개에서 농흉증의 영상의학적 접근 (Medical Imaging of Pyothorax in a Dog)

  • A 3-years-old female Pointer was evaluated for anorexia, dyspnea and exercise intolerance for 10 days. Auscultation revealed muffling of heart sound. There were leukocytosis, anemia, hypoalbuminaemia and hyperglobulinaemia on the blood profiles. Radiography showed severe pleural effusion. Thorax ultrasonographs described traped pleural effusion, pericardial effusion and collapsed right lung lobe(s). On CT images, thoracic neoplasia and lung torsion were ruled-out. Thoracocentesis with thoracoscopy and bacterial culture revealed pyothorax and pleuritis. The diagnosis of pyothorax in dogs and cats is based on clinical signs, radiography, thoracocentesis and cytology and culture of the exudate. In this case, various diagnostic medical imaging techniques (radiography, ultrasonography, CT and thoracoscopy) were used for diagnosis of pyothorax.

A Huge Intra-Abdominal Mass Associated with Long-Term Surgical Gauze Retention in a Toy Breed Dog

  • A 1.83 kg, 9-year-old, spayed female Yorkshire Terrier was referred to the Veterinary Medical Teaching Hospital of Konkuk University for evaluation of an intra-abdominal mass with a week history of vomiting and diarrhea. On physical examination, survey radiography, abdominal ultrasonography, and computed tomography, a huge firm mass was identified in the mid-caudal abdomen. Surgical exploration of the abdominal cavity was performed to remove the mass. The encapsulated mass adhered to the mesentery, tail of the spleen, small intestine, omentum, and right lobe of the pancreas was removed using blunt dissection with dry gauze and cotton swabs. Macroscopic and histopathological examination revealed that the mass was foreign-body granuloma consistent with gauze fiber. Plain abdominal radiography demonstrated no remarkable findings 8 months post-operatively. There was no evidence of vomiting, diarrhea, coughing, difficulty breathing, and cyanosis on exertion 13 months post-operatively.

치과방사선영상 검사는 위험한 술식인가? (Are dental radiographs dangerous?)

  • Radiographs can help in the diagnosis and treatment planning, but the exposure to ionizing radiation may elevate the risk of developing cancer in a person's lifetime. The objective of this review is to briefly summarize 1) radiation risk, especially cancer risks associated with diagnostic imaging, 2) linear, non-threshold (LNT) hypothesis, 3) the risks of radiation exposure to a fetus, and 4) the campaign of Image Gently. The individual risk of radiation-related cancer from any single medical imaging procedure is extremely small and it is not likely to be cancer risk at doses lower than 100 mGy, but patients may be harmed by avoiding diagnostic imaging due to fear of radiation hazard. Dentists need to understand the radiation doses delivered by various radiographic techniques and the acceptable exposure thresholds to effectively advise the patient and to reduce the unnecessary radiation

치아 우식증 진단시 필름 방사선사진상과 디지털 방사선영상의 비교:CCD, CMOS, PSP와 film (A comparison of film and 3 digital imaging systems for natural dental caries detection: CCD, CMOS, PSP and film)

  • Purpose: To evaluate the diagnostic accuracy of occlusal and proximal caries detection using CCD, CMOS, PSP and film system. Materials and Methods : 32 occlusal and 30 proximal tooth surfaces were radiographed under standardized conditions using 3 digital systems; CCD (CDX-2000HQ, Biomedysis Co., Seoul, Korea), CMOS (Schick, Schick Inc., Long Island, USA), PSP (Digora/sup (R)/FMX, Orion Co./Soredex, Helsinki, Finland) and I film system (Kodak Insight, Eastman Kodak, Rochester, USA). 5 observers examined the radiographs for occlusal and proximal caries using a 5-point confidence scale. The presence of caries was validated histologically and radiographically. Diagnostic accuracy was evaluated using ROC curve areas (Az). Results: Analysis using ROC curves revealed the area under each curve which indicated a diagnostic accuracy. For occlusal caries, Kodak Insight film had an Az of 0.765, CCD one of 0.730, CMOS one of 0.742 and PSP one of 0.735. For proximal caries, Kodak Insight film had an Az of 0.833, CCD one of 0.832, CMOS one of 0.828 and PSP one of 0.868. No statistically significant difference was noted between any of the imaging modalities. Conclusion: CCD, CMOS, PSP and film performed equally well in the detection of occlusal and proximal dental caries. CCD, CMOS and PSP-based digital images provided a level of diagnostic performance comparable to Kodak Insight film.

어린이 방사선검사 보조기구의 개발 (Development of Children's X-ray Imaging Assist Device)

  • Examination with medical radiography is the most significant advances in medicine in the 21st century. Diagnostic imaging occupies an important position in all medical subjects, surgical and internal medicine, including radiology, orthopedics, and neurosurgery. However, the invention associated with conventional radiography devices has focused only on emphasizing the efficient aspects of the examination or obtaining clearer images without considering the age and disease-induced constraints of the patient's posture. The newly developed X-ray aids to solve this problem can effectively perform X-ray imaging of children's who have difficulty communicating and controlling. It is also thought to be a good way to provide accurate imaging information and minimize radiation exposure for children.

