  • 한국인의 암 사망률 1위를 차지하는 폐암은 발견되기 전까지 별다른 증상이 없어 환자는 병을 쉽게 인지하지 못하고, 기존의 진단법 또한 초기단계에는 적용이 어렵다. 해결책으로서, 분자수준에서의 체액분석을 폐암진단에 도입하는 방안이 제시되고 있다. 이를 위한 분석기기 가운데 대표적으로는 칩 기반 바이오센서가 있으며, 이 센서의 큰 장점으로는 고가의 분석장비나 숙련된 분석인력이 없이도 현장에서의 진단이 가능하다는 점이다. 본 미니총설에서는 폐암 진단에 활용가능한 혈액 내 바이오마커와 바이오칩 센서의 연구현황을 소개하고 이들의 발전가능성에 대해 논의하고자 한다.

Specific Detection of Acanthamoeba species using Polyclonal Peptide Antibody Targeting the Periplasmic Binding Protein of A. castellanii

  • Acanthamoeba keratitis (AK) is a rare ocular disease, but it is a painful and sight-threatening infectious disease. Early diagnosis and adequate treatment are necessary to prevent serious complications. While AK is frequently diagnosis via several PCR assays or Acanthamoeba-specific antibodies, a more specific and effective diagnostic method is required. This study described the production of a polyclonal peptide antibody against the periplasmic binding protein (PBP) of A. castellanii and investigated its diagnostic potential. Western blot analysis showed that the PBP antibody specifically reacted with the cell lysates of A. castellanii. However, the PBP antibody did not interact with human corneal epithelial (HCE) cells and the other 3 major causative agents of keratitis. Immunocytochemistry (ICC) results revealed the specific detection of A. castellanii trophozoites and cysts by PBP antibodies when A. castellanii were co-cultured with HCE cells. PBP antibody specificity was further confirmed by co-culture of A. castellanii trophozoites with F. solani, S. aureus, and P. aeruginosa via ICC. The PBP antibody specifically reacted with the trophozoites and cysts of A. polyphaga, A. hatchetti, A. culbertsoni, A. royreba, and A. healyi, thus demonstrated its genus-specific nature. These results showed that the PBP polyclonal peptide antibody of A. castellanii could specifically detect several species of Acanthamoeba, contributing to the development of an effective antibody-based AK diagnostics.

Prospective evaluation of the clinical utility of whole-exome sequencing using buccal swabbing for undiagnosed rare diseases

  • Purpose: Whole-exome sequencing (WES) has been a useful tool for novel gene discovery of various disease categories, further increasing the diagnostic yield. This study aimed to investigate the clinical utility of WES prospectively in undiagnosed genetic diseases. Materials and Methods: WES tests were performed on 110 patients (age range, 0-28 years) with suspected rare genetic diseases. WES tests were performed at a single reference laboratory and the variants reported were reviewed by clinical geneticists, pediatricians, neurologists, and laboratory physicians. Results: The patients' symptoms varied with abnormalities in the head or neck, including facial dysmorphism, being the most common, identified in 85.4% of patients, followed by abnormalities in the nervous system (83.6%). The average number of systems manifesting phenotypic abnormalities per patient was 3.9±1.7. The age at presentation was 2.1±2.7 years old (range, 0-15 years), and the age at WES testing was 6.7±5.3 years (range, 0-28 years). In total, WES test reported 100 pathogenic/likely pathogenic variants or variants of uncertain significance for 79 out of 110 probands (71.8%). Of the 79 patients with positive or inconclusive calls, 55 (50.0%) patients were determined to have good genotype-phenotype correlations after careful review. Further clinical reassessment and family member testing determined 45 (40.9%) patients to have been identified with a molecular diagnosis. Conclusion: This study showed a 40.9% diagnostic yield for WES test for a heterogeneous patient cohort with suspected rare genetic diseases. WES could be the feasible genetic test modality to overcome the diversity and complexity of rare disease diagnostics.

