A study on the Current of Hospital Practices in Clinical Dietetics (임상영양사의 업무수행 현황에 관한 연구)

  • A study was undertaken to elucidate the clinical dietetic performance level and to identify the difference between the actual role performance and perception of important level of the clinical dietitian. The survey was carried out for the 21 general hospital over 100 beds in Seoul, Pusan and Daegu area. The results of the study can be summarized as follows. 1. One dietitian was responsible for the feeding and clinical dietetic activity of 166 in-patients on the average. 2. In the actual role performance of clinical dietetic practices, of the 26 items listed, only 3 were always or almost or almost performed by 75% of the dietitians, 4 additional items were always or almost performed by 60% of them. 3. In the perception of importance level of clinical dietetic practices, more than 75% of the dietitians indicated that 23 of the 26 tasks were important practices. 4. The performance average score of nutrition assessment was 15.85, of nutrition care plan development was 19.72, of patient counseling was 12.22, of research activity was 19.62, and of education was 21.27 in the dietetic department was a division for the clinical dietetic activity, while 10.96, 15.24, 9.54, 15.97, 18.50 respectively in the dietetic department has not a division for it.

Identifying Current Hospital Practices in Clinical Dietetics (병원 영양사의 임상업무 실태 조사(I))

  • The purpose of this study were to examine clinical dietetic performance level and to determine factors affecting clinical dietetic performance level. The survery was carried out for the 33 general hospitals over 400 beds in Seoul We established 13 factors which amy affect clinical dietetic performance level and investigated the relationship between these factors and clinical dietetic performance level. The average score of performance and perception of importance levels of clinical dietetic practices were 46.0${\pm}$6.9 and 73.2${\pm}$4.8 respectively. None of the demographic variables were significantly correlated to clinical dietetic performance level. Clinical dietetic performance level depended upon whether dietetic department had a division or not for clinical dietetic activities.

  • The foodservice operation in SEVERANCE dietetic department was assessed to evaluate and improve the foodservice quality product in and served. The survey questionnaire consisted of the general background / its evaluation of quality / sanitary condition / patient's taste. The survey has been done since 1987. The following results are based on the survey done on the July second, 1994. It was done for every patient and the percentile of data collection was 80.8. All data were analyzed through Chi-square, t-test, ANOVA, Correlation, Multiple Regression(ENTER). The results are as follows 1. Patient's perception, variety in fooditems, food's temperture was not bad'. 2. The intake of meals was 60-65% for meat, 65-70% for vegetable, and 50-55% for kimchi. Bad appetite(30.2 %), poor displays of food( 13.2%), and lack of variety of fooditems were responsible for the residue. 3. Foodservice and sanitary condition were excellent. 4. The patient's demend for nutritional deucation was high though only a few(7.6%) were educated. 5. Patients' satisfaction with the department was significantly(p<0.001) different from that with other medical department. 6. Patient's satisfaction was influenced and determined by menu's variety, sanitary condition, age, length of stay and food's temperature.

A comparison of in-patients' satisfaction in hospital foodservice method (급식관리방법에 따른 환자 만족도 비교연구)

  • The purpose of this study were to improve in foodservice system, and to evaluate the satisfaction of patient meal. Two General Hospitals (A & B) in Ewha Womans University college of Medicine were selected for this study. The survey questionnaire consisted of general backgrounds and foodservice evaluation. We chosed factors which may influence meal satisfaction such as meal time, amount, optimal temperature, taste, quality, sanitary condition, and employee' s kindness. One-way ANOVA and Pearson' s correlation were used as statistical methods. The results can be summarized as follows 1. In meal time, for the A&B hospitals were 3.46$\pm$0.93 3.63$\pm$0.76 respectively. 2. In cold foods for the A&B hospitals were 3.23$\pm$0.89 3.52$\pm$0.78 respectively while in hot foods 3.29$\pm$1.02 3.27$\pm$0.90 respectively for A&B hospitals. 3. In taste for the A&B hospitals were 2.81$\pm$0.96 3.01$\pm$0.95 respectively. 4. In quality were 2.93$\pm$0.92 3.25$\pm$0.91 respectivly. 5. In amount were 3.38$\pm$0.95 3.36$\pm$0.98 respectively. 6. In sanitary condition for the A&B hospitals were 3.55$\pm$0.88 3.12$\pm$0.97 respectively. 7. In kindness were 3.11$\pm$0.87 3.32$\pm$0.76 respectively. 8. Quality, taste, meal time, kindness, sanitary condition, temperature, amount, age, duration, and room grade were significantly correlated to the satisfaction of patient meal in order.

Case Report of Metabolic Syndrome Patients Improved with Oriental Medical Treatment and Dietetic Therapy (한방치료와 식이요법을 병행하여 호전된 대사증후군환자 증례보고)

  • Objectives This study was to report the effect of oriental medical treatment and dietetic therapy on metabolic syndrome. Method We treated three cases of metabolic syndrome patients with herbal medicine, electroacupuncture, cupping a boil, dietetic therapy, aerobic exercise were done during the treatment period. Result After treatment the diagnostic index(waist circumference, triglycerides, HDL cholesterol, blood pressure, fasting glucose) was improved. Conclusion The patient with Metabolic syndrome was improved using oriental medical treatment and dietetic therapy.

