국내에서 판매되는 임산부용 비타민·무기질 보충제에 함유된 영양소의 종류와 함량 및 적절성 평가 (Nutrient Composition and Content of Vitamin and Mineral Supplements and Their Appropriateness for Pregnant and Lactating Women in Korea)

  • Objectives: Use of dietary supplements containing vitamins and minerals is growing in Korean adults, especially in pregnant and lactating women. Vitamin and mineral supplements are available in different composition and in a wide range of contents. The purposes of the study were to examine nutrient composition and content of vitamin and mineral supplements for pregnant and lactating women and assess their appropriateness as dietary supplements. Methods: Information on the name, manufacturer, nutrient composition, and usage of vitamin and mineral supplements for pregnant and lactating women were obtained from the homepage of the Food Safety Information Portal managed by the Ministry of Food and Drug Safety, and Korean Index of Medical Specialties. A total of 264 products were identified. Results: Among 264 products, 26.1% were single nutrient products, and 73.9% were multinutrient products. The most commonly included nutrient was iron (70.1%), folic acid (66.3%), vitamin $B_{12}$ (45.8%), vitamin C (38.6%), and vitamin $B_6$ (38.6%). Although more than 50% of products contained nutrients less than 150% of Recommended Nutrient Intakes or Adequate Intakes for daily use, some products contained inappropriately high amounts of nutrients. When a maximum daily dose of supplements was taken as described on the label, iron in 73 products (39.5%), folic acid in 14 products (8.0%) were likely to be consumed in amounts greater than Tolerable Upper Intake Levels. Most products were assessed as inappropriate for pregnant women due to the possibility of excessive intake of vitamins or minerals when compared with Dietary Reference Intakes. Conclusions: Pregnant and lactating women need to carefully select dietary supplements containing adequate amounts of vitamins and minerals. Nutritionists should provide guidelines regarding selection of appropriate vitamin and mineral supplements for pregnant and lactating women.

우리나라 성인의 비타민 A 섭취현황 : 2007~2012년 국민건강영양조사 자료를 이용하여 (Estimated dietary intake of vitamin A in Korean adults: Based on the Korea National Health and Nutrition Examination Survey 2007~2012)

  • 본 연구에서는 우리나라 19세 이상 성인의 비타민 A 섭취 실태와 그 역학적 특성을 파악하기 위해 제 4 5기 국민건강영양조사 (2007~2012) 자료의 24시간 회상법 자료와 상용식품 중 레티놀 및 카로티노이드 함량 데이터베이스를 연계하여 1일 레티놀, 6종의 카로티노이드 (${\alpha}$-카로틴, ${\beta}$-카로틴, 리코펜, ${\beta}$-크립토잔틴, 루테인/제아잔틴) 및 비타민 A 섭취량을 추정하였다. 그 결과 대상자 33,069명의 평균 비타민 A 섭취량은 레티놀 당량으로 $788.4{\mu}g\;RE/day$였으며, 레티놀 활성당량으로는 $488.9{\mu}g\;RE/day$였다. 대상자의 비타민 A 섭취의 적절성을 평가하기 위해 2010년 한국인 영양섭취기준과 새로 개정된 2015년 한국인 영양소 섭취 기준의 비타민 A 기준과 비교한 결과, 전체 대상자 중 2010년의 RE 기준으로는 42.9%가, 2015년의 RAE 기준으로는 70.6%가 평균필요량 미만으로 섭취하고 있었다. 75세 이상 대상자 중 80% 이상이 새로 개정된 기준에 따르면 평균필요량 미만으로 섭취하고 있었고, 소득 수준이 낮을수록 비타민 A 섭취 부족 위험이 증가하였다. 각 식품군 별 비타민 A의 섭취량을 산출하였을 때에는, 채소류, 조미료류, 해조류, 과일류 순으로 기여율이 높았다. 새로 제정된 영양섭취기준에 따라 우리나라 성인의 비타민 A 섭취량을 평가한 결과, 종전에 비해 매우 부족한 것으로 평가되었으므로, 균형 잡힌 식사를 통한 충분한 양의 비타민 A를 섭취하도록 교육하고 권고할 필요성이 대두된다. 본 연구는 우리나라 성인의 비타민 A 섭취량을 추정하고, 섭취의 적절성을 평가한 연구로서 의의가 있으며, 비타민 A의 섭취 실태를 확인한 본 연구에서 더 나아가 우리나라 국민의 비타민 A 및 레티놀, 카로티노이드의 섭취량과 대사지표 및 부족 증상의 발생 등과의 연관성에 대한 후속 연구가 필요할 것으로 사료된다.