Evaluation of Clinical Efficacy and Safety Following Kyungokgo-Gamibang Administration in Dogs with Skin and Joint Diseases

  • Skin and joint diseases are relatively common in dogs. Nutritional complementation is one of the various management strategies for these disorders. This study evaluated the safety and clinical efficacy of Kyungokgo-gamibang in dogs with skin and joint diseases. Thirty dogs with diseases were included and divided into three groups: control group (n = 15), skin group (n = 10), and joint group (n = 5). The skin and joint groups were fed skin and joint gums composed of Kyungokgo-gamibang extract with standard treatment for four weeks. The control group included dogs with skin diseases who were administered standard skin infection treatment for 4 weeks. The physical and laboratory results showed no remarkable adverse effects of Kyungokgo-gamibang extract after its administration in dogs. Clinical efficacy was evaluated using quality of life scale, and levels of cytokines, including interferon-γ, interleukin (IL)-2, IL-6, IL-8, IL-10, monocyte chemoattractant protein-1, and tumor necrosis factor-α, for 4 weeks in all groups. Dermatologic clinical scales were performed for 4 weeks in the control and skin groups. Both the control and skin groups had significantly decreased dermatologic clinical scales, including pruritus and erythema scales (p < 0.05). Among the cytokine levels, only IL-2 concentration was significantly decreased in the skin group after 4 weeks of administration of the Kyungokgo-gamibang extract (p = 0.032). There was no significant difference between the levels of cytokines on days 0 and 28 in the joint group. The quality of life scale was significantly increased after week 4 compared to week 0 in the skin (p = 0.008) and joint groups (p = 0.041). This study suggests that Kyungokgo-gamibang extract can be applied in managing dogs affected by skin and joint diseases without adverse effects.

  • Objective : We have studied the part, definition, diagnostic generaliztion and pathological viewpoint of Qi mouth and Renying by refering literatures Methods : I refered to records from anciant to modern. The results are followings : 1. Qi mouth and Renying is ancient diagnostic method and the origin of diagnostic method through the pulsation of radial artery. And Qi mouth is recognized as the right and left Qi mouth and Renying is located on pulsation points of both common carotid arteries. 2. The yin of five viscera, nutritional blood and viscous liquid are diagnosed by Qi mouth which is inclued lung meridian. And the yang of six organs, guarding Qi and the rise and fall of stomach energy are diagnosed by Renying which is inclued stomach meridian. 3. It is possible that the position, process and transformational condition of disease are decided by comparing and examining Qi mouth and Renying.

Fabrication and Simulation of Fluid Wing Structure for Microfluidic Blood Plasma Separation

  • Human blood consists of 55% of plasma and 45% of blood cells such as white blood cell (WBC) and red blood cell (RBC). In plasma, there are many kinds of promising biomarkers, which can be used for the diagnosis of various diseases and biological analysis. For diagnostic tools such as a lab-on-a-chip (LOC), blood plasma separation is a fundamental step for accomplishing a high performance in the detection of a disease. Highly efficient separators can increase the sensitivity and selectivity of biosensors and reduce diagnostic time. In order to achieve a higher yield in blood plasma separation, we propose a novel fluid wing structure that is optimized by COMSOL simulations by varying the fluidic channel width and the angle of the bifurcation. The fluid wing structure is inspired by the inertial particle separator system in helicopters where sand particles are prevented from following the air flow to an engine. The structure is ameliorated in order to satisfy biological and fluidic requirements at the micro scale to achieve high plasma yield and separation efficiency. In this study, we fabricated the fluid wing structure for the efficient microfluidic blood plasma separation. The high plasma yield of 67% is achieved with a channel width of $20{\mu}m$ in the fabricated fluidic chip and the result was not affected by the angle of the bifurcation.

