  • Degradation diagnosis of XLPE insulated URD cables was accomplished through out new method, which was to be analyzed by non-electrical experiments and synthesized by degradation points. To supplement this method, It was also carried out using several electrical analyses. Tan$\delta$ had commonly a different tendency by means oftemperature and frequency and also appeared higher at the outer part rather than innerpart of insulator. PD-q increased generally in proportion to the applied voltage andshowed regular patterns in relation to the thickness of insulator. Breakdown voltageswere measured and breakdown lifetimes were predicted appling for Weibull distribution function. As a result, breakdown lifetime in failure cables was shorter up to$\fraction one-third$ times than that in general cables. It was very available to estimate cable degradation using above method, but it needs further study on XLPE insulated URD cables in order to improve reliability.

  • This paper presents an advanced estimation method for obtaining the probability density functions of a damage parameter for valve leakage detection in a reciprocating pump. The estimation method is based on a comparison of model data which are simulated by using a mathematical model, and experimental data which are measured on the inside and outside of the reciprocating pump in operation. The mathematical model, which is simplified and extended on the basis of previous models, describes not only the normal state of the pump, but also its abnormal state caused by valve leakage. The pressure in the cylinder is expressed as a function of the crankshaft angle, and an additional volume flow rate due to the valve leakage is quantified by a damage parameter in the mathematical model. The change in the cylinder pressure profiles due to the suction valve leakage is noticeable in the compression and expansion modes of the pump. The damage parameter value over 300 cycles is calculated in two ways, considering advance or delay in the opening and closing angles of the discharge valves. The probability density functions of the damage parameter are compared for diagnosis and prognosis on the basis of the probabilistic features of valve leakage.

Diagnosing the Cause of Operational Faults in Machine Tools with an Open Architecture CNC

  • The conventional computerized numerical controller (CNC) of machine tools has been increasingly replaced by a PC-based open architecture CNC (OAC) that is independent of a CNC vendor. The OAC and machine tools with an OAC have led to a convenient environment in which user-defined applications can be efficiently implemented within a CNC. This paper proposes a method of diagnosing the cause of operational faults. The method is based on the status of a programmable logic controller in machine tools with an OAC. An operational fault is defined as a disability that occurs during the normal operation of machine tools. Operational faults constitute more than 70 percent of all faults and are also unpredictable because most of them occur without any warning. To quickly and correctly diagnose the cause of an operational fault, two diagnostic models are proposed: the switching function and the step switching function. The cause of the fault is logically diagnosed through a fault diagnosis system using diagnostic models. A suitable interface environment between a CNC and developed application modules is constructed to implement the diagnostic functions in the CNC domain. The results of the diagnosis were displayed on a CNC monitor for machine operators and transmitted to a remote site through a Web browser. The proposed diagnostic method and its results were useful to unskilled machine operators and reduced the machine downtime.

  • Inherited platelet function disorders (IPFDs) are a disease group of heterogeneous bleeding disorders associated with congenital defects of platelet functions. Normal platelets essential role for primary hemostasis by adhesion, activation, secretion of granules, aggregation, and procoagulant activity of platelets. The accurate diagnosis of IPFDs is challenging due to unavailability of important testing methods, including light transmission aggregometry and flow cytometry, in several medical centers in Korea. Among several IPFDs, Glanzmann thrombasthenia (GT) is a most representative IPFD and is relatively frequently found compare to the other types of rarer IPFDs. GT is an autosomal recessive disorder caused by mutations of ITGA2B or ITGB3. There are quantitative or qualitative defects of the GPIIb/IIIa complex in platelet, which is the binding receptor for fibrinogen, von Willbrand factor, and fibronectin in GT patients. Therefore, patients with GT have normal platelet count and normal platelet morphology, but they have severely decreased platelet aggregation. Thus, GT patients have a very severe hemorrhagic phenotypes that begins at a very early age and persists throughout life. In this article, the general contents about platelet functions and respective IPFDs, the overall contents of GT, and the current status of genetic diagnosis of GT in Korea will be reviewed.

