A 57-year old man who was admitted to an emergency room of a tertiary hospital with hemoptysis developed malarial fever 19 days later and then died from severe falciparum malaria 2 days later. He had not traveled outside of Korea for over 30 years. Through intensive interviews and epidemiological surveys, we found that a foreign patient with a recent history of travel to Africa was transferred to the same hospital with severe falciparum malaria. We confirmed through molecular genotyping of the MSP-1 gene that Plasmodium falciparum genotypes of the 2 patients were identical. It is suggested that a breach of standard infection control precautions resulted in this P. falciparum transmission between 2 patients in a hospital environment. This is the first report of a nosocomial transmission of falciparum malaria in Korea.
Purpose: The purpose of this study was to evaluate the effectiveness of pulmonary rehabilitation on lung function and fatigue in persons with chronic obstructive pulmonary disease (COPD). Methods: Thirty one persons with COPD participated in this study. Four groups were allocated as follows: experimental group 1 (under 10 years of the post-disease period), control group 1 (under 10 years of the post-disease period), experimental group 2 (over 10 years of the post-disease period), and control group 2 (over 10 years of the post-disease period). Results: Forced expired volume in one second (FEV1) % pred and lactic acid showed improvement in experimental group 1 and experimental group 2 after training. Control group 1 and control group 2 did not show improvement of FEV1% pred and lactic acid after training. However, FEV1% pred and lactic acid in experimental group 1 were not significantly different, compared with the experimental group. Conclusion: Results of our study suggest that implementation of a pulmonary rehabilitation program resulted in improvement of lung function and fatigue in persons with COPD not related to the post-disease period.
Objectives: Legionnaires' disease (LD) is a severe type of pneumonia caused by inhalation of aerosols contaminated with Legionella. On September 22, 2016, a single case of LD was reported from a newly built apartment building in Gyeonggi province. This article describes an epidemiologic investigation of LD and identification of the possible source of infection. Methods: To identify the source of LD, we interviewed the patient's husband using a questionnaire based on the Legionella management guidelines from the Korea Centers for Disease Control and Prevention. Water samples from the site were collected and analyzed. An epidemiological investigation of the residents and visitors in the apartment building was conducted for 14 days before the index patient's symptoms first appeared to 14 days after the implementation of environmental control measures. Results: Legionella pneumophila serogroup 1 was isolated from the heated-water samples from the patient's residence and the basement of the apartment complex. Thirty-two suspected cases were reported from the apartment building during the surveillance period, yet all were confirmed negative based on urinary antigen tests. Conclusions: The likely source of infection was the building's potable water, particularly heated water. Further study of effective monitoring systems in heated potable water should be considered.
Purpose: Health management programs should provide comprehensive health services for students and staffs at schools. Health management programs are critical for helping students become adults with physically, mentally, and socially good health conditions. The aim of this study was to identify the past and present history of health management programs and health laws to help schools develop future health plans. Methods: This study was conducted by analyzing reference data collected using data from Ministry of Education and Science Technology, Korean Educational Development Institute, Korea Centers for Disease Control and Prevention, and National Statistical Office as well as legal documents from the Legislative Office related to school health from 2001 to 2010. Results: Health management programs in schools included three sections: disease prevention and control, physical examinations, and prevention of communicable diseases. Disease prevention and controls consisted of obesity control, drug abuse prevention and control, and management of students' mental health. Various strategies and services were developed to improve health status of students for 10 years from 2001 to 2010. School health laws and systems have been established for disease prevention, physical examinations and communicable disease control as well, to improve students' health as well. Conclusion: The history of health management programs has a number of implications to help design future plans for school health programs and services for students and staffs.

Children are the most nutrition sensitive sub-group of a population. The nutritional status of children should be especially emphasized at all levels. This study was performed to investigate the current status, trend, and influencing factors to malnutrition of infants and children in China. The study was mainly based on the China Health and Nutrition Survey which is a longitudinal study conducted in 8 provinces and the data on growth of children under 7 years of age in 9 cities in China. The result of this study showed that one fifth of the children under 5 years of age are still suffering from stunted growth and one tenth suffering from underweight. The nutrition intervention on children under 2 years of age, especially on those under 18 months should be emphasized. Better supplementary food can improve the nutritional status to decrease the prevalence of stunted children. Therefore, the development of supplementary food should be the priority and should be emphasized with breastfeeding.
Growth, oxidative stress and antioxidant capacity of Fucus serratus exposed to high doses of Cd were examined. Two sites in Southwest England (Restronguet Point and Bantham Quay) were selected since they had different histories of metal contamination. 1~10 mg Cd $L^{-1}$ were treated to Aquil medium for up to 14 days. Similar levels of lipid peroxidation but different values of relative growth rates, cupric ion reducing antioxidant capacity (CUPRAC) and 2,2-diphenyl-1-picrylhydrazyl (DPPH) free radical scavenging capacity indicated that F. serratus has population-dependent antioxidant strategies. F. serratus demonstrated cadmium resistance with no visual symptoms for 14 days and the population from the polluted area seemed to have more powerful antioxidant strategies. However Fucus from the conserved area also showed protective anti oxidative mechanism.
Objectives: The number of cases of hepatitis A virus (HAV) infections has sharply increased in Korea, especially among young adults. In this study, an HAV outbreak in a facility for disabled people was investigated, and we found epidemiological differences both between 2 different generations and between generally abled and disabled groups. Methods: We analyzed the incubation period and attack rate of an HAV outbreak and investigated the prevalence of HAV antibodies among the staff and residents of a facility for the disabled. We performed a retrospective cohort study during the HAV outbreak, which lasted from February 8 to 25, 2019, including examinations of HAV antibody tests and post-exposure HAV vaccination for the staff or residents of the facility. Results: There were 9 confirmed cases in 2 staff members and 7 residents. Among 53 people (30 staff and 23 residents), except for the 9 confirmed cases and 1 staff member with a known history of HAV infection, HAV seroprevalence was seen in 16.7% of the staff under 40 years of age and 95.2% of those over 40 years of age, while the corresponding rates in the residents were 0.0% and 58.8%, respectively. Conclusions: This result implies that it is necessary to prioritize HAV vaccination for vulnerable groups and workers of residential care facilities.

