Thromboembolic Events Associated with Electrolytic Detachment of Guglielmi Detachable Coils and Target Coils : Comparison with Use of Diffusion-Weighted MR Imaging

  • Objective : The purpose of this study was to retrospectively evaluate and compare the incidence of diffusion-weighted image (DWI) lesions between the Guglielmi detachable coil (GDC) and the Target coil for treating unruptured intracranial aneurysm. Methods : From 2010 to 2011, consecutive 222 patients with an intracranial aneurysm underwent coil embolization. Inclusion criterias were : 1) unruptured intracranial aneurysm, 2) one or more GDC or Target coils used with or without other coils, 3) DWI examination within 24 hours after coiling, and 4) coiling performed without a balloon or stent. Results : Ninety patients (92 cases) met the inclusion criteria. DWI lesions were detected in 55 (61.1%) of 90 patients. In the GDC group (n=44), DWI lesions were detected in 31 (70.5%). The average number of DWI lesions was $5.0{\pm}8.7$ (mean${\pm}$SD; range, 1-40) in aneurysm-related territory. In the Target coil group (n=48), DWI lesions were detected in 24 (50.0%). The number of DWI lesion was $2.1{\pm}5.4$ (range, 1-32) in aneurysm-related territory. There was no significant correlation between a number of coils and DWI lesions. No significant differences were also observed in the number of DWI lesions in each group. Conclusion : The GDC and Target coils, which have an electrolytic detachable system, showed no differences in the incidence of DWI lesion.

탈착형 계류시스템 배치에 따른 부유식 해양구조물의 운동 및 계류성능에 관한 연구 (A Study on the Global Motion Performance of Floater and Mooring Due to Arrangement of Detachable Mooring System)

  • In this study, the global response characteristics of floater and mooring for floating offshore wind turbine with a detachable mooring system are performed. Global motion and structural response result extracted from the coupled motion analysis of 10MW DTU floating offshore wind turbine with detachable mooring system modeled by high-order boundary element model and finite element mesh, were used to study the characteristics of tension on mooring lines subjected to three different types of ocean loads. Breaking limit of mooring line characterized by wind, current and wave load has a major effect on the distribution of mooring tension found in time domain analysis. Based on the numerical results of coupled motion analysis, governing equation for calculating the motion response of a floater under ocean loads, and excitation force and surge motion and tension respectively are presented using excursion curve. It is found that the response of floater is reliable and accurate for calculating the tension distributions along the mooring lines under complex loadings. This means that the minimun breaking limit of mooring system satisfied a design criteria at ultimate ocean environmental loading condtions.

Endovascular Graft-Stent Placement for Treatment of Traumatic Carotid Cavernous Fistulas

  • Detachable balloon-based endovascular fistula occlusion is a widely accepted treatment for traumatic carotid cavernous fistulas (CCF). However, more recently coils have been used to obliterate the lesion, especially in case detachable balloon is not available. We failed balloon-assisted coil embolization for CCF because of large fistulas and herniation of coil loops into the parent artery. The authors describe our experiences of balloon-expandable graft-stents to treat CCF, and place emphasis on arterial wall reconstruction. Three traumatic CCF patients were treated using a graft-stent with/without coils, and underwent angiographic follow-up to evaluate the patency of the internal carotid artery (ICA). In all cases, symptoms related to CCF regressed after stent deployment and did not recur during follow-up. Follow-up angiography revealed good patency of the ICA in all patients. Graft-stents should be considered as an alternative means of treating CCF and preserving the parent artery by arterial wall reconstruction especially in patients with a fistula that cannot be successfully occluded with detachable balloons or coils.