The ability of panoramic radiography in assessing maxillary sinus inflammatory diseases

  • Purpose : To evaluate the relative diagnostic accuracy of panoramic radiography and Water's projection in maxillary sinus inflammatory diseases by comparing the radiodensities of the images with those of CT. Materials and Methods : Panoramic radiographs, Waters' projection, and CT images from 55 subjects (11O sinuses) were included in this retrospective study. The radiodensity of each maxillary sinus in panoramic radiography was recorded separately as upper and lower divided horizontally by hard palate. In Waters' projection, the overall sinus radiodensity was recorded. The CT images were considered as gold standard. Results : In panoramic radiography, 83 sinuses had same upper and lower radiodensity and 72 of these were consistent with those of CT, 26 sinuses had different upper and lower radiodensity and 15 of these, upper radiodensity was consistent with CT, the remaining 11, lower radiodensity was consistent with CT. One sinus had upper radiolucency with lower radiopacity and both were consistent with those of CT. Altogether 73 (66.4%) among 110 sinuses in panoramic radiography showed full agreement with CT, 26 (23.6%) showed partial agreement with CT. 9 sinuses had no lower image under the hard palate in panoramic radiography due to the smaller size of sinus. In Waters' projection, the radiodensity of 105 sinuses (95.5%) were consistent with that of CT. Conclusion : The panoramic radiography showed 90.0% of the sinus conditions fully or partially which may appear less accurate than that of Water's view (95.5%) but with more detailed information of the inferior part of sinuses.   • A 6-year-old intact male Doberman pinscher presented with a thin soft stool and urinary signs. On radiography, three masses were observed in the caudal abdomen. One mass was described as a large, round, mineralized mass, with an "eggshell" appearance. The second mass was located caudal to the mineralized mass, and the third mass was located between the two masses. The second and the third masses had a soft tissue density. Ultrasonography was performed to identify the features and the origins of the masses. The first mass contained a large amount of anechoic fluid and had a thin wall; it was determined to be a cyst. Strong acoustic shadowing artifact was created by the mineralized cystic wall. Caudal to the cyst, the second mass was shown to be the prostate. A hypoechoic stalk connected the prostate and the cyst, which represented the cyst as a paraprostatic cyst with mineralization. The third mass was the normal urinary bladder. The cyst was removed surgically and confirmed by histopathologic examination. This report described typical clinical findings, diagnostic imaging, and treatment of a paraprostatic cyst.

Development of an evidence-based clinical imaging diagnostic guideline for implant planning: Joint recommendations of the Korean Academy of Oral and Maxillofacial Radiology and National Evidence-based Healthcare Collaborating Agency

  • Purpose: This study was conducted to develop an evidence-based clinical imaging diagnostic guideline for implant planning, taking into account efficacy, benefits, and risks. Materials and Methods: The guideline development process employed the adaptation methodology used for Korean clinical imaging guidelines(K-CIG). Core databases(Ovid-Medline, Ovid-Embase, National Guideline Clearinghouse, Guideline International Network) and domestic databases (KoreaMed, KMbase, and KoMGI) were searched for guidelines. The retrieved articles were analyzed by 2 reviewers, and articles were selected using well-established inclusion criteria. Results: The search identified 294 articles, of which 3 were selected as relevant guidelines. Based on those 3 guidelines, 3 recommendations for implant planning were derived. Conclusion: We recommend radiography or cone-beam computed tomography (CBCT) scanning for individual patients judged to require a cross-sectional image after reading of a panoramic X-ray image and a conventional intraoral radiological image. Various steps should be taken to raise awareness of these recommendations among clinicians and the public, and K-CIG should be regularly reviewed and revised.

감염성 골질환의 핵의학 영상진단 (Nuclear Medicine Imaging Diagnosis in Infectious Bone Diseases)

  • Infectious and inflammatory bone diseases include a wide range of disease process, depending on the patient's age, location of infection, various causative organisms, duration from symtom onset, accompanied fracture or prior surgery, prosthesis insertion, and underlying systemic disease such as diabetes, etc. Bone infection may induce massive destruction of bones and joints, results in functional reduction and disability. The key to successful management is early diagnosis and proper treatment. Various radionuclide imaging methods including three phase bone scan, Ga-67 scan, WBC scan, and combined imaging techniques such as bone/Ga-67 scan, WBC/bone marrow scan add complementary role to the radiologic imaging modalities including plain radiography, CT and MRI. F-18 FDG PET imaging also has recently been introduced in diagnosis of infected prosthesis and chronic active osteomyelitis. Selection of proper nuclear medicine imaging method will improve the diagnostic accuracy of infections and inflammatory bone diseases, based on understading of pathogenesis and radiologic imaging findings.
