Tethered Cord Syndrome; Surgical Indication, Technique and Outcome

  • Objective : The authors tried to reveal some unique features of lipomeningomyelocele (LMMC), including clinical presentation, factors precipitating onset of symptoms, pathologic entities of LMMC associated with tethered cord syndrome, and surgical outcome in LMMC patients. Methods : Seventy-five patients with LMMC were enrolled in this study. Neuro-imaging and intraoperative findings allowed classification of LMMC into three Types. The patients were divided into two groups by age : A (51 patients), from birth to 3 years, and B (24 patients), from 3 to 24 years. For prevention of retethering of the cord, a mega-dural sac rebuilding procedure was performed in 15 patients. Results : During a mean postoperative follow-up period of 4 years, the surgical outcome was satisfactory in terms of improved pain and motor weakness, but disappointing with reference to the resolution of bowel and bladder dysfunction. Among these 75 patients with LMMC, preoperative deficits were improved after surgery in 29 (39%), remained stable in 28 (37%), changed slightly in 13 (17%), and worsened in 5 (7%). Patients in group A achieved better outcomes than those in group B. Depending on the type of lesion, patients with types I and II LMMC have better outcomes than those with type III LMMC. Finally, retethering of the cord with neurological deterioration occurred in 4 (5.3%) of the 75 patients, but no retethering was found in the 15 patients who were recently treated with a mega-dural sac rebuilding procedure. Conclusion : Our data continue to support the opinion that early diagnosis and optimal surgery are still essential for the treatment of patients with LMMC, since there is a high likelihood of residual neurological functions that can be preserved. Based on our surgical experience of untethering and decompression of lipomas, a mega-dural sac repair is useful to prevent retethering of the cord.

Effect of Electrical Stimulation using ABR and ECochG Analysis based on Jastreboff Tinnitus Mocel (Jastreboff 이명 모델에서의 ABR과 ECochG 신호분석을 통한 전기자극의 효과)

  • Many researches have been performed whether electrical stimulation could be used for diagnosis and treatment on the auditory system impairment. Unfortunately, there were no standard methods or theoretical background for choosing stimulus conditions because of the lack of understanding on the transmission of electrical stimulation through the auditory pathway. This research was conducted to observe the effect of electrical stimulation on the tinnitus-induced animals. Nine guniea pigs were used for the experment and divided into two groups, five animals for the experimental group(A) and four animals for the control group(B). Experimental conditions were divided into four steps, before tinnitus induction and 1, 6, 12 hours after tinnitus induction using salicylate based on the Jastreboff model. In each experimental condition, ABR and ECochG were obtained, and autocorrelation coefficients were calculated from normalized waveforms based on rms values. Sum of all the autocorrelation coefficients was extracted as a parameter to observe the changes between before and after the electrical stimulation. As a result, ABR parameter values were rapidly increased 6 hours after tinnitus induction, the gradually returned back to the initial state. On the other hand, when electrical stimulation was applied, parameter values did not change compared with the initial sate. Parameter values of ECochG showed that the effect of electrical stimulation appeared 12 hours after the tinnitus induction. It was concluded that an electrical stimulation to the tinnitus-induced model changes the correlation coefficients of ABR and ECochG waveforms.

  • This study was conducted to identify the factors affecting the quality of life of cancer patients based on demographic characteristics, health status, and mental health using the results of the 6th National Health and Nutrition Survey. The research tool used the demographic characteristics, health status, mental health, and quality of life of the KNHANES VI-2. Data analysis was conducted using SAS 9.3 version. There were significant differences in the quality of life in the study results by age, gender, economic activity, individual income quartile, subjective health status, mental health status, activity limitation, stress perception, and experience of depression symptoms. Factors influencing the quality of life of cancer patients included age, sex, income quintiles(individual), subjective health status, activity limitation, and experience of depression symptoms, which together explained 39.1% of the quality of life. To improve the quality of life of cancer patients, comprehensive management is required from diagnosis to treatment to return to society. It is also necessary to develop and apply long-term programs including multidimensional approaches and improvement of quality of life, early screening of cancer and cancer prevention education, management of depression symptoms, and social support.

  • Cleidocranial dysplasia(CCD) is a congenital disorder of skeletal and dental abnormality, which is mesodermal dysfunction influencing many tissues and organs. Skeletal abnormalities in CCD are delayed closure of cranial suture and fontanelle, presence of wormian bone and clavicle aplasia. And CCD has an effect on the long bones, phalanges, spine, pelvis, muscles, and central nervous system. Dental manifestations include retention of deciduous teeth, multiple supernumerary teeth, delayed or absent eruption of permanent teeth and formation of cysts around nonerupted teeth. But as a result in common with a lack of medical and physical disability patient may have no substantive complaint, there are many masticatory and psychological problem by absent eruption of permanent teeth after exfoliation of deciduous teeth. For this reason CCD is necessary fo early diagnosis and must be improvement of the patient's appearance as well as provision of a functioning masticatory mechanism by treatment of surgical removal of supernumerary teeth, followed orthodontically eruption of the natural permanent teeth at adequate time.

