관상동맥질환자에게 제공한 의료인의 식이 및 운동권고, 운동자기효능감이 규칙적인 운동이행에 미치는 영향 (Effect of Diet and Exercise Recommendations Provided by Medical Staff to Patients, and Exercise Self Efficacy with Coronary Artery Disease on Adherence to Regular Exercise)

  • 허혈성 심장질환은 재발의 위험이 높고, 재발 시 사망률이 더욱 증가하므로, 재발 예방을 위해서 운동과 식이요법 등 건강행위의 실천이 중요하다. 관상동맥질환자의 규칙적인 운동에 대한 동기를 유발하기 위해서는 의료인의 건강행위 권고가 필요하다고 알려져 있으나, 의료인의 권고가 대상자들의 운동이행에 어떠한 영향을 미치는 지에 대한 연구는 드물다. 이에 의료인의 식이 및 운동 권고가 규칙적인 운동이행에 미치는 영향을 파악하고자, 2018년 7월 4일부터 2019년 7월 30일까지 서울 소재 2개 대학병원 순환기내과에 관상동맥조영술을 위해 입원한 환자 219명을 대상으로 서술적 조사 연구를 실시하였다. 연구 결과, 의료인으로부터 운동이행 권고를 받은 경우(OR 3.52, p=.036), 식이이행 권고를 받은 경우(OR 6.48, p=.022), 운동자기효능감이 높은 경우(OR 1.36, p=.001), 경제 상태가 좋은 경우(OR 7.59, p=.007), 주관적인 건강 상태가 좋은 경우(OR 0.31, p=.047), 규칙적인 운동이행에 긍정적인 영향을 주었다. 따라서 의료인은 규칙적인 운동과 건강한 식이에 대해 환자 맞춤형 권고를 강화할 필요가 있고, 운동자기효능감 증진 프로그램을 개발하여 참여를 유도할 필요가 있다. 향후 의료인 권고의 정도나 방법에 따라 관상동맥질환자들의 자기 관리 실천에 미치는 영향을 파악하는 연구를 제언한다.

통보리 분쇄 사료 급여가 비육후기 한우 채식행동에 미치는 영향 (Effects of Whole Grain Barley Cracked Feed on the Eating Behavior of Hanwoo Steers During Finish Fattening Period)

  • 통보리 분쇄 사료를 급여 수준별로 5개 처리구(C:무처리구, T1: 10% 첨가, T2: 20%, T3: 30%, T4: 40% 첨가)를 두고, 비육후기 5개월 동안(각 처리 구별 6두, 총 30두 공시) 처리구별로 각각 급여한 후 48시간 육안 관찰 한 그 결과를 요약하면 다음과 같다. 총 채식량(농후사료+조사료)은 T4 (9.96kg) > T1 (9.42kg) > C (9.22kg) > T3 (8.91kg) > T2 (8.51kg) 순으로 높게 나타났다(P<0.05). 채식시간은 T4 > T3 > C > T1 > T2구 순이였으며 (P<0.01), 반추시간은 T4 > T1 > C > T3 > T2구 순으로 나타났다 (P<0.05). 그러나 휴식시간은 반추시간과 반대로 T2 > T1 > C > T3 > T4 순으로 나타났다(P<0.01). 채식시간과 반추시간을 합한 저작 시간은 T4 (392.5 min.) > T3 (331.4 min.) > C (329.4 min) > T1 (327.2 min) > T2 (276.3 min.) 순으로 나타났다(P<0.01). 식괴수, 총 저작수, 식괴당 저작수, 사료가치지수는 각각 190.9개, 9,129.4회, 47.8회, 39.4 (분/kg)으로 T4구가 가장 높게 나타났다(P<0.01). 식괴당 반추시간은 C구가 가장 높고 T3구가 가장 낮게 나타났으며(P<0.05), 분당 식괴수는 T3 가장 높고 C구가 가장 낮게 나타났다(P<0.05). 채식율과 저작효율은 T2구에서 가장 높았지만(P<0.01, 0.05), 반추효율에서는 T1구가 가장 높았다. 그리고 군 행동에서는 휴식(78.5%) > 반추(11.4%) > 채식(10.1%)순으로, 군 행동 중 서서 휴식은 T3 > C > T2 >T1 >T4 구 순으로 높게 나타났다.

