The Effect of the Health Belief and Efficacy Expectation Promoting Program on Osteoporosis Preventive Health Behavior in Women with Rheumatoid Arthritis (건강신념 및 효능기대증진 프로그램이 류마티스 관절염환자의 골다공증 예방행위에 미치는 영향)

  • Osteoporosis has been known as a common complication of rheumatoid arthritis and a major preventable health problem. Lots of studios have demonstrated that changes in life style can help delay or prevent osteoporosis. Therefore nursing intervention related osteoporosis prevention have consisted of education programs aimed at changing dietary and exercise habit. However knowledge gained from education haven't always leaded to behavior change. Therefore it is important to consider other psychological variables in effecting behavior change. Numerous research have found self efficacy and health belief to be an important factor in individual decision making behavior. The purpose of the study was to develop health belief and efficacy expectation promoting program based on Health Belief Model & Self Efficacy Model and to investigate its effects in women with rheumatoid arthritis. For this purpose, one group pretest-post design was used. The subject of the study were 16 women with rheumatoid arthritis in Pusan city and data collection was carried out from April, 1997 to May, 1998. The intervention program was consisted of educating on osteoporosis and enhancing and reinforcing self efficacy by verbal persuasion during the period of 4 weeks. The instruments were used to collect data in this study were Osteoporosis Health Belief Scale, Osteoporosis Self Efficacy Scale, and Osteoporosis Preventive Behavior Scale. Data was analyzed by Wilcoxon signed rank test using SPSS $PC^+$ program. The results are as follows : 1) The behavior should be increased after intervention was supported(Z=-3.5162, p=.0004, diet : Z=-3.2942, p=.0010, exercise). 2) The sub-hypothesis that perceived sensitivity should be increased after intervention was supported (Z=-2.3854, p=.0171). 3) The sub-hypothesis that perceived severity should be increased after intervention was rejected(Z=-1.4327, p=.1520). 4) The sub-hypothesis that perceived benefit should be increased after intervention was supported(Z=-2.6410, p=.0083). 5) The sub-hypothesis that perceived barrier should be decreased after intervention was supported (Z=-2.4138, p=.0158). 6) The sub-hypothesis that efficacy expectation should be increased after intervention was supported(Z=-3.5162, p=.0004). As a conclusion, it was found that health belief and self efficacy promoting program was an effective nursing intervention for preventing osteoporosis of rheumatoid arthritis.

Rapidly Increasing Prevalence of Obesity and Their Confident Determinants in Korea

  • Objectives: The 2003 WHO/FAO technical report described that Korea has largely maintained its traditional high-vegetable diet despite major social and economic changes, and had lower than expected levels of obesity prevalence than other industrialized countries. However, the prevalence of obesity in Korea has recently been rapidly increasing. The aim of this study was to elucidate the determinants of this rapid growth of obesity prevalence in Korea and to compare the results of national nutrition surveys between Korea and Japan. Methods: The trends of the National Health and Nutrition Survey in Korea instituted every 3 years and that in Japan conducted every year were compared. The results of obesity prevalence defined as more than 25 of the Body Mass Index, the percentage of habitual exercisers and the results of the nutritional surveys were examined from 1992 to 2005 in Korea and from 1992 to 2004 in Japan. Results: The prevalence of obesity in males has been gradually increasing in both Korea and Japan since 1992. Though until 1995 the prevalence of obesity in the Korean male population was less than that in Japan, after 1998 Korea surpassed Japan and a markedly increasing trend was observed. In females, the increasing trend of obesity was slower than males in both Korea and Japan. However, the prevalence of obesity was much higher in Korea compared with that in Japan. The percentage of exercisers was much lower in Korea than in Japan. Although, the definition of an exerciser varies with the survey year in Korea and is different from that in Japan, almost 70% of the population was not regularly engaging in moderate or hard intensity exercise in Korea. From 1995, the total energy intake was increased by 9.8% in Korea but it was decreased by 6.9% in Japan. Presently, the energy intake per capita per day in Korea exceeded that in Japan. Remarkable increases in the intake of meat and poultry, vegetable oils and fats, and milk and dairy products were observed in Korea from 1995 to 2005. On the other hand, these values decreased during the same period in Japan. Conclusion: The prevalence of obesity in Korea is increasing and has surpassed that of Japan. The current trends could be attributed to the low prevalence of habitual exercisers, and an increase in energy intake and the proportion of energy intake from fat.

