Diffuse alveolar hemorrhage is a rare but serious and frequently life-threatening complication of a variety of conditions. The first goal in the management of patients with diffuse alveolar hemorrhage is to achieve or preserve stability of the respiratory status. Subsequently, the differential diagnosis is aimed at the identification of a remediable cause of the alveolar hemorrhage. The most common causes of diffuse alveolar hemorrhage with glomerulonephritis are microscopic polyangiitis and Wegener's granulomatosis, followed by Goodpasture syndrome and systemic lupus erythematosus. Microscopic polyangiitis (MPA) is a distinct systemic small vessle vasculitis affecting small sized vessels with few or no immune deposits and with no granulomatosus inflammation. The disease may involve multiple organs such as kidney, lung, skin, joint, muscle, gastrointestinal tract, eye, and nervous system. MPA is strongly associated with antineutrophil cytoplasmic autoantibody (ANCA) that is a useful serological diagnostic marker for the most common form of necrotizing vasculitis. Our report concerns a case of microscopic polyangiitis with diffuse alveolar hemorrhage in a 54-year-old man. He was admitted to our hospital due to dyspnea upon exertion and recurrent hemoptysis. Laboratory findings showed hematuria, proteinuria and deterioration of renal function. In the chest CT scan, diffuse ground glass appearance was seen in both lower lungs. A lung biopsy revealed small vessel vasculitis with intraalveolar hemorrhage and showed a positive reaction to against perinuclear ANCA. The patient was treated with prednisolone and cyclophosphamide. Chest infiltration decreased and hemoptysis and hypoxia improved. He is still being followed up in our hospital with a low dose of prednisolone.
Purpose: Diffuse pigmented villonodular synovitis (PVNS) is an uncommon aggressive synovial proliferative disorder of unknown etiology affecting the joint linings. Though a histologically benign inflammatory process, because of its aggressive growth with bone destruction or recurrence, it is frequently suggested to occur as a low malignant neoplasm. Optimal treatment is surgery, but the local recurrence rate after radical synovectomy for diffuse PVNS is relatively high due to the infiltrative growth pattern. External beam radiotherapy with moderate doses or intra-articular instillation of radioactive isotopes may improve the likelihood of local control and long-term function in patients with incompletely resected or recurrent diffuse PVNS. I report one case of diffuse PVNS of the right knee joint treated with arthroscopic synovectomy and external beam radiotherapy is presented.

Objective : This study was conducted to investigate the correlation between the degrees of injury on brain magnetic resonance imaging (MRI) and the time interval to recovery of consciousness in patients with diffuse axonal injury. Methods : From January 2004 to December 2008, 25 patients with diffuse axonal injury were treated at our hospital. We retrospectively investigated the patients' medical records and radiological findings. We divided the patients into three groups according to the grade of MRI finding : grade I, small scattered lesions on the white matter of the cerebral hemisphere; grade II, focal lesions on the corpus callosum; and grade III, additional focal lesions on the brain stem. Result : Seven patients belonged to the grade I group; 10 to the grade II group; and 8 to the grade III group. The mean Glasgow Coma Scale (GCS) score of all patients at the time of admission was 7.28. Recovery of consciousness was observed in 23 of the 25 patients; the remaining two patients never regained consciousness. The time interval to recovery of consciousness (awake status) ranged from 1 day to 125 days (mean 22.1 days) : grade I group patients, within approximately 1 week (mean 3.7 days); grade II group patients, within approximately 2 weeks (mean 12.5 days); and grade III group patients, within approximately 2 months (mean 59.5 days). Conclusion : Our study results suggest a correlation between the mean time interval to recovery of consciousness in patients with diffuse axonal injuries and the degrees of brain injuries seen on MRI. Patients with grade I and II diffuse axonal injuries recovered consciousness within 2 weeks, while patients with grade III injuries required approximately 2 months.

Object : We intended to investigate the relationship between the degree of injury on MRI and the outcome of the patients with diffuse axonal inury. Method : From january, 1995 to march, 1999, 22 patients were supposed to have diffuse axonal injuries by means of their neurologic signs and MRI. We investigated their prognosis according to CT, MRI and initial neurologic findings. Result : 1) The lesions were mainly located at white matter of cerebrum, corpus callosum, brainstem, and basal ganglia. 2) The lesions of white matter were most commonly in the frontal lobe and temporal lobe. 3) The majority of corpus callosal lesions were located in the posterior body and splenium, but anterior corpus callosal lesions combined with posterior lesions were not found. 4) Brainstem lesions, all non-hemorrhagic, were mostly located in the dorsolateral aspect, not be found on CT. 5) The brainstem lesions were found in 10 cases among total 22 cases, and corpus callosal lesions were accompanied with 8 cases of brainstem lesions. 6) The patients with brainstem lesions had worse prognosis. Conclusion : It is important and reasonable to take brain MRI to identify the brainstem lesions in any cases of suspicious diffuse axonal injury, and we should remind that the diffuse axonal injury with stem lesion has worse prognosis.