  • This study was conducted to evaluate the effect of current dietetic internship program. The survey sample was dietetic practitioners who were charged with a training for dietetic students. The questionnaire requested information about demographic information, experience of guidance for the dietetic student, self evaluation as trainer, fortification Items in university education, and difficulties of the internship program. Data was analyzed using SAS for windows(version 6.12), Descriptive statistics were used such as means, standard deviations, frequency distribution and percentages. Comparison of mean differences was analyzed using t-test and ANOVA. Participants identified how effective the current internship program was. Result indicated that the effectiveness was not high(2.81), and under 29 years old age group, under 6 years work experience group, and contracted foodservice group had positive opinion of the program significantly. Under 29 years old age group, evaluated themselves as a good trainer. The results revealed that respondents wanted to be fortified the practical training courses in school curriculum.

Work Analysis and Time Measurement of Dietians in Employee Feeding Facilities by Work Sampling Methodology (워크샘플링에 의한 사업체 급식소 영양사 업무 분석 및 소요시간 산출)

  • The purposes of this study were to: a) investigate the percentage distribution and the time spent of dietetic activities and b) estimate dietitian's staffing needs in employee foodservices. In 6 employee foodservices, the dietetic activities were analyzed by work sampling methodology. The results of this study were as follows: 1. The percentage distributions of dietetic activities, delay and non-dietetic activities were 79.06, 20.39, and 1.55%, respectively; 2. The major activities of dietitians in employee foodservice were production management 21.00%, purchasing management 16.73%, record keepig 14.40%, and menu management 6.30%, 3. The total labor time per week was 3,310 min (55.16 hr) and specially the time spent on 13 dietetic activities was 2,626 min (43.77 hr). 4. The time spent per week on major activities of dietitians such as production management, purchasing management record keeping, and menu management were 693.93, 554.83, 483.99, and 205.22 min, respectively.

Developing standardized dietetic staffing indices in employee foodservice by job analysis methodology (사업체 급식소 영양사 직무분석 ( 제 2 보 ) : 업무수행시간 및 적정인원산출)

  • The purposes of this study were to : a) investigate actual time spent and expected labor time spent on management activities, and b) develop standardized indices of dietetic staffing needs in employee foodservice. A job analysis questionnaires were developed and mailed to 65 dietitians who were members of The Korean Dietetic Association Practice Group, members with management responsibilities in employee foodservices. Completed questionnaires were received from 32 dietitians for a response rate of 49%. The questionnaire contained two parts with a total of 99 statements. Statistical data analysis was completed using the SAS programs for descriptive analysis, wilcoxon signed ranks test, wilcoxon rank sum test, and pearson correlation. The results of this study can be summarized as follows. 1. The actual time spent on management activities by dietitians in employee foodservice was 69.80 hours and expected labor time spent was 61.81 hours. And they were significantly different (p<0.05). 2. ILO allowance rate( 11%) was applied: The standardized working hours per week of dietitians working in employee foodservice with manufacturing and industrial plants, and office building were 79.61 and 64.25 respectively ; Staffing need indices were 1.81 and 1.46 respectively on the base of 44 working hours. 3. The average standardized working hours per week was 68.61 hours and staffing need indices was 1.56

A study of satisfaction with nutrition counseling service for consumers (영양상담서비스에 대한 소비자의 만족도 조사 연구)

  • The purposes of this study were to analyze the performance in nutrition counseling service and to develop a desirable methodology for the nutrition counseling. Data were collected through a survey on satisfaction of 221 patients( 111 males and 110 females) in the nutrition counseling service conducted in seven general hospitals of over 450 beds in Seoul. The results are as follows Most(93.9%) of the respondents agreed on the necessity of nutrition counseling and 96.8% of them recognized the importance of the role of a therapeutic diet. The mean satisfaction ratings were 4.34/5.00 in the dietitian's customization, 4.19/5.00 in the overall nutrition counseling service, 3.90/5.00 in the detailed presentation of the menu, and 3.66/5.00 in the explanations of recipes for the food. Total mean satisfaction ratings were 38.57/55.00 for liver disease patients, 40.29/55.00 for hypertensives, 42.54/55.00 for renal disease patients, 43.28/55.00 for gastrointestinal disease patients, and 44.09/55.00 for diabetics.

Usage of Foodservice Systems Management Competencies by Korean Dietitians

  • The purpose of this study was: 1) to investigate whether Korean dietitians are engaged in Foodservice Systems Management (FSM) competencies in their current jobs, and 2) to identify the relationship between types of dietetic practice areas and Korean dietitians' usage of FSM competencies. Fifteen FSM competencies were widely used, 17 were somewhat used, 13 were seldom used and the remaining two competencies were little or not used by the dietitians. The most used competency was related to menu planning (98%). Whereas, the least used competency was related to marketing skills (17%). As a whole, the categories of 'technical and communication skills' (78%) and 'production and distribution management' (77%) were more widely used than other FSM categories. Of 47 competencies, 34 competencies were found to be significantly associated with the type of practice area. Considering the findings, the type of dietetic practice area was a strong factor of Korean dietitians' usages of FSM competencies. Of 34 competencies associated with the type of dietetic practice area, 25 FSM competencies were more likely to be used by the dietitians from health care facilities.
