변연형태에 따른 IPS Empress 도재관의 적합도에 관한 연구 (A STUDY ON THE FITNESS OF THE IPS EMPRESS CERAMIC CROWN ACCORDING TO MARGIN TYPE)

  • The purpose of this study was to compare the fitness of the IPS Empress ceramic crowns according to margin type such as bevel, shoulder, rounded shoulder, shoulder with bevel, rounded shoulder with bevel and grooved bevel. After 10 experimental dies were constructed for each group, the IPS Empress ceramic crowns were fabricated on the dies, cemented with resin cement(Bistite resin cement, Tokuyama Soda Co. Ltd., Japan) and embeded in clear epoxy resin. Each specimen was sectioned with a low speed diamond saw in both the faciopalatal and the mesiodistal direction. The cement film thickness was measured with a measuring microscope. The obtained results were as follows : 1. The fitness at the external margin was good in the following order: rounded shoulder(28.99), shoulder with bevel(31.33), grooved bevel(31.79), rounded shoulder with bevel(34.65), bevel(35.02) and shoulder(35.08). However there is no significant statistical difference. 2. The fitness at the internal margin of bevel(25.75), rounded shoulder(31.24), shoulder with bevel(32.07) and rounded shoulder with bevel(34.45) was better than that of shoulder(47.98). 3. The fitness at the axial surface of bevel(26.58) was better than that of shoulder(49.59). 4. The fitness at the central fossa was good in the following order: shoulder with bevel(38.88), grooved bevel(40.46) rounded shoulder with shoulder(46.29), shoulder(48.91), bevel(49.47) and rounded bevel(52.07). However there is no significant statistical difference. 5. Overall fitness of bevel(29.71), rounded shoulder(32.49) and shoulder with bevel(34.47) was better than that of shoulder(44.73).

전부주조금관 치경부 변연의 형태가 치경부 변연적합에 미치는 영향 (THE EFFECT OF VARIOUS FINISH LINE PREPARATIONS ON THE MARGINAL SEAL OF FULL CROWN PREPARATIONS)

  • 현재 임상에서 사용되고 있는 니켈-크롬합금(Washiloy Soft, Kamemizu Chemical Inc. Ltd., Japan)을 이용하여 Chamfer, shoulder, shoulder with a $45^{\circ}$ bevel의 각 치경부 변연의 형태에 따른 시멘트층 피막후경을 교합면 및 치경부에서 측정, 비교하여 다음과 같은 결론을 얻었다. 금관의 교합면 장착정도는 chamfer가 가장 우수하고, 다음으로는 shoulder, shoulder with a $45^{\circ}$ bevel의 순이며, 이들 간에는 유의성 있는 차이를 나타내었다(p<.05). 금관의 치경부 적합도는 chamfer가 가장 우수하고, 다음으로 shoulder with a $45^{\circ}$ vevel, shoulder의 순이며, 이들 간에는 유의성 있는 차이를 나타내었다(p<.05).

무릎관절 해면뼈의 기계적 물성 (The Mechanical Properties of Trabecular Bone in Knee Joint)

  • In this study, we performed the compressive strength test of trabecular bone in knee joint for measuring the elastic modulus and ultimate strength. The main knee joint is femorotibial articulation between the lateral and medial femorotibial condyle. In the case of osteoarthritis, some patients have only medial condylar osteoarthritis. We performed the mechanical test for comparison the difference of the each condylar strength. We used diamond core-drill and linear precision saw for making the specimens. Specimens were cored from both condyle in distal femur and proximal tibia in fresh cadaver (male 10, female 12), and tested by universal test machine with temperature control saline circulation system. Results of the test in distal femoral parts, averaged elastic modulus was $360.61{\pm}159.40MPa$ for male, $150.89{\pm}70.65MPa$ for female. Averaged ultimate strength was $6.79{\pm}2.91MPa$ for male, $2.89{\pm}1.31MPa$ for female. Male was 2.4 times stronger than female. In the proximal tibial parts, averaged elastic modulus was $108.80{\pm}52.88MPa$ for male, $73.45{\pm}55.06MPa$ for female. Averaged ultimate strength was $2.59{\pm}1.39MPa$ for male, $1.75{\pm}1.16MPa$ for female. Male was 1.5 times stronger than female. In the distal femoral condyle, medial condyle had more strength than lateral condyle at middle region. But lateral condyle had more strength than medial condyle at anterior & posterior regions (p<0.02). In the proximal tibial condyle, medial condyle had more strength than lateral condyle. (p<0.01).

