노인 낙상군과 노인 비낙상군의 비교: 균형능력과 우울감 및 삶의 질 (Comparison of Elderly Fallers and Elderly Non-Fallers: Balancing Ability, Depression, and Quality of Life)

  • Fall is one of the most intimidating health conditions in elders. Comprehensive assessment is necessary to understand the individual and environmental aspects of the falls such as balancing abilities, depression, and quality of life. The purpose of this study was to compare the balancing ability, depression, and quality of life between elderly fallers and elderly non-fallers. Thirty-two community-dwelling elders (fifteen males and seventeen females between 65 and 83 years old), who have experienced fall on walking during last twelve months, were involved in the elderly fallers group. And twenty-four males and twenty-two females between 65 and 83 years old of community-dwelling elders, who have no experienced fall on walking during last twelve months, were involved in the elderly non-fallers group. Berg balance scale (BBS), timed up and go test (TUG), and functional reach test (FRT), were used to evaluate the ability of the physical balance. 'Beck depression scale in Korean' questionnaire was used to assess the depression. 'Korean version of World Health Organization Quality of Life Assessment Instrument-Bref' questionnaire was used to assess the quality of life. The results were as follows: 1) Balancing abilities measured by the BBS, TUG in the elderly fallers group were meaningfully lower than that of the elderly non-fallers group (p<.05), whereas no significant difference in the FRT was found (p>.05). 2) Depression level in the elderly fallers group was significantly higher than that of the elderly non-fallers group (p<.05). 3) Quality of life in the elderly fallers group was significantly lower than that of the elderly non-fallers group, excluding environment domain (p<.05). Therefore, in order for clinical evaluation of the community-dwelling elders those with reduced balancing ability, it is necessary to evaluate and understand the fall experience, depression, and quality of life.

노인의 사회적관계 요인이 우울 궤적에 미치는 영향 -잠재성장모형을 이용한 종단연구 (Influential Factors of Social Relation on the Change in the Depression Level of Elderly -Longitudinal Analysis using a Latent Growth Model)

  • 사회적관계 요인이 노인의 우울 수준과 밀접한 관련이 있다는 것은 선행연구를 통해 확인되고 있으나, 우울 수준 궤적에 영향을 미치는 사회적관계 하위요인에 대한 구체적인 연구는 이루어지지 않고 있다. 이러한 한계를 고려한 본 연구는 노인의 사회적관계 하위요인이 노인의 우울 궤적에 미치는 영향요인을 분석하는데 연구 목적이 있다. 연구를 위해 한국고용정보원이 제공하고 있는 고령화연구패널자료(Korean Longitudinal Survey of Ageing: KLoSA)의 3, 4, 5, 6차 자료를 활용 했으며, 65세 이상 4차시 모두 설문에 응답한 2,484명을 최종 분석대상으로 활용하였다. 65세 이상 노인의 우울 수준에 대한 종단적 연구에서 시간이 지날수록 개인별 우울 수준이 낮아지고 있어 긍정적 변화가 있음을 확인했고, 잠재성장모형의 조건부 모형을 통해 노인의 우울 궤적에 미치는 구체적인 사회적관계 요인을 분석하였다. 분석 결과, 배우자유무, 가구원수, 친한 사람과의 만남, 경제활동 유무, 종교 유무 등이 노인의 우울 수준 초기값에 영향을 미치는 요인으로 나타났으며, 가구원수, 친한 사람과의 만남, 삶에 대한 기대감 등이 노인의 우울 수준 궤적에 영향을 미치는 요인으로 나타났다. 연구결과를 통해 노인의 우울수준을 지속적으로 낮출 수 있는 구체적인 프로그램 및 지원의 필요함 등을 제언하였다.

신체활동과 우울이 지역사회 거주 노인의 수면의 질에 미치는 영향: 전기노인과 후기노인의 비교 (The Influence of Physical Activity and Depression on Sleep Quality in Community-dwelling Older Adults: A Comparison between Young-old and Old-old)

  • Purpose: The purpose of this study was to identify the influence of physical activity and depression on sleep quality among the young-old and old-old community-dwelling elderly. Methods: Participants were 216 community-dwelling older adults in Korea aged 65 or above. Data were collected using structured questionnaires with face-to-face interviews that included demographic and health-related characteristics, International Physical Activity Questionnaires (IPAQ), the Short Form Geriatric Depression Scale (SGDS) and the Pittsburgh Sleep Quality Index (PSQI). A hierarchical multiple regression was conducted to examine whether physical activity and depression would predict sleep quality under other controlled factors. Results: There were differences in demographic and health-related characteristics, physical activity, and depression by age groups, but not in sleep quality. In the young-old elderly, physical activity (${\beta}=-0.22$, p=.043) and depression (${\beta}=0.31$, p=.002) were significantly associated with sleep quality (F=4.46, p=.001, Adjusted $R^2=.16$). In the old-old elderly, physical activity (${\beta}=-0.29$, p=.001) and depression (${\beta}=0.41$, p<.001) were significantly associated with sleep quality (F=10.79, p<.001, Adjusted $R^2=.29$). Conclusion: These finding highlight physical activity and depression as important contributors to sleep quality in both young-old and old-old elderly.