비출혈을 동반한 외상성 내경동맥 가성동맥류 1례 : Detachable Coil에 의한 색전술 (A case of traumatic pseudoaneurysm of the internal carotid artery accompanying massive epistaxis: treated with detachable Coil)

  • Traumatic pseudoaneurysm of the intracavernous internal carotid artery(ICA) is extremely rare, but it is life threatening condition because of massive recurrent epistaxis. Unfortunately, the prompt diagnosis and treatment of this disease are frequently delayed due to its rarity and variable latent period, so this disease is regarded as a challenge to clinicians. Optimal therapy for this diseases demands rapid suspicion for it and is essential in order to give the best functional outcome with minimizing its morbidity and mortality. The authors present a case of male patient with traumatic pseudoaneurysm of intracavernous ICA accompanying severe epistaxis. This patient was a 37-year-old male with unilateral blindness and recurrent massive epistaxis after suffering trauma to head. Computed tomography, MRI and carotid artery angiogram showed pseudoaneurysm of intracavernous ICA with sphenoid bone fracture. The patient was effectively managed with occlusion of the pseudoaneurysmal circulation by endovascular interventional embolization technique utilizing mechanically detachable tungsten coils.

  • 본 논문에서는 현재 웨어러블 디바이스 기술을 둘러싼 이슈 중에서 가장 중요한 (1)짧은 배터리 수명, (2)스마트폰에 의존할 수밖에 없는 네트워크 범위로 인한 호환성 및 확장성 부족, (3)크기와 무게로 인한 사용자의 개성을 살릴 수 없는 디자인 문제를 해결하기 위하여 "사용자의 용도에 따라 심박, NFC, 교통카드, GPS와 같은 센서를 선택하여 장착할 수 있는 탈부착형 스마트 기기 제어 디스플레이 메인 모듈과 배터리를 장착한 액세서리 모듈로 분리 가능한 웨어러블 디바이스"를 개발한다.

  • 비접촉식 충전기에 사용되는 변압기는 비교적 큰 공극으로 인해 낮은 결합계수를 가지므로 전력전달 능력이 떨어진다. ㄷ ㅓ 형 코어로 비접촉식 충전기용 변압기를 만들면 표준형의 EE, EC, UU 코어로 만든 변압기에 비해 높은 결합계수를 얻을 수 있다. 본 논문에서는 ㄷ ㅓ 형 코어로 만든 변압기의 특성을 연구하였다. 누설 인덕턴스와 여자 인덕턴스가 미치는 영향을 정현파 구동이나 구형파 구동을 통하여 분석하였고 코어에 따른 전력 전달 특성을 비교하였다. 분석의 타당함을 확인하기 위하여 ZVS 변환기에 삽입하여 그 동작을 확인하였다. 본 논문에 사용된 분석과 비교하는 방법은 공극이 큰 PCB 변압기 등에도 적용할 수 있다.

연삭 휠 형상 복합가공시스템의 구조 안전성에 관한 연구 (A Study on Structural Safety of Integrated Machine for Grinding Wheel Forming)

  • This study evaluated the structural safety of a heavy-duty integrated machine for grinding wheel forming. Structural analysis was performed to evaluate the structural safety of the base. The base was designed by dividing the single base and detachable base. The analysis conditions were applied to the own weight and the load of component parts. From the structural analysis results, although the stress of the detachable base was decreased, the amount of deformation was increased. If the deformation of the detachable base decreases, it is expected to be safer than the single base.

Cyclic testing of short-length buckling-restrained braces with detachable casings

  • Buckling-restrained braced frames (BRBFs) are commonly used as lateral force-resisting systems in the structures located in seismic-active regions. The nearly symmetric load-displacement behavior of buckling-restrained braces (BRBs) helps in dissipating the input seismic energy through metallic hysteresis. In this study, an experimental investigation has been conducted on the reduced-core length BRB (RCLBRB) specimens to evaluate their hysteretic and overall performance under gradually increased cyclic loading. Detachable casings are used for the concrete providing confinement to the steel core segments of all test specimens to facilitate the post-earthquake inspection of steel core elements. The influence of variable core clearance and the local detailing of casings on the cyclic performance of RCLBRB specimens has been studied. The RCLBRB specimen with the detachable casing system and a smaller core clearance at the end zone as compared to the central region exhibited excellent hysteretic behavior without any slip. Such RCLBRB showed balanced higher yielding deformed configuration up to a core strain of 4.2% without any premature instability. The strength-adjustment factors for the RCLBRB specimens are found to be nearly same as that of the conventional BRBs as noticed in the past studies. Simple expressions have been proposed based on the regression analysis to estimate the strength-adjustment factors and equivalent damping potential of the RCLBRB specimens.