치약 종류에 따른 유치의 표면 변화 (Change in surface of primary tooth using different type of toothpaste)

  • Objectives : The aim of this study is to evaluate the surface changes of enamel specimen, tooth structure by toothpastes in child and adult. Methods : Experimental teeth were collected from extracted human primary teeth. 120 enamel specimens were prepared by cutting the teeth into $2{\times}3{\times}2mm$ blocks using diamond saw and the specimens were assigned to 3 groups. Group 1 was used as control with no treatment. Group 2 was treated with child toothpaste and Group 3 was treated with adult toothpaste on primary enamel surface for 3 minutes daily over 4 weeks. The specimens were immersed into individual container having artificial saliva and the artificial saliva was changed every day. The electron probe micro analyzer(EPMA) provided weight percent(wt%) of calcium(Ca) and phosphorous(P) on enamel surface. The morphology was analyzed by scanning electron microscopy(SEM). Data were analyzed by one-way analysis of variance(ANOVA) and Tukey's test post-hoc test using SPSS(Version 20, SPSS Inc., Chicago, USA). Level of significance was set at 0.05. Results : The surface changes of the primary teeth revealed a significant difference during 4 weeks. Calcium(Ca) and phosphorous(P) levels were found the weight percent difference and a rough enamel surface was seen on SEM after adult toothpaste application. Conclusions : The changes in Ca and P and the morphological surface were affected by the primary tooth treated with adult toothpaste. Enamel surface showed significant differences during 4 weeks.

상아질 표면처리방법이 compomer의 전단결합 강도에 미치는 영향에 관한 연구 (EFFECT ON THE SHEAR BOND STRENGTH OF A COMPOMER TO DENTIN ACCORDING TO SURFACE CONDITIONING)

  • The purpose of this study was to evaluate the shear bond strength of the Compoglass Carvifil bonded on the dentin surface according to etching or non-etching and two time application or three time application of single component. Human non-carious 60 extracted 3rd molar were used. The occlusal dentin surfaces of all teeth were exposed with Diamond Wheel Saw and polished with Lapping & Polishing machine(South Bay Technology Co., U.S.A). The teeth were then distributed randomly into four groups of 15 teeth each and dentin surface were conditioned as following. Control group : Non-etching, two times application of Syntac Single Component. (According to manufacture's instruction) Experimental group 1 : Non-etching, three times application of Syntac Single Component. Experimental group : 2 Etching, two times application of Syntac Single Component. Experimental group 3 : Etching, three times application of Syntac Single Component. Compoglass were bonded to exposed dentin surfaces and all samples were placed in distilled water for 7 days. The shear bond strengths were measured by universal testing machine (SHIMADAZU AUTOGRAPH, AGS-4D., Japan). The results were as follows : 1. Experimental group 3 revealed the highest value (30.75${\pm}$4.74 MPa) and control group revealed the lowest value(14.85${\pm}$2.69 MPa). There was significant difference of shear bond strength among four groups(P<0.01) 2. The acid-etching groups (experimental group 2, 3) had higher shear bond strengths than non etching groups(control group and experimental group 1). 3. The additional application of Syntac single component groups revealed a higher bond strength than two times application groups (control group and experimental group 2).

$1\alpha$, Dihydroxycholecalciferol의 골다공증성 골절 치유효과 : 광학현미경 및 주사전자현미경적 관찰 (Effects of $1\alpha$, 25 Dihydroxycholecalciferol on Osteoporotic Fracture : Light Microscopic and Scanning Electron Microscopic Observation)

  • 12주령 랫트의 정상 비골골절의 치유과정을 이해하고, 난소적출 후 비골골절의 치유과정과 난소적출 후 비골골절의 치유에 미치는 $1\alpha$, 25 dihyoxycholecalciferol의 영향을 알아보고자 실험을 실시한 결과 다음과 같은 결론을 얻었다. 비골의 골절은 골절유발 후 5주에 성숙된 신생골 조직으로 충만되어 조직형태학적으로 완전한 치유가 이루어졌다. 난소적출 후 골절의 치유는 정상골절의 치유보다 지연되었으나, $1\alpha$, 25 dihydroxycholecalciferol 투여 결과 골절유발 후 5주에 성숙된 신생골 조직으로 골절단이 충만되어 정상상태의 골절과 비슷하게 치유가 되었다. 이상의 결과를 종합해보면 랫트의 난소를 적출하여 여성호르몬의 결핍을 유발하면 난소적출 후 7주에는 골다공증이 발생되고, 골다공증은 정상상태의 골절에 비하여 약 1주일 이상 골절의 치유를 지연시켰으며, $1\alpha$, 25 dihydroxycholecalciferol을 투여하였을 경우 정상상태의 골절치유와 비슷한 경과를 나타내어 $1\alpha$, 25 dihydroxycholecalciferol은 골다공증성 골절의 치료에 있어서 효과있는 치료제라는 결론을 내릴 수 있었다.

