중국 학령기 아동과 청소년의 학교급식과 가정식 점심식사의 질 비교: 2011년 '중국건강영양조사' 자료 분석 (Dietary Quality Comparison of the School and Home Lunches Consumed by Chinese School-Age Children and Adolescents: Analysis of the 2011 China Health and Nutrition Survey)

  • Objectives: The number of schools offering school lunches has increased in China. This study examined the dietary quality of the lunches consumed by Chinese school-age children and adolescents, with a focus on comparing school lunches with home lunches. Methods: The first weekday 24-hour dietary recall data of 6~17-year-old students (n=1,084) from the 2011 China Health and Nutrition Survey were analyzed. The subjects were divided into the school lunch group and the home lunch group, and the dietary quality of lunches was compared between the two groups among 6~11-year-old students (n=634; 177 in the school lunch group and 457 in the home lunch group) and 12~17-year-old students (n=450; 144 in the school lunch group and 306 in the home lunch group), respectively. Frequently consumed foods, amount of food group intake, food group intake pattern, Dietary Diversity Score (DDS), and Dietary Variety Score (DVS) were examined. Results: The most frequently consumed foods in both lunch groups were rice and pork. An excessive intake of meat and insufficient intake of seafood were noted in both lunch groups. The school lunch group showed a lower level of vegetable consumption than the home lunch group (P=0.017 in 6~11-year-old students, P=0.003 in 12~17-year-old students). Although more students ate meals with a better dietary pattern in the school lunch group than the home lunch group, there were no significant differences in DDS and DVS between the two groups. Conclusions: Overall, the dietary quality of lunches was not superior in the school lunch group compared to the home lunch group. This suggests that much room remain for improving dietary quality of school lunches in China.

중국 연변 조선족 성인과 한국 구리지역 성인의 영양소 및 식품섭취 비교 평가 (Comparative Assessments of Dietary Intake of Korean-Chinese and Koreans)

  • This study was conducted to assess dietary intake and quality obtained by 24-hour recall method between Korean-Chinese in Yanbian, China(Korean-Chinese) and Koreans in Kuri, Korea(Koreans). The subjects consisted of 730 Korean-Chinese and 696 Koreans. Mean daily energy intake of Korean-Chinese was 1,788kcal and that of Korean was 1,844kcal. Proportions of energy intake from carbohydrate, fat, and protein were 62.6: 16.9: 14.2 for Korean-Chinese and 65.4: 16.3: 14.4 for Koreans. Nutrient intake levels of Koreans were higher than those of Korean-chinese in most nutrients. Calcium and vitamin $B_2$ intakes as percent of Korean RDA were less than 60% in Korean-Chinese. Koreans consumed calcium and vitamin A in the level less than 80% of Korean RDA. Iron intake level of Korean females was less than 75% of Korean RDA. Korean-Chinese consumed 90.4% of food from plant origin, and Koreans consumed 82.5% of food from plant origin. Mean dietary diversity score(DDS) of Korean-Chinese, $3.00{\pm}0.39$, was significantly lower than that of Koreans, $3.73{\pm}0.76(p<0.05)$. Mean dietary variety score(DVS) of Korean-Chinese, 13.6, was significantly lower than that of Koreans, 20.1(p<0.001). According to these results, some adults in the both Korean-Chinese and Koreans did not consume enough nutrients, especially calcium and vitamin $B_2$ intake for Korean-Chinese and calcium and vitamin A intake for Koreans. Dietary qualities of Korean-Chinese subjects and Korean females were not adequate. In the future, further research to promote dietary quality of Koreans including Korean-Chinese must consider the results of this study.