기능성 소화불량증 환자의 영양소 섭취 및 식행동 (Nutrient Intake and Dietary Behaviors of Patients with Functional Dyspepsia)

  • As a disease that reduces quality of life, functional dyspepsia (FD) is associated with foods that may worsen its symptoms or cause it. The purpose of this study was to examine the nutritional status and dietary behaviors of FD patients. We investigated food intake, food intake frequency, and dietary habits of 45 FD subjects according to the Rome III Diagnosis Criteria. Average age and body mass index (BMI) were 47.7 years and $22.6kg/m^2$ (males: $23.4kg/m^2$, females: $22.1kg/m^2$), respectively. Average energy intake was 77% of Korean Dietary Reference Intake (KDRI), and it was less than that of the Korea National Health and Nutrition Examination Surveys (KNHANES). Other nutrient intake levels were similar to the KNHANES. Energy intake proportion of carbohydrates : protein : fat was 56 : 18 : 26, and the ratio of fat intake was higher than that of the KNHANES. Beans, laver, tomato, and yogurt were consumed very frequently. In the results of meal regularity, dietary behaviors and composition of diet were relatively good. It is likely that the patient controlled their diet by eating cautiously and by reducing alcohol drinking and smoking. Our results indicate that FD patients' nutritional status by consumption of nutrients was at a level of normal healthy people and that dietary habits were better than normal adults. However, their fat intake levels were somewhat higher than normal people. Therefore, further research is required to identify the relationship between dietary intakes and FD.

Nutritional Assessment of the Oldest-Old Population Aged 85 Over: Based on 2009 Korea National Health and Nutrition Examination Survey

  • As Korea is expected to become super high aged society in 2026, nutrition is important elements of health in the elderly and affects their life. In epidemiological surveys, anthropometric and biochemical measurements represent important components of nutritional assessment in elderly. In the present study, we investigated anthropometric index, biochemical markers, and nutrient intake for elderly aged over 85 by using data on 71 elderly (24 men, 47 women) over 85 years which was obtained from Korea National Health and Nutrition Examination Survey in 2009. According to BMI values, the prevalence of underweight and obesity was 9.7% and 34.5% in men, and 7.9% and 26.4% in women. The prevalence of sarcopenia was 70.7% in men and 25.0% in women. However, plasma biochemical data are normal range in the older population. In nutrients intake, daily Intake of energy, protein, dietary fiber, water, thiamin, riboflavin, niacin, calcium, and potassium was inadequate considering the Dietary Reference Intakes for Koreans(KDRIs). In conclusion, elderly need to be educated nutrition and their health dietary life to prevent malnutrition, and standard of nutrition intake should be rearranged in elderly aged 85 over considering their antrhopometric index.

코덱스 영양소 기준치 설정시 영양섭취기준의 적용 방안 (Application of Dietary Reference Intakes for Codex Nutrient Reference Values)

  • 현재 코덱스 NRVs 중에는 새로이 각국에서 제정된 영양 섭취기준과 상당히 차이가 있는 영양소들이 있어서 개정을 위한 검토 작업이 시급히 필요하다. DRIs 중 EARs 혹은 RDAs를 코덱스 NRVs 기준으로 설정하는 것에는 각각이 장점과 단점이 있으므로, 각 기준에 대한 장점과 문제점을 충분히 고려하여 선정하여야 한다. 각 나라의 DRIs를 NRVs 기준으로 이용하거나 국제적으로 영양소별로 단일 값을 정한 후 각 나라의 인구 구조를 근거로 NRVs 기준을 설정하는 것은 국가마다 서로 다른 기준을 설정하는 결과를 초래하고 이로 인한 소비자의 혼란 및 국가 간 교역에 혼란을 초래할 가능성이 크므로, 각 나라의 인구 구조를 적용한 값을 NRVs기준으로 설정하는 것은 문제가 있다고 사료된다. 코덱스 NRVs 설정시 인구 구조에 근거한 RDA를 사용할 경우 연령 집단 간 편차가 크면 산출된 값이 lowest UL 보다 높을 수 있으므로, 성인 이외의 연령층 (어린이)에 대해서 바람직한 적용 방안을 후속 연구를 통해 별도로 고려해 볼 필요가 있다. 따라서 기존의 Codex 기준과 같이 국제적으로 새로이 설정된 DRIs를 전면적으로 검토하고 이를 최대한 반영하여 영양소별로 단일 값을 제시하여 각 나라에서 이용하도록 하는 것이 바람직하다고 사료된다.