상아질처리제(象牙質處理劑)에 의(依)한 상아질(象牙質) 표면(表面) 도말층(塗抹層)의 제거(除去)에 관(關)한 연구(硏究) (A STUDY ON THE REMOVAL OF DENTINAL SMEAR LAYER BY DENTIN CONDITIONER)

  • The purpose of this study was to observe the effect on the removal of dentinal smear layer and morphological changes of reduced dentin surfaces by various dentin surface conditioners. Thirty-two healthy human premolars extracted due to periodontal or orthodontic reasons were used. The teeth were cross-sectioned to expose dentin at the middle portion of the crown with diamond rotary saw. The specimens were then divided into 8 groups. The sectioned dentin surfaces in group 1 to 4 were grinded with No. 400 grit silicone abrasive paper and those in group 5 to 8 were cut with #700 carbide bur under air-water spray. The grinded or cut dentin surfaces were conditioned with 3% $H_2O$, Dentin Conditioner(GC Inter. Corp., Japan), and Scotchprep(3M Dent Prod., U.S.A) according to manufacturer's directions. All the specimens were dried in room temperature for 48 hours, and gold-coated with Eiko ion coater(Eik-engineering Co.), and observed in Hitachi S-450 Scanning electron microscope at 15-25 KV. The following results were obtained; 1. The dentin surfaces grinded with the silicon abrasive paper were rougher in texture and heavier in amount of smear layer than those cut with the carbide bur. 2. Scrubbing of 3% $H_2O_2$ was not effective in removing dentinal smear layer. 3. 20-second conditioning of Dentin Conditioner (GC Inter. Corp) resulted in the removal of a significant amount of the smear layer without removing the tubular plugs and dissolving the peritubular dentin. 4. Scotchprep removed the smear layer very effectively. But at the same time it dissolved the peritubular dentin. 5. Irrespective of the uses of the silicon carbide abrasive paper or the carbide bur the morphological changes of dentin surfaces treated with the same conditioning agents were similar.

5급 와동에서 수종의 상아질 접착제에 따른 미세누출의 비교 (THE COMPARISON OF MICROLEAKAGE OF CLASS V COMPOSITE RESIN RESTORATIONS WITH VARIOUS DENTIN BONDING SYSTEMS)

  • The purpose of this study was to evaluate microleakage of six current dentin bonding systems. In this in vitro study, class V cavities were prepared on buccal and lingual surfaces of thirty extracted human molars. Each margin was on enamel and dentin/cementum. Experimental teeth were randomly divided into six groups of 5 each. Group 1 : Scotchbond Multi-Purpose; Group 2 : Single Bond; Group 3 : Prime&Bond NT ; Group 4 : Clearfil Liner Bond 2 ; Group 5 : MAC Bond II ; Group 6 : One-up Bond F. The bonding agent and composite resin were applied to class V cavities according to manufacturer's directions. After thermocycling, the specimens were immersed in 0.5% basic fuchsin dye solution for 6 hours and sectioned longitudinally through the center of the restoration with a low speed diamond saw. The degree of microleakage was measured as the extent of dye penetration under the stereomicroscope at $\times$20. The data were analyzed using one way ANOVA. When significant differences found. multiple comparisons were made using Duncan's Multiple Range Test. The results of this study were as follows: 1. In all groups, leakage value seen at the enamel margin was significantly lower than that seen at the dentin margin(P<0.001). 2. At the enamel margin, none of the dentin bonding systems used in this study showed statistically significant difference in leakage values(P<0.05). 3. At the cementum margin, group 3 showed the highest leakage value, and others were decreased as group 5, 6, 4 in that order, and group 1, 2 showed the lowest leakage value. There was statistically significant difference between group 3 and the other groups except for group 5(P<0.05).