파킨슨병 환자의 영양 상태에 따른 식사의 질 및 삶의 질에 관한 연구 (Study on the qualities of diet and life in Parkinson's disease patients according to their nutritional status)

  • Assessment regarding the nutritional status of Parkinson's disease (PD) patients is important because their nutritional status influences the outcome of disease. The purpose of this study investigated the dietary habits, dietary quality, and quality of life for Korean PD patients according to their nutritional status. Seventy PD patients were recruited from K University Hospital (KMC IRB#0918-07) from February 2010 to October 2011. To diagnose PD, the United Kingdom Parkinson's Society Brain Bank standards were used by a neurology physician. The participants were interviewed and assessed using various tools, including a general questionnaire, anthropometrics, questionnaires for dietary habits, dietary diversity score (DVS), dietary variety score (DVS), diet quality index-international (DQI-I), beck depression inventory (BDI), the Korean version of mini mental state examination (K-MMSE), and 36-item short-form health survey (SF-36). The results of the study were as follows: 1) Most of the PD patients (69.0%) were at risk of malnutrition, and 8.5% and 22.5% of the subjects were malnourished and well-nourished, respectively. 2) Clinical symptoms (swallowing and chewing difficulties) were obvious, and the quality of diet (DDS, DVS, DQI-I) was low in the malnourished group (p < 0.01). 3) The malnourished group had significantly lower scores regarding SF-36 (quality of life) as well as that of cognitive function (p < 0.05). 4) The nutritional status of PD patients was significantly correlated with their depression (p < 0.05) and quality of life (p < 0.01). These results indicate that appropriate nutritional management will improve the nutritional status and quality of life as well as the cognitive function in Parkinson's disease patients.

2001년도 국민건강영양조사를 이용한 한국인의 혈중 지질 이상에 따른 식생활 비교 및 평가 (Analysis of Dietary Intake of Koreans by Dyslipidemia using 2001 Korean National Health and Nutrition Examination Survey Data)

  • To find out what foods and dishes are being consumed by people with dyslipidemia, we have researched which food groups and dish groups are utilized in order to use for nutrition educations. The data was obtained from participants in 2001 KNHNS using the 24hours recall method. Ages over 30 years old are used for the analysis. Food and dish group intakes are analyzed by gender, age, residence, education, and economic status. Dyslipidemia are divided into A, B and C groups using guideline of dyslipidemia. The differences in food consumption were analyzed and assessed by GMDVF, DDS and DVS. SAS and SPSS were used for the data analysis. Overall, In food group, people in B and C (with dyslipidemia) have higher intake levels of vegetables, while people in A (without dyslipidemia) have higher intake levels of fruits (p < 0.05). In dish group, people in B and C (with dyslipidemia) have higher intake levels of rice dish, soups and kimchi, while people A (without dyslipidemia) have higher intake levels of breads and snacks, salad and fruits (p < 0.05). In the dietary patterns of main food group (GMDVF), the pattern excluding dairy (11011) revealed the highest proportion in all groups. DDS = 4 has the highest proportion in all groups. The proportions of subjects with the low dietary diversity score increased in B and C (with dyslipidemia). Food groups that most people do not consume were dairy and fruits. The level of DVS in A (without dyslipidemia) is higher than in B and C (with dyslipidemia). With these results, it is shown that people with dyslipidemia had worse quality patterns of food intake than those without dyslipidemia. Thus we should emphasize the balanced diet and educate people how to choose foods. So it is necessary to develop food guide for people with dyslipidemia.

아토피 피부염 환자의 영양상태.식이형태 및 식사의 질 평가 (Study of Nutritional Status, Dietary Patterns, and Dietary Quality of Atopic Dermatitis Patients)

  • The prevalence of atopic dermatitis (AD) continues to increase in the industrialized countries related to Western lifestyle and dietary patterns. The purpose of this study was to evaluate the nutritional status, dietary patterns and dietary quality of AD patients. AD patients (n=50) and gender age matched healthy control (HC) were studied in case-control clinical trail. Preference and frequencies of consumption for 98 foods were estimated by questionnaires. Dietary pattern, current nutrients intake, and dietary quality were evaluated by the 3-day food records. As a result, anthropometric measurements from HC and AD patients were not significantly different. AD patients had significantly lower preference for buckwheat, some fishes (shellfish, flatfish, salmon), egg yolk, some vegetables (onion, garlic, Chinese-cabbage, radish, pepper, mushroom), coffee, and snack than those of HC had (p<0.05). AD patients had significantly lower frequency for pea, some fishes (shellfish, herring, flatfish, salmon, codfish), egg, some vegetables (spinach, pumpkin, mushroom), margarine/butter, nuts, coffee and apple juice, fried chicken, coke than those of HC had (p<0.05). The intake of vitamin $B_1$, vitamin $B_2$ (p<0.05) and vitamin E (p<0.001) of AD patients were significantly lower than those of HC. The intake for Ca and Zn of AD patients were remarkably lower than RDA for each nutrient. Moreover, AD patients' index of nutritional quality (INQ) and nutrient adequacy ratio (NAR) of vitamin E, vitamin $B_1$, vitamin $B_2$ were lower than those of HC. AD patients' dietary quality including dietary diversity score (DDS), dietary variety score (DVS), DMGFV (dairy, meat, grain, fruit, vegetable), dietary quality index (DQI) was poor. This results indicate that atopic dermatitis patients had significantly different food preference and food frequency when these were compared with those of healthy people. The dietary diversity, dietary variety and dietary quality of atopic dermatitis patients were very limited.