노인의 스트레스와 우울 간의 관계에서 식습관의 매개 효과 분석 (Analysis of Mediating Effects of Eating Habits on the Relationship between Stress and Depression in the Elderly)

  • 배경: 노인의 식습관에 따른 스트레스와 우울 간의 상호관계를 이해하는 것은 필요하다. 목적: 노인을 대상으로 스트레스와 우울과의 관계에서 식습관의 매개 효과를 실증적으로 검증하는 것이다. 방법: 국내의 대도시 및 중소도시에 거주하는 만 60세 이상 남녀를 대상으로 Google On-line 설문지를 배포하였으며, 총 365명의 설문지를 회수하여 분석에 활용하였다. 결과: 첫째, 노인의 스트레스는 우울에 정(+)적인 영향을 미치는 것으로 나타났다. 즉 노인의 스트레스가 상승하면 우울감 또한 상승하는 것으로 나타났다. 둘째, 노인의 식습관은 스트레스와 우울 간의 관계를 부분 매개하였다. 결론: 본 연구는 노인의 스트레스와 우울 간의 관계에서 식습관의 매개 효과를 실증적으로 검증하였으며, 우울을 감소시키는 방안으로 건강한 식습관의 필요성을 확인했다는 점에 의의가 있다. 따라서 상담 기관에서는 노인의 우울 정도를 상담할 때 매일 섭취하는 식습관을 점검할 필요가 있다. 또한, 식습관은 스트레스에 의해 영향을 받기 때문에 잘못된 식습관을 지닌 노인의 경우 개인의 스트레스 지수를 파악할 필요가 있다. 더 나아가 평소 올바른 식습관을 가질 수 있도록 기관과 지역사회에서 지속적인 영양교육을 할 필요가 있다.

재가 노인의 삶에서의 음식 만족도와 우울, 고독감, 사회적지지, 생활만족도와의 관계 (The Relationships among Satisfaction with Food-Related Life, Depression, Isolation, Social Support, and Overall Satisfaction of Life in Elderly South Koreans)

  • Food is an essential component of good health and a high quality of life. This is especially true for the elderly, where health and quality of life can vary due to eating issues. This study attempts to measure the satisfaction with food-related life in the elderly of South Korea, while examining differences in social support, depression, isolation, and life satisfaction based on the level of satisfaction with food-related life. Data collection was conducted using one-on-one surveys from December 2011 to January 2012. A total of 390 elderly (aged 65 and over) who live in Korea participated in this study. The results showed that the satisfaction with food-related life was high (3.52 out of 5). The elderly who had company during a meal or a meal preparer showed a higher satisfaction with their food-related life. Social support was high (5.32 out of 7) and the feelings of isolation were low (3.85 out of 5). The average satisfaction with life was 4.42 out of 7, indicating a fair satisfaction with life, while the average depression score was relatively low (8.77 out of 26). In general, the elderly with a higher satisfaction with food-related life had a higher social support and overall satisfaction with their life. On the other hand, the elderly with a lower satisfaction with food-related life generally reported higher levels of depression and isolation. A positive correlation was observed between satisfaction with food-related life, social support, and satisfaction of life, while a negative correlation was found between satisfaction with food-related life, isolation, and depression. The results of this study indicate that managing the satisfaction with food-related life is an important component of mental health in the elderly.

요양병원 입원 노인의 우울 관련요인 (The Factors Related to Depression of the Elderly Patients in Geriatric hospital)

  • Objective : The study was designed to identify the factors related to depression of the elderly patients in geriatric hospital. Method : The subjects consisted of 198 old people who admitted in three geriatric hospital located in Daegu City. The Data was collected through interview with questionnaire from July 16 to August 16, 2012. Results : The general characteristics which significantly affected depression was perceived health status(F=18.158, p=.000). There was a slightly negative correlation between activity daily living & depression(r=-.378,p=.000) and strongly negative correlation between life satisfaction & depression(r=-.573, p=.000). Among the factors related to depression of the subjects, life satisfaction had highest expanatory power of 32.8%. Conclusion : The finding of this study provide useful information for constructing an intervention program and care for the elderly patients in geriatric hospital with depression.