Effects of a Self-management Program for Metabolic Syndrome - A Metabolic Syndrome Management Program in Seoul - (지역사회 대상의 대사증후군 자가관리 프로그램 중재 효과 - 서울특별시 대사증후군 관리 시범사업 -)

  • Objectives: The aim of the study was to identify the effects of a community-level metabolic syndrome self-care management program. Methods: Data included baseline demographics, abdominal circumference, blood pressure, blood sugar, triglyceride, and HDL-cholesterol collected at 15 public health centers in Seoul from January to March, 2010. A data obtained(584 men and 410 women) from Metabolic Syndrome Management Program in Seoul was analyzed using Paired t-test and Linear mixed model. Results: The group with 6-month intervention resulted in significant decrease in abdominal circumference, systolic and diastolic blood pressure, blood sugar, and triglyceride. In moderate intervention group with 3-month follow-up showed tendency that abdominal circumference(-1.27(${\beta}$)) and diastolic blood pressure(-1.66(${\beta}$)) are significantly reduced. In the group with aggressive intervention, the more counseling led to significant reduction in abdominal circumference(-0.22(${\beta}$)). Conclusions: These results indicate that a self-care for metabolic syndrome management can improve metabolic syndrome status over a short-term period. Further structured research is needed to develop a systematic program on health behaviors focusing on exercise, diet, and intervention.

Analysis of Factors Affecting Breakfast Eating Behavior of Children in Indonesia: An Application of the Health Belief Model (인도네시아 아동들의 아침식사하기에 영향을 미치는 요인 분석 -건강신념모델을 이용하여-)

  • Objectives: This study investigates the current state of consuming breakfast among elementary school students residing in Malang, East Java, Indonesia, and to identify factors that influence breakfast behavior. Methods: The research model was set up as per the health belief model, and slightly modified by adding the subjective normative factors of the theory of planned behavior. The survey was conducted from July 17 to August 15, 2017 using a questionnaire, after receiving the permission PNU IRB (2017_60_HR). Results: The subjects were 77 boys (49.4%) and 79 girls (50.6%) suffering from malnutrition with anemia (21.2%) and stunting ratio of Height for Age Z Score (HAZ) (11.5%). Furthermore, moderate weakness (14.8%) and overweight and obesity (12.3%) by Body Mass Index for Age Z Score (BMIZ) were coexistent. According to the results obtained for breakfast, 21.8% did not eat breakfast before school, with 18.8% of the reasons for skipping breakfast being attributed to lack of food. Even for subjects partaking breakfast, only about 10% had a good balanced diet. The average score of behavioral intention on eating breakfast was 2.60 ± 0.58. The perceived sensitivity, perceived severity, perceived benefits, and self-efficacy of the health belief model correlated with breakfast behavior. Of these, self-efficacy (β=0.447, R2=0.200) and perceived sensitivity (β=0.373, R2=0.139) had the greatest effect on breakfast behavior. Mother was the largest impact person among children. Conclusions: In order to increase the level of breakfast behavior intention among children surveyed in Indonesia, we determined the effectiveness by focus on education which helps the children recognize to be more likely to get sick when they don't have breakfast, and increase their confidence in ability to have breakfast on their own. We believe there is a necessity to seek ways to provide indirect intervention through mothers, as well as impart direct nutrition education to children.

