현재 불안 장애의 분류 : 타당한가? (DSM-IV Diagnostic Criteria for Anxiety Disorder: Discriminant Validity)

  • The Diagnostic and Statistical Manual 4th edition (DSM-IV) has been widely accepted and used for international classification of mental disorder. The DSM has been changed to improve diagnostic reliability and validity through descriptive and categorical approaches which was undertaken atheoretically. The authors reviewed current studies about the DSM-IV classification system and the diagnostic issues of representative categories of anxiety disorder. The authors concluded that the anxiety disorder classification system in DSM-IV has limitations such as a lack of empirical consideration for overlapping features of anxiety disorders and a lack of discriminant validity. To improve diagnostic validity and revise the current DSM-IV classification system, the authors suggested 1) more longitudinal studies for collecting empirical evidence, 2) decreasing the dependence upon operational criteria, 3) deceasing diagnostic boundary blurring, 4) developing disease specific biological diagnostic techniques and 5) continued collaboration between the DSM and International Classification of Diseases (ICD) systems.

Epidemiological investigation of equine hindlimb ataxia with Setaria digitata in South Korea

  • Background: Since 2013, the number of requests for diagnosis for horses based on neurological symptoms has increased rapidly in South Korea. The affected horses have commonly exhibited symptoms of acute seasonal hindlimb ataxia. A previous study from 2015-2016 identified Setaria digitata as the causative agent. Objectives: This study is an epidemiological investigation to find out risk factors related to the rapid increase in hindlimb ataxia of horses due to aberrant parasites in South Korea. Methods: An epidemiological investigation was conducted on 155 cohabiting horses in 41 horse ranches where the disease occurred. The surrounding environment was investigated at the disease-causing horse ranches (n = 41) and 20, randomly selected, non-infected ranches. Results: Hindlimb ataxia was confirmed in nine cohabiting horses; this was presumed to be caused by ectopic parasitism. Environments that mosquitoes inhabit, such as paddy fields within 2 km and less than 0.5 km from a river, had the greatest association with disease occurrence. Conclusions: Most horse ranches in South Korea are situated in favorable environments for mosquitoes. Moreover, the number of mosquitoes in the country has increased since 2013 due to climate change. Additional research is required; however, these data show that it is necessary to establish guidelines for the use of anthelmintic agents based on local factors in South Korea and disinfection of the environment to prevent disease outbreaks.

DSM 진단 기준을 이용한 ADHD 진단 전문가시스템 구현 (Implementation on ADHD Diagnostic Expert System based on DSM Diagnostic Criteria)

  • 본 논문에서는 ADHD 진단을 해주는 전문가시스템을 설계 및 구현한다. DSM-IV-TR을 이용하여 ADHD 진단기준을 연령대에 따라 단어를 바꾸어 구체화한다. 이 진단지를 가지고 오브젝트와 해당 값을 설정하고 규칙을 생성한다. 그리고 'ADHD 진단 시스템 엔진'과 '사용자 질의응답 프로그램'으로 구성된 진단시스템을 설계한다. 'ADHD 진단 시스템 엔진'은 규칙 기반 추론 엔진으로 Prolog 언어로 구현하여, INPUT을 '사용자 질의응답 프로그램'으로부터 받는다. INPUT에 의해 규칙은 ADHD 진단기준을 기반으로 점화되며 진단결과를 추론해서 OUTPUT을 다시 '사용자 질의응답 프로그램'으로 보낸다. '사용자 질의응답 프로그램'은 Python 언어로 구현하여 사용자와의 대화를 처리하는 인터페이스 역할을 한다. 'ADHD 진단 시스템 엔진'과 '사용자 질의응답 프로그램'의 중간다리 역할을 Pyswip 라이브러리를 통해서 수행한다. 결과적으로 ADHD 진단 전문가시스템을 통해 진단비용 절감과 간편한 이용으로 치료계획에 도움을 주고자한다.

ELM 기반의 지능형 알고리즘과 퍼지 소속함수를 이용한 유입변압기 고장진단 기법 (Diagnosis Method for Power Transformer using Intelligent Algorithm based on ELM and Fuzzy Membership Function)

  • Power transformers are an important factor for power transmission and cause fatal losses if faults occur. Various diagnostic methods have been applied to predict the failure and to identify the cause of the failure. Typical diagnostic methods include the IEC diagnostic method, the Duval diagnostic method, the Rogers diagnostic method, and the Doernenburg diagnostic method using the ratio of the main gas. However, each diagnostic method has a disadvantage in that it can't diagnose the state of the power transformer unless the gas ratio is within the defined range. In order to solve these problems, we propose a diagnosis method using ELM based intelligent algorithm and fuzzy membership function. The final diagnosis is performed by multiplying the result of diagnosis in the four diagnostic methods (IEC, Duval, Rogers, and Doernenburg) by the fuzzy membership values. To show its effectiveness, the proposed fault diagnostic system has been intensively tested with the dissolved gases acquired from various power transformers.

