Constitution Therapy of Adolescent Ulcerative Colitis: A Case Report (청소년궤양성 대장염 환자의 사상체질요법에 대한 증례보고)

  • Objectives: This study discusses the effects of constitution therapy on adolescent ulcerative colitis. Methods: A 12-year-old male patient was treated with western medication for six months and herbal medicine for one year and six years; however, his abdominal pain, mucousy stool, bloody stool, and diarrhea persisted. He was diagnosed as having Taeumin according to Sasang constitution classification and treated with Sasang constitutional medicine (i.e., Yeuldahanso-tang and food restrictions based on constitutional medicine theory). Before the first treatment, his Pediatric Ulcerative Colitis Activity Index (PUCAI) score was 35, but this score decreased to 15 within six months and was maintained at 10 by 18 months of treatment. He took the herbal medication for 38 months, and his PUCAI score was 0 at the end of treatment. His liver functioned normally despite long-term drug use, and a follow-up colonoscopy showed no ulcerative colitis except melanosis, which was presumed to be caused by the herbal medicine. Ulcerative colitis in pediatric adolescents requires treatment with pharmacotherapy and dietary control based on constitutional medicine to maintain disease remission. Conclusions: Constitutional therapy is effective for treating adolescent ulcerative colitis. More clinical data are needed for patients with ulcerative colitis.

Effects of Replacing Spray Dried Porcine Plasma with Solpro500 on Performance, Nutrient Digestibility and Intestinal Morphology of Starter Pigs

  • The objective of this study was to evaluate Solpro500 (a wheat hydrolysate containing a high level of glutamine) as a replacement for spray dried porcine plasma (SDPP) in diets fed to nursery pigs. One hundred and eight pigs (Dalland, $5.39{\pm}0.80$ kg BW) weaned at 21 days were assigned to one of three treatment groups for a 28 day feeding trial. The experimental diets were based on corn and soybean meal and were supplemented with either 8% SDPP, 4% SDPP plus 4% Solpro500 or 8% Solpro500. Each treatment was fed to six pens with six pigs per pen (4 barrows and 2 gilts). The experimental results indicated no significant difference (p>0.05) in daily gain, feed intake or feed efficiency for pigs fed the three experimental diets. However, the diarrhea index for pigs fed either 4% SDPP and 4% Solpro500 or 8% Solpro500 was lower (p<0.05) than that for pigs fed 8% SDPP. No differences (p>0.05) were found in the apparent fecal digestibility of dry matter, organic matter or crude protein between pigs fed the three diets. The intestinal morphology (villous height, villous width and crypt depth) was not affected by diet treatments (p>0.05). In conclusion, Solpro500 SDPP can replace SDPP without any negative effects on nursery pig performance.

Effects of Biocom as a Replacement of Glutamine on Performance and Blood Biochemical Indexes of Early Weaned Piglets

  • The objective of this study was to evaluate Biocom (a protein source containing a high level of glutamine and alanyl-glutamine) as a replacement for glutamine (Gln) in nursery pig diets. Forty-two pigs (fourteen pigs per treatment) weaned at 28 d of age were used in a 28-d performance trial using three dietary treatments: control (no Gln), control supplemented with Gln or Biocom. The control diet was composed of corn, soybean meal, whey and fish meal. Individual body weight, pen feed disappearance and diarrhea were monitored. On d 0, 2, 7 and 14 postweaning, respectively, five pigs per treatment were selected and bled from the anterior vena cava to obtain five replicate samples of blood on each dietary treatment for determination of blood biochemical index. Dietary supplementation of Gln and Biocom did not influence performance, plasma Gln and total serum protein concentration (p>0.05). However, the addition of Gln and Biocom could prevent serum urea nitrogen and serum cortisol from increasing on d 2 postweaning (p<0.05). There were no significant differences (p>0.05) in any of the examined parameters between Gln- and Biocom-supplemented diets. In conclusion, dietary Gln did not influence the performance of early-weaned piglets owing to the complex diet containing whey, but could prevent the increase of serum urea and cortisol. Biocom could be used as a replacement for free pure Gln without any negative effect on early-weaned piglets.

Alloferon Alleviates Dextran Sulfate Sodium-induced Colitis

  • Dysfunction of gut immune regulation is involved in mucosal damage in inflammatory bowel disease (IBD). However, there is still no efficacious immune-regulator for the treatment of IBD. Alloferon is a novel immune-modulatory peptide that was originally isolated from infected insects. It shows anti-inflammatory effects by the regulation of cytokine production by immune cells and their activities. Therefore, we investigated the effect of alloferon in a mouse model of colitis using dextran sulfate sodium (DSS). Colitis was induced by administration of DSS in drinking water for 7 consecutive days. It was confirmed by the presence of weight loss, diarrhea, hematochezia, and colon contraction. Alloferon was injected 4 days after DSS administration. We found that alloferon improved the pathogenesis of IBD based on the reduced disease activity index (DAI) and colon contraction. Edema, epithelial erosion, and immune cell infiltration were found in mice administered DSS, but the phenomena were reduced following alloferon treatment. The plasma level of IL-6, a classical pro-inflammatory cytokine in colitis, was also decreased by alloferon. Moreover, alloferon inhibited the TNF-${\alpha}$-induced degradation and phosphorylation of $I{\kappa}B$ in Colo205 colon cancer cells. Taken together, these results show that alloferon has anti-inflammatory effects and attenuates DSS-induced colitis.