Effects of Dietary Liquid Chitosan on the Performance of Broiler Chickens (액상 키토산의 급여가 육계의 생산성에 미치는 영향)

  • This study were conducted to evaluate effects of drinking concentrated liquid chitosan on performance of broiler. A total of 240 broiler chicks at 7 days of age were fed the commercial diet and water until 47 days of age, drinking water divided into three treatments, 0 ppm (control), 200 ppm (200) and 400 ppm (400) of concentrated liquid chitosan. The body weight gain and feed conversion ratio were not different by drinking concentrated liquid chitosan until 28 days of age, but body weight gain and feed conversion ratio were improve by drinking concentrated liquid chitosan over 28 days of age. Both body weight gain and feed conversion ratio were tend to improve and significantly improved (p<0.05) in 200 and 400 than control at 29 to 35 days of age, respectively. While body weight gain was tended to improve and significantly improved (p<0.05) in 400 and 200 than control at 36 to 47 days of age, respectively. There was a decrease in the microflora population of E coli. in the cecum contents and feaces of broilers by drinking concentrated liquid chitosan. This effect is higher in 400 than 200. The digestibility of dry matter and crude fat of feed were tend to increased in broiler by drinking concentrated liquid chitosan. Digestibility of crude protein of feed of broiler in 200 and 400 was significantly improved (p<0.05) as compared with those in control. These results indicated that when broiler chicks drinking concentrated liquid chitosan of 400 ppm, the improvement of the body weight gain, feed conversion ratio and feed nutrients digestibility, decreasing of microflora population of both E coli. and Salmonella in the cecum contents and feaces.

The Health and Dietary management of Impaired elderly by ADL in Gyeonggi, Korea (ADL에 의한 도움필요 노인의 건강과 식생활관리 -경기지역 농촌노인을 중심으로-)

  • The purpose of this study is to provide information searching for health promotion, nutrition improvement, and health care of the impaired elderly by ADL(Activity of Daily Living) and IADL(Instrumental Activity of Daily Living). The subjects were divided into the Assistant Needed Group and No Assistant Needed Group for living in line with the responses of ADL(10 items like dressing, washing, move etc.) and IADL(10 items like housekeeping, using transportation, shopping, Phone call etc.). Survey was made for health behavior, health risk habit, dietary management status and diet intake by 24 hr-recall and questionnaire method. 242 subjects were collected in 12 cities or Gun districts in Gyeonggi Province, S. Korea. Survey was carried out by regional home extension workers using interview method. Statistical analyses were made using SAS (Version 8.1). Chi-Square Tests and General Linear Models. The subjects of impaired ADL elderly was 26.5% and it composed 30% of the total male and 22.2% of the total female. The demographic status of the impaired ADL elderly showed no difference from that of the normal elders, elementary school educated (73.4%), with spouse (43.8%) or with adult children(37.5%), using monthly living cost of 500-1,000 thousand won(35.9%). Mean age was 74.05 years compared to 72.25 years of normal elders. However, there was no significant difference from the normal and impaired ADL group, regular exercise(60.0%), with walking (90.0%), no-smoke(54.7%) and no-drink(48.4%). Kind of disease was not different from the one in impaired and normal group, with cardiovascular disease(32.3%), with diabetes mellitus(8.1%), joint lumbago neuralgia(32.3%) and osteoporosis(9.7%). Gastrointestinal complaints of the impaired ADL group were nausea(57.8%), chronic indigestion (23.4%), constipation (14,0%) and vomiting(3.7%). Sleeping time required for the impaired was longer than that for the normal group by 10hours(4.7%) or 8-10hours(20.3%), which consisted 1.7% and 16.6% respectively. Nutrient intake of the impaired ADL group was low compared to normal range elders: Energy(1260kca1), Protein(52.75g). There was gender difference in nutrient intake; the male impaired group showed no significant difference from the normal group but it was significantly lower in female impaired group. These results suggest that low quality of life and low economic status of the impaired ADL elderly require congregate meal in village hall to cover the lack of side dishe variety. And nutrition education program including community assistance would be required for the impaired ADL elderly together with the sufficient food and exercise practice. By operating nutrition education program, the impaired ADL elderly would maintain more enhanced quality of life and ameliorate the ADL capability.

