Objective: There exist gender differences in pulsatile contour waveform. Women have a greater age-related increase in left ventricular mass than do men and more likely to experience symptomatic heart failure after infarction. SDPTG (the second derivative of photoplethysmogram waveform) is a noninvasive method for evaluating the pulse wave and is correlated with age and other risk factors for atherosclerosis. We studied the effect of gender on SDPTG and made clear why the gender differences appear. Methods: To study the effects of effect factors, including height and blood pressure, on SDPTG in the fourth decade, data on height, weight, PTD (pulse transit distance), blood pressure, serum lipid levels, and SDPTG were collected in 115 laboratory healthy men and women. SDPTG is derived from double-differential processing of fingertip photoplethysmography and consists of a, b, c, and d waves in systole and an e wave in diastole; SDPTG aging index (AI) was calculated as (b-c-d-e)/a. Results: There were significant gender-related differences of SDPTG AI, height, and blood pressure. Age, height, and mean blood pressure were respectively and significantly correlated with SDPTG AI. SDPTG is dependent upon age, height, and blood pressure. Restricting analysis to SDPTG AI, age, height, and mean blood pressure, yielded that there were gender-related differences in SDPTG AI (P<0.05) which were derived from those of height (F<0.001, df=l, P=0.994). Conclusions: These new data may help to explain previous findings about age-related differences in pulsatile contour waveforms and why gender differences of SDPTG appear. The results of this study suggest that SDPTG AI, used for evaluation of biological vascular aging, should be calibrated by height as well as age and blood pressure.
Objective : There is accumulating data that cardiovascular diseases, including hypertension, cerebro-vascular accident, coronary heart disease, relate to atherosclerotic changes. Increased serum levels of lipids could play a role in these circumstances. the SDPTG(the second derivative of photoplethysmogram waveform) is noninvasive methods for evaluating the pulse wave and is correlated with age and other risk factors for atherosclerosis. Methods : To study the effects of serum lipid levels on the SDPTG in the middle-age adults, data on serum lipid levels and the SDPTG were collected in 80 healthy middle age with varying serum lipid concentrations. Biological vascular age was assessed using the SDPTG. the SDPTG consists of an a, b, c, and d wave in systole and an e wave in diastole; an SDPTG aging index(Al) was calculated as (b-c-d-e)/a. Results : Serum triglyceride, total cholesterol, total lipid, phospholipid, HDL cholesterol levels were respectively and significantly correlated with SDPTG Al. Serum lipid levels and blood pressure are age-dependent and the SDPTG is blood pressure-dependent. Restricting analysis to age and blood pressure yield strong positive correlations between serum triglyceride, total lipid levels and SDPTG Al, but no correlations between serum total cholesterol, phospholipid levels and SDPTG Al. Conclusion : The results of this study suggest that preventive acupuncture programs or herbal medications for reducing serum lipid, especially triglyceride, levels in middle-age should include a evaluation of biological vascular aging with the second derivative of photoplethysmogram waveform.

목적 : 심초음파는 비침습적이므로 반복적으로 정확히 심질환의 경과를 관찰하여 치료효과 및 수술시기를 정할 수 있는 검사로서 임상적으로 매우 유용하다. 실시간 심근조영심초음파에 의한 time intensity 평가는 부위별로 수행됨으로 연속적으로 위치하는 관심영역이 intensity에 있어 심장의 움직임 변화에 영향을 받는다. Time intensity 곡선의 최적의 곡선맞춤을 위해 주기적인 심장 운동 매개변수를 조합해 기존의 모델을 보정한 안정적인 측정방법을 제시한다. 방법 : 심장의 운동에 의한 특징적인 정보를 설명하기 위해 기존의 문헌에 제시된 지수 함수에 주어진 심박수로 만들어진 시간에 관한 일반적인 정형파 함수를 추가한다. In image-based CFD modeling of carotid bifurcation hemodynamics, it is often not possible (or at least not convenient) to impose measured velocity profiles at the common carotid artery inlet. Instead, fully-developed velocity profiles are usually imposed based on measured flow rates. However, some studies reported a pronounced influence of inflow boundary conditions that were based on actual velocity profiles measured by magnetic resonance imaging which showing the unusual presence of a high velocity band in the middle of the vessel during early diastole inconsistent with a Dean-type velocity profile. We demonstrated that those velocity profiles were induced by the presence of modest secondary curvature of the inlet and set about to test whether such more "realistic" velocity profiles might indeed have a more pronounced influence on the carotid bifurcation hemodynamics. We found that inlet boundary condition with axisymmetric fully-developed velocity profile(Womersley flow) is reasonable as long as sufficient CCA inlet length of realistic geometry is applied.
A 4-years-old, intact male Golden retriever dog was presented with abdominal distension and dyspnea. Physical examination revealed arrhythmia and cardiac murmur. Generalized cardiomegaly, pleural effusion and ascites were shown on thoracic and abdominal radiographs. Two-dimensional echocardiography revealed abnormal mitral and tricuspid valve motion, mitral and tricuspid regurgitation, left ventricular eccentric hypertrophy and left atrial dilation. Color-flow Doppler imaging revealed turbulent flow extending into the left ventricle during diastole from the mitral valve orifice, and into the left atrium during systole. Spectral Doppler recordings revealed highly increased early diastolic mitral valve inflow and prolonged pressure half-time of mitral inflow. Based on the echocardiographic examination, the diagnosis was made as the mitral valve dysplasia concurrent with mitral valve stenosis and tricuspid valve dysplasia.
Objectives : The second derivative of photoplethysmogram waveform(SDPTG) is a simple, convenient and non-invasive technique for pulse wave analysis. This study was designed to investigate the differences in the SDPTG between the affected side and the unaffected side in hemiparetic patients after stroke, and the effects of acupuncture at Palsa(BaXie) in hemiparetic patients after stroke. Methods : To evaluate the differences between the affected side and the unaffected side in hemiparetic patients after stroke, their SDPTG were recorded by using a Cardio Peri SA6000. To evaluate the effects of acupuncture at Palsa(BaXie) in 20 hemiparetic patients after stroke and 20 healthy subjects, their SDPTG were recorded 2 times(pre-acupuncture and post-acupuncture). The SDPTG consists of an a, b, c and d wave in systole and an e wave in diastole. Sano aging index was defined as (b-c-d)/a. Results : 1. The affected side in hemiparetic patients after stroke had higher average b/a ratio(p<0.05)and Sano aging index(p<0.05) than the unaffected side 2. In a comparison of pre-acupuncture and post-acupuncture at the Palsa(BaXie), b/a ratio(p<0.05) and Sano aging index(p<0.05) was decreased significantly in both hemiparetic patients and healthy subjects. Conclusions : These findings suggest that acupuncture at Palsa(BaXie) may be effective method of reverting some of the deleterious effects on vascular function produced by stroke.

