Gender Differences in Paediatric Patients of the Swiss Inflammatory Bowel Disease Cohort Study

  • Purpose: Gender differences in paediatric patients with inflammatory bowel disease (IBD) are frequently reported as a secondary outcome and the results are divergent. To assess gender differences by analysing data collected within the Swiss IBD cohort study database since 2008, related to children with IBD, using the Montreal classification for a systematic approach. Methods: Data on gender, age, anthropometrics, disease location at diagnosis, disease behaviour, and therapy of 196 patients, 105 with Crohn's disease (CD) and 91 with ulcerative or indeterminate colitis (UC/IC) were retrieved and analysed. Results: The crude gender ratio (male : female) of patients with CD diagnosed at <10 years of age was 2.57, the adjusted ratio was 2.42, and in patients with UC/IC it was 0.68 and 0.64 respectively. The non-adjusted gender ratio of patients diagnosed at ${\geq}10$ years was 1.58 for CD and 0.88 for UC/IC. Boys with UC/IC diagnosed <10 years of age had a longer diagnostic delay, and in girls diagnosed with UC/IC >10 years a more important use of azathioprine was observed. No other gender difference was found after analysis of age, disease location and behaviour at diagnosis, duration of disease, familial occurrence of IBD, prevalence of extra-intestinal manifestations, complications, and requirement for surgery. Conclusion: CD in children <10 years affects predominantly boys with a sex ratio of 2.57; the impact of sex-hormones on the development of CD in pre-pubertal male patients should be investigated.

Cat Monitoring and Disease Diagnosis System based on Deep Learning (딥러닝 기반의 반려묘 모니터링 및 질병 진단 시스템)

  • Recently, several ICT-based cat studies have produced some successful results, according to academic and industry sources. However, research on the level of simply identifying the cat's condition, such as the behavior and sound classification of cats based on images and sound signals, has yet to be found. In this paper, based on the veterinary scientific knowledge of cats, a practical and academic cat monitoring and disease diagnosis system is proposed to monitor the health status of the cat 24 hours a day by automatically categorizing and analyzing the behavior of the cat with location information using LSTM with a beacon sensor and a raspberry pie that can be built at low cost. Validity of the proposed system is verified through experimentation with cats in actual custody (the accuracy of the cat behavior classification and location identification was 96.3% and 92.7% on average, respectively). Furthermore, a rule-based disease analysis system based on the veterinary knowledge was designed and implemented so that owners can check whether or not the cats have diseases at home (or can be used as an auxiliary tool for diagnosis by a pet veterinarian).

Effect of the Sensor Location on Magnetocardiography (심자도 센서의 위치 효과에 대한 연구)

  • Magnetocardiography (MCG) is a non-contact, non-invasive, and harmless diagnostic tool to detect the abnormal electrical conductivities of the heart caused by the various coronary artery disease or cardiac muscular disease. The purpose of this study is to identify whether MCG signals and MCG parameter values vary depending on the location of sensor assembly. It will be an important reference for the standard measurement. Four healthy male subjects (33.3$\pm$6.3 years) participated in this study. Basal recording was made at 20 mm apart from the chest surface. All subjects were requested to take a regular breathe while MCG was taken. The gap between the chest surface and the bottom of the sensor assembly was 20, 40, 60, and 80 mm. Recording was made using 64 channel MCG system (Axial type, first order gradiometer) developed by Korea Research Institute of Standards and Science (KRISS). After resting for two minutes in a supine position on the bed in magnetically shielded room, MCG were recorded for 30 s. As the sensor location is getting away from the chest surface signal, the amplitude of R and T wave peak decreases to 70% (at 40 mm gap), 50% (at 60 mm), and 37% (at 80 mm) of the reference strength measured (y = $1.3903e^{-0.0169x}$, $R^2$ = 0.99; where y=amplitude remained after reduction, x=distance between chest surface and sensor location). The regression equations may be used as a good reference to calculate how much strength will be decreased by the distance. In MCG parameters, most values of parameters were decreased as the gap was increased. As an example, the current moment at T-wave peak reduced to 52% (at 40 mm gap), 33% (at 60 mm), and 19% (at 80 mm). However, the difference caused by the gap could be reduced by considering the distance when the MCG parameters were calculated. The study results can be used as a useful reference to design the baseline and the sensor location.