Study on the Utilization of Complementary Alternative Therapy in elder Arthritics (노인 관절염 환자의 보완.대체요법 이용실태)

  • The purpose of this study was to investigate the rate of utilization, kinds and effective complementary-alternative therapy in elder arthritics, and then utilize the results as basic data for nursing intervention for elder arthritics. Study subjects consisted of 157 elder arthritics over 60 years old, data were collected through a structured questionnaire and face to face interviews. Data collection was done from July 2001 to August 2001. Subjects were sampled out from outpatients of department of rehabilitation of a university hospital in S city, outpatients of a local hospital in D city, and outpatient at public heath center in K and S city. Sexual distribution of subjects showed male 19.1% and the female 80.9%. The diagnosis distribution showed degenerative arthritis at 91% and reumatoid arthritis at 8.9% Duration of arthritics was 10 years over by 46.5%, duration of hospital treatment was 1-5 years by 41% The degree of pain by arthritis pointed out a mean point of 3.37 on a 5-point numeric scale 94.2% of subjects have experience complementary-alternative therapies used. Of the kind the subjects used, physiotherapy occupied 38.2%, Oriental medicine 36.3%, physical exercise 35.7%, nutritional therapy 22.3%, animal diet 8.9%, herbal diet 3.8%. The hardest thing due to arthritis represented disability in daily life by 59.8% and the pain problem by 30.5%. In conclusion, results of the study reveal that elder arthritics have used physiotherapy, Oriental medicine, physical exercise. Concrete strategies for nursing intervention about these complementary-alternative therapy are required to the established soon.

Assessment of Damages for Non-pecuniary Loss and Compensation for Damages in Medical Accidents - Overview for Cases of Medical Injury Relief in Korea Comsumer Agency - (의료사고의 손해배상과 위자료 산정 -한국소비자원 의료피해구제 사례들의 일별-)

  • There are two opinions on the legal characteristics of damages for non-pecuniary loss, a private sanctions theory and complementary function of damages for non-pecuniary loss, briefly. There is a close connection between the legal characteristics and the function of damages for non-pecuniary loss. The functions of damages for non-pecuniary loss are consist of satisfaction, prevention( sanctions) and complementation. Several cases of medical injury relief reported to Korea Comsumer Agency are categorized as follows, 1) cases of death after having an operation, 2) diagnosed with disability after a medical accident, 3) extended damages happening related to delayed diagnosis, 4) et cetera(a plastic surgery, a treatment with oriental medicine), and the damages for non-pecuniary loss in respect to each cases are examined. In the case of occurring death or disability, Korea Comsumer Agency has set up guidelines for assessment of damages for non-pecuniary loss by classifying into major and collateral violation for a duty of care. Furthermore, the damages for non-pecuniary loss in the case of all sorts of cancers, are assessed in accordance with the degree of responsibility subsequent to dividing cancer into good and poor prognosis. When it comes to a complementary function of damages for non-pecuniary loss in the actual work, it is hard to assess the damages as it is difficult to objectify non-pecuniary loss, such as emotional distress. Though compensation for damages is major legal characteristics of consolation money, preventing a damage(private sanctions) through consolation for a victim or sanctions against an assailant also has great significance. Therefore, it is necessary to approach flexibly for mutual agreement by considering specialty( concrete facts) of individual issue thoroughly. If considering this aging society that limits the possible age for work to 60 years old, it is needed to have a complementary function of consolation money in mind not to make it less meaningful for victims due to small sum of consolation money.

  • There are several respiratory diseases that show chronic granulomatous inflammation for the histologic finding. Among them, sarcoidosis and tuberculosis are not easy to differentiate when the clinical and radiological features present similar patterns. The increasing incidence of nontuberculous mycobacteria pulmonary disease is making it more difficult for clinicians to arrive at a proper diagnosis. A 69 year old male patient visited our hospital with chronic cough as his chief compliant. His radiologic findings were multiple enlarged mediastinal lymphadenpathies with innumerable micronodules and multiple patch infiltrations. The spleen biopsy finding showed chronic granulomatous inflammation, and Mycobacterium avium was identified on the bronchoscopic culture. Because of these findings, we treated him with drugs for nontuberculous mycobacteria disease other than sarcoidosis. However, during the treatment, his symptoms and radiological features became aggravated. Thus, we reviewed the radiologic and pathologic findings and decided to treat him with steroid, which relieved his symptoms and improved the radiologic findings. We report here on a case of sarcoidosis that was initially misdiagnosed as nontuberculous mycobacteria pulmonary disease.