A Study on the Weight Length Index and Dental Caries of Elementary School Students (초등학생들의 체중신장지수(WLI)와 치아우식증에 관한 연구)

  • In Korea, changes in children's diet patterns accelerate their physical growth and development: frequent snacking has been presumed to be a major cause of increasing dental caries. The present study attempts to clarify the relationship between the physical development of growing children and their tooth decay. For this purpose, 632 six-grade children in 4 elementary school located in Urban(Seoul) and Rural(Po-gok, Yang-In) were classified into three groups based on the Weight Length Index(WLI), known to reflect the nutritional conditions of school-age children, and the relationship was analyzed between each group and the variables considered to be related with dental caries. The result is as follows: The average weight and height of the male is $44.88{\pm}10.89$ kg, $148.49{\pm}7.33$ cm and female is $43.35{\pm}9.60$ kg, $149.23{\pm}6.73$ cm, respectively, which are in the similar level with the Korean Physical Standard. The classification of the children by the WLI reveals a relatively high distribution of over-weighted child ren - 212 persons, 335% of the entire population. The DMFT Index was a little high in the rural area(3.15 teeth in urban and 3.31, in rural). Among the groups of children classified by the WLI, the over-weight group have the highest DMFT index(3.69 teeth). The relationship between the frequency of taking in basic nutrients and the DMFT index is also found: the relationship is not evident in case of the foods containing rich calcium, protein, as well as fruits and vegetables. But, in the protein-rich food, higher frequency of its intake means significantly lower DMFT index in the normal-weight group of the urban children. In case of carbohydrate, higher frequency of its intake means significantly higher DMFT index in all the groups of the rural children. The DMFT index has some correlations with the relevant variables: the index has a positive correlation with the frequency of snacking, and a negative correlation with the economic status. That is, the higher the frequency of snacking is, and the lower the economic status is, the higher the DMFT index may be. In the logistic multiple regression analysis conducted with the presence of DMFT as a dependent variable, only the frequency of tooth brushing is turned to be a variable affecting the presence of either decayed, missing, or filled teeth. Based on the above result, the variables affecting the DMFT index are a time spent on eating, frequency of intake of protein and carbohydrate for a week, frequency of snacking, regular dental check-ups, preventive behaviors for oral health(fluoride gargling, tooth brusing after each meal, proper tooth brushing method). These variables have a relationship with the DMFT index, but the degree is somewhat different between the groups classified either by the region or by the WLI. Therefore, appropriate nutrition management should be conducted according to the individual's nutritional conditions when the services like nutritional education are provided based on the closely-examined characteristics of each target group. And, at the same time, oral health education should be strengthened, and its importance should also be emphasized so that people can pay attention to their own oral health.

Weight Control Behaviors and Correlates in Korean Adolescents (한국 청소년의 체중조절행동 영향요인)

  • The objective of this study was to investigate the prevalence rate and correlates of weight control behaviors among adolescents in Korea. We analyzed data on 37,472 adolescents aged 12-18 years from the 2011 7th Korea Youth Risk Behavior web-based Survey. The results of this study was as following. First, the prevalence rate of healthy, unhealthy and extreme weight control behaviors was significantly higher in girls than in boys. Second, in both genders, healthy and unhealthy weight control behaviors were more common among obese adolescents than non-obese adolescents. But, use of diet pills were not prescription by doctor was more common in underweight boys and obese girls than others. Third, gender, school, family affluence scale, mother's education level, body shape perception(BSP), weight status predicted prevalence rate of healthy weight control behaviors. Gender, school, region, father's education level, BSP predicted prevalence rate of unhealthy weight control behaviors. Gender, school, BSP, weight status predicted prevalence rate of extreme weight control behaviors. These findings suggest the needs to design appropriate prevention program that can induce healthy weight control behaviors.