12~36개월 유아의 식사 질 평가 (The Assessment of Dietary Quality in Toddler of 12-36 Months)

  • This research was conducted to evaluate the general dietary quality(the balance and diversity of food intake, nutrient intake and adequacy) of 12 to 36-month-old toddler, through examining their actual food intake. It was conducted for 664(male-357, female-307) healthy toddler who were outpatients from Mar. to Aug., 1999 at university hospitals and private hospitals in Seoul and kyunggi province. The anthropometric parameters were measured. The 24-hr recall method were utilized to analyze the nutrient intake by months, based on the classification of The Korea Child Gorwth Standard. As for the subjects of this research, we found that the younger the age, the more frequent the pattern of intake of only 3 kinds of food groups. The results are that 1) the group with 1 point of the DDS(Dietary Diversity Score) mostly took only foods from the milk group, 2) the group with 2 points only took foods from the grain and milk group, 3) the group with 3 points did not take foods from the fruit and vegetable group, and 4) the group with 4 points took just about all kinds of food groups except for the vegetable group. This means that most of them were not taking various kinds of food even after the age of 12 months, and that they took more than twice the recommended amount of foods from the milk group until the age of 20 months. Thus, one could find a tendency of dependance on the milk group for food intake. Food intake for the vegetable group was severely under the recommended allowance, regardless of the age. The intake of iron was less than two thirds of the recommended allowance, except for the group of 12 to 14-month-olds who had been fed with infant formula. The Nutrient Adequacy Ratio(NAR) of major nutrients(protein, calcium, phosphorous, iron, vitamin A, vitamin B$_1$, vitamin B$_2$, niacin, vitamin C) and the Mean Adequacy Ratio(MAR, shows the general quality of dietary intake) was 0.86$\pm$0.12, but the NAR of iron was 0.62$\pm$0.27, which was extremely low compared to those of other nutrients. Therefore, concerning the subjects of this research, the MAR value alone does not provide enough ground to conclude that the subjects dietary quality is satisfactory. 2001년도 국민건강영양조사 자료를 이용한 고혈압 유무에 따른 식생활 비교 및 평가 (Comparative Analysis and Evaluation of Dietary Intake between with and without Hypertension Using 2001 Korea National Health and Nutrition Examination Survey (KNHANES))

  • The purpose of this study was to analyze and evaluate dietary intake between with and without hypertension. Study subjects were more than 30 years old adults (n = 3,806) who participated in the 2001 Korea National Health and Nutrition Examination Survey. People who have 'self-recognition about hypertension' and 'having diet for hypertension' were excluded. Using the WHO standard, subjects were divided into the hypertensive group (SBP> 140, DBP> 90) and the normal group (SBP< 140, DBP< 90). The Body Mass Index (BMI) and the Waist-Hip Ratio (WHR) of a hypertensive group were higher than those of a normal group (p < 0.01). The distribution of the subjects for smoking, alcohol consumption, exercise, stress, preference of salty food were not significantly different between a normal group and a hypertensive group (p>0.05). Dietary intakes were investigated by the 24-hour recall method. When food and dish intakes analyzed by sociodemographic factors, normal group consumed more fruits than those of a hypertensive group. Statistical significant were shown at female group, residences in metropolitan area and having elementary school education (p<0.05). Hypertensive group consumed more alcoholic beverages than those of a normal group at 'age 39-39', 'aged 50-64', 'high economic status', 'low economic status' and 'residences in metropolitan' (p<0.05) The amount of intakes for fat, potassium, thiamin, vitamin C and alcohol were significantly different between the normal group and the hypertensive group (p<0.05). The highest score of the Dietary diversity score (DDS) was 4 in both normal group and hypertensive group. Normal group showed high Nutrition Density (ND) of vitamin C and hypertensive group showed high ND of sodium. In summary, these results showed that significant difference for people with hypertension were intakes of fruits, alcohol, thiamin, vitamin C. And these results differed by sociodemographic groups. Therefore, the differential approach in each group is demanded for prevention and control of the hypertension.