The effects of Sa-Mul-Tang (Si-Wu-Tang), a Traditional Chinese Medicine, on Phenylhydrazine-induced Anemic Rats

  • Sa-Mul-Tang (Si-Wu-Tang, SMT), a kind of Chinese medicine, has been used for the hemato-deficient disease for hundreds of years. In this work, investigations on the anti-anemic activity of an aqueous extract of SMT were undertaken in order to find the pharmacological basis for the ethnomedical use of the formulation. Three kinds of Angelicase species, such as Angelica sinensis, Angelica acutiloba, and Angelica gigas, were used for preparing the water extracts of SMT. Anemic model rats were induced by the treatment of phenylhydrazine (40 mg/kg/day, i.p.) for 4 days. After the treatment of phenylhydrazine, rats were divided into several groups for their different treatment of three kinds of SMT. Red blood cell (RBC), hemoglobin (Hg), and hematocrit (Hct) were determined on the day 0, 3, 6, 10, 14 after the treatment of SMTs and erythrocytes deformabilities were also determined at the end of experiments. Oral administration of SMT (1g/kg/day) for 14 days did not ameliorate drug-induced anemic states evaluated by RBC counts, Hg contents, and Hct values. However, the erythrocyte deformabilities were improved in phenylhydrazine-treated group by the administragest that SMTs (p<0.05). Especially, these effects were high in the Angelica acutiloba group. These results suggest that SMTs have an ameliorative effect on blood rheology related to the blood stasis syndrome in oriental diagnostics not on the blood deficient states related to the anemic syndrome.

Non-invasive Transcutaneous pCO2 Gas Monitoring System for Arterial Blood Gas Analysis

  • Monitoring the carbon dioxide concentration in arterial blood is vital for the evaluation and prevention of pulmonary disease. Yet, domestic pure arterial blood carbon dioxide sensor technologies are not being developed, instead all sensors are imported. In this paper, we develop a real time monitoring system for arterial blood partial pressure of carbon dioxide($pCO_2$) gas from the wrist by using a carbon micro-heater. The micro-heater was fabricated with a thickness of 0.3 ${\mu}m$ in order to collect the carbon dioxide under the skin. The micro-heater has been designed to perform temperature compensation in order to prevent damage to the skin. Two clinical trials of the system were undertaken. As a result, we demonstrated that a portable, transcutaneous carbon dioxide analysis($TcpCO_2$) device produced domestically is possible. In addition, this system reduced the analysis time significantly. Carbon films could reduce the unit price of these sensors by replacing the gold film used in foreign models. Also, we developed a real time monitoring system which can be used with optical biosensors for medical diagnostics as well as gas sensors for environmental monitoring.

소증(素證)과 맥진(脈診)에서의 개체성(個體性) 관찰(觀察)에 대한 문헌적(文獻的) 고찰(考察) (The literature study on the individual characteristic factor by the interrogation of history taking and palpation)

  • Objectives : For the excellent differentiation of syndromes, we study on the individual characteristic factor by the interrogation of history taking and palpation. Methods : To the subject of diagnosis special books and diagnostics textbook of korean medicine, we arrange the individual characteristic factor by the interrogation of history taking and palpation. Results : The interrogation of history taking on the individual characteristic factor was arranged dispositional symptoms and individual characteristic of syndromes. And the pulse taking and palpation on the individual characteristic factor was arranged the moderate person's pulse condition ; floating pulse, deep and sinking pulse, replete and forceful pulse, intermittent pulse, long pulse, slippery or smooth pulse, relaxed or loose pulse (浮沈實大長滑緩脈), six Yin and six Yang pulse (六陽脈, 六陰脈). Conclusions : As the results, the individual characteristic factor is very important item of the four methods of diagnosis and the differentiation of syndromes. And therefore, we have to divide the signs of individual characteristic factor and the signs of disease in the process on four methods of diagnosis and differentiation of syndromes.

  • Stroke, also called an attack on the brain, is a complex disease that results from the interaction of many genetic and environmental factors. StrokePortal is a comprehensive resource for information on stroke that integrates and provides essential findings regarding stroke pathology, diagnostics, and treatments, based on Oriental and Western medicine. The stroke information was collected from various sources, such as journal articles, books, websites, and news stories, and it was refined, classified, and stored into a relational database system by automatic classification and manual curation. To provide the stored information effectively to users, a specialized retrieval system, based on web interfaces, was implemented. StrokePortal provides cutting-edge information to experts; interested people, including patients and their families; and investigators to broaden their knowledge of effective treatments for patients and offer many preventive measures. It provides a specialized feature with which users can upload their information and opinions to StrokePortal, which will enrich and mature the content even further.
