Association between Alzheimer's Disease and Cancer Risk in South Korea: an 11-year Nationwide Population-Based Study

  • Background and Purpose: Previous studies have suggested a decreased cancer risk among patients with Alzheimer's disease (AD). There remains a lack of data on the specific types of cancer and risk factors for developing cancer in AD. We evaluated the association between AD and cancer risk, and we examined specific types of cancer. Methods: A population-based longitudinal study was conducted using the National Health Insurance Service-Senior cohort for 2002-2013. A total of 4,408 AD patients were included in the study, as were 19,150 matched controls. Potential associations between the risk of cancer and AD were analyzed using Cox proportional hazard regressions. Results: Cancer developed in 12.3% of the AD group patients and in 18.5% of control group subjects. AD was associated with a reduced risk of cancer (hazard ratio [HR], 0.70; 95% confidence intervals, 0.64-0.78). The risk of head and neck cancers was significantly reduced (HR, 0.49), as were risks for cancers of the digestive tract, including stomach cancer (HR, 0.42), colorectal cancer (HR, 0.61), liver and biliary tract cancers (HR, 0.68), and pancreatic cancer (HR, 0.55). Lung and prostate cancer risks were also significantly lower for the AD group (HR, 0.52 and HR, 0.72, respectively). Conclusions: Our results showed an inverse association between AD and cancer. Further research involving a large number of patients in a hospital based-study is needed to address the biological associations between cancer development and dementia, including AD.

Feasibility Study of Cylindrically Diffusing 532 nm Wavelength for Treatment of Pancreatic Cancer

  • Laser ablation may provide a minimally invasive palliative treatment for pancreatic cancer. The aim of the current study was to assess the feasibility of a 532-nm laser equipped with a cylindrical light diffuser for the treatment of pancreatic cancer. Monolayers of BxPC-3 human pancreatic cancer cell were exposed to 532 nm laser light. Power levels of 5 - 7 W were used to uniformly target the entire cell colonies for 60 and 120 seconds. The cells were incubated for 24 hours after treatment and viabilities were determined by using a MTT assay. Laser ablation was performed by using the cylindrical light diffuser on six pancreatic tumor tissues obtained from pancreatic cancer xenograft mouse models, which were exposed to the 532 nm light at 5W or 7W for 10 to 30 seconds. In the in vitro study, the survival rates of the pancreatic cancer cells were reduced by 6.6% to 98.9% after the treatment, and the survival rates were reduced by increasing laser power and/or irradiation time. In the pancreatic tumor tissues, a homogenous circular ablation zone was observed in all tumors and the ablation distance induced by the laser irradiation showed to be constant from the diffuser to all directions (standard deviation, 0.3 - 1.3 mm). Ablation distance and area increased with increasing laser power and/or irradiation time. The 532 nm laser effectively killed pancreatic cancer cells, and the cylindrical light diffuser was found to be suitable for laser ablation as it provided uniform ablation in pancreatic cancer.

Anti-Wrinkling Effect of Noni (Morinda citrifolia) by Antioxidant and Anti-Inflammatory Properties

  • Noni has been used for medicinal purposes for more than 2,000 years in South Pacific Polynesia, China and India, and has been heavily ingested as an extract for its excellent antioxidant and anti-inflammatory effects. However, a recent study found that the noni extract causes digestive disorders, kidney problems, and liver diseases, which made it necessary to use it for other purposes than as an extract. In this study, we want to evaluate the potential of noni as an anti-oxidant, anti-inflammatory and anti-wrinkling agent. Methods: Noni was freeze-dried, extracted in water, and concentrated. Skin cells were treated with the noni extract for 24 hrs and then were exposed to UVB (55 mJ/cm2). After 48 hrs of incubation, pro-inflammatory cytokine, elastase, MMP-1 and type-1 procollagen levels were measured by ELISA. Results: To find out the antioxidant effect of the noni extract, the DPPH and ABTS radical scavenging activity experiments were conducted and the noni extract showed 97.0 % and 92.0 % antioxidant efficacy at 200 ㎍/mL respectively. The noni extract (50 and 100 ㎍/mL) decreased IL-6 and TNF-α in RAW 264.7 cells induced by LPS in a concentration-dependent manner. In the RT-PCR experiment involving NO production, the noni extract (50 and 100 ㎍/mL) inhibited NO production by strongly inhibiting iNOS mRNA expression, and also inhibited the elevation of MMP-1 and elastases caused by UVB irradiation by 25.0 % and 7.0 % respectively. In addition, type-1 procollagen was elevated by 20.0 % by the noni extract treatment in HaCaT cells. Conclusion: The noni extract has photoprotective ability by reducing proinflammatory mediators, elastase and MMP-1 production, and elevation of collagen synthesis. Our findings suggest that the noni extract might be a good natural substance to protect against UVB-induced premature skin aging.

