Review of the Nomenclature of the Retaining Ligaments of the Cheek: Frequently Confused Terminology

  • Since the time of its inception within facial anatomy, wide variability in the terminology as well as the location and extent of retaining ligaments has resulted in confusion over nomenclature. Confusion over nomenclature also arises with regard to the subcutaneous ligamentous attachments, and in the anatomic location and extent described, particularly for zygomatic and masseteric ligaments. Certain historical terms-McGregor's patch, the platysma auricular ligament, parotid cutaneous ligament, platysma auricular fascia, temporoparotid fasica (Lore's fascia), anterior platysma-cutaneous ligament, and platysma cutaneous ligament-delineate retaining ligaments of related anatomic structures that have been conceptualized in various ways. Confusion around the masseteric cutaneous ligaments arises from inconsistencies in their reported locations in the literature because the size and location of the parotid gland varies so much, and this affects the relationship between the parotid gland and the fascia of the masseter muscle. For the zygomatic ligaments, there is disagreement over how far they extend, with descriptions varying over whether they extend medially beyond the zygomaticus minor muscle. Even the 'main' zygomatic ligament's denotation may vary depending on which subcutaneous plane is used as a reference for naming it. Recent popularity in procedures using threads or injectables has required not only an accurate understanding of the nomenclature of retaining ligaments, but also of their location and extent. The authors have here summarized each retaining ligament with a survey of the different nomenclature that has been introduced by different authors within the most commonly cited published papers.

Clinical Study of 61 cases Benign Lung Tumor (폐양성 종양 61례의 외과적 고찰)

  • Benign lung tumors compose a heterogeneous group of solid growths that present variations in clinical features, depending on whether the origin is within the bronchus on lung parenchyma or from visceral pleura. Benign tumors of the lung are relatively uncommon, and series are to be found in the literature and the classification of benign tumors of the lung continues to be controversial because of disagreement concerning the origin and prognosis of many common lesions. We adopt Liebows original classification but excluded bronchial adenoma which no longer considered as benign tumor and added pulmonary A-V fistula and congenital cystic adenomatoid malformation. We analyzed 61 cases of benign tumors which were composed of 16 original Korean cases and 45 cases which were reported on journal of Thoracic & Cardiovascular Surgery. The results were. [1] Incidence; Of 61 cases, chondromatous hamartoma was 2 cases [41 %], congenital cystic adenomatoid malformation 10 cases [16.4 %], pulmonary A-V fistula 5 cases [8.1 %], sclerosing hemangioma 4 cases [6.5 %], teratoma, plasma cell granuloma & mesothelioma were 3 cases [4.9%], Castlemans disease 2 cases [3.3%], and mucous gland adenoma, paraganglioma, and leiomyoma 1 case [1.6 %]. [2] Age & Sex distribution; Male 30 cases and female 31 cases. Mean age was 31.4 years old. [3] Main symptom; was coughing, 32.8%, and no symptom, 24.6%. [4] Sixty eight percentage of chest film showed mass density, and 4 cases showed calcification, 2 cases had lobulation. [5] Size of mass was large and multiple mass was 2 cases. Endobronchial tumors were 9 cases, 14.9 %. [6] Three cases of endobronchial tumor were preoperatively diagnosed by bronchoscopy and 2 cases of pulmonary A-V fistula were diagnosed by pulmonary arteriography. [7] Seven cases, 11.5%, had associating diseases such as bronchogenic cyst, thymic cyst, Schwannoma, situs inversus, bronchiectasis and bronchogenic carcinoma. [8] Minor resection such as excision 8i: wedge resection were 15 cases, 26.2 %, and 6 cases, 75.4 %, of lobectomy were performed including 5 cases of pneumonectomy 5 cases had. [9] Postoperative complications; One case, 1.6 %, expired due to respiratory insufficiency. Two cases had re-operation due to bleeding and hemoptysis.

A Study on Personal Color Therapy practice to Skin Care (체질에 따른 컬러테라피의 피부관리 적용방법의 안(案))

  • This study focuses on practical appliance of color therapy to skin care. After visiting the notion of color therapy and skin care, this study considers the ideas and background theories for diagnosis of constitution and skin. Cosmic duals force five element theory developed in China was the first introduction; Ayurvedic medicine and seven chakra notion are examined. Cosmic duals force five element theory, an idea describing the composition and movement rule of the cosmic components, adopted five colors(white, yellow, red, blue, and black) for cure purposes. In future, mental effect of the color is examined in the latter part of this study. The practice of the color therapy assigns solarized cream treatment and photo therapy, simultaneously proposes the use of color therapy to the skin care. For the diagnosis of the constitution, cosmic duals force five element theory and ayurvedic medicine are followed in a sequence. Cosmic duals force five element theory introduces five colors. Constitution medicine considers four types, ayurvedic medicine adopts three types, and seven colors are mentioned as chakra colors. Such a variations lead to possible disagreement on constitution analysis and driven colors. This study adopts the above methodologies for selecting potential color therapy for skin care. Adopt of selected cases are purely a part of the proposal. Initially, examined theories serve as a pilot for selecting a representative hypothesis. Followed selection of constitution and matched color for the use of the skin care are the scope of this study. In summary, background methodologies are implemented for the calculation of color therapy.