의료분야 진단용방사선발생장치의 품질관리 (Quality Control of Diagnostic X-ray Equipment in Medical Field)

  • 진단용방사선발생장치를 이용한 검사는 영상의학과에서 인체 내 정보를 파악하기 위한 가장 유용한 진단장비 중 하나로 신뢰도가 매우 높다. 이와 같은 이유로, 최근에는 영상의학과 영역의 진단 검사 장비를 이용한 검사 건수가 매년 증가하고 있다. 또한, 검사 건수 증가는 장치의 빠른 노화를 불러일으키고 이로 인해 장치에 대한 정도관리(quality control, QC)의 중요성이 대두된다. 특히, 진단용방사선발생장치를 이용한 검사에서 정도관리란 검사시작부터 검사가 끝날 때 까지 발생할 수 있는 모든 문제점을 사전에 알아내고 대처하여 신뢰가 확보된 영상을 획득하여 제공하는 과정에 있는 일련의 모든 활동을 의미한다. 이에 본 연구에서는 진단용방사선발생장치를 이용한 검사에서 정도관리에 대해보고 한다.

강아지에서 장외 대장균 감염에 의한 치명적 폐렴 사례 (Fatal pneumonia caused by extraintestinal pathogenic Escherichia coli in a young dog)

  • This paper describes a fatal case of pneumonia in a 14-day-old dog caused by extraintestinal pathogenic Escherichia coli (ExPEC). The necropsy showed that almost all of left lobes of the lungs had dark-red consolidation. A histopathology examination revealed moderate acute fibrino-hemorrhagic necrotizing pneumonia with intralesional bacterial colonies. Non-suppurative epicarditis, congestion in the liver, and necrosis in the white pulp of the spleen also were found. E. coli with cytotoxic necrotizing factor 1 and α-hemolysin was isolated from the lung. This case was confirmed to have fatal pneumonia caused by ExPEC that led to a systemic infection.

  • Medical institutions wishing to install and operate diagnostic radiation generators must complete appointment training within one year of appointment based on the 「Medical Act」 and the 「Rules on Safety Management of Diagnostic Radiation Generator Devices」 which will come into effect on January 1, 2024. Additionally, You must receive supplementary education every three years from the date you received it. The strengthening of safety management for diagnostic radiation generators used in medical institutions means that although the radiation exposure that may occur when using diagnostic radiation generators is low, the risk of carcinogenesis may be higher than previously evaluated. In addition, safety management of diagnostic radiation generators can be said to be an essential requirement because it has been reported that the incidence of leukemia and other diseases is increasing in diagnostic radiation tests. However, the safety management training targets and programs for radiation exposure management operated by other organizations other than diagnostic radiation generators are significantly different. In addition, since the public institutions that are responsible for radiation safety management are divided, there is a risk of duplicative, excessive, and under-administrative application to medical institutions and educational institutions that install and operate diagnostic radiation generators. Therefore, we would like to determine their consistency by comparing domestic and foreign related cases and the provisions of the 「Medical Act」 and the 「Nuclear Safety Act」.

Activation of JNKs is essential for BMP9-induced osteogenic differentiation of mesenchymal stem cells

  • Although BMP9 is highly capable of promoting osteogenic differentiation of mesenchymal stem cell (MSCs), the molecular mechanism involved remains to be fully elucidated. Here, we explore the possible involvement and detail role of JNKs (c-Jun N-terminal kinases) in BMP9-induced osteogenic differentiation of MSCs. It was found that BMP9 stimulated the activation of JNKs in MSCs. BMP9-induced osteogenic differentiation of MSCs was dramatically inhibited by JNKs inhibitor SP600125. Moreover, BMP9-activated Smads signaling was decreased by SP600125 treatment in MSCs. The effects of inhibitor are reproduced with adenoviruses expressing siRNA targeted JNKs. Taken together, our results revealed that JNKs was activated in BMP9-induced osteogenic differentiation of MSCs. What is most noteworthy, however, is that inhibition of JNKs activity resulted in reduction of BMP9-induced osteogenic differentiation of MSCs, implying that activation of JNKs is essential for BMP9 osteoinductive activity.

요부 후지낵측지 고주파 열응고술: 진단적 차단과의 연관성 (Radiofrequency Facet Joint Denervation in the Treatment of Low Back Pain: Relationship with the Diagnostic Block)

  • Background: Response to diagnostic blocks does not consistently predict the outcome of interventional facet denervation. We investigated the relationship between pain relief by the percutaneous radiofrequency denervation of the lumbar zygapophysial joints with the result of facet joint diagnostic local anesthetic injection in patients with back pain originating from the lumbar zygapophysial joint. Methods: There were 35 patients enrolled, with ranging in age from 25 to 76 years ($52.6{\pm}12.7$ years, mean ${\pm}$ SD). We studied 7 men (20%) and 28 women (80%). All patients underwent double diagnostic block of $L_{3/4}$, $L_{4/5}$ and $L_5-S_1$ facet joint with 0.5% bupivacaine. The 35 patients fell into the following group. (1) Group A (n = 16): those who felt clear relief (pain free with Likert scale) from the double diagnostic block (2) Group B (n = 19): 11 patients who were always equivocal in their response to the double diagnostic block and 8 patients who were either pain free or equivocal in their response to the double diagnostic block. All 11 patients were done the facet joint denervation. The effect on the pain was evaluated with 4 point Likert scale 1, 6 and 12 weeks after the procedure. We evaluated the relationship between the pain response to diagnostic block and the pain relief with facet joint denervation. Results: Significant correlation was observed between the response to diagnostic block and pain relief with facet denervation (P < 0.05). We found no correlation between the categories of spinal operation and pain response to facet denervation (P value > 0.05). Conclusions: A satisfactory result of lumbar facet joint denervation can be obtained in many patients, especillay in patients whose pain were relieved by the diagnostic double facet joint block. It may be said that facet joint denervation for mechanical low back pain using radiofrequency thermocoagulation is a safe, easy, and repeatable technique.