The Effectiveness of Arthroscopy in Complicated Knee Arthroplasty (합병증이 병발된 슬관절 치환술에 시행한 관절경술의 효용성)

  • Purpose: To investigate the outcome of arthroscopic treatment for the complications of knee arthroplasty and to evaluate the effectiveness of arthroscopy. Materials and Methods: We analyzed 25 patients who underwent arthroscopy to treat complications of knee arthroplasty between May 1992 and June 2008. Nineteen cases out of 25 had total knee arthroplasty (TKA) and the remaining 6 cases had unicompartmental knee arthroplasty (UKA). Before arthroscopy, physical examinations and radiographic evaluations were carried out to find out the causes of complications. Joint fluid aspiration and hematologic evaluation were added when infection was suspected. Results: Among the diagnosis at the time of arthroscopy, there were 11 cases of infection, 6 cases of stiffness due to adhesion and fibrosis, and 2 cases of soft tissue impingement in the cases of TKA. Among the cases of UKA, one for each case of meniscal tear, subluxation of mobile insert, hemarthrosis, cement loose body, soft tissue impingement, and stiff knee was found. Nine out of 11 patients who had infection were treated successfully with arthroscopy but the remaining 2 cases underwent revision arthroplasty. Seven patients with arthrofibrosis had improved range of motion from 65 preoperatively to 105 postoperatively. The others also showed successful results after arthroscopy. Conclusion: Arthroscopy to treat complications after knee arthroplasty was a safe and effective method when appropriate selection of patients was made.

A Case of Naturally Acquired Cheyletiellosis in a Rabbit: Therapeutic Trial of Selamectin (애완토끼에서 발생한 Cheyletiellosis 피부염에 대한 selamectin 치료 1례)

  • An about 5-month-old, female rabbit was presented with pruritus, alopecia and mucopurulent ocular discharge. On physical examination mild to moderate scales on whole body were observed. Dermatological lesions such as alopecia, erythema, papules, pustules and crusts were observed in the eyelids, nose, inner pinna, medial sites of four feet, metacarpal and metatarsal areas, and cranial site of left stifle joint. Also, mild conjunctivitis, blepharitis and keratitis were found. For extensive dermatologic diagnostic evaluation skin scraping, tape stripping, impression smear, combing, wood' light, bacterial culture and fungal culture were performed. Finally, Cheyletiella spp. was found by combing. Many heterophils and eosinophills were appeared in impression smear. The result of fungal culture was negative. Pasteurella spp was cultured. Definitive diagnosis of Cheyletiellosis and secondary Pasteurella spp infection were established. The rabbit was treated with 6 mg/kg of selamectin topically every two weeks and restricted in cage for one month. During one month of initial treatment, clinical signs such as pruritus, alopecia, scales, papules, pustules and crusts were remarkably improved. However, in spite of good therapeutic response of selamectin the rabbit was dead suddenly at 22 days after second administration of selamectin. This case showed that selamectin was possibly effective for cheyletiellosis in rabbit; safety of selamectin for rabbit was not identified.

Design of UWB/WiFi Module based Wireless Transmission for Endoscopic Camera (UWB/WiFi 모듈 기반의 내시경 카메라용 무선전송 설계)

  • Ultra-wide-angle wireless endoscopes are demonstrated in this paper. The endoscope is composed of an ultra-wide-angle camera module and wireless transmission module. A lens unit with the ultra-wide FOV of 162 degrees is designed and manufactured. The lens, image sensor, and camera processor unit are packaged together in a 3×3×9cm3 case. The wireless transmission modules are implemented based on UWB- and WiFi-based platform, respectively. The UWB-based module can transmit HD video to a computer in resolution of 2048×1536 (QXGA) and the frame rate of 15 fps in MJPEG compression mode. The maximum data transfer rate reaches 41.2 Mbps. The FOV and the resolution of the endoscope is comparable to a medical-grade endoscope. The FOV and resolution is ~3X and 16X higher than that of a commercial high-performance WiFi endoscope, respectively. The WiFi-based module streams out video to a smart device with th maximum date transfer rate of 1.5 Mbps at the resolution of 640×480 (VGA) and the frame rate of 30 fps in MJPEG compression mode. The implemented components show the feasibility of cheap medical-grade wireless electronic endoscopes, which can be effectively used in u-healthcare, emergency treatment, home-healthcare, remote diagnosis, etc.
