inducible factor 1 alpha (HIF-$1{\alpha}$) in gastric cancer is still controversial. We thus undertook a systematic review to assess the relationship. Method: A systematically literature search of Pubmed, Embase, Web of Science, China Biological Medicine Disc and Cochrane Library was undertaken in February 2013, and the reference lists of articles were retrieved. Results: 12 trials (1,555 participants) were included to assess the association between HIF-$1{\alpha}$ expression and survival. Summary hazard ratios (HRs) were calculated. HIF-$1{\alpha}$ expression was significantly correlated with poor overall survival of gastric cancer patients (HR=1.34, 95%CI: 1.13-1.58; P=0.0009), but not with poor disease free survival of gastric cancer patients (HR=1.67, 95%CI: 0.99-2.82; P=0.06). Conclusion: HIF-$1{\alpha}$ was associated with poor OS, but not DFS, especially for Asian patients. But studies evaluating relationships of HIF-$1{\alpha}$ with OS and DFS in non-Asian gastric cancer patients appear needed.

Antiallergy drugs from Oriental medicines

  • Although Oriental medicines have long been used effectively in treating many diseases throughout the world, the pharmacological mechanisms of most Oriental medicines used have not been defined. As part of our continuing search for biologically active antiallergic drugs from natural sources, Oriental medicines were analyzed. Some Oriental medicines have been used against various allergic diseases for generations, and still occupies an important place in traditional medicine in Korea. It is also still unclear how Oriental medicine prevents allergic disease in experimental animal models. Some Korean folk medicines inhibited the mast cell-mediated allergic reaction. This review summarizes the effective folk medicine in experimental effect of allergic reaction. Potential antiallergic folk medicines include: Poncirus trifoliata; Siegesbeckia glabrescence; Solanum lyratum; Aquilaria agallocha; Ulmi radicis; Polygonum tinctorium; Hwanglyun-Haedok-Tang; Rehmannia glutinosa; Kum- Hwag-San; Syzygium aromaticm; Spirulina platensis; Sosiho-Tang; Sinomenium acutum; Schizonepta tenuifolia; Shini-San; Magnoliae flos; Sochungryoung-Tang; Oryza sativa; Cryptotympana atrata; Salviae radix; Rosa davurica; Asiasari radix; Chung-Dae-San; and Cichorium intybus. Understanding the mechanisms of action for these Oriental medicines can permit drug development and laying of the ground-work for evaluating potential synergistic effects by addition and subtraction of prescriptions.