한국인 성인 남녀에서 허리 둘레 기준 복부비만에 따른 영양섭취상태 평가: 2010-2012 국민건강영양조사 자료를 이용하여 (Evaluation of nutrient and food intake status, and dietary quality according to abdominal obesity based on waist circumference in Korean adults: Based on 2010-2012 Korean National Health and Nutrition Examination Survey)

  • 본 연구는 국민건강영양조사 데이터를 활용하여 우리나라 성인에서 성별 허리둘레를 기준으로 한 복부비만 여부에 따른 혈액지표, 생활습관, 식사섭취 상태 및 식사의 질을 분석한 결과, 남성과 여성 모두에서 허리둘레 기준 복부비만군의 경우 정상군에 비해 대사적 지표 (혈중 콜레스테롤, 중성지방, 혈당 및 혈압)가 모두 유의적으로 높게 나타났으며, 여성 허리둘레 기준 복부비만군에서 비타민 $B_2$, 칼슘, 인 및 칼륨 등 미량영양소 섭취의 질이 정상군에 비해 유의적으로 낮았고, 식품 섭취의 다양성을 의미하는 식품군 점수 및 총 식품점수도 비만군이 정상군에 비해 유의적으로 낮게 나타났다. 또한 여성의 경우 식품 섭취의 다양성 (총 식품점수)이 증가할수록 허리둘레 기준 복부비만 위험도가 유의적으로 낮게 나타난 의미 있는 결과를 도출하였다. 이와 같은 결과는 비만의 관리 및 예방을 위해 영양소의 과부족이 없고 미량영양소의 적절한 공급을 가능하게 하는 다양한 식품의 섭취를 권장할 수 있는 근거자료 및 영양교육의 기초자료로 활용할 수 있을 것으로 사료된다.

  • This study was conducted to identify the characteristics of Korean diet in the view of food and food group intakes. The study subjects consisted of preschool children(PC), elementary school children(EC), high school students(HS), college students(CS), and adults over 30 years old living Seoul and the surrounding areas. The subjects over 30 years old were classified to 30-49 years, 50-69 years, and over 70 years. A dietary survey was conducted using 24-hour recall method and data were collected from 2392 subjects. Rice was a major contributor to many nutrients intake because of the amount of consumption. Vegetable oil made great contribution to fat intakes and eggs, squid, and anchovy supplied much cholesterol compared to other foods in all the age groups. Milk was major contributor of fat and cholesterol as well as calcium in younger age group, and seasoning gave much contribution to iron intakes. Mean dietary variety score(DVS) was 32 for EC group and 17 for subjects over 70 years, and tended to decrease along the increasing age. EC group consumed most variable number of foods of all groups but they consumed many junk foods, therefore the MAR value was low despite their high DVS value. Mean dietary diversity score(DDS) of each age group from PC group to subjects over 70 years was 4.34, 4.28, 3.79, 3.89, 3.76, 3.56, and 3.25, respectively. Food group, which was consumed below the minimum amount, was mainly fruits in all the age groups, and many subjects in HS group and over 30 years consumed dairy products below the minimum amount. From these results, age specific nutritional problems were found. PC group and subjects over 50 years seemed to need more diverse food intake than now, EC group did nutrient dense food, and HS group and subjects over 30 years did dairy products. Also, subjects took various nutrients from rice and seasoning contributed to some nutrients intakes. (Korean J Nutrition 34(5) : 568∼579, 2001)