식품교환법을 활용한 영양교육의 효과: 초등학생의 영양지식, 식생활 태도, 식이섭취 변화 (Effects of Nutrition Education Using Food Exchange System: Changes in Elementary Students' Nutrition Knowledge, Dietary Attitude and Nutrients Intake)

  • The purpose of this study was to investigate the effects of nutrition education using Food Exchange System on elementary students' nutrition knowledge, dietary attitude and nutrients intake. Nutrition education lessons (40 min /lesson, 4 times), '5 major nutrients and functions', '6 food groups and sources of 6 food groups', 'good choice of snacks and eating out' as class lesson, 'daily needed energy and food exchange units' as individual lesson, were provided to 70 elementary students (4th grade 33 students, 5th grade 37 students) in Jeonbuk Province. We assessed the changes in nutrition knowledge, dietary attitude, food habit using a questionnaire and nutrient intake using 24 hr recall method by nutrition education. In nutrition knowledge, there were significant increases in scores of 'functions of carbohydrate', 'functions of protein', 'functions of lipid', 'foods of carbohydrate', and 'foods of lipid'. In dietary attitude, there were significant increases in scores of 'taking a meal with joy', 'taking a meal at ease', 'taking a meal with sufficient protein intake' and 'taking a meal without spicy foods' by nutrition education. There were significant changes in the type of breakfast and in the frequency of snacks. After education, in type of breakfast, it showed higher number of students ate rice oriented meal than they did before education. and in frequency of snacks, it showed lower number of students ate snacks 'over 3 times' than they did before education. In Carbohydrate : Protein : Fat (CPF) ratio (%), it was significantly changed from 55.8 : 17.7 : 26.6 to 63.6 : 15.3 : 21.1. In evaluation of nutrient intake by Dietary Reference Intakes for Koreans (KDRIs), vitamin A, thiamin, niacin, vitamin $B_6$, calcium, phosphate, iron and zinc showed positive changes in distribution of number of children by intake level. That is, there were significant improvements in intakes of vitamin A, thiamin, niacin, vitamin $B_6$, calcium, phosphate, iron and zinc. These results showed that nutrition education using Food Exchange System for elementary students improved nutrition knowledge, dietary attitude and nutrients intake. It suggest that nutrition education using Food Exchange System may improve dietary behaviors and reduce an incidence of obesity in elementary students.

흡연 노인 남성의 흡연기간에 따른 신체적, 생화학적 특성, 영양소섭취 및 골밀도 차이 : 2008~2011년 국민건강영양조사 자료 분석 (Anthropometric, Biochemical Characteristics, Nutrient Intakes and Bone Density by Smoking Period in Elderly Male Smokers: Analysis of Data from Korea National Health and Nutrition Examination Survey (KNHANES), 2008~2011)

  • This study investigated anthropometric and biochemical characteristics, nutrient intakes, and bone density of Korean elderly men (over 65 yrs). Data on bone density and anthropometric (height, weight, waist circumference, body mass index and blood pressure) and biochemical (total cholesterol, high-density lipoprotein (HDL)-cholesterol, low-density lipoprotein (LDL)-cholesterol, triglyceride and hemoglobin) characteristics, nutrient intakes, and nutrient density were obtained from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES, 2008~2011). Subjects were categorized into smoking and non-smoking groups, and smoking groups were divided into three groups by smoking period (under 20 yrs, 21~40 yrs and over 41 yrs). Serum triglyceride concentrations of the smoking group aged 21~40 yrs were higher than those of other groups (P<0.01), whereas other biochemical factors were not different. Intake ratios of energy, protein, phosphorus and sodium in subjects were over 100% of Dietary Reference Intake for Koreans (KDRI). Nutrient densities according to intakes of thiamin, riboflavin, and niacin per 1,000 kcal were significantly different among the groups (P<0.05). Bone density of subjects decreased according to smoking period (T-score of total femur in non-smoking group -0.3108, and -0.2918, -0.4941, -0.6847 in smoking group, respectively). Ratio of osteoporosis was 38.1% in the non-smoking group and 44.4%, 51.1%, and 64.0% in the smoking group, respectively. The findings of the present study show that smoking may be associated with bone health, higher ratio of osteoporosis, and low nutrient density in elderly men. Therefore, practical and systematic non-smoking programs are required to improve the bone density of elderly men as well as maintain healthy bone levels and desirable lifestyle.