  • Background and purpose: Cardiovascular disease will undoubtedly rise along with the aging of the 'baby-boom' generation. The purpose of this study is to find the new index of the cardiovascular aging. Methods: The effects of aging on the heart and the arterial system are surveyed in the point of structure and function. Results: Arterial stiffening is due to the fatiguing effects of periodic stress on the arterial wall and is the main reason for increasing pulse wave velocity. The systolic hypertension is caused by the early return of wave reflection. The increased after-load by the arterial change leads to the development of left ventricular hypertrophy. The reduction in left ventricular compliance cause the impairments of the diastolic function. In contrast to the lower limb, aging effect in the upper limb are almost due to the ascending aortic pressure wave and the reflected wave from the lower limb. Conclusion: We have the following points. (1) The change of physiological pulse pattern by age can be explained by the early returning of reflected wave. (2) The atrial pulse in old age are generated by the left ventricular hypertrophy.

Mucopolysaccharidoses in Taiwan

  • Mucopolysaccharidoses (MPSs) are a group of rare inherited metabolic disorders caused by specific lysosomal enzyme deficiencies leading to the sequential degradation of glycosaminoglycans, causing substrate accumulation in various cells and tissues and progressive multiple organ dysfunction. The rare disease medical care team at Mackay Memorial Hospital in Taiwan has been dedicated to the study of MPSs for more than 20 years. Since 1999, more than 50 academic papers focusing on MPSs have been published in international medical journals. Topics of research include the following items regarding MPSs: incidence, natural history, clinical manifestations, gene mutation characteristics, cardiac function, bone mineral density, sleep studies, pulmonary function tests, hearing assessments, percutaneous endoscopic gastrostomy, anesthetic experience, imaging analysis, special biochemical tests, laboratory diagnostics, global expert consensus conferences, prenatal diagnosis, new drug clinical trials, newborn screening, and treatment outcomes. Of these published academic research papers, more than half were cross-domain, cross-industry, and international studies with results in cooperation with experts from European, American and other Asian countries. A cross-specialty collaboration platform was established based on high-risk population screening criteria with the acronym "BECARE" (Bone and joints, Eyes, Cardiac and central nervous system, Abdomen and appearance, Respiratory system, and Ear, nose, and throat involvement). Through this platform, orthopedic surgeons, rheumatologists, ophthalmologists, cardiologists, rehabilitation physicians, gastroenterologists, otorhinolaryngologists, and medical geneticists have been educated with regards to awareness of suspected cases of MPSs patients to allow for a further confirmative diagnosis of MPSs. Because of the progressive nature of the disease, an early diagnosis and early multidisciplinary therapeutic interventions including surgery, rehabilitation programs, symptom-based treatments, hematopoietic stem cell transplantation, and enzyme replacement therapy, are very important.

A study on the validity to make a diagnosis of Soeumin by QSCC II (Questionnaire for the Sasang Constitution Classification II) (사상체질분류검사지(四象體質分類檢査紙)(QSCC) II 에 의한 소음인(少陰人) 진단(診斷)의 타당성(妥當性) 연구(硏究))

  • The purpose of this study is to examine validity to Soeum scales by QSCC II. For this study, the objects are 336 patients who were made a diagnosis of Soeumin and 422 patients of other constitutions in Oriental Medicine & Western Medicine Cooperative Health Examination Center KyungHee University Medical Center. A systematic analysis has been made and the results are as follows. 1. We made a frequency analysis in 758 examinees' questions and concluded that 35 questions, among which 30 questions were existing and 5 were new, had statistical significance in making a diagnosis of Soeumin by more than 10% in comparison with answer frequency of other constitutions. 2. Because confidence coefficient of 0.8011 revised Soeum scales(35) rises up as compared with that of 0.6319 existing Soeum scales(51), I think revised Soeum scales reflect trait of Soeumin well and it is liable to shorten question time by controling questions. 3. Soeumin's discrimination probability in Soeumin was 55.4% by QSCCII, but that rises up to 64.3% by discriminant function made by revised Soeum scales. So there is statistical significance in discriminant function. 4. When questions in QSCCII are divided into body-spirit(體形氣像), looks-attitude(容貌詞氣), temper- talent(性質才幹), continuous mind(恒心), avarice(心慾), physiological symptoms(生理的 證) and pathological symptoms(病理的 證), response frequency and constitution discrimination probability falls in questions about avarice and pathological symptoms.