Application of DEM with Coarse Graining Method to Fluidal Material Behavior Analysis (유동성 재료의 동적 거동 해석을 위한 입자확대법 기반 DEM의 적용)

  • PURPOSES : In this paper, the applicability of DEM to a coarse graining method was evaluated by simulating a series of minicone tests for cement paste. METHODS : First, the fundamental physical quantities that are used in a static liquid bridge model were presented with three basic quantities based on the similarity principle and coarse graining method. Then, the scale factors and surface tensions for six different sizes of particles were determined using the relationship between the physical quantities and the basic quantities. Finally, the determined surface tensions and radii were utilized to simulate the fluidal behavior of cement paste under a minicone test condition, and the final shape of the cement paste with reference DEM particle radii was compared with the final shape of the others. RESULTS : The simulations with adjusted surface tensions for five different radii of particles and surface tension showed acceptable agreement with the simulation with regard to the reference size of the particle, although disagreement increases as the sizes of the particle radii increase. It seems reasonable to increase the particle radii by at least 0.196 cm considering the computational time reduction of 162 min. CONCLUSIONS : The coarse graining method based on the similarity principle is applicable for simulating the behavior of fluidal materials when the behavior of the materials can be described by a static liquid bridge model. However, the maximum particle radius should be suggested by considering not only the scale factor but also the relationship of the particle size and number with the radius of the curve of the boundary geometry.

  • Cancer of the cervix is the most common malignancy in women in developing countries and the second most common cancer in women throughout the world with approximately 500,000 new cases each year. Prevention of this large number of premature deaths among women is, therefore, a goal worthy of urgent and serious consideration. Due to its high diagnostic disagreement among pathologists and large quantity of specimens, it is necessary to develop an automatic screening system measuring morphologic and densitometric features of the samples. Many research works have been published but most of them used Feulgen stained specimens which are not a usual staining method used in clinics. In this thesis, an automatic cancerous nucleus detection method essential to a screening system with papanicolaou stained specimens called Pap-smear is proposed which employs image processing techniques. It uses edge information to segment objects and morphologic as well as densitometric information to distinguish cancerous nuclei from dirts or normal nuclei. It has produced useful results in our study.

  • Objectives : The aim of this study is to assess the quantity and quality of randomized controlled clinical trials(RCTs) published in the journal of oriental rehabilitation medicine(JORM). Methods : After searching RCTs from all the articles published in the JORM from the inception(1991) to Oct, 2012, quantity assessment were made on the study design, sample size, main intervention, intervention versus control design and medical condition. Quality assessment were made on the cochrane risk of bias(RoB) check list. Assessment was performed by 2 independent reviewers and disagreement was discussed based on concensus of all authors. Results : Among the 1013 articles, total 59 RCTs were published. First RCT was published in 1994. 49 RCTs were parallel 2-arm designed. Average sample size was 35.1 per study and 16.3 per arm. The most common medical condition is musculoskeletal disease(30.5%). Quality from cochrane RoB was generally low. Conclusions : Though RCTs published in JORM were increasing, the quality remains low. Researchers should make a effort to follow the RoB checklists and improve the quantity and quality of studies.

  • This study is considered how to use newspapers to apply education by the way of analyses of environment coverage in newspapers. Data for the study were gathered by content analyses of KINDS(Korean Integrated News Database System) established by Korean Press Institute. The environment coverage is mainly placed in social and regional magazines of newspapers, and the news story are mainly assigned to straight/feature magazines in type. The news of environment coverage is mostly gathered from data by government-informer, and the news is positive/agreement or negative/disagreement in tenor. The news gathering methods of planning/magazine newspaper serial are scientific and objective, and they are of the firsthand data by news reporter, contributions by experts and interviews. The spaces of the news are specially edited. The environment news is often negative/disagreeable in tenor because the news is mostly of straight ones written by non-experts. Applying newspapers in education is a useful learning method which students could develop thinking power and induce concerning and interest by themselves. From the results of the study, the useful suggestions to apply newspapers to learning are as follows. At first, spaces and types of news must be read in detail. Secondly, it is hopeful that indirect news by not writer himself might be possibly avoided in learning. Thirdly, the themes of news would be picked up in relation with learning contents. Lastly, it suggests that the tenor of news is neutral or, in cases, positive and negative together possibly.