  • Various stem cells or progenitor cells are being used to treat cardiovascular disease in ischemic heart disease, stem ceil therapy is expected to regenerate damaged myocardium. To evaluate effects of stem cell treatment, the method to image stem cell location, distribution and differentiation is necessary. Optical imaging, MRI, nuclear imaging methods have been used for tracking stem cells. The methods and proglems of each imaging technique are reviewed.

Study on Common Conceptual Terms as a Premise for Korean Classification of Disease in Oriental Medicine in Connection with ICD-10 (ICD 연계 한의질병분류를 위한 전제로서의 공통개념어 연구)

  • In order to classify diseases of oriental medicine in liaison with International Classification of Diseases, there should be intermediation and sharing concepts between the two in addition to proper classification. Classification units were settled for differentiation of diseases or syndromes first. And second, the standard forms of disease classification system were proposed. Third, this classification system was made of serial groupings of syndrome under the traditional disease name. Fourth, the location of disease and the interrelation between different syndromes were depicted with diagram in order to define more clearly. As the results and conclusion, The classification units were composed of 2 categories; topology, organ, meridian, somatic structure, body fluid units for description and various regulatory unit terms of western and traditional medicine for explanation. The mixed classification model of western diseases and traditional syndromes(證) was adopted as a fundamental classification system containing disease by exterior pathogen, systemic internal diseases, psychoneuronal diseases, metabolic diseases, diseases of sense organs, supportive structure diseases, obstetric-gynecology diseases, child diseases, 4-type constitutional diseases. And those were differentiated with generalized, localized, functional, oncogenic, environmental features in detail. The cause, site, condition, dispositions must be expressed in each disease name too. The types of diagnosis using classification system are principal and final diagnosis, principal procedure, main conditions, and these are applied to this Korean classification system equally. For more clarification of differentiation, a plane topological map and three dimensional coordinates were proposed to manifest the location, features and relation of disease itself or each other.

  • According to the literature study on Nae-gwan and Kongson, reviewing the oriental medical books from Hung-Ti-Nei-Ching $\ll$黃帝內經$\gg$ to recent books and other 35 kinds of literatures, the following results are obtained. 1. The location of Nae-gwan is the superior 2 cun Tae-nung point between Tendon of flexor carpiradials and Tendon of palmaris longus; the location of Kongson is the 1st Metatarsal, medial, dented and posterior 1 cun T'aebaek point. 2. The effects of Nae-gwan are relaxing mind, nutrition of heart, peaceful chest, invigorate vital energy, transmitting triple energy; the effects of Kongson are steadying spleen harmonious stomach clearing away dampness, controlling ch'ung-im, regulating blood. 3. Nae-gwan is often used for circulatory organs disease, digestive organs disease, neuropsychiatry disease; Kongson is often used for digestive organs disease, urinary organs disease, neuropsychiatry disease, therefore, these double points are used for internal disease wholly. 4. The Needle-steadying depth of Nae-gwan is 0.5-1 cun, Kongson is 0.3-2 cun; the Moxibustion dosage of Nae-gwan is 3-7 zhuang, Kongson is 3-5 zhuang. 5. Nae-gwan belongs to pericardium Merdian, Kongson belongs to Spleen Meridian, therefore, these double points are combined in yin Meridian upper and lower sides. These points can be used for treating front body part, such as heart, chest and stomach.