버섯잔사 첨가급여가 육성기 한우 채식행동에 미치는 영향 (Effects of Spent Mushroom Substrates Addition on Eating Behavior of Growing Hanwoo)

  • 본 실험은 NDF 및 ADF 함량이 높은 버섯폐배지(버섯잔사)에 대한 볏짚 대체 가능성을 채식 행동 관점에서 검토할 목적으로 실시하였으며, 시험사료인 농후사료+볏짚(Control: 5.32kg+자유채식), 농후사료+버섯폐배지+볏짚(T1 : 5.32kg+0.82kg+자유채식) 및 농후사료+버섯폐배지+ 볏짚 (T2 : 5.32 kg+1.64 kg+자유채식)를 급여하여 채식 행동 조사를 하였다. 공시가축은 평균체중 357.0+2.9kg인 한우 18두를 이용하였으며, 사료급여에 있어서는 농후사료와 버섯페배지는 1일 2회로 반량씩 급여하였으며 볏짚은 자유채식토록 하였다. 볏짚 채식량은 Control구가 2.66kg으로서 가장 높게, T2구가 2.03 kg으로 가장 낮게 나타난 반면, 총채식량 및 NDF 채식량은 T2 > T1 > Control구 순으로 높게 나타났다. 채식시간은 Control구가 289.3분, T2구가 290.5로 비슷했고 T1구가 236.0분으로 가장 적었다(p<0.05). 반추시간에 있어서는 버섯폐배지 첨가량을 0%(0.0kg), 15%(0.82kg) 및 30%(1.64kg)로 증가 시켜줌에 따라 반추시간은 451.0분에서 359.5분으로 직선적으로 감소하였다(p<0.05). 이는 버섯폐배지의 사료입자 크기가 매우 작았던 것에 기인된 것으로 생각된다. 1일 저작시간, 식괴수, 저작회수와 식괴당 저작수 및 사료가치지수는 볏짚을 급여한 대조구에서 모두 높게 나타났다(p<0.05). 그러나 식괴당 반추시간은 T2(69.4분)에서, 분 당 식괴수는 T1구(1.07번)에서 높게 나타났다(p<0.05). 채식율, 반추효율 및 저작효율성에 있어서는 버섯폐배지 첨가 급여구(T1, T2)가 무첨가 급여구(Control)에 비하여 높게 나타났으며, 이는 높은 채식량과 버섯폐배지의 작은 입자도가 관여한 것으로 생각된다. 군 행동은 모든 구(Control, T1 및 T2)에서 농후사료 급여시에 통일된 행동을 보였고, 반추 및 휴식에 있어서 군 행동은 처리 간 큰 차이가 없는 것으로 나타났다. 이상의 결과 육성우 배합사료에 버섯폐배지 15% 및 30%를 첨가 급여함으로서 반추시간, 식괴수, 저작수 및 FVI치는 다소 낮은 결과치를 보였으나, 채식행동에 큰 문제가 될 정도는 아니며, 오히려 채식율, 반추효율 및 저작효율에 있어서 무첨가구에 비하여 첨가구가 높았던 점을 감안 할 때 한우 육성우 조사료원으로서 대체 가능성을 기대할 수 있다.

청소년의 가공식품 섭취실태 및 구매행동에 관한 연구 (A Study on Intake and Purchasing Behavior of Processed Food among Adolescents)

  • 본 연구에서는 자주적인 소비 주체인 청소년을 대상으로 가공식품 섭취실태와 구매행동대한 인식을 분석함으로써 청소년을 위한 식생활 교육프로그램 개발 연구의 기초자료를 제공하고 학교현장의 가정과 교사들이 효과적인 식생활 교육을 할 수 있도록 조력하는 것에 그 목적을 두었다. 가공식품 선택 시 고려사항으로 맛이 있는 것과 가격이 싼 것 순이었으며, 표시정보 확인 시 중요하게 보는 것은 유통기간과 가격 순으로 높게 나타났다. 가공식품 구입 시 식품첨가물 표시정보는 56%가 확인하지 않으며, 절반 이상의 학생들이 식품첨가물이 적게 든 것을 구입하려는 노력은 하지 않는 것으로 나타났다. 조사대상자의 일반적인 특성에 따른 차이를 분석한 결과, 학교에서 배운 영양지식 활용정도는 여학생일수록, 학년이 낮을수록, 가공식품 구입비용(1일)이 낮을수록 학교에서 배운 영양지식을 더 잘 활용하는 것으로 나타났다. 청소년들은 합리적인 가공식품 구매행동이 아닌 단순하고 본능적인 구매행동을 하고 있어 가공식품 섭취와 관련된 청소년 식행동에 대해 기초적인 영양 교육부터 선행되어야 할 필요가 있다. 또한 실생활에서 활용 가능한 가공식품 관련 영양교육이 더욱 체계적이고 효율적으로 진행되어야 하겠다.