Chondroprotective Effects of a 30% Ethanol Extract of Sargassum fulvellum (모자반(Sargassum fulvellum) 주정 30% 추출물의 In Vitro에서의 연골 보호효과)

  • Osteoarthritis (OA) is an inflammatory disease due to wear caused by the continuous use of cartilage. Although many drugs for treating OA are being studied, they have side effects, such as digestive disorders and cardiovascular diseases. Glucosamine, a drug derived from natural products, is known to be less effective. Therefore, the marine organism, Sargassum fulvellum, was studied to determine whether it contains substances with a chondroprotective effect on the inflammatory response of chondrocytes induced by interleukin-1β (IL-1β). A 30% ethanol extract of S. fulvellum (SF30%EtOH) has therapeutic and few side effects. We first confirmed the presence of nitric oxide (NO) and the expression of inducible nitric oxide synthase (iNOS), which is expressed during inflammatory reactions. We then examined the expression of collagen type II, which is the main component of the extracellular matrix and cartilage. Finally, the expression of extracellular matrix degrading enzymes, MMPs and ADAMTS-4 and -5, was confirmed. The results showed that SF30%EtOH reduced the expression levels of NO, iNOS, MMPs, and ADAMT-4 and -5, and increased the expression level of collagen type II in chondrocytes induced with IL-1β. Therefore, SF30%EtOH has a chondroprotective effect against inflammation, indicating its potential use for the prevention and treatment of OA.

A study on the implementing the Movement of <Life Nurturing Method and Conduction Exercise(養生方導引法)> of 《Pathogenesis and Manifestations of Food Stagnation and Indigestion(宿食不消病諸候)》 in 『Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論)』 (『제병원후론(諸病源候論)』 《숙식불소병제후(宿食不消病諸候)》에서 <양생방도인법(養生方導引法)>의 동작(動作) 구현(具顯)에 관한 고찰)

  • Objective : We examined the literature of <Life Nurturing Method and Conduction Exercise(養生方導引法)> of 《Pathogenesis and Manifestations of Food Stagnation and Indigestion(宿食不消病諸候)》 in 『Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論)』 and actually implement its movement to present conduction exercise based on the principle of Korean medicine treatment. Method : We used web services for traditional Eastern Asian medical classics and some literature on 『Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論)』 and its <Life Nurturing Method and Conduction Exercise(養生方導引法)〉. The original text was interpreted and the movements of <Life Nurturing Method and Conduction Exercise> were implemented according to the literature. A qi-gong(氣功) expert demonstrated the movements and we took pictures. Results : <Life Nurturing Method and Conduction Exercise(養生方導引法)> was written in the symptom of Food Stagnation and Indigestion(宿食不消候) and the symptom of food damage and excessive eating(食傷飽候) among the chapters of 《Pathogenesis and Manifestations of Food Stagnation and Indigestion(宿食不消病諸候)》. With respect to the movements and breathing, stretching your back allows qi(氣) energy to enter the lower cinnabar field(下丹田) immediately, and when the lower stomach is full of qi it can be transported upper body, leading to better communication of qi around the body. Conclusion : The practical implementation of the movements and postures will contribute to understanding the characteristics, pathology, and contents of diseases related to indigestion, and furthermore it can be widely used clinically for prevention and treatment of digestive disorders.