  • This paper shows an intelligent disease diagnosis system for public. Our system deals with 30 diseases and their typical symptoms selected based on the report from Ministry of Health and Welfare, Korea. Technically, the system uses a modified FCM algorithm for clustering diseases and the input vector consists of the result of user-selected questionnaires. The modified FCM algorithm improves the quality of clusters by applying symmetrically measure based on the fuzzy theory so that the clusters are relatively sensitive to the shape of the pattern distribution. Furthermore, we extract the highest 5 diseases only related to the user-selected questionnaires based on the fuzzy membership function between questionnaires and diseases in order to avoid diagnosing unrelated disease.

Evaluation of Sex and Age Factors Contributing to the Diagnosis of Oral Frailty in Community-Dwelling Older Adults

  • Background: With increasing interest in health in old age, aspects of oral aging are being considered. The Korean Academy of Geriatric Dentistry recently proposed the diagnostic criteria for oral frailty in older adults in Korea. This study aimed to conduct a cross-sectional survey of factors related to oral frailty among community-dwelling older adults and identify differences in oral frailty status according to age and sex. Methods: Among 217 older adults aged ≥60 years who visited a senior center in Wonju, 206 completed all tests for oral frailty. Among them, data from those with a Korean Version of the Modified Barthel Index score ≥90 were used in the final analysis. After evaluating oral frailty diagnostic factors such as chewing ability, occlusal force, tongue pressure, oral dryness, oral cleanliness, and swallowing function, oral hypofunction was determined according to the oral frailty diagnostic criteria. Subsequently, the evaluation results were compared based on sex and age. Results: Significant differences in chewing ability, maximum occlusal pressure, and maximum tongue pressure were observed between sexes. However, these differences did not affect oral frailty diagnosis. All diagnostic factors of oral frailty, except for the risk of oral dryness and swallowing dysfunction, showed significant differences with age. However, no significant difference was observed in the prevalence of oral frailty. Additionally, this study found no relationship between sex and oral frailty factors using the oral frailty diagnostic criteria. However, it also found that age plays a significant role as an oral frailty diagnostic indicator, in addition to oral dryness and swallowing function. Conclusion: Sex and age did not affect oral frailty diagnosis. However, patients' chewing ability, occlusal force, and tongue pressure were affected by sex and age. Therefore, sex and age should be considered when diagnosing and intervening in oral frailty in the future.

  • This article introduces the five functions of the national innovation system(NIS). As one of social systems in the national level, the five generic functions of open system - production, boundary spanning, maintenance, adaptation, management functions - are applied to the NIS. The production function is the primary process, which produces innovative products and services of the NIS. The boundary spanning function is the function of procuring input and disposing the innovation output or aiding in these processes. Experienced R&D human resources, R&D funds, technology etc. are some of the components of the input of the NIS. The maintenance function is responsible for maintaining smooth operation and upkeeping the system in terms of various conditions. The adaptation function is to help the system change and adapt, and scan the environment for problems, opportunites, and technological developments. It has outward orientation, from the long-term view for the survival of the system. The management function carries out planning and control of the overall activities for the other four functions in order to effectiving run the system as a whole. Finally, this article discusses implications of the diagnosis of the national innovation system and the decision making process of S&T policy.