경기 일부지역 초등학생의 채소와 과일 섭취 및 관련 인식, 자아효능감, 영양지식과 식행동 (Intakes and Beliefs of Vegetables and Fruits, Self-Efficacy, Nutrition Knowledge, Eating Behavior of Elementary School Students in Kyunggi Area)

  • The purpose of this study was to examine vegetable & fruit (V/F) intakes, beliefs and self-efficacy regarding V/F consumption, nutrition knowledge and eating behavior of elementary students. A survey was conducted to the 4th graders (n = 234) at two elementary schools in Guri, Kyunggi-do. About one-fourth of subjects were overweight or obese. Subjects had 4.2 servings of V/F a day, consuming 340.2 g of V/F. Girls consumed significantly more amounts of vegetables than boys (p < 0.05). Girls were more favorable regarding V/F consumption (p < 0.01), and believed more strongly on advantages of having V/F such as “good for skin” constipation prevention (p < 0.001) and cancer prevention (p < 0.05). Boys felt more strongly in disadvantages or barriers of eating V/F, including mother's cooking time constraints (p < 0.01), lack of past experience of eating V/F, and family members' disliking of V/F (p < 0.05). Girls felt more confident in eating V/F (p < 0.05) than boys; they also felt more confident in specific items of "eating fruits/salads instead of cookies/chips for snack" and "eating fruit juice/vegetable juice instead of soda"(p < 0.01). Subjects showed low level of nutrition knowledge, especially in items such as balanced meals, recommended servings of V/F and vitamin deficiency. Compared to boys, girls had more desirable eating behavior such as eating adequate amount of meals (p < 0.001), having a variety of foods, eating fruits daily, and having fatty foods less frequently (p < 0.05). Intakes of Ca and K were quite below the recommended level, while the intakes of protein, Na and vitamin A intake were much above the Dietary Reference Intakes (DRI) for 9-11 old children. Nutrient intakes expressed as %DRI was higher in girls for vitamin A (p < 0.01), energy and riboflavin (p < 0.05). High V/F consumption group (${\geq}5$ servings of V/F a day) compared to the counterparts showed higher self-efficacy and had better eating behaviors. Nutrition education for children should focus on increasing consumption of V/F, by helping them to increase self-efficacy for eating V/F and to recognize the benefits and reduce the barriers of eating V/F, especially in boys. It is also needed to provide nutrition information for balanced meals or increasing V/F consumption, and help the children to adopt desirable eating behavior.

남녀 대학생의 영양지식과 다이어트 행동에 관한 연구 (A Study of the Nutritional Knowledge and Diet Practice Behavior of College Students)

  • The principal objective of this study was to determine the effects of diet knowledge and perception on diet practice behavior among university students. Data were collected between March and April of 2005 from 523 male and female university students in Seoul and Chungbuk, via a self-administered questionnaire. The subjects included 217 male students (41.5%) and 306 female students (58.5%), and 52.0% of the subjects were 21-23 years old. With regard to residence type, 42.6% of the subjects lived in the dormitory, home (29.1%) self-boarding (19.5%) and meal (8.8) were in the order. The average weights and heights of the subjects were as follows: 70.9$\pm$9.0 kg and 174.3$\pm$4.4 cm in the male students and 53.9$\pm$5.9 kg and 161.3$\pm$4.5 cm in the female students. The average BMI values of the male and female students were 23.4$\pm$2.5 and 20.8$\pm$2.1, respectively. The average nutritional knowledge scores were as follows: 1.96 out of 3.0, dietary habit (2.13), nutrition (2.0), exercise (1.98), preferred food (1.90), and chronic disease (1.79). We noted no significant differences between the male and female subjects in terms of scored points. Nutritional knowledge by gender and residence type were shown to be significantly different--the male subjects had higher scores for exercise than did the female subjects. The self-boarding subjects had significantly lower scores with regard to nutritional knowledge and chronic disease. The experience and duration of previous diet practice were also significantly different with regard to gender and residence type. The male subjects tended to conduct diet practice for longer periods than females. The self-boarding students reported more attempts at diet behavior, but for shorter durations. The results show that the nutritional knowledge and diet practice behavior of the subjects were influenced by gender and residence type.

