개 췌장 위낭종에서 초음파 유도 흡인 생검을 통한 진단 및 치료 2례 (Diagnosis and resolution of pancreatic pseudocysts with percutaneous ultrasound-guided aspiration in two dogs)

  • Pancreatic pseudocyst is one of the most common pancreatic mass lesions developed following acute or chronic pancreatitis. Two dogs were presented with nonspecific clinical signs such as abdominal pain, vomiting and depression and diagnosed as pancreatic pseudocysts through percutaneous ultrasound-guided aspiration. Pancreatic pseudocyst may contain pancreatic juice, so leads the deteriorate state of patient through pertinent inflammation and autodigestion of pancreas. In this study, the clinical signs and pancreatic lesion were improved after aspiration. Percutaneous ultrasound-guided aspiration can provide the diagnostic information through cytology and lipase and amylase concentration, and be selected as the first choice for treatment.

  • The continued interest in diagnostic applications of ultrasound has led to a need for more detailed knowledge of the ultrasonic properties of biological tissues. In this paper, temperature dependence of ultrasound velocity and attenuation is considered through a fundamental experiment. Acryl and Pig Liver are used as specimen. Data is collected over the temperature range of 20-50 C. As a result, we know that ultrasound velocity and attenuation coefficient are uniformly changed in proportion as specimen's temperature.

  • The difference in echogenicity between cancerous and normal tissues is not quite distinguishable in ultrasound B-mode imaging. However, tumor or cancer in breast or prostate tends to be stiffer than the surrounding normal tissue. Thus, imaging the stiffness contrast between the two different tissue types is helpful for quantitative diagnosis, and such a method of imaging the elasticity of human tissue is collectively referred to as ultrasound elasticity imaging. Recently, elasticity imaging has established itself as an effective diagnostic modality in addition to ultrasound B-mode imaging. The purpose of this paper is to present various elasticity imaging methods that have been reported up to now and to describe their principles of operation and characteristics.

Primary Angle-Closure Glaucoma in a Maltipoo Dog

  • A two-year-old spayed female Maltipoo dog was presented with a two-month duration of glaucoma in the right eye. On the first presentation, menace response and dazzle reflex were absent in the right eye, but it was present in the left eye. Slit-lamp biomicroscopy showed buphthalmia, mild corneal edema, and conjunctival and episcleral hyperemia in the right eye. The intraocular pressures were 70 mmHg and 30 mmHg in the right and left eyes, respectively. On the gonioscopic and high-resolution ultrasound examinations, both the iridocorneal angle and ciliary cleft were completely closed in the right eye. However, gonioscopy revealed an opened iridocorneal angle even with some broader base of pectinate ligament fiber, but high-resolution ultrasound showed a narrowed ciliary cleft and increased contact of the iris base with the limbal cornea in the left eye. Based on these results, a diagnosis of primary angle-closure glaucoma was made in both eyes. This case report highlighted the importance of evaluating the ciliary cleft with high-resolution ultrasound as a critical diagnostic and prognostic role in canine glaucoma.

Automated Breast Ultrasound Screening for Dense Breasts

  • Mammography is the primary screening method for breast cancers. However, the sensitivity of mammographic screening is lower for dense breasts, which are an independent risk factor for breast cancers. Automated breast ultrasound (ABUS) is used as an adjunct to mammography for screening breast cancers in asymptomatic women with dense breasts. It is an effective screening modality with diagnostic accuracy comparable to that of handheld ultrasound (HHUS). Radiologists should be familiar with the unique display mode, imaging features, and artifacts in ABUS, which differ from those in HHUS. The purpose of this study was to provide a comprehensive review of the clinical significance of dense breasts and ABUS screening, describe the unique features of ABUS, and introduce the method of use and interpretation of ABUS.

  • Nonalcoholic fatty liver disease, characterized by excessive accumulation of fat in the liver, is the most common chronic liver disease worldwide. The current standard for the detection of hepatic steatosis is liver biopsy; however, it is limited by invasiveness and sampling errors. Accordingly, MR spectroscopy and proton density fat fraction obtained with MRI have been accepted as non-invasive modalities for quantifying hepatic steatosis. Recently, various quantitative ultrasonography techniques have been developed and validated for the quantification of hepatic steatosis. These techniques measure various acoustic parameters, including attenuation coefficient, backscatter coefficient and speckle statistics, speed of sound, and shear wave elastography metrics. In this article, we introduce several representative quantitative ultrasonography techniques and their diagnostic value for the detection of hepatic steatosis.