Nutrient intakes of infants with atopic dermatitis and relationship with feeding type

  • BACKGROUND/OBJECTIVES: The prevalence of atopic dermatitis in infants is increasing worldwide. However, the nutrient intake status of infants with atopic dermatitis has not been studied properly. This study was conducted to compare the nutrient intake status of infants in the weaning period with atopic dermatitis by feeding type. MATERIALS/METHODS: Feeding types, nutrient intake status and growth status of 98 infants with atopic dermatitis from age 6 to 12 months were investigated. Feeding types were surveyed using questionnaires, and daily intakes were recorded by mothers using the 24-hour recall method. Growth and iron status were also measured. RESULTS: The result showed that breastfed infants consumed less energy and 13 nutrients compared to formula-fed or mixed-fed infants (P < 0.001). The breastfed group showed a significantly lower intake rate to the Dietary Reference Intakes for Koreans than the other two groups (P < 0.001). In addition, they consumed less than 75% of the recommended intakes in all nutrients, except for protein and vitamin A, and in particular, iron intake was very low, showing just 18.7% of the recommended intake. There was no significant difference in growth by feeding type, but breastfed infants showed a significantly higher rate of iron deficiency anemia (P < 0.001). CONCLUSIONS: Continuous management programs should be prepared for breastfed infants with atopic dermatitis, who are in a period when rapid growth takes place and proper nutrient intake is essential.

국민기초생활보장사업 수급 가정에서의 식품공급안정성과 아동의 영양상태 (Food Security and Children′s Nutritional Status of the Households Supported by the National Basic Livelihood Security System)

  • In order to provide basic data for the means to improve food situation and nutritional status of those supported by the National Basic Livelihood Security System (NBLSS), we examined household food insecurity and nutritional status of children under the support of NBLSS. This study included 209 children aged 3-12 years (99 boys and 110 girls) and their caretakers. We measured house food insecurity using Radimer/cornell Scale, children's body sizes and nutrient intake by semi-quantitative food frequency questionnaire, and caretakers' nutritional management skills. Only 9.6% of the households were in food secured (FS) while 8.1% were in household food insecured, but without hunger (HFI), 42.1% were in adult food insecured with hunger (AFI), and 40.2% were in child hungry (CH). Important predictors of food security were nutritional management skills of the caretakers as well as their education, but neither income nor food expenditure of the households. Mean energy intake of the children was 86.0% of the Korean Recommended Dietary Allowance (RDA). Intakes of protein, phosphorous, vitamin A and B$_1$ were relatively high ranging from 112.3% to 124.4% of the RDAs while those of calcium, iron, niacin, vitamin C were low showing 74.8-83.3% of the RDAs. Height, weight and weight/height ratio were close to the reference levels. Lower nutrient intakes of children were observed as the households were more food insecured. However, nutrient intakes and body sizes of children did not differ as a function of household socioeconomic status representing by income, food expenditure and caretakers' education. Results of this study suggest the importance of food security and nutritional management skills for the children's nutrient intakes. Concerning this matter, a need for nutrition education in the program for NBLSS was discussed.

영양균형식단 작성을 위한 식품교환표와 식사구성안 개발 (Development of a Food Exchange Table and Food Pattern for Nutritionally Balanced Menu Planning)

  • Objectives: The purpose of this study was to develop new meal planning tools for a nutritionally balanced diet. Methods: Based on the food exchange list for diabetes, we adjusted the food group classification system to reflect the suggested nutritional factors for chronic disease prevention and health promotion. We developed a nutritionally balanced dietary profile for adults and compared it with the dietary reference intakes for Koreans (KDRIs) and the food pattern recommended by the Korean Diabetes Association. Results: The newly developed menu planning tools are the LOHAS food exchange table and the LOHAS food pattern. Our recommended daily 1,800 kcal dietary composition for adults is as follows: The carbohydrate food group consists of 4 'whole grains', 3 'refined grains', 2 'sugars', 9 'vegetables', 3 'starchy vegetables', 2 'fruits' and 1 'high sugar fruits'. The protein food group includes 3 'plant protein foods', 3 'animal protein foods (low-fat)', and 1 'animal protein foods (high-fat)'. The fat food group consists of 2 'oils and nuts' and 1 'solid fats'. The total number of calories is estimated at 1,840 kcal and the energy ratio is 62% carbohydrate, 18% protein, 20% fat, 6.8% saturated fat and 13.2% sugars. Using the LOHAS food exchange table, it is possible to estimate values of saturated fat, unsaturated fat, dietary fiber, and sugars besides carbohydrate, protein and fat. It is also possible to compose a dietary design considering carbohydrate, sugars, saturated fat and dietary fiber. The LOHAS food pattern provides benefits for the management of both institutional food services and individual meals, as it can help reduce the levels of saturated fat and sugar intake and help develop healthy meals rich in unsaturated fats and dietary fiber. Conclusions: The LOHAS food exchange table and LOHAS food pattern are expected to be practical tools for designing and evaluating nutritionally balanced diets.