Geohashed Spatial Index Method for a Location-Aware WBAN Data Monitoring System Based on NoSQL

  • The exceptional development of electronic device technology, the miniaturization of mobile devices, and the development of telecommunication technology has made it possible to monitor human biometric data anywhere and anytime by using different types of wearable or embedded sensors. In daily life, mobile devices can collect wireless body area network (WBAN) data, and the co-collected location data is also important for disease analysis. In order to efficiently analyze WBAN data, including location information and support medical analysis services, we propose a geohash-based spatial index method for a location-aware WBAN data monitoring system on the NoSQL database system, which uses an R-tree-based global tree to organize the real-time location data of a patient and a B-tree-based local tree to manage historical data. This type of spatial index method is a support cloud-based location-aware WBAN data monitoring system. In order to evaluate the proposed method, we built a system that can support a JavaScript Object Notation (JSON) and Binary JSON (BSON) document data on mobile gateway devices. The proposed spatial index method can efficiently process location-based queries for medical signal monitoring. In order to evaluate our index method, we simulated a small system on MongoDB with our proposed index method, which is a document-based NoSQL database system, and evaluated its performance.

Prognostic Factors and Therapeutic Outcomes in 22 Patients with Pleomorphic Xanthoastrocytoma

  • Objective : Pleomorphic xanthoastrocytoma (PXA) is a rare primary low-grade astrocytic tumor classified as WHO II. It is generally benign, but disease progression and malignant transformation have been reported. Prognostic factors for PXA and optimal therapies are not well known. Methods : The study period was January 2000 to March 2012. Data on MR findings, histology, surgical extents and adjuvant therapies were reviewed in twenty-two patients diagnosed with PXA. Results : The frequent symptoms of PXA included seizures, headaches and neurologic deficits. Tumors were most common in the temporal lobe followed by frontal, parietal and occipital lobes. One patient who died from immediate post-operative complications was excluded from the statistical analysis. Of the remaining 21 patients, 3 (14%) died and 7 (33%) showed disease progression. Atypical tumor location (p<0.001), peritumoral edema (p=0.022) and large tumor size (p=0.048) were correlated with disease progression, however, Ki-67 index and necrosis were not statistically significant. Disease progression occurred in three (21%) of 14 patients who underwent GTR, compared with 4 (57%) of 7 patients who did not undergo GTR, however, it was not statistically significant. Ten patients received adjuvant radiotherapy and the tumors were controlled in 5 of these patients. Conclusion : The prognosis for PXA is good; in our patients overall survival was 84%, and event-free survival was 59% at 3 years. Atypical tumor location, peritumoral edema and large tumor size are significantly correlated with disease progression. GTR may provide prolonged disease control, and adjuvant radiotherapy may be beneficial, but further study is needed.

  • Objective: The 19 Mechanisms of Disease were first described in Huangdi Neijing. When examining Yixuexinwu, three descriptions that were different from Huangdi Neijing were found. Methods and Results: After examining the two books, three differences in the description of the 19 Mechanisms of Disease were discovered: 1) The upper and lower locations of dyspnea (喘症) are reversed. 2) The ratio of provisions for "fire (火)" and "cold (寒)" is different. 3) The organ affiliation for pain is different. Conclusion: 1) The upper and lower locations of dyspnea (喘症) are reversed. : Regarding external contraction dyspnea (外感喘), Yixuexinwu used the disease of the upper location (lung 上, 肺) in the same way as in Huangdi Neijing. In terms of internal damage (內傷喘), it was separately classified as a disease of the lower location (kidney 下, 腎). 2) The ratio of provisions for "fire (火)" and "cold (寒)" is different. : Arithmetically, if you round off the number after calculating the fraction, the difference between the two ratios becomes the same. Theoretically, five provisions of "fire" are replaced by four provisions of "heat (熱)", and the difference in ratios is exactly the same. Empirically, it emphasizes that there are more illnesses from fire and heat than from cold. 3) The organ affiliation for pain is different. : Huangdi Neijing described general inflammatory pain, while the Yixuexinwu described only stress-related abdominal pain (肝木乘脾 腹痛).

  • Inflammatory bowel disease (IBD) develops during childhood or adolescence in approximately 25% of patients with IBD. Recent studies on pediatric IBD have revealed that early-onset IBD has distinct phenotype differences compared to adult onset IBD. Pediatric early-onset IBD differs in many aspects including disease type, location of the lesions, disease behavior, gender preponderance and genetically attributable risks. This review examines the currently published data on the clinical, epidemiological and genetic differences between early-onset and adult-onset IBD. And finally, therapeutic considerations in the management of pediatric-onset IBD are also discussed.