2015-2017년 국민건강영양조사 자료를 활용하여 영양소 섭취와 식이다양성이 중년 이후 성인과 노인의 골다공증에 미치는 영향 (Effect of nutrient intake and dietary diversity score on osteoporosis of middle-aged adults and elderly based on 2015-2017 Korean National Health and Nutrition Examination Survey data)

  • 본 연구는 제 6기 3년차 (2015년), 제 7기 1, 2년차 (2016년, 2017년) 국민건강영양조사 자료를 활용하여 50세 이후 연령층을 대상으로 사회경제적 수준, 음주, 흡연 및 신체활동 및 BMI 중 골다공증 발생에 영향을 미치는 변인을 통제변인으로 적용한 후 영양소 섭취 수준과 DDS가 골다공증 발생에 미치는 영향을 파악하고자 하였다. 골다공증으로 의사 진단을 받은 경험이 있는 경우 골다공증 환자군, 골다공증 의사 진단을 받은 경험이 없는 경우 정상군으로 분류하였으며, 정상군으로 분류된 대상자 중 환자군으로 분류한 대상자와 성, 연령 및 사회경제적 수준에 매칭되는 경우만을 자료 분석에 활용하였다. 본 연구결과 연령에 따라 골다공증 발생의 위험도는 75세 이상 연령군에 비해 50-64세 연령군에서 2.38배 정도 낮았으며, 모든 연령군에서 여자의 골다공증 발생의 위험이 8.85배 이상 높았다. 골다공증 발생에 영향을 미치는 통제변인으로 50-64세 연령군의 경우 성, 연령, 가구내 소득수준, 교육수준이었고, 65-74세 연령군의 경우 성, 교육수준, 음주, BMI이었으며, 75세 이상 연령군의 경우 성, 연령, 현재 흡연, 음주, BMI, 에너지 섭취량이었다. 모든 연령군에서 탄수화물, 단백질 (동물성, 식물성), 지방, 식이섬유, 칼슘, 인, 나트륨, 칼륨, 비타민 B1, 비타민 B2, 니아신, 콜레스테롤 섭취량 증가는 골다공증 발생의 위험을 낮추었으나 연령군별 통제변인을 적용할 경우 완전통제 되어 영양소 섭취량은 골다공증 발생에 영향을 미치지 않았다. 단백질, 인, 철, 비타민 B1을 EAR 이상 섭취 시 모든 연령군에서 골다공증 발생의 위험을 19% 이상, 비타민 B2와 칼슘을 EAR 이상 섭취 시 65세 이상 연령군에서 골다공증 발생의 위험을 31% 이상 예방하는 효과가 있었으나 통제변인 적용 시 65-74세 연령군에서 비타민 B2만이 부분통제 되었고, 이외 영양소는 모든 연령군에서 완전통제 되었다. DDS 수준은 50-64세, 75세 이상 연령군에서 2점 이하의 점수를 보일 경우 5점을 보인 대상자에 비해 골다공증 발생의 위험이 각각 1.765배, 3.595배 높았으나, 각 연령군별 통제변인 적용 후 50-64세 연령군에서는 완전 통제되었고, 75세 이상 연령군에서는 부분통제 되었다. 본 연구결과 50세 이상 연령군에서 통제변인 적용 시 영양소 섭취량은 골다공증 발생의 위험에 완전통제되었으나, 75세 이상의 노인 연령군에서 DDS는 통제변인에 의해 부분통제되는 것으로 나타나 노인 연령층에서 DDS가 2점 이하의 낮은 점수를 보일 경우 골다공증 발생의 위험을 증가시겼다. 즉, 노인기 골다공증 예방 및 뼈 건강을 위한 식이치짐으로 곡류군, 육류·어류·콩류군, 채소군, 과일군, 우유 및 유제품군을 포함한 다양한 식품의 균형된 섭취를 권장할 필요가 있으며, 본 연구에서 선정된 통제변인은 추후 50세 이상 연령군을 대상으로 골다공증 관련 연구 수행 시 보정변인으로 활용될 수 있을 것으로 사료된다.
