  • Facial palsy is a common disease in clinic, which can be classified into central and peripheral according to the causes. The central facial palsy is caused by cerebral vascular accident, brain tumor, etc. The peripheral facial palsy comes from cold stimulus on face, regional infection of virus, suppurative tympanitis, inflammation on mastoid process, pathological teeth, trauma like cranial fracture, and so forth, They have distinctive features in diagnosis. While the central facial palsy is followed by hemiplegia and articulation disorder, the peripheral one by the disappearance of wrinkles on the forehead and rising of eyeball on paralyzed side when closing the eyes. Most of the cases in this thesis are peripheral palsy. The social classes and ages of the patients are so various that the treatments must be applied from various standpoints. The statistical data shows that the functional weakness of the whole body is the fundamental condition of the facial palsy. Therefore it is very important to find and the exact pathology and treatment appropriate for Hyungsang of the patients.

Moebius syndrome - About Pathogenesis, Clinical manifestations, Diagnosis, and Treatment of Moebius - (뫼비우스 증후군 - 발병기전, 임상양상, 진단 및 치료 - )

  • Objectives: To review the concept of Moebius syndrome. Methods: Literature search was done to study definition, epidemiology, pathophysiology, clinical feature, and treatment of Moebius syndrome. Pubmed, RISS, Google scholarship and uptodate scholastic were used in the research. Search words were 'Moebius syndrome', 'treatment of Moebius syndrome'. Only English and Korean studies were assessed. Results: Moebius syndrome is rare disease characterized by nonprogressive congenital uni- or bi-lateral facial (VII cranial nerve) and abducens (VI cranial nerve) palsy. This facial palsy is found across the world, and its incidence is approximately 1 per 250,000. Moebius is diagnosed by clinical features. Facial palsy, eye abduction problem, limb deformities, global cerebral nerve impairment can be shown. Rehabilitation, smile surgery, and acupuncture can be used to treat this. Conclusion: Moebius syndrome's epidemiology, pathogenesis, treatment is still not fully revealed. It is known to be a congenital disease which didn't have exact treatment except surgery. But, it needs further study about exact treatment, diagnosis, and pathogenesis.

Characteristic Changes in First-Visit Patients with Peripheral Facial Palsy before and during COVID-19 Pandemic: Focused on a Korean Medicine Hospital

  • This study aimed to analyze whether the coronavirus disease 2019 (COVID-19) pandemic affected the characteristics of first-visit patients with peripheral facial palsy (PFP) and observe changes in their characteristics. This study analyzed the electronic medical records of 2,310 first-visit patients with PFP who visited the Facial Palsy Center, Kyung Hee University Korean Medicine Hospital from January 1, 2019, to December 31, 2021, in terms of demographic characteristics, disease phase, residence locations, hospital visit route, and patient care. During COVID-19, the proportion of acute patients increased by 5.3%, the proportion of visits by residents in Seoul increased by 3.8%, and the proportion of patients receiving only outpatient treatments increased by 12.8%. Significant relationships were present between the presence of the COVID-19 pandemic and the number of patients by disease phase (p = 0.043), residence locations (p = 0.003), and patient care (p = 0.003). Thus, several differences in the characteristics of first-visit patients with PFP visiting a Korean medicine hospital during the COVID-19 pandemic in terms of demographic characteristics, disease phase, residence locations, and patient care.

  • Object : The purpose of this study is to evaluate the effect of Miso Facial Rejuvenation Acupuncture on Facial Palsy Sequela. Miso Facial Rejuvenation Acupuncture is an Korean medical treatment technique used for face lifting and facial disease. Method : We tereated a male patients who has Facial palsy sequela include contraction, spasm, synkinesis with Miso Facial Rejuvenation Acupuncture. The treatment was performed one times a week for thirteen times. The effects of the treatment were evaluated VAS(visual analogue scale). Result : After the treatment the grade of VAS was decreased and clinical symptoms were gradually disappaered. Conclusion : Miso Facial Rejuvenation Acupuncture showed improvement in sequela of facial palsy.

  • Facail nerve palsy is a common disease in the face. This disease is treated by many methods. But it is not treated by Facial Acupunture. Therefore we performed to evaluate the effect of Facial Acupuncture with oriental dedical treatment in facail nerve palsy. This clinical study was carried out 2 caese of facail nerve palsy which had been admitted our hospital. In this study the patients were treated by Facail Acupuncture. Other treatment were herbal medication, regabilitative therapy. H-B scale(House-Brackmann facial nerve grading system) and Digital Infrared Thermal Imaging were measured from admission to discharge. After being treated by our methods, Patients' H-B scale and Inequality of Facial DITI were improved in cases compared to existing. In the study, The treatment of facial nerve palsy using Facial acupuncture was more effective, especially H-B scale, Digital Infrared Thermal Imaging and treatment period, compared to existing.