Diagnostic Value of Ultrasound-Guided Fine Needle Aspiration Biopsy in Malignant Thyroid Nodules: Utility for Micronodules

  • Background: The diagnostic approach to thyroid nodules involves ultrasound-guided fine needle aspiration biopsy (US-FNAB). We especially aimed to evaluate the contribution and the place of US-FNAB in preoperative evaluation of the malignant cases and draw attention to discordant cases diagnosed with papillary thyroid microcarcinoma (PTMC). Materials and Methods: A total of 276 cases were retrospectively reviewed who were subsequently diagnosed with a malignancy and who underwent US-FNAB. Results: Some 45 were found to have previously undergone the US-FNAB procedure. Of the patients in whom the surgical specimen was diagnosed with a malignancy, 21 (46.7%) were diagnosed as malignant or suspicious for malignancy, and 24 (53.3%) were concluded as benign or insufficient for diagnosis. Patients with the diagnosis of PTMC outnumbering the others was a striking finding (11 cases, 24%). Conclusions: We suggest performing repeat aspiration biopsy considering sampling errors in cases where inconsistency exists between clinical findings and cytological results in thyroid nodules smaller than 10 mm in diameter and with suspicious findings on ultrasonography.

Diagnostic Value of Endorectal Ultrasound in Preoperative Assessment of Lymph Node Involvement in Colorectal Cancer: a Meta-analysis

  • Background: Nodal invasion by colorectal cancer is a critical determinant in estimating patient survival and in choosing appropriate preoperative treatment. The present meta-analysis was designed to evaluate the diagnostic value of endorectal ultrasound (EUS) in preoperative assessment of lymph node involvement in colorectal cancer. Materials and Methods: We systematically searched PubMed, Web of Science, Embase, and China National Knowledge Infrastructure (CNKI) databases for relevant studies published on or before December 10th, 2014. The sensitivity, specificity, likelihood ratios, diagnostic odds ratio (DOR) and area under the summary receiver operating characteristics curve (AUC) were assessed to estimate the diagnostic value of EUS. Subgroup analysis and meta-regression were performed to explore heterogeneity across studies. Results: Thirty-three studies covering 3,016 subjects were included. The pooled sensitivity and specificity were 0.69 (95%CI: 0.63-0.75) and 0.77 (95%CI: 0.73-0.82), respectively. The positive and negative likelihood ratios were 3.09 (95%CI: 2.52-3.78) and 0.39 (95%CI: 0.32-0.48), respectively. The DOR was 7.84 (95%CI: 5.56-11.08), and AUC was 0.80 (95%CI: 0.77-0.84). Conclusions: This meta-analysis indicated that EUS has moderate diagnostic value in preoperative assessment of lymph node involvement in colorectal cancer. Further refinements in technology and diagnostic criteria are necessary to improve the diagnostic accuracy of EUS.

  • For efficient and accurate diagnosis of ultrasound images, the time gain compensation (TGC) and dynamic range (DR) control of the ultrasound echo signal are important. TGC is for compensating the attenuation of the ultrasound echo signal along the depth, and DR is used to control the image contrast. In this paper, we propose an algorithm for finding the optimized values of TGC and DR automatically. For TGC, the degree of compensation is determined along the depth based on the effective attenuation estimation of ultrasound signal. For DR optimization, we introduce a novel cost function on the basis of the characteristics of ultrasound image, which provides the minimum value at the optimal DR. Experiments have been performed by applying the proposed algorithm to a real US imaging system. The results show that the algorithm automatically can determine the values of TGC and DR in realtime so that the subjective quality of the corresponding US image may be good enough for diagnosis.

Clinical Application of an Image-Guided Intervention in Three Dogs

  • A 9-month-old, 11.3 kg, intact, male, mixed-breed dog was referred for treatment of cor triatriatum dexter (CTD); a 5-month-old, 1.9 kg, intact, male Maltese for pulmonic stenosis (PS); and a 3-year-old, 6.62 kg, intact, female West Highland white terrier for esophageal stricture with regurgitation. A balloon catheter intervention was performed in the dog with CTD, and subsequent color Doppler ultrasound and abdominal ultrasound showed normal blood flow across the perforated membrane dividing the right atrium and the disappearance of the severe ascites present before treatment. Balloon catheter intervention in the dog with PS reduced the blood flow through the stenosis from 5.82 m/s to 3.97 m/s. In the dog with esophageal stricture, balloon catheter intervention widened the esophagus and no subsequent regurgitation was observed. Balloon catheter intervention is an interventional radiology procedure that represents a definitive treatment option for various stenotic lesions in dogs, including CTD, PS, and esophageal stricture. Although interventional radiology procedures for these diseases have already been reported, details of procedures and successful outcome have not been reported in Korea.