S대학 치위생과 실습실에 내원한 환자의 치아우식상태와 구강위생관리능력, 구강건강행위 실태 (A Study on the Oral Health Status, PHP Index and Oral Health Behavior of Patients in 'S' College Dental Clinic)

  • 본 연구는 구강건강행위와 구강건강상태, 구강위생관리능력과의 상호관계를 분석하여, 치과위생관리를 위한 기초자료를 제공하고자 2007년 9월 5일부터 12월 1일까지 S대학 치위생과 구강위생실습실을 내원한 스켈링 실습환자를 대상으로 실시하였으며, SPSS WIN 12.0 프로그램을 이용하여 분석하였다. 1. 연령이 낮을수록 우식경험처치치아의 수(FT index)가 높은 것으로 조사되었으며, 구강위생용품 영역, 정기적인 치과방문 영역, 식이조절 영역에서는 연령이 증가할수록 구강건강행위가 높은 것으로 조사되었다. 2. 대상자의 구강건강행위 정도는 5점 만점에서 2.62로 대체적으로 낮은 수준으로 조사되었으며, 잇솔질 영역에서는 '위아래로 치아를 닦고 잇몸과 혀도 닦는다', 구강위생용품 영역에서는 '구강양치액(가그린)을 사용 한다', 정기적인 치과방문 영역에서는 '아픈 경우 미루지 않고 치과에 방문 한다', 식이조절 영역에서는 '우식성식품 보다 청정식품을 섭취 한다', 교육 및 관심 영역에서는 '흡연이 구강건강에 좋지 않음을 교육받는 적이 있다'가 가장 높게 조사되었다. 3. 구강건강행위가 높을수록 우식경험미처치치아(DT index)의 수가 낮아지는 것으로 조사되었으며, 구강건강행위가 높을수록 우식경험처치치아의 수는 높은 것으로 조사되었다. 치면세마 실습실에 내원한 환자의 구강위생관리능력 수준을 분석한 결과 '양호' 25%, '보통' 36.3, 불량 30.4, 매우불량 8.3%로 조사되어 대부분의 환자들의 구강위생관리 능력이 불량한 것으로 조사되었으며, 구강건강행위가 높을수록 구강위생관리능력 지수는 낮아지는 것으로 조사되었다. 4. 구강건강행위와 우식경험미처치치아(DT index) 간에 음(-)의 상호관련성이 있는 것으로 분석되어, 구강건강행위가 높을수록 우식경험미처치치아(DT index)의 수는 낮아지는 것으로 조사되었다(p < .01). 구강건강행위와 우식경험처치치아(FT index) 간에 양(+)의 상호관련성이 있는 것으로 분석되어, 구강건강행위가 높을수록 우식경험처치치아(FT index) 수는 높아지는 것으로 조사되었다(p<.01). 구강건강행위와 구강위생관리능력 지수(PHP index) 간에 음(-)의 상호관련성을 보이고 있어 구강건강행위가 높을수록 구강위생관리능력 지수(PHP index)는 낮아지는 것으로 조사되었다(p < .01).

무용전공 여고생에서 섭식장애 인지와 비만도에 따른 체중조절 태도 및 식이 자기효능감, 섭식행동 (The Weight Control-Related Attitudes, Dietary Self-Efficacy and Eating Behaviors according to the Perception of Eating Disorder and Obesity Index in Dancing Major High School Girls)

  • This study was performed in order to compare the differences in dietary self-efficacy and eating behaviors as well as the relationship of weight control attitudes according to eating disorder perception and obesity index for high school girls majoring in dance. Almost half (45.5%) of the subjects perceived eating disorders, and the rate of underweight group was 45.7%. Forty-four percent of the group that perceived eating disorders were in modern dance, while 55.0% of the group that did not perceived eating disorders were in Korean dance (p < 0.05). For the obesity index, a similar rate was shown between the different types of dance for the underweight group, but in the normal group, 54.5% were in Korean dance (p < 0.05). Most (82.0%) of the group that perceived eating disorders had fears on obesity, while only 53.3% of the non-perceiving group had fears on obesity (p < 0.01). Over half (58.3%) of the underweight group had fears on obesity, while 77.2% of the normal group showed fear in that (p < 0.05). For necessity of weight loss to do dance performances, 25.0% of the underweight group and 57.9% of the normal group need that (p < 0.05). Most of the group that perceived eating disorders (92.0%) answered that they had experience in very low calorie diets, while only 40.0% of the non-perceiving group had experience (p < 0.001). For necessity of calorie education, 42.0% of the group that perceived eating disorders and only 20.0% of the non-perceiving group answered is needed (p < 0.05). For eating behavior, the group that perceived eating disorders had lower total scores compared to the non-perceiving group (p < 0.001), The group that perceived eating disorders showed lower scores for restraint eating (p < 0.01) and emotional factors (p < 0.001) except by external eating. This study showed that high school girls majoring in dance have high interest in weight control and problems with wrongful body images and eating behaviors, as well as need for calorie education were suggested. ADL에 의한 도움필요 노인의 건강과 식생활관리 -경기지역 농촌노인을 중심으로- (The Health and Dietary management of Impaired elderly by ADL in Gyeonggi, Korea)