  • Non-alcoholic fatty liver disease (NAFLD), especially including non-alcoholic steatohepatitis (NASH) is one of the common diseases with 25% of prevalence globally, but there is no thera-peutic access available. Amomum villosum var. xanthioides (Wall. ex Baker) T.L.Wu & S.J.Chen (AX), which is a medicinal herb and traditionally used for treating digestive tract disorders in Asia countries. We aimed to examine pharmacological effects of ethyl acetate fraction of AX (AXEF) against ER stress-induced NASH mice model using C57/BL6J male mice by tunicamycin (TM, 2 mg/kg) injection focusing on the oxidative stress. Mice were orally administrated AXEF (12.5, 25, or 50 mg/kg), silymarin (50 mg/kg) or distilled water daily for 5 days, and outcomes for fatty liver, inflammation, and oxidative stress were measured in serum or liver tissue levels. AXEF drastically attenuated hepatic ER stress-induced NASH which were evidenced by decreases of li-pid droplet accumulations, serum liver enzymes, hepatic inflammations, and cell death signals in the hepatic tissue or serum levels. Interestingly, AXEF showed potent antioxidant effects by quenching of reactive oxidative stress and its final product of lipid peroxide in the hepatic tissue, specifically increase of metallothionein (MT). To confirm underlying actions of AXEF, we ob-served that AXEF increase MT1gene promoter activities in the physiological levels. Collectively, AXEF showed antioxidant properties on TM-induced ER stress of NASH by enhancement of MTs.

  • Newborn gastrointestinal tract emergency diseases include various disorders that occur anywhere along the digestive tract. Most of them are congenital malformations, not acquired diseases. Many of them cannot be confirmed on prenatal examination, and consequently, postnatal evaluation is required. Unlike adults, in newborn babies, detailed evaluations with computed tomography (CT) and magnetic resonance imaging (MRI) are often limited, so making the diagnosis based on early abdominal radiography is most important. Therefore, it is necessary to be familiar with the radiologic findings that may be seen on imaging studies. In addition, it is important to understand abdominal ultrasound and fluoroscopy findings, which can be used in addition to simple radiography and indications that can be used for further diagnosis and appropriate treatment. Therefore, this study describes and organizes the various imaging findings that may occur in neonatal gastrointestinal emergencies.

Comparative Study on Dietary Life and Nutrient Intakes of Elderly Persons at Nursing Home or Their Home in Chungnam (충남지역 일부 시설노인과 재가노인의 식생활 및 영양섭취상태 비교)

  • The purpose of this study was to compare the dietary life and nutrient intakes among elderly persons residing at nursing homes and their homes in order to provide fundamental data for improved foodservice and dietary management. We recruited 150 subjects at a nursing home stay and 150 home stay elderly persons for a survey. The results were as follows. There was no significant difference in frequency of napping between the two groups. The frequency of going out was higher in the home stay subjects, but regular exercise was higher in nursing home stay subjects. Subjects possessed the following diseases: dementia and digestive disorders for nursing home stay elderly men, blood vessel disorder and diabetes for home stay elderly men, digestive and blood vessel disorders for nursing home stay elderly women, and blood vessel and heart disease for home stay elderly women. In a health-related assessment, smoking, drinking, and teeth condition of nursing home stay subjects were not higher than those of home stay subjects. The ratio of subjects who ate meals everyday was higher in nursing home stay elderly women than in home stay women. While most nursing home subjects ate adequate amount of meals, home stay subjects ate until they were full. The daily energy intakes of men and women were 2,229.9 kcal and 2,302.1 kcal for nursing home subjects and 1,894.0 kcal and 1,885.9 kcal for home subjects, respectively. Nutrient intake was also higher in nursing home than home stay subjects. In summary, the dietary life of elderly persons residing in a nursing home was higher than that of elderly persons residing at their home from the view point of meal frequency, meal intake, and nutrient intakes. Therefore, systematic assessment and management of nutrition for elderly staying at a nursing home or at home alone should be conducted continually.

Postpartum Reproductive Tract Recovery and Prevalence of Health Problems in Dairy Cows (젖소의 분만 후 생식기의 회복 및 질병 발생 조사)

