Exercise Prescription and Dietary Modification for Prevention and Treatment of Chronic Degenerative Disease II. On Arteriosclerosis and Hypertension (성인병의 예방과 치료를 위한 영양과 운동처방 II. 고혈압과 동맥경화에 미치는 영향)

  • Regular exercise is effective in preventing coronary disease such as angina pectoris and infarction, inside it can lower the blood pressure and aids in weight control and release of stress. Risk factors of arteriosclerosis is hypertension, hyperlipidemia, diabetes, obesity, physical inactivity and excessive smoking. Arteriosclerosis begins at young age worsens with age, particulary in male. For people with risk factors of arteriosclerosis, it is important to prevent arteriosclerosis-related disease with dietary, living pattern and exercise prescription. Dietary fibers promote exercise of the digestive tract and shortens the time food remains inside the digestive tract. It can prevent obesity, hyperlipidemia, arteriosclerosis and colin cancer by blocking the absorption of cholesterol. Various vegetables and sea foods are lichen unsaturated fats and prevent the absorption of cholesterol inside the digestive tract. Essential fatty acids and unsaturated fats which are contained in vegetable oils, promotes metabolism while preventing absorption. In fruits, pectin water-soluble fiber, is present and lowers the level of cholesterol. By consuming foods that low in cholesterol and saturated fats, and rich in unsaturated fats, aliomentotherapy alone can reduce the plasma cholesterol by 10~l5$\%$. For ideal exercise, it should be aerobic with intensity of 60~80$\%$ HRmax, duration of 15~60min/day. The frequancy of 3~6/week is desirable the better exercise prescription is endurance aerobic exercise. To get more effect exercise, exercise consistency is very important.

The effects of weight loss by a low-calorie diet and a low-calorie plus exercise in overweight undergraduate students (과체중 대학생에서 저열량식이 또는 저열량식이와 운동병합에 의한 체중 감량 효과)

  • Korean undergraduate students attempt to lose weight but often fail due to utilizing ineffective weight loss strategies. Some diet programs have succeeded, yet, they have not provided adequate skills for long-term weight maintenance. The aim of the study was to determine the effect of a low calorie diet and exercise with nutritional education on weight loss, serum lipid profiles, nutrient intakes, and dietary behavior modification in overweight and obese undergraduate students. The subjects in the low calorie diet group (LCD; n = 12) and the low calorie diet plus exercise group (LCDE; n = 13) had the same goal of losing 4 kg during a 12 week program. Nutrient intakes were assessed by the 24 hour recall method. Also, food habits and dietary behaviors were investigated by self-administered questionnaires before and after the weight control program and one month after completing the program. LCD and LCDE groups lowered body weight by 2 kg and 1 kg, respectively, although they decreased calorie intake by 355 and 287 kcal per day compared to intakes prior to the study. Body fat mass decreased in both the LCD and LCDE groups; however, the decrease was greater in the LCDE group. In addition, only the LCDE group increased muscle mass. The LCD group had a slightly better effect in reducing body weight, body fat, and waist circumference than the LCDE group. However, their decrease was reversed after the mid-study check in the LCD group; the reduction was better maintained and decreased more in the LCDE group. However, serum lipid profiles were already in borderline prior to the study; moreover, they were not modified after losing weight. The dietary behavior program helped students to develop better dietary habits. In conclusion, the combination of a low calorie diet and exercise is necessary in order to maintain longer weight loss by increasing muscle mass and decreasing body fat.

Selecting items of a food behavior checklist for the development of Nutrition Quotient (NQ) for children (어린이 영양지수 (NQ, Nutrition Quotient) 개발을 위한 평가항목 선정)

  • The objective of this study is to select a simple and easy measurable food behavior checklist for the development of Nutrition Quotient (NQ) for children, which reflects children's diet quality, as well as to evaluate the validity of the items in the food behavior checklist. The first 36 items in the checklist were established by an expert review, modifying the preliminary 50 items in the checklist, which had been selected by a literature review and the Korean National Health and Nutrition Examination Survey data. 341 children in 5th and 6th grades at an elementary school participated in a one-day dietary record survey, and later responded to 36 food behavior questions of the checklist. Pearson's correlation coefficients between the responses to the food behavior checklist items along with the mean nutrient intakes of the children were calculated. From the result, in which responses of food frequency and food behavior items showed certain association with the dietary record data, a second checklist with 22 items was selected. A survey was conducted by using the second checklist. 1,393 children in the 5th and 6th grades at 12 elementary schools in metropolitan cities, such as Seoul, Busan, Gwangju, Daegu, Daejeon, and Incheon, participated in the survey. Further, an exploratory factor analysis was performed. After the analysis, 19 items (10 items from food frequency and 9 items from food behavior) were finalized as the food behavior checklist items for the NQ. The final 19 food behavior checklist items were composed of 5 factors: 'Balance', 'Diversity', 'Moderation', 'Regularity', and 'Practice'. This study is a significant first trial to establish a comprehensive system for evaluating children's food habit and diet quality. This checklist might need continuous modification and revision reflecting the change of children's dietary life and the social environment.