  • The purpose of this study is to provide information searching for health promotion, nutrition improvement, and health care of the impaired elderly by ADL(Activity of Daily Living) and IADL(Instrumental Activity of Daily Living). The subjects were divided into the Assistant Needed Group and No Assistant Needed Group for living in line with the responses of ADL(10 items like dressing, washing, move etc.) and IADL(10 items like housekeeping, using transportation, shopping, Phone call etc.). Survey was made for health behavior, health risk habit, dietary management status and diet intake by 24 hr-recall and questionnaire method. 242 subjects were collected in 12 cities or Gun districts in Gyeonggi Province, S. Korea. Survey was carried out by regional home extension workers using interview method. Statistical analyses were made using SAS (Version 8.1). Chi-Square Tests and General Linear Models. The subjects of impaired ADL elderly was 26.5% and it composed 30% of the total male and 22.2% of the total female. The demographic status of the impaired ADL elderly showed no difference from that of the normal elders, elementary school educated (73.4%), with spouse (43.8%) or with adult children(37.5%), using monthly living cost of 500-1,000 thousand won(35.9%). Mean age was 74.05 years compared to 72.25 years of normal elders. However, there was no significant difference from the normal and impaired ADL group, regular exercise(60.0%), with walking (90.0%), no-smoke(54.7%) and no-drink(48.4%). Kind of disease was not different from the one in impaired and normal group, with cardiovascular disease(32.3%), with diabetes mellitus(8.1%), joint lumbago neuralgia(32.3%) and osteoporosis(9.7%). Gastrointestinal complaints of the impaired ADL group were nausea(57.8%), chronic indigestion (23.4%), constipation (14,0%) and vomiting(3.7%). Sleeping time required for the impaired was longer than that for the normal group by 10hours(4.7%) or 8-10hours(20.3%), which consisted 1.7% and 16.6% respectively. Nutrient intake of the impaired ADL group was low compared to normal range elders: Energy(1260kca1), Protein(52.75g). There was gender difference in nutrient intake; the male impaired group showed no significant difference from the normal group but it was significantly lower in female impaired group. These results suggest that low quality of life and low economic status of the impaired ADL elderly require congregate meal in village hall to cover the lack of side dishe variety. And nutrition education program including community assistance would be required for the impaired ADL elderly together with the sufficient food and exercise practice. By operating nutrition education program, the impaired ADL elderly would maintain more enhanced quality of life and ameliorate the ADL capability.

Nutritional aspects of night eating and its association with weight status among Korean adolescents

  • BACKGROUND/OBJECTIVES: A growing body of research has indicated that night eating could be associated with poor diet quality and negative health outcomes. This study examined the nutritional aspects of night eating, its related factors, and the association between night eating and body weight among Korean adolescents. SUBJECTS/METHODS: This study analysed the data from a one day 24-hour dietary recall as well as a demographic survey of 1,738 Korean adolescents aged 12 to 18-years-old obtained from the 2010-2012 Korea National Health and Nutrition Examination Survey. 'Night eating' was defined as consuming 25% or more of one's daily energy intake between 21:00 and 06:00. Subjects complying with the preceding condition were classified as 'night eaters', whereas the rest were considered 'non-night eaters'. Logistic regression analysis examined factors related to night eating. Multiple linear regression analyses were used to examine the relationship between night eating and BMI z-scores, whereas multinomial logistic regression analysis was used to examine the relationship between night eating and weight status. RESULTS: About 21% of Korean adolescents appeared to be night eaters. Night eaters showed increased breakfast skipping (P = 0.001), higher energy intake from snacks (P < 0.001), greater proportion of energy intake from fat (P = 0.029), and lower Dietary Diversity Scores (P = 0.008) than non-night eaters. Male adolescents presented 1.9 times higher odds of being night eaters than females. Adolescents whose both parents were night eaters were 4.4 times as likely to be night eaters as those whose neither parents were. Female adolescents showed a significant relationship between night eating and BMI z-scores (${\beta}=0.28$, P = 0.004). However, night eating did not increase odds of being overweight or obese in adolescents. CONCLUSIONS: Night eating in Korean adolescents was related to undesirable dietary behaviours and low diet quality in general as well as higher BMI z-scores in females. Male gender and parental night eating appeared to be the factors that significantly increased odds of night eating. These results suggest that night eating should be considered when designing nutrition education or intervention programs targeting adolescents.
