Foramen transversarium enlargement caused by vertebral artery tortuosity: Diagnosis with cone-beam computed tomography and magnetic resonance angiography

  • A markedly enlarged foramen transversarium was discovered incidentally on a cone-beam computed tomography scan of a 72-year-old male patient who was referred for dental implant placement. Further evaluation with magnetic resonance angiography revealed that the foramen enlargement was caused by a tortuosity in the course of the vertebral artery. This case report highlights the importance of recognizing significant incidental findings on diagnostic images and the potential need for additional imaging as part of the complete interpretative process.

Advanced performance evaluation system for existing concrete bridges

  • The management of existing concrete bridges has become a major social concern in many developed countries due to the large number of bridges exhibiting signs of significant deterioration. This problem has increased the demand for effective maintenance and renewal planning. In order to implement an appropriate management procedure for a structure, a wide array of corrective strategies must be evaluated with respect to not only the condition state of each defect but also safety, economy and sustainability. This paper describes a new performance evaluation system for existing concrete bridges. The system evaluates performance based on load carrying capability and durability from the results of a visual inspection and specification data, and describes the necessity of maintenance. It categorizes all girders and slabs as either unsafe, severe deterioration, moderate deterioration, mild deterioration, or safe. The technique employs an expert system with an appropriate knowledge base in the evaluation. A characteristic feature of the system is the use of neural networks to evaluate the performance and facilitate refinement of the knowledge base. The neural network proposed in the present study has the capability to prevent an inference process and knowledge base from becoming a black box. It is very important that the system is capable of detailing how the performance is calculated since the road network represents a huge investment. The effectiveness of the neural network and machine learning method is verified by comparing diagnostic results by bridge experts.

Objective and Subjective Voice Examination in Korean Medicine

  • Objectives: When a person speaks, voice problems usually include pain or discomfort and/or difficulties in terms of the pitch, the loudness and the quality of the voice. When patients with voice problems induced by stroke, Parkinson's disease, and systemic diseases involving the voice are examined, generally, of the Four Diagnoses (四診), a Diagnosis of Hearing can be used in current Korean medicine. The effects of acupuncture and herb medicine on voice problems have been reported for over 20 years. However, when it comes to improvements, objective and subjective evaluation methods need to be explained. Methods: Subjective methods for evaluating voice were studied through a literature search of old medicinal books containing Korean medicine diagnostics, and an objective evaluation method using Praat software is presented. Results: Korean medicine doctors analyze the patient's voice in clinical settings unconsciously on a daily basis. However, most voice diagnoses depend on the doctor's subjective evaluation. Voice qualities can be evaluated by using the Eight Principles (八綱), including Yin-Yang; the Five Elements (Phases); the Grade, Roughness, Breathy, Asthenic, Strained (GRBAS) score, and the Visual Analogue Scale (VAS) as subjective methods, and an acoustic analysis using the Praat program can be used as an objective method. Conclusion: A more complete voice examination can be achieved by using subjective and objective methods at the same time. For an objective explanation and management of patient's voice problems or systemic disorders, an objective method should be used in Korean medicine, which already has many subjective diagnostic methods. More research needs to be conducted, and more clinical evidence needs to be collected in the future.

Assesment and Diagnosis of Attention Deficit Hyperactivity Disorder(ADHD) - Focusing on Behavior Rating Scales - (주의력결핍과잉행동장애의 진단 및 평가 - 행동평정척도들을 중심으로 -)

  • Objective : This study is to investigate the method for assesment and diagnosis of ADHD, especially focusing on behavior rating scales. Methods : We searched the recent date of the publication and paper in ADHD. Results : For Assesment and Diagnosis of ADHD, various method such as interview with parents, child and teacher, behavior observation, behavior rating scales and neuropsychological test are used. The structured interview consists of the restrictive questions and response, and then have diagnostic algorithm, consequently can be used by untrained clinicians. Of the structured interview, standardization of K-SADS in Korean version is finished. Behavior rating scales, the form of parent, teacher and self-report questionnaires, are used as diagnosis and treatment evaluation of ADHD. Behavior rating scales consist of both ADHD-specific scales and broad-band scales designed to screen for various symptoms (including ADHD symptoms). ADHD-specific scales are useful in differential diagnosis, discrimination of subtype, treatment evaluation, However, broad-band scales are useful in preliminary examination. The neuropsychological tests can evaluate attention deficit and effect of attention deficit on cognitive function and academic performance. The neuropsychological tests also used in diagnosis and treatment evaluation of ADHD. Conclusion : For Assesment and Diagnosis of ADHD, various method are used, especially behavior rating scales are both useful and simple tool for diagnosis and treatment evaluation.

  • In this study aims to provide operational guidelines based on risk assessment and safety evaluation in the operation of non-traditional oil production plants, serving as alternative resources in comparison to conventional oils within the construction sector. Presently, operational facilities within these plants are active. However, integrating comprehensive management through sectional and device-specific diagnostic and evaluation systems in similar platform-based operational management systems proves to be a challenging reality. Consequently, this research introduces an evaluation methodology tailored for an operation-centric management system using an integrated management approach, and evaluates its practical applicability within the construction field.