This study investigates the effects of moisture content on diffuse ultrasound to be applied for the evaluation of micro-structural damage in concrete subjected to various environmental conditions. We monitored diffuse wave parameters for concrete samples in process of water saturation for 5 days. Dried samples were immersed in a water bath, and the change of moisture content in concrete were estimated by measuring the change of mass. For the diffuse wave analysis, a frequency range of 500 kHz, which represents a scattering regime of ultrasound in concrete, was selected. The test results reveal that the ultrasonic diffusivity slightly changed, and the ultrasonic dissipation significantly increased by approximately 120% in the process of water saturation. Therefore, the moisture content in concrete should be considered for the evaluation of micro-structural damage using diffuse wave techniques.

Global solar radiation consists of direct and diffuse radiations. Both components are necessary for not only atmospheric science and solar energy domains but also agricultural applications. In this study, the data of direct and diffuse radiations are uploaded to Github. It was observed in Naju during May 2019 to November 2020. Using this data, the previous empirical equations using the relation between clearness index and diffuse ratio were validated. All coefficients of determination (R2) and RMSE were similar as 0.79~0.80 and 0.13~0.15. However, to get the lower RMSE, other non-linear approaches will be required with more observation data.
Objective : There were few reports on the treatment of Adult-onset Still's disease and Diffuse cerebral dysfunction. This oriental medical treatment shows the possibility of healing Adult-onset Still's disease and Diffuse cerebral dysfunction, therefore we announce this. Methods : The acupuncture, herbal medicine, herbal acupucture(Jahageo, Nockyong) therapies were applied for treating this patient's chief symptom(chest discomfort, palpitation, rigidity, mentality change, global aphasia, dysphagia, coughing and sputum) Results : 1. Adult-onset Still's disease is a rare systemic inflammatory disorder of unknown etiology, characterised by salmon-colored rash, intermittent spiking high fever, arthralgia and variety of systemic features. This is one type of Juvenile arthritis. 2. Diffuse cerebral dysfunction is an anoxic-ischemic encephalopathy, and most cause of this is the hypoxia caused by hypotention or respiratory distress. 3. After oriental medical treatments, his chief symptoms were improved. Conclusion : The more study about oriental medical treatment on Adult-onset Still's disease and Diffuse cerebral dysfunction is needed.
The diagnostic criteria for diffuse thyroid disease are ambiguous and there are many errors due to the subjective diagnosis of experts. Also, studies on ultrasound imaging of thyroid nodules have been actively conducted, but studies on diffuse thyroid disease are insufficient. In this study, features were extracted by applying the GLCM algorithm to ultrasound images of normal and diffuse thyroid disease, and quantitative analysis was performed using the extracted feature values. Using the GLCM algorithm for thyroid ultrasound images of patients diagnosed at W hospital, 199 normal cases, 132 mild cases, and 99 moderate cases, a region of interest (50×50 pixel) was set for a total of 430 images, and Autocorrelation, Sum of squares, sum average, sum variance, cluster prominence, and energy were analyzed using six parameters. As a result, in autocorrelation, sum of squares, sum average, and sum variance four parameters, Normal, Mild, and Moderate were distinguished with a high recognition rate of over 90%. This study is valuable as a criterion for classifying the severity of diffuse thyroid disease in ultrasound images using the GLCM algorithm. By applying these parameters, it is expected that errors due to visual reading can be reduced in the diagnosis of thyroid disease and can be utilized as a secondary means of diagnosing diffuse thyroid disease.
This study was carried out to investigate the dimensional stability of wood treated with PEG for better utilization of domestic small-diameter logs. Four species of ring- porous wood(Quercus mongolica FISCH, Quercus variabilis BLUME, Fraxinus rhynchophylla HANCE, Paulownia coreana UYEKI,) and four species of diffuse-porous wood(Prunus sargentii REHDER, Betula davurica PALL, Populus tomentiglandulosa T. LEE, Cornus controversa HEMSLEY) were used for this experiment. The shrinkage of wood decreased with increasing the concentration of PEG. The shrinkage of diffuse- porous woods was lower than that of ring-porous woods. The weight gain increased in proportional to the PEG concentration. Diffuse-porous woods showed a little higher weight gain than ring-porous woods. Bulking effect also increased with increasing the PEG concentration, but was in inverse proportional to the molecular weight of PEG. Diffuse-porous woods showed higher bulking effect than ring-porous woods. Consequently, diffuse-porous woods showed better dimensional stability than ring- porous woods. It was considered that dimensional stability was affected by characteristics of wood such as vessel and tylosis, and density.