Dietary Patterns of Middle School Girls Living in Teajon City : Comparisons between Groups of Divorced Single Mother's Family and Both Parent's Family (대전 지역 편모가정과 정상가정 여중생의 식생활양상 비교)

  • This study was aimed at comparing the dietary pattern of middle school girls according to family type. As for subjects, sixty girls of M middle school of Taejon city were chosen as a total. They were devided into two groups consisted of thirty subjects respectively ; those who live in single mother's family(SMF) and both parent's family(BPF). In this study, intakes of food and nutrient and dietary behaviors of each group were investigated through two-day dietary records and questionnaire. The results obtained in this study were as follows : 1) Mean daily intake of all nuitrients except calcium were more than RDAs in two groups. However, intake of calcium and rtio of Ca / P were lower than RDAs in two groups. And proportion of less than 2/3 of RDAs of calcium were higher than other nutrients. Intake of iron was more than RDAs, but nutritional status of iron might be poorer considering the food sorce of iron taken by subjects. According to family type, girls of SMF seemed to have more serious to have more serious problem in these nutrients than those of BPF. 2) The average distribution ratios of breakfast, lunch, dinner and snack to total calorie intake was 25.2, 31.3, 24.6 and 18.9 in SMF, and 19.7, 33.1, 24.3 and 22.9 in BPF. Thus it proved that lunch contributed more energy intake than any other meal, and that snack was an important means to supply nutrients for girls of two groups. And energy intake through breakfast of SMF was more than BPF. 3) The major dietary problems of two groups were unbalanced diet, missing meal, and overeating. The degree of sharing meal with family was lower in girls of SMF than those of BPF. The degrees of skipping (p<0.05), or irregularity, or preparing meal by girls(P<0.05) in the case of dinner were tended to be more frequent in SMF than BPF. And number of food taken per day was less various in SMF of 15.2 kinds than BPF of 17.1 kinds(P<0.001). As a conclusion, subjects should take more calcium or iron contained food which is well absorved, and eat diverse foods to improve their nutritional status. And it is also necessary that calorie intake through snack should be diminished, and dinner should be eaten regularly and shared with family if possible. In view of family type, girls of SMF proved to have poorer food habit than those of BPF. Thus, these results shows that dietary pattern of girls is different atcording to family type. Therefore, we should carry out nutritional education for girls considering the difference of dietary pattern by family type. In addition, nutritional education for girls of SMF should be regarded more importantly than those of BPF because girls of SMF might have more chance to manage their meal by themselves and also have more dietary problems.

Lifestyle intervention might easily improve blood pressure in hypertensive men with the C genotype of angiotensin II type 2 receptor gene

  • BACKGROUND/OBJECTIVES: Recent studies have reported an association of the angiotensin II type 2 receptor (AT2R) 3123Cytosine/Adenine (3123C/A) polymorphism with essential hypertension and cardiovascular diseases. The purpose of the study was to investigate whether the AT2R 3123C/A polymorphism affects blood pressure for free-living hypertensive men during a 5-month intervention period. SUBJECTS/METHODS: The subjects were free-living hypertensive Japanese men aged 40 to 75 years who agreed to intervention in the period from 2004 to 2011. Detection of the AT2R 3123C/A polymorphism was determined by polymerase chain reaction. The dietary intervention was designed to decrease salt level and to increase potassium level through cooking instructions and self-monitoring of the diet. The exercise session consisted of activities such as stretching, resistance training, and walking. Blood pressure, urinary sodium and potassium excretion, dietary and lifestyle data, and non-fasting venous blood sample were collected at baseline and after the intervention period. RESULTS: Thirty nine subjects were eligible for participation and the follow-up rate was 97.4%. The C allele proportion was 57.9%. AT2R 3123C/A polymorphism was X-chromosome-linked, therefore we analyzed the C and A genotypes. At baseline, no significant differences were observed between the genotype groups. After the intervention, there were no significant differences in lifestyle habit between the groups. Nevertheless, the estimated salt excretion (g/day) was significantly decreased only in the C genotype (13.0-10.3, P = 0.031). No significant change was observed in systolic blood pressure (SBP) (mmHg) in the A genotype, but a significant decrease was observed in the C genotype (150.0-141.5, P = 0.024). CONCLUSTIONS: In the C genotype, it might be easy to improve SBP through lifestyle intervention in free-living hypertensive Japanese men, however generalization could not be achieved by the small sample size.