  • This study estimated the degree of uterine inflammation and resumption of ovarian cyclicity in postpartum dairy cows. In addition, the prevalence of health problems during calving and the postpartum period was investigated. A total of 224 Holstein dairy cows from four dairy farms in Chungcheong Province were used for the study. Uterine discharge was scored on a 0 to 5 rank scale (metricheck score) using a metricheck instrument from 1 to 8 weeks postpartum and uterine cytology (neutrophil level) was examined at 4, 6 and 8 weeks postpartum to evaluate uterine inflammation. Resumption of ovarian cyclicity was evaluated based on progesterone analysis (${\geq}1ng/mL$) at 4, 6 and 8 weeks postpartum. Postpartum disorders were diagnosed by veterinarians in the research team. The metricheck score decreased linearly (p < 0.0001) from weeks 1 ($3.4{\pm}0.1$) to 4 ($2.0{\pm}0.1$) postpartum, and then remained at that level through week 8 postpartum ($1.6{\pm}0.1$, p > 0.05). The neutrophil levels determined by uterine cytology assays also decreased (p < 0.0001) from week 4 ($22.3{\pm}1.8%$) to 6 ($12.1{\pm}1.4%$) postpartum, and then remained at that level through week 8 postpartum ($9.2{\pm}1.4%$, p > 0.05). The proportion of cows exhibiting the ovarian cyclicity increased linearly (p < 0.0001) from weeks 2 (12.1%) to 8 (74.3%) postpartum. The prevalence of dystocia and retained placenta were 20.5% and 30.4%, respectively. The prevalence of ketosis, milk fever, abomasal displacement, septicemic metritis, endometritis, subclinical endometritis, pyometra, and ovarian cysts were 18.8%, 3.6%, 4.0%, 36.2%, 29.5%, 17.0%, 4.5%, and 14.7%, respectively. The prevalence of digestive problems and mastitis were 5.4% and 9.6%, respectively. The prevalence of culling and death were 4.0% and 3.6%, respectively. The prevalence of dystocia, retained placenta, septicemic metritis, endometritis, ovarian cysts, digestive problems, mastitis, and culling differed among farms (p < 0.05 to 0.01). In conclusion, proper examinations for uterine inflammation and ovarian cyclicity during 4 to 8 weeks postpartum are useful clinical tools to maintain herd reproductive health. Dystocia, retained placenta, septicemic metritis, and endometritis were predominant disorders in dairy cows of the survey area.

A study on the degree of need of the knowledge of pathophysiology, clinical microbiology and mechanisms and effects of drugs in clinical nurses (기초간호자연과학의 병태생리학, 병원미생물, 약물의 기전과 효과 내용별 필요도에 대한 연구)

  • The purpose of this study was to define the content of the requisite knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for clinical knowledge for nursing practice. Contents of knowlege on pathological physiology, clinical microbiology, and mechanisms and effects of drugs were constructed from syllabus of basic nursing subjects in 4 colleges of nursing, and textbooks. The degree of need of 72 items was measured with a 4 point scale. The subjects of this study were college-graduated 136 nurses from seven university hospital in Seoul and three in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, and hospice ward. The results were as follows : 1. The highest scored items of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice were side effects of drugs, anticoagulants, mechanisms of drugs, antihypertensive drugs, tolerance and addiction of drugs, interactions among drugs, hospital infection in the order of importance. The lowest scored item was structure of microorganisms. 2. The highest order of need according to unit was repair in tissue injury unit, definition etiology classification of inflammation in inflammation unit, transplantation and immunologic response in alterations in immunity unit, thrombus and thrombosis in disorders of cardiovascular function unit, gene disorders in genetic disorders unit, hospital infection in infection unit, virus in microorganisms unit, side reactions of drugs in introduction unit, anticonvulsants in drugs for central nervous system unit, local anesthesia in anesthesia unit, anticoagulants in drugs for cardiovascular system unit, anti-inflammatory drugs in antibiotics unit, anti-ulcer drugs in drugs for digestive system unit, and bronchodilators in drugs for respiratory system unit. 3. The common content of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for all clinical areas in nursing were side effects of drugs, anticoagulants, interactions among drugs, and hospital infection. However, the degree of need of each pathological physiology, clinical microbiology, clinical microbiology, and mechanisms and effects of drugs was different depending on clinical areas. 4. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as tissue changes due to injurious stimuli, degenerative changes of tissue, alterations in metabolism of carbohydrates, ischemia, hyperemia and congestion, hospital infection, structure of microorganism, classification of microorganism, bacteria, virus, antidepressants, antipsychotic drugs, antiemetic drugs, antiparkinsonism drugs, antianxiety drugs, antibiotics, tuberculostatics, antiviral drugs, antifungal drugs, parasiticides, antiulcer drugs, antidiarrheais, and anti constipation drugs were shown according to the work area. 5. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as transplantation and immunologic response, alterations in the metabolism of uric acid, structure of microorganism, classification of microorganism, immunosuppressants, drugs for congestive heart failure were demonstrated according to the duration of work. Based on these findings, all the 72 items constructed by Korean Academic Society of Basic Nursing science should be included as contents of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs.