Aortic regurgitation occurs when there is a leakage of the valve backward into the left ventricle during diastole. Chronic aortic regurgitation may be present for decades before any symptoms occur. The left ventricle is able to compensate for the large volume of blood that flows backward by enlarging the cavity and increasing the thickness of the muscle. This mechanism allows the heart to pump out both the amount of blood required by the body and the blood that has gone backward into the left ventricle. One patient who had chest pain from exercise was treated. He was diagnosed with aortic regurgitation LVH by echocardiography. Signs showed him to be of Taeumin-type within Sasang Constitutional Medicine. He was prescribed Gamijoripewontang for 75 days, and improvement of clinical symptoms were observed. Results support prescription of Gamijoripewontang as an effective treatment for Taeumin-type patients suffering chest pain caused by aortic regurgitation.
A shunt from the left ventricle to the left anterior descending artery is being developed for coronary artery occlusive disease, in which the shunt or conduit connects the the left ventricle (LV) with the diseased artery directly at a point distal to the obstruction. To aid in assessing and optimizing its benefit, a computational model of the cardiovascular system was developed and used to explore various design conditions. Computational fluid dynamic analysis for the shunt hemodynamics was also done using a commercial finite element package. Simulation results indicate that in complete left anterior descending artery (LAD) occlusion, flow can be returned to approximately 65% of normal, if the conduit resistance is equal for forward and reverse flow. The net coronary flow can increase to 80% when the backflow resistance is infinite. The increases in flow rate produced by asymmetric flow resistance are enhanced considerably for a partial LAD obstruction, since the primary effect of resistance asymmetry is to prevent leakage back into the ventricle during diastole. Increased arterial compliance has little effect on net flow with a symmetric shunt, but considerably augments it when the resistance is asymmetric. The computational results suggest that an LV-LAD conduit will be beneficial when the resistance due to artery stenosis exceeds 27 PRU, if the resistance is symmetric. Fluid dynamic simulations for the shunt flow show that a recirculating region generated near the junction of the coronary artery with the bypass shunt. The secondary flow is induced at the cutting plane perpendicular to the axis direction and it is in the attenuated of coronary artery.

Wavelength selection and prediction algorithm for determining hematocrit are investigated. A model based on the difference in optical density induced by the pulsation of heart beat is developed by taking approximation of Twersky's theory on the assumption that the variation of blood vessel size is small during arterial pulsing[1]. A device is constructed with a five-wavelength LED array as light source. The selected wavelengths are two isobestic points and three in compensation for tissue scattering. Data are collected from 549 out-patients who are randomly grouped as calibration and prediction sets. The range of percent hematocrit was 19.3∼51.8. The ratio of the variations of optical density between systole and diastole at two different wavelengths is used as a variable. We selected several such variables that show high reproducibility among all variables. Multiple linear regression analysis is made. The relative percent error is 8% and the standard deviation is 3.67 for the calibration set. The relative % error and standard deviation of the prediction set are 8.2% and 3.69 respectively. We successfully demonstrate the possibility of non-invasive hematocrit measurement, particularly, using the wavelengths below 1000nm.
To regulate cardiac output of the Total Artificial Heart(TAH) physiologically, the hemodynamic information must be toed back to the controller. So far, our group has developed an automatic cardiac output control algorithm using the motor current waveform, It is, however difficult to detect the preload level such as a filling status of ventricular inflow and the variation of atrial pressures within normal physiologic range(0-15 mmHg) by analyzing the motor current which simultaneously reflects the afterload effect. On the other hin4 the interventricular volume pressure(IVP) which is not influenced by arterload but by preload is a good information source for the estimation of preload states. In order to find the relationship between preload and IVP waveform, we set up the artificial heart system on the Donovan type mock circulatory system and measured the IVP waveform, right and left atrial pressures, inflow and outflow waveforms and the signals represented the information of moving actuator's position. We shows the feasibility of estimating the hemodynamic changes of inflow by using IVP waveform. fife found that the negative peak value of IVP waveform is linearly related to atrial pressures. And we also found that we could use the time to reach the negative peak in IVP waveform, the time to open outflow valve, the area enclosed IVP waveform as unfu parameters to estimate blood filling volume of diastole ventricle. The suggested method has advantages of avoiding thrombogenesis, bacterial niche formation and increasing longterm reliability of sensor by avoiding direct contact to blood.