  • Purpose : To assess the usefulness of cardiac MR imaging (MRI) in the diagnosis of acute myocardial infarction and in the assessment of myocardial viability in comparision with T1-201 SPECT. Materials and Methods : We retrospectively studied 17 patients who complained of chest pain and dyspnea with cardiac MRI . The patients were evaluated for the presence or absence of high signal intensity on T2-weighted image (T2wI), abnormal wall motion on 2D-FIESTA, perfusion defect on Gd-DTPA enhanced T1WI, and delayed myocardial enhancement on 15-minutes delay Gd-DTPA enhanced T1WI. The results were correlated with the images on T1-201 SPECT, taken at rest and stress, through which reversibility of perfusion defect was assessed. Results : Both cardiac MRI and T1-201 SPECT proved to be useful methods for diagnosing acute myocardial infarction. In order of decreasing correspondence, T2WI, T1-201 SPECT, delayed enhancement study, and wall motion images all showed significant statistical correlation with the clinical diagnosis of myocardial infarction. Perfusion MRI, on the other hand, showed no significant statistical difference was found between T1-201 SPECT and cardiac MRI. The results on T2WI showed high accordance with those on Tl-201 SPECT, while delayed myocardial enhancement and wall motion studies showed no agreement with Tl-201 SPECT. Conclusion : Cardiac MRI is useful method for diagnosis of acute myocardiac infarction. With respect to the assessment of myocardial viability, the results obtained on cardiac MRI showed high agreement with those on Tl-201 SPECT. However, further study is necessary at this point for standardization and establishment of the methods for assessing myocardial viability on cardiac MRI.

A Literatural Investigation of Diagnosis Methods and Evaluation Outcomes for the Clinical Trials on Temporomandibular Disorders (측두하악장애 임상연구에 사용된 진단 및 평가기준에 대한 문헌적 고찰)

  • Objectives The aim of this review is to provide fundamental data for temporomandibular disorders diagnosis and evaluation criteria which can be used in clinical trial. Methods We investigated the clinical studies on temporomandibular disorders via PubMed. Also, we searched domestic articles through "OASIS", "NDSL", "KISS", "Korean Traditional Knowledge Portal (KTKP)". The articles we focused on were the recent decade from 2007 to 2016. A total of 139 studies were analyzed: 42 domestic articles and 97 overseas articles. This study focuses on the diagnosis and evaluation criteria on temporomandibular disorders. Results 1) In diagnosis criteria, Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) was used 14 times in domestic articles and 63 times in overseas articles. Clinical symptoms were used 13 times in domestic articles, 17 times in overseas articles. 2) In evaluation criteria, Visual Analog Scale (VAS) was used as a pain scale 12 times in domestic articles, 63 times in overseas articles. Pressure Pain Threshold (PPT) was used 16 times in overseas articles. Whereas, no clinical trials used PPT in domestic articles. In psychological scale, Symptom Checklist-90-Revised (SCL-90-R) was used 6 times. However, SCL-90-R was used 2 times in domestic articles. ROM(Maximum Mouth Opening (MMO), Maximum Comfortable Opening (MCO) were used 24 times in overseas. In domestic articles, MMO was counted 12 times, while MCO was counted 5 times. Conclusions This research reviewed the tendency of using diagnosis methods and evaluating outcomes of the clinical on TMD. It is expected that this investigation would develop further treatment for TMD in the Korean Medicine.

  • This study is to evaluate the performance and suggest alternative plans of the public libraries in Taegu Metropolitan City. Regression analysis is used to fit a log-log equation to the total budget-output for all the public libraries. The results of the performance evaluation confirm that there is diseconomies of scale in library operations(${\Sigma}b_n$ 3,732). Therefore, all the public libraries should be improve productivity by restructuring of library identity, rational selection of physical sites and new branch library planning, userbased budget allocation and collection development, staffing and reengineering, optimalization of web site, and enforcement of service functions, etc.

  • Fault detection and diagnosis is the most important technology in condition-based maintenance(CBM) system that usually begins from collecting signatures of running machines using multiple sensors for subsequent accurate analysis. With the quick development in industry, there is an increasing requirement of selecting special sensors that are cheap, robust, and easy-installation. This paper experimentally investigated performances of four types of sensors used in induction motors faults diagnosis, which are vibration, current, voltage and flux. In addition, diagnostic effects of five popular classifiers also were evaluated. First, the raw signals from the four types of sensors are collected at the same time. Then the features are calculated from collected signals. Next, these features are classified through five classifiers using artificial intelligence techniques. Finally, conclusions are given based on the experiment results.

Pulmonary Blastoma - A Case Report (폐아세포종;1례 보고)

  • Pulmonary blastoma is a very rare, primary, malignant neoplasm of the lung. The tumor typically is large, well-defined mass and arises in the periphery of the lung. Histologically, it resembles fetal lung tissue evoking the concept that these tumors are drived from the primitive pluripotential pulmonary blastema, but controversy also exit about the nature of this tumor. Clinical evaluation is inconsistant and unreliable as to the diagnostic determination of the tumor type and degree of malignancy. Therefore, diagnosis is usually based on histologic findings. The prognosis after adequate resection appears to be better than for other malignant tumors of the lungs. We experienced a case of pulmonary blastoma occurring in 56-year-old female. She was hospitalized for evaluation of an abnormal shadow in the left lower lung field which found incidentally on chest roentgenogram. We approached the lesion though left posterolateral thoracotomy and performed the left lower lobectomy. The microscopic findings of resected specimen revealed characteristic appearance of pulmonary blastoma. The postoperative course was uneventful without complication.