상아질 접착제의 전단결합강도와 미세누출에 관한 연구 (A STUDY ON THE SHEAR BOND STRENGTHS AND MICROLEAKAGES OF THREE BONDING AGENTS ON DENTIN)

  • New bonding agent systems have been supplied which operators can simply apply to conditioned tooth surfaces. The purpose of this study was to evaluate the shear bond strengths and the microleakages of three bonding agents and composite resins to dentin. Seventy-five extracted human maxillary and mandibular molar teeth were used in this study. For the shear bond strength test, the entire occlusal dentin surfaces of thirty teeth were exposed with Diamond Wheel Saw and smoothed with Lapping and Polishing Machine (South Bay Technology Co., U.S.A). For the microleakage test, Class V cavities were prepared in the buccal surfaces of fourtyfive teeth. They were randomly assigned into 3 groups according to dentin bonding agents ($Scotchbond^{TM}$ Multi-Purpose plus, ONE-$STEP^{TM}$ and Prime & $Bond^{TM}$)and composite resins (Z-100, $Aelitefil^{TM}$ and TPH $Spectrum^{TM}$) to be used. Bonding agents and composite resins were bonded to exposed dentin surfaces of the tooth crown and to Class V cavities on the buccal surfaces respectively according to manufacturer's directions. The shear bond strengths were measured by universal testing machine($U^{TM}$ AGS-100, Japan). In addition, the degree of micro leakage at the occlusal and gingival margin was examined by 2 % methylene blue and stereomicroscope(Olymous SZH 10, Japan). The results were as follows: 1. The shear bond strength to dentin was the highest value in SBMP-Plus group($16.68{\pm}7.38$ MPa) and the lowest value in Prime & Bond group($11.61{\pm}5.82$ MPa), but there was no significant difference of shear bond strength among three groups. 2. The degree of microleakage at both occlusal and gingival margin was showed the lowest in SBMP-Plus group and the highest in ONE-STEP group. 3. At both occlusal and gingival margin, there was significant difference of microleakage between SBMP-Plus and ONE-STEP/ Prime & Bond groups(p<0.05), but no significant difference of microleakage between ONE-STEP and Prime & Bond group(p>0.05).

중합환경에 따른 복합레진의 물리적 성질에 관한 연구 (The effect of cavity configuration on the mechanical properties of resin composites)

  • 이 연구의 목적은 와동의 형태가 굴곡강도와 탄성계수 등 복합레진의 물리적 성질에 미치는 영향을 평가하는 것이다. 복합레진은 Clearfil$^{TM}$ AP-X(Kuraray, Japan)와 Esthet-X$^{TM}$(Dentsply, USA)가 이용되었으며, 상아질 접착제는 Clearfil$^{TM}$ SE Bond(Kuraray, Japan)와 Prime & Bond NT$^{TM}$(Dentsply, USA)를 사용하였다. 대조군의 시편은 split steel mold(25mm$\times$2mm$\times$2mm) 내에 상기 2종류의 복합레진을 충전하여 2개의 대조군 시편을 제작하였으며, 2.4 및 3.4의 C-factor를 부여하기 위한 유리 모형와동을 제작하고, 와동 내에 상기 2종류의 복합레진을 충전하기 전 유리와동의 내면은 sandblasting 처리하고 각각의 복합레진과 동일회사 제품의 상기 상아질 접착제로 처리한 후, 복합 레진을 각각 충전하여 4개의 실험군을 제작하였다. 제작된 실험군 시편은 저속 diamond saw로 충전된 복합레진 부위의 중심부를 통과하도록 절단하여 레진기둥(25mm$\times$2mm$\times$2mm)이 되도록 제작하였다. 제작된 시편을 37$^{\circ}C$의 증류수에 24시간 동안 보관 후, 만능시험기(EZ Test, Shimadzu, Japan)를 이용하여 분당 1mm의 crosshead speed로 3점 굴곡강도를 측정하였다. 또 Linometer(R&B, Korea)를 이용하여 복합레진의 중합수축량을 측정하였으며 굴곡강도측정 후 시편의 파단면은 주사전자현미경(S-2300, Hitachi, Japan)을 이용하여 관찰하였다. 실험결과의 통계분석은 95% 수준의 one-way ANOVA/Tukey's test를 이용하여 결과를 얻었다. 실험에 이용된 2종류 복합레진의 굴곡강도와 탄성계수는 C-factor치 증가에 따라 감소하였으며, 파단면 또한 C-factor의 증가에 따라 더 불규칙해지는 양상을 나타내었다. 본 실험의 결과 hybrid형 복합레진이 micro-hybrid형 복합레진에 비해 C-factor의 영향을 더 많이 받는 것으로 나타났으며, 와동의 C-factor증가가 굴곡강도나 탄성계수와 같은 복합레진의 물리적 성질을 저하시킨다는 것을 의미하였다.