Although many fungicides are registered for use to control powdery mildew on cucurbits, management of resistance to fungicides in pathogen populations still remains a major challenge. Two biopesticides Regalia SC and HMO 736 were evaluated in the greenhouse and field for their efficacy against powdery mildew in squash. In greenhouses, Regalia SC alone significantly (P < 0.05) reduced powdery mildew compared to the nontreated control, and was as effective as the chemical standard Procure 480SC (triflumizole). In alternation with Procure 480SC, Regalia SC demonstrated greater or equivalent effects on reducing the disease. HMO 736 alone showed varying levels of disease control, but alternating with Procure 480SC significantly improved control efficacy. In addition, application of Regalia SC or HMO 736 each in alternation with Procure 480SC significantly increased the chlorophyll content in leaves and the total fresh weight of squash plants, when compared with the water control, Regalia SC and HMO 736 alone. In field trials, application of Regalia SC and HMO 736 each alone significantly reduced disease severity in one of two field trials during the early stage of disease development, but not during later stages when disease pressure became high. Both Regalia SC and HMO 736 each applied in alternation with Procure 480SC significantly improved the control efficacy compared to Procure 480SC alone. Results from this study demonstrated that an integrated management program can be developed for powdery mildew in squash by integrating the biopesticides Regalia SC, HMO 736 with the chemical fungicide Procure 480SC.
This study was performed to investigate prevalence of clonorchiasis among the inhabitants living in villages along the 4 major rivers, Nakdong-gang (= river), Seomjin-gang, Youngsan-gang, and Guem-gang in southern Korea. From January to December 2006, a total of 24,075 stool samples (1 sample per an inhabitant) were collected in 23 localities and examined by the formalin-ether sedimentation technique. Of the inhabitants examined, 3,441 (14.3%) were found to harbor various types of intestinal parasite eggs, cysts or larvae. Numbers of infected people were 2,661 (11.1%) for Clonorchis sinensis, 431 (1.8%) for heterophyids, 226 (0.9%) for Entamoeba spp., 57 (0.2%) for Giardia lamblia, 30 (0.1%) for Trichuris trichiura, and 18 (0.07%) for echinostomes. Prevalence rates of clonorchiasis according to the river basin were 17.1% in Nakdong-gang, 11.2% in Seomjin-gang, 5.5% in Youngsan-gang and 4.6% in Guem-gang. Of the 2,661 C. sinensis egg-positive cases, 57.7% was male. The present findings suggest that clonorchiasis is still highly prevalent among inhabitants in the riverside areas of southern Korea, and it is necessary to implement a systematic control program in the endemic areas.

Stem rot is a serious disease in Jerusalem artichoke (JA). To reduce the impact of this disease on yield and quality farmers often use fungicides, but this control method can be expensive and leave chemical residues. The objective of this study was to evaluate the efficacy of two biological control agents, Trichoderma harzianum T9 and Bacillus firmus BSR032 for control of Sclerotium rolfsii under field conditions. Four accessions of JA (HEL246, HEL65, JA47, and JA12) were treated or notreated with T. harzianum T9 and B. firmus BSR032 in a 4 × 2 × 2 factorial experiment in two fields (environments), one unfertilized and one fertilized. Plants were inoculated with S. rolfsii and disease was evaluated at 3-day intervals for 46 days. T. harzianum T9 and B. firmus BSR032 reduced disease incidence by 48% and 49%, respectively, whereas T. harzianum T9 + B. firmus BSR032 reduced disease incidence by 37%. The efficacy of T. harzianum T9 and B. firmus BSR032 for control of S. rolfsii was dependent on environments and genotypes. The expression of host plant resistance also depended on the environment. However, HEL246 showed consistently low disease incidence and severity index in both environments (fertilized and unfertilized). Individually, T. harzianum T9, B. firmus BSR032, or host plant resistance control stem rot caused by S. rolfsii in JA. However, no combination of these treatments provided more effective control than each alone.