Comparison of Harmonic scalpel and monopolar cautery for capsulectomy at the second stage of expander/implant breast reconstruction

  • Background Capsular contracture is a common complication of two-stage expander/implant breast reconstruction. To minimize the risk of this complication, capsulectomy is performed using monopolar cautery or ultrasonic surgical instrumentation, the latter of which can be conducted with a Harmonic scalpel. To date, there is disagreement regarding which of the two methods is superior. The purpose of this study was to compare postoperative outcomes between a group of patients who underwent surgery using a Harmonic scalpel and another group treated with monopolar cautery. Methods A retrospective chart review was conducted of patients who underwent capsulectomy as part of two-stage breast reconstruction between January 2018 and February 2019 and who received at least 1 month of follow-up after surgery. Operative time and postoperative outcomes, including drainage duration, were analyzed. Results In total, 36 female patients underwent capsulectomy. The monopolar group consisted of 18 patients and 22 breasts, while the Harmonic scalpel group consisted of 18 patients and 21 breasts. There was no statistically significant difference in demographics between the two groups. The Harmonic scalpel group had a significantly shorter mean drainage duration (6.65 days vs. 7.36 days) and a smaller mean total drainage volume (334.69 mL vs. 433.54 mL) than the monopolar cautery group (P<0.05). No statistically significant difference was observed with regard to seroma or hematoma formation. Conclusions The Harmonic scalpel approach for capsulectomy reduced the total drainage volume and drainage duration compared to the monopolar cautery approach. Therefore, this approach could serve as a good alternative to electrocautery.

The Effects of Mycoplasma Infection on Semen Parameters (in Normal Human Semen) (남성에서 Mycoplasma 감염이 정액검사소견에 미치는 영향에 관한 연구)

  • Mycoplasmas have long been suspected of contributing to involuntary infertility in couples. However considerable disagreement exits concerning the role of genital mycoplasma infection in human infertility. Several investigators have noted abnormalities in the semen analysis of men with positive mycoplasma cultures, and early epidemiologic studies indicated that Ureaplasma urealyticum was linked to human reproductive failure on the basis of higher frequencies of isolation from infertile versus fertile couples and successful pregnancies in infertile couples after doxycycline therapy. However, subsequent investigators have questioned these findings because there are many studies in which treatment for mycoplasma in the male or female did not demonstrate an improved pregnancy rate, and semen samples from unexplained infertile men containing ureaplasmas have not revealed poorer motility, fewer spermatozoa and more aberrant forms. The objective of this study were to investigate the incidence rate of mycoplasma in semen and to investigate whether the presence of mycoplasma in semen makes significant difference to the semen volume, sperm motility and sperm counts. The results were that the rate of isolation of mycoplasma species was 70.3%. Semen volume is $2.84{\pm}1.01ml$ for culture negative and $3.15{\pm}1.42ml$ for culture positive group. Sperm motility is $46.23{\pm}15.80%$ for culture negative and $50.09{\pm}15.69%$ for culture positive group, and sperm count is $95.47{\pm}47.14({\times}(P)10^6/ml)$ for culture negative and $86.73{\pm}47.59({\times}10^6/ml)$ for culture positive group. In conclusion, we suggest that the presence of mycoplasma in semen makes no significant differences to the sperm parameters.

DEVELOPMENT AND EVALUATION OF A PHANTOM FOR MULTI-PURPOSE DOSIMETRY IN INTENSITY-MODULATED RADIATION THERAPY

  • A LEGO-type multi-purpose dosimetry phantom was developed for intensity-modulated radiation therapy (IMRT), which requires various types of challenging dosimetry. Polystyrene, polyethylene, polytetrafluoroethylene (PTFE), and polyurethane foam (PU-F) were selected to represent muscle, fat, bone, and lung tissue, respectively, after considering the relevant mass densities, elemental compositions, effective atomic numbers, and photon interaction coefficients. The phantom, which is composed of numerous small pieces that are similar to LEGO blocks, provides dose and dose distribution measurements in homogeneous and heterogeneous media. The phantom includes dosimeter holders for several types of dosimeters that are frequently used in IMRT dosimetry. An ion chamber and a diode detector were used to test dosimetry in heterogeneous media under radiation fields of various sizes. The data that were measured using these dosimeters were in disagreement when the field sizes were smaller than $1.5{\times}1.5\;cm^2$ for polystyrene and PTFE, or smaller than $3{\times}3\;cm^2$ for an air cavity. The discrepancy was as large as 41% for the air cavity when the field size was $0.7{\times}0.7\;cm^2$, highlighting one of the challenges of IMRT small field dosimetry. The LEGO-type phantom is also very useful for two-dimensional dosimetry analysis, which elucidates the electronic dis-equilibrium phenomena on or near the heterogeneity boundaries.