우리나라 노인의 우울이 삶의 질 및 구강건강과의 관련성: 국민건강영양조사 제8기 1차년도(2019)자료 활용 (Relationship of depression with the quality of life and oral health ofthe elderly Koreans: data from the 8th National Health and Nutrition Examination Survey)

  • Background: This study was aimed at reducing depression and improving the quality of life and oral health of the elderly Koreans by elucidating the effect of depression on their life quality and oral health. Methods: Original data from the 8th National Health and Nutrition Survey (2019) were analyzed and implemented. The elderly (age≥65years) Koreans were enrolled. The oral health status was determined using the Decayed, Missing, Filled Teeth (DMFT) index and subjective self-report. The quality of life was quantified as the sum of score of each item in the Euro Qol-5 dimension (EQ-5D) and Health-related Quality of Life Instrument with 8 Items (HINT-8). The t-test, one-way analysis of variance, correlation analysis, and multiple regression analysis were performed to statistically compare the quality of life, oral health status, and oral health status according to the depression status. Results: EQ-5D and HINT-8 scores differed significantly with the history of depression, current depression, stress perception, depression for 2 weeks, and annual suicidal intention (p≤0.05). The DMFT index score differed significantly with the depression diagnosis and depression for 2 weeks (p≤0.05). EQ-5D, HINT-8, and DMFT index differed significantly with all oral health behavior variables (p≤ 0.05). Conclusions: The results of this study could serve as a basis to formulate oral health programs for the elderly to reduce depression and improve the quality of life.

재가노인의 건강행위 관련 특성, 자아존중감, 일상생활 수행능력 및 가족지지가 우울에 미치는 영향 (The Effects of Health Behavior-related Characteristics, Self-esteem, Activities of Daily Living, and Family Support on Depression in the Community-dwelling Elderly)

  • Purpose: The purpose of this study was to investigate the effects of health behavior-related characteristics, self-esteem, activities of daily living, and family support on depression in the community-dwelling elderly. Methods: The participants were 229 elders sampled from Seoul and other five provinces. A structured questionnaire was used to collect data, and data were analyzed by applying t-test, ANOVA, Pearson correlation coefficient, and stepwise multiple regression with SAS 8.12. Results: There was a negative correlation between depression, MMSE-K, self-esteem, ADL, IADL, and family support. Depression and health behavior-related characteristics showed a significant difference according to stress level and the reason of stress. Major factors that affected the elders depression were self-esteem, the amount of alcohol drinking, sleeping hours, ADL and the cognition of health status, which explained 59.4%. Conclusion: Based on the findings of this study, self-esteem and health behavior-related characteristics including alcohol drinking, sleeping hours and ADL were the influencing factors of depression in the community-dwelling elderly. Therefore, effective psychological and physical health promotion methods need to be developed and applied in nursing interventions to prevent depression in the community-dwelling elderly.

지역사회 만성질환노인의 약물사용과 우울예방 교육 프로그램 효과 (The Effects of an Education Program on the Knowledge of Medication and Prevention of Depression in the Elderly with Chronic Disease at a Local Community)

  • Purpose: This study was conducted to evaluate the effects of an education program on the knowledge of medication and prevention of depression in the elderly at a local community. Methods: This study utilized the nonequivalent control group pretest-posttest design. Thirty consecutive people were included in this study for the experimental group, and another 30 people were allocated to the control group. The program was performed once a week for 3 weeks. Data were collected from March 15 to April 30, 2010 and statistical analyses were performed by ${\chi}^2$-test and independent t-test using the SPSS/WIN 12.0 program. Results: There were statistically significant differences in the knowledge of medication, depression and medication compliance between the experimental and control groups. Conclusion: This study demonstrated that an education program for the knowledge of medication and prevention of depression in the elderly with chronic disease could improve their knowledge of medication and their medication compliance, and decrease their depression. These results suggest that education of knowledge with social psychology can be an effective and practical method of management to the elderly with chronic disease at local communities.

저소득층 재가노인의 근골격계 만성통증과 우울이 건강관련 삶의 질에 미치는 영향에 관한 연구 (The Effects of Chronic Musculoskeletal Pain and Depression on Health-related Quality of Life by Gender in Community-dwelling Older Adults)

  • Purpose: The aim of this study was to investigate the effects of chronic musculoskeletal pain and depression on health-related quality of life (HRQoL) according to gender in community-dwelling older adults. Methods: The subjects of this study were 209 elderly individuals who were receiving visiting nursing services from a public health center located in Gangwon-do. Data were collected using a structured questionnaire from March to April, 2008. SPSS/WIN 13.0 was used for data analysis. Results: Depression and HRQoL showed a significant difference between male and female subjects. There was a negative correlation between chronic musculoskeletal pain, depression, and HRQoL. In the male elderly, depression was the most significant predictor of HRQoL, while in the female elderly, chronic musculoskeletal pain was the most significant predictor of HRQoL. Conclusion: The study showed that chronic musculoskeletal pain was the variable with the highest explanatory power for HRQoL in the female elderly. Therefore, chronic musculoskeletal pain needs to be assessed and managed first in nursing interventions to improve HRQoL of the female elderly.