Bilateral Facial Palsy in a Patient with Guillain-Barre Syndrome after COVID-19 Vaccination (Covid-19 백신 접종 후 발생한 길랑-바레 증후군에 동반된 양측성 안면마비 1례)

  • Objectives : The purpose of this study is to report a case of bilateral facial palsy in a patient with Guillain-Barre Syndrome after coronavirus disease 2019(COVID-19) vaccination. Methods : The patient diagnosed as Guillain-Barre Syndrome and Bilateral facial palsy after COVID-19 vaccination. We applied Korean medical treatment including acupuncture and herbal medicine for bilateral facial palsy. We used HBGS(House-Brackmann Grading System), VAS(Visual Analog Scale) to estimate the symptoms. Results : After the treatment, both facial palsy showed big improvement. Compared to the onset, HBGS improved by Grade 1/2 and VAS by 2. Conclusions : This case report shows effect of Korean medicine on Bilateral facial palsy and suggests a possibility of Korean medicine to treat adverse events following COVID-19 vaccination.

Improved Symptoms of Peripheral Facial Nerve Palsy in ChAdOx1 nCoV-19 Vaccine Recipients Following Complex Korean Medicine Treatment

  • Peripheral facial nerve palsy is a neurological condition/disease characterized by unilateral paralysis of the facial muscles. We report the effects of complex Korean medicine treatment in 2 patients diagnosed with Bell's palsy who developed symptoms 3-4 days following vaccination with ChAdOx1 nCoV-19 and who received oral steroids and antiviral medications. Complex Korean medicine treatments including acupuncture, electroacupuncture, bee venom acupuncture, cupping therapy, herbal medicine, and physical therapy were performed. Symptom improvement was evaluated daily using the Numerical Rating Scale, House-Brackmann Grade, and Yanagihara's Score. Patient 1 was hospitalized for 22 days and Patient 2 visited the outpatient clinic 20 times. In both cases, postauricular pain caused by Bell's palsy disappeared, and symptoms of facial paralysis improved following completion of treatment. These findings could suggest that combined Korean medicine treatment may improve symptoms of peripheral facial nerve palsy.

The Usefulness of Blink Reflex in the Evaluation of Early Stage of Bell's Palsy (발병초기 벨마비 환자의 진단에서 순목 반사(blink reflex)의 유용성)

  • Background: Bell's palsy is the most common disease of cranial nerve. While most electrodiagnostic tests can detect the abnormality of facial nerve several days later in Bell's palsy, blink reflex usually reveals the abnormality earlier than other tests. Therefore, we investigated the diagnostic usefulness of blink reflex in the early stage of Bell's palsy. Methods: We performed a prospective investigation in patients with facial palsy. We enrolled patients with Bell's palsy who were evaluated within 7 days of symptom onset and excluded patients with secondary causes of facial palsy. We analyzed the findings of blink reflex according to age, sex, evaluation time, and severity of facial palsy. Results: Of 320 consecutive patients with facial palsy, a total of 204 patients were enrolled. Blink reflex was normal for 10 patients and abnormal for 194 patients. The time interval between the symptom onset and the evaluation time was not associated with the result of blink reflex, but House-Brackmann grade was significantly related to the result of blink reflex (P<0.001). Patients with mild Bell's palsy often showed normal blink reflex. Conclusions: Our study shows that blink reflex is useful diagnostic test regardless of evaluation timing in the early stage of Bell's palsy, although it could be normal in patients with mild Bell's palsy.

A Case Study of Oriental Medicine Treatment on Peripheral Facial Palsy (말초성 안면마비 환자의 한방치료 치험례)

  • Objective: Facial nerve palsy is caused by damage to the 7th cranial nerve. It is the main symptom of facial muscle paralysis on the affected side. Usually, recovery from this disease begins 2-3 weeks after onset and most patients recover in 4-8 weeks. If the patients cannot receive proper treatment, severe permanent impairments, both physical and mental, may remain, so this disease should be treated appropriately. In this study, a patient with facial nerve palsy was admitted to the Korean medicine hospital for treatment. We report on the patient's progress and the effects of treatment. Methods: We cured the patient with herbal medicines, acupuncture, herbal acupuncture therapy, and physical therapy. We used a numerical rating scale, the House Brackmann grading system, and a weighted regional grading system to assess symptom changes. Result: The patient with facial nerve palsy was hospitalized for 23 days and recovered from symptoms without significant problems on the face or in motor function.