Synergic Effect of Methanol extracts of Schizandrae Fructus and Mume Fructus on Experimental Mouse Colitis Induced by Dextran Sulfate Sodium (Dextran Sulfate Sodium 유도 마우스 대장염에 미치는 오미자와 매실의 상승효과)

  • The fruits of Schisandra chinensis and Prunus mume have been traditionally used in the Oriental countries as an astringent against diarrhea and abdominal pain, a protectant for liver disease, an antimicrobial, and a blood tonic. However, little is known about the extract of Schizandrae Fructus and Mume Fructus (SMF-Ex) on dextran-sulfate sodium (DSS)-induced colitis in mice. In this study, we investigated the protective effects of SMF-Ex on DSS-induced colitis in mice. An experimental colitis was induced by daily treatment with 5% DSS. SMF-Ex was orally administrated the single dose (80 mg/kg, body weight/day) for 7 days with one time per day. SMF-Ex reduced significantly clinical sign of DSS-induced colitis, including body weight loss, shorten colon length, increased disease activity index (DAI), and histological colon injury. SMF-Ex also inhibited significantly nitric oxide (NO) and prostaglandine $E_2$ ($PGE_2$) productions in DSS-induced colitis mice. Furthermore, SMF-Ex increased significantly an superoxide anion (SOD), catalase, and glutathione peroxidase (Gpx) activity of the colon tissue in DSS-induced colitis mice. These results suggest that SMF-Ex administration could reduce significantly the clinical signs and inflammatory mediators, and increase antioxidant activity in DSS-induced colitis model mice and is a good candidate for further evaluation as an effective anti-ulcerative agent.

  • Purpose : We have carried out this study to investigate association of DSOM scores and women infertility in oriental medicine for systematization diagnosis and treatment of women infertility. Methods : We analyzed the results of medical records for 151 patients who were diagnosed as women infertility. We investigated the possession of symptom index of women infertility, recognition association of DSOM. Results : 1. In comparison with 151 clinical trial's data. deficiency of Yin(陰虛), coldness(寒), spleen(脾) are associated positively and deficiency of Yang(陽虛), phlegm(痰) are associated negatively. 2. In pathogenesis(病機) indicator of average, cold inside the body is associated positively and diarrhea, kidney are associated negatively. 3. In pathogenesis five score-point of value, Heat(熱), kidney(腎) are associated negatively. 4. In pathogenesis(病機) indicator. constipation(便秘) is associated positively and hardness of lower abdomen, tingly weakness of lower back and knee question are associated negatively. 5. In 140 questions, nine questions are associated positively and ten question are associated negatively.

Long-term Functional and Patient-reported Outcomes Between Intra-corporeal Delta-shaped Gastroduodenostomy and Gastrojejunostomy After Laparoscopic Distal Gastrectomy

  • Purpose: This study aimed to compare the long-term functional and patient-reported outcomes between intra-corporeal delta-shaped gastroduodenostomy and gastrojejunostomy after laparoscopic distal gastrectomy for gastric cancer. Materials and Methods: We retrospectively reviewed clinicopathological data from 616 patients who had undergone laparoscopic distal gastrectomy for stage I gastric cancer between January 2015 and September 2020. Among them, 232 patients who had undergone delta-shaped anastomosis and another 232 who had undergone Billroth II anastomosis were matched using propensity scores. Confounding variables included age, sex, body mass index, physical status classification, tumor location, and T classification. Postoperative complications, nutritional outcomes, endoscopic findings, and quality of life (QoL) were compared between the 2 groups. Results: No significant differences in postoperative complications or nutritional parameters between the two groups were observed. Annual endoscopic findings revealed more residual food and less bile reflux in the delta group (P<0.001) than in the Billroth II group. Changes of QoL were significantly different regarding emotional function, insomnia, diarrhea, reflux symptoms, and dry mouth (P=0.007, P=0.002, P=0.013, P=0.001, and P=0.03, respectively). Among them, the delta group had worse insomnia, reflux symptoms, and dry mouth within three months postoperatively. Conclusions: Long-term nutritional outcomes and QoL were comparable between the delta and Billroth II groups. However, more residual food and worse short-term QoL regarding insomnia, reflux symptoms, and dry mouth were observed in the delta group. Longer fasting time before endoscopic evaluation and short-term symptom management would have been helpful for the delta group.