Development of Nutritional Counseling Internet Program for Weight Reduction Using Expert System (전문가 시스템을 이용한 인터넷 체중 감량 상담 프로그램 개발)

  • The purpose of the study was to develop a nutritional counseling program using expert system to assist obese people to lose weight through behavior modification in the internet. The counseling internet program for weight loss was developed by the accumulation of knowledge for dealing with eating habits and exercising behaviors into expert system tool, Knowledge Engineering Agent (KEA) by a dietitian without any help of computer expert. KEA was built based on the theory of Multiple Classification Ripple Down Rules. To accumulate knowledge into KEA, survey was performed in 150 obese people, the dietitian reviewed and consulted each survey case, and the consulted contents were learned and accumulated into KEA. Survey questionnaires were the same as those of the internet consulting program, and they included general characteristics, dietary habits, lifestyle, and exercise patterns related to obesity. KEA was used for nutritional counseling of obese people after KEA had enough knowledge for weight loss based on behavior modification by the dietitian. To accumulate knowledge to KEA, the dietitian selected proper factors inferred from the survey questionnaire of each case, and added the conclusions for them. Conclusions were made for helping clients to correct bad eating behaviors and accumulate good behaviors for losing weight. When clients answered survey questionnaires in a counseling internet program, KEA gave the recommendation how to eat, to exercise and the deal with stress in a real time for each case. If KEA did not have enough knowledge for a specific case, the conclusion window wrote no conclusion and the dietitian needed to add conclusions for the case. The conclusions for the new case added to the KEA knowledge base. In conclusions, a counseling internet program for weight reduction can be used for give advices how to deal with obesity in a man-to-man way in a real time using KEA where nutritional knowledge based on behavior modification for weight loss was accumulated.

Production of Sikhae Fermented Beverage using a Dextran Producing Isolate from Kimchi and Takju Yeast (김치에서 분리한 Dextran 생성균 및 탁주 효모를 이용한 식혜 발효음료 제조)

  • A fermented alcoholic beverage made from Sikhae, a Korean traditional fermented rice beverage, has been developed using a dextran producing isolate from Kimchi and Takju yeast. When both of yeast and the isolate inoculated for fermented beverage production it produced around 4% (w/v) ethanol and oligosaccharides during fermentation. Inoculation of Takju yeast and the dextran producing isolate showed a similar fermentation profile with case of yeast inoculation only, but the rate of sucrose use was slower than the case of yeast only. TLC analysis showed that oligosaccharides were produced during the fermentation of Takju yeast and the isolate from Kimchi. Therefore, it is possible to produce a functional fermented beverage by modification of dextran fermentation.

  • This study was performed to determine dietary and lifestyle factors associated with hypertension in Korean adolescents. Study subjects were 12~19 years (n = 521) adolescents who participated in the 2005 Korean National Health and Nutrition Examination Survey (KNHANES III). Subjects were divided into the hypertensive group (HG, n = 102) and normotensive group (NG, n = 419) by '2007 Korean children and adolescents growth standard' and the relationships between blood pressure and physical measurement, nutrients intakes, eating behaviors and health related factors were analyzed. HG showed significantly higher levels in weight, waist circumference and BMI than NG. The amount of nutrient intakes was not different between NG and HG. Index of nutritional quality (INQ) for phosphate was higher in HG compared with NG. In both male and female HG, INQ for iron was higher but INQ for vitamin B1 was lower than NG. HG revealed higher consumption frequencies of snack, yoghurt, and ice cream compared with NG. In eating and behavioral factors, 'dinner with family', 'eat proper amount', 'keep Korean traditional diet', alcohol drinking, and mean alcohol intake were significantly different between the two groups. By logistic regression method, risk factors for hypertension revealed in this study were gender (male), age (15~19 years), BMI (${\geq}\;85$ percentile), and not keeping Korean traditional diet. These results suggest that education program for hypertension prevention in adolescents should include eating habits improvement and lifestyle modification as well as weight control.

Changes in Food Companies' Product and Promotion Activities after Restriction of TV Advertising of Energy-Dense and Nutrient-Poor Foods in Korea (고열량.저영양 식품의 TV 광고 제한에 따른 식품회사의 제품 및 촉진 활동 변화)

  • This study determined the effects of television (TV) advertising restriction of Energy-Dense and Nutrient-Poor Foods on product and promotion activities by food companies producing or selling children's favorite foods. A survey using a selfadministered questionnaire was conducted via on-line or fax by marketers or R&D managers from 108 food companies. The data from 55 respondents (50.9%) were analyzed. Restriction exerted positive effects on food products with respect to compliance with labeling requirements (4.0 out of 5 points) and reinforcement of nutritional contents examination of new products (3.7 out of 5 points). Reformulations of products such as reduction of nutrients like sodium, sugar and fat were also reported. In addition, food companies underwent diverse changes in promotional activities, including modification of package designs and displays in stores, offering free gifts, discounts, etc. In conclusion, restriction of TV food advertising may contribute to improvement of children's food environment by encouraging food companies to make favorable product changes. On the other hand, the results also revealed that food companies adopt diverse marketing channels that are not yet under regulation. Hence, to make policies more effective, regulation needs to be extended from TV to other marketing channels to which children are easily exposed.

  • The principal objective of this study was to develop the optimum oligonucleotide primers for the simple detection of Campylobacter in food samples. In order to achieve this goal, a variety of oligonucleotide primers were designed via the modification of 16S rDNA, ceuE and mapA sequences of Campylobacter. Through the subsequent analysis of the specificity and sensitivity of primers, two types of oligonucleotide primers, CB4 and CJ1, were selected for Campylobacter genus-specific and C. jejuni species-specific primers, respectively. The detection limit was found to be $10^0{\sim}10^1$ cells per reaction with the prepared cell suspension, however, the sensitivity in the meat samples was less, at $10^1{\sim}10^2$. We suggested that PCR inhibitors such as hemoglobin or immunoglobulin in pork or beef influenced.

Effect of an abdominal obesity management program on dietary intake, stress index, and waist to hip ratio in abdominally obese women - Focus on comparison of the WHR decrease and WHR increase groups - (복부비만 관리프로그램이 성인 비만여성의 식이섭취, 스트레스지수 및 복부비만율에 미치는 영향 - 복부비만감소군과 복부비만증가군의 비교를 중심으로 -)

  • This study was conducted to evaluate the effects of an abdominal obesity management program on dietary intake, stress index, and waist to hip ratio (WHR) in abdominally obese women. The subjects were 195 adult abdominally obese women (WHR ${\geq}$ 0.80) who had been participating in a nutrition education (total of nine times) and dietary habits and life style modification programs (total of six times) for 12 weeks. The abdominal obesity management program focused on the nutrition provided by breakfast, lunch, and dinner, proper dietary habits, and practices to improve life style. The subjects were divided into a WHR decrease group and a WHR increase group according to changes in the WHR. Daily nutrient intake was assessed with a 3-day food record, body measurements and blood vessel age, stress index, and a health index that were measured at baseline and after 12 weeks. After the intervention, weight, waist circumference, hip circumference, WHR, and body mass index (BMI) decreased significantly in the WHR decrease group. Energy intake increased from 1486.2 kcal to 1541.4 kcal with a significant improvement in nutrient density for animal protein, total fat, animal fat, fiber, calcium, phosphorus, zinc, vitamin $B_6$, vitamin C, vitamin E, and saturated fatty acids in the WHR decrease group. Additionally, dietary diversity increased significantly in the WHR decrease group compared to that in the WHR increase group. The WHR decrease group showed a significant improvement in the stress and health indices. Changes in WHR were correlated with changes in nutrient intake (animal protein, total fat, animal fat, plant fat, fiber, calcium, iron, potassium, vitamin A, vitamin $B_2$, vitamin $B_6$, vitamin C, and folate) and medical index profiles (stress and indices) adjusted for age, birth status, baseline BMI, and baseline WHR. These results show that an abdominal obesity management program was effective not only for reducing the WHR but also to improve dietary intake and the stress index in abdominally obese women.

Treatment of Obese Women with Low Calorie Diet, Aerobic Exercise and Behavior Modificaiton (저열량균형식, 운동 및 행동수정에 의한 비만여성의 치료에 관한 연구)

  • A multidisciplinary weight control program was conducted for obese women. The major components of the program included low calorie diet therapy, exercise, behavior modification and nutritional education and counseling. Sixteen healthy volunteers in excess of body fat, above 30%, were enrolled in the group support program. But 5 person were dropped out in the 2nd week of treatment. During the 1st week of group orientation, individual cause of obesity was assessed through a computer program including survey of dietary intake, activity, eating habits and life styles. During the 5 weeks of treatment, 4.8kg of average weight loss was accomplished using a following program ; low calorie diet(1200kcal/day with all essential nutrients), low impact aerobic exercise(50~60% of $VO_{2max}$, 1 hour/day in a group, 3~5 days/week), behavior modification of individual life styles and eating habits causing obesity and nutritional education concerning nutrition, role of exercise such as brisk walking, importance of slow eating in regular meal pattern and internal motivation for weight reduction, health risk of obesity and rapid weight loss, weight recycling and yo-yo syndrome, etc. Nutritional conseling was conducted 3 times per week with checking self-records of foods, activity, emotional state and tiredness. Before and immediately after 5 weeks of treatment, blood pressure, fasting blood glucose, cholesterol and triglyceride were measured and comparied with paired t-test. After 5 weeks of treatment, body weight, body mass index, body fat and circumferences of waist, upper arm and hip were significantly decreased. Also LDL-cholesterol was significantly decreased after obesity treatment.