  • Uterus myoma is a benign tumor of smooth muscle in the wall of the uterus, In oriental medicine, we used to made an effort to management this patients without surgical operation. Doctors have treated patients of uterus myoma mainly by checking over each symptom they have. Then we think that patients have some symptoms in relation to an etiological cause. So I have carried out this study to investigate association of DSOM scores and an attack of uterus myoma in oriental medicine. We chose 3 groups, the first one is 257 uterus myoma patients who visited Dongeui University Oriental Medical Center from May 2001 to June 2006, the second one is 558 outpatients who didn't have uterus myoma from May 2005 to June 2005, the third one is 129 clinical trials who volunteered for Sasang constitutional medicine. Then we made up 3 groups to checkup DSOM, and investigated the All DSOM Fluents which effect uterus myoma patients using regression model. Logistic regression analysis indicate as follows ; In comparison with 558 outpatients data, blood stasis(血瘀), dryness(燥) is associated positively and insufficiency of Yang(陽虛), spleen(脾), phlegm(痰) negatively, and mean of the index for pathogenic factor(病機指標 平均) of deficiency of qi(氣虛), heart(心) negatively. In comparison with 129 clinical trials data, blood stasis(血瘀) is associated positively and phlegm(痰) negatively, and mean of the index for pathogenic factor(炳機指標 平均) of deficiency of Yin(陰虛), liver(肝), diarrhea positively, heart(心) negatively. 3. In investigation of DSOM items, items of blood stasis(血瘀), deficiency of Yin(陰虛), coldness(寒) is associated positively and items of heart(心), spleen(脾), Phlegm(痰) negatively.

A Case of Adult Otitis Media with Effusion Accompanied by Atopic Dermatitis (아토피피부염을 동반한 성인 삼출성 중이염 치험례)

  • Objectives : This study is to report a case of otitis media with effusion accompanied by atopic dermatitis improved by Korean medicine monotherapy. Methods : A 31-year-old woman patient visited our clinic for aural fullness, hearing loss, and tinnitus of the left ear on August 14th, 2021. She also had abdominal pain, diarrhea, and general weakness because of side effects of antibiotics. Besides, she had nasal congestion, rhinorrhea, and dry skin. Through otoscopy, her tympanum was fully filled with yellow effusion. She only took Korean medicine therapy including acupuncture and herbal medicine weekly. The treatment continued for about 4 months(total 14 visits) until December 4th, 2021. Otoscopy, numerical rating scale(NRS), scoring atopic dermatitis(SCORAD) index were used to assess the symptoms. Results : On August 30, 2021, her intestinal symptoms improved. The left tympanum was recovered to normal condition as the treatment continued. The effusion disappeared on October 16th, 2021. On November 6th, 2021, peritympanic blood congestion also almost alleviated. Subjective symptoms including aural fullness, hearing loss, and tinnitus disappeared on October 16, 2021. Nasal symptoms and dry skin also improved. The SCORAD index dropped from 15 to 4. Conclusions : These results suggests that otitis media with effusion can be sufficiently managed by Korean medicine therapy. Further, active application of Korean medicine therapy to common otolaryngological diseases is necessary.

An Epidemiological Investigation on an Outbreak of Shigellosis in a Special School for Handicapped in Yeongcheon-si and in a Rehabilitation Facility in Gyeongsan-si, Korea, 2008 (2008년 영천시 지적장애인 학교와 경산시 재활원에서 발생한 세균성이질에 관한 역학조사)

  • Objectives: In September 2008, an outbreak of diarrhea occurred among students attending Y school in Yeongcheon-si. Shigella sonnei was cultured from some of the rectal swabs. An epidemiological investigation was carried out to determine the source of the infection and the mode of transmission of the shigellosis outbreak. Methods: The index case lived in the D rehabilitation facility in Gyeongsan-si and an additional epidemiological investigation was carried out there. The cases could not be questioned due to their mental handicaps. The teachers were interviewed instead. A patient case was defined as a resident with diarrhea more than one time a day from September 18 to September 26, 2008 or a resident with confirmed Shigella sonnei at the Y school or the D rehabilitation facility. Results: The attack rate was 1.2% (8 persons) among 659 persons in the Y school and D rehabilitation facility. Five persons were microbiologically confirmed to have the infection and three persons were diagnosed on the basis of symptoms. Shigella sonnei was cultured from five of the 659 rectal swabs. However, 80 environmental specimens including drinking water, preserved foods, and cooking utensils were negative. All eight patients were Y school students and had been living in group boarding and lodging. Six of them lived in the D rehabilitation facility and two lived in the dormitory at the Y school. Five cases showed pulsed-field gel electrophoresis patterns that were identical for Shigella sonnei. Conclusions: The results of this study showed that the infection source of the shigellosis outbreak, in the two places, were identical. It is likely that the infections initially spread from a teacher or volunteer and then among the students.
