척수손상환자의 우울성향에 관한 연구

  • Depression in Spinal Cord Injury Patients Kang, Soon Hee, M.Ed., R.P.T. Dept. of Rehabilitation Medicine, Kyung-Hee University Medical Center Depressions have the potential to affect adversely the rehabilitation of patients with spi-nal cord iniury. The present study examined depression, functional independence, and demographic and medical characteristics for 49 spinal cord injury(SCI) patients. This investigation involved the administration of the Beck Depression Inventory, the Modified Barthel Index, and aquestionnaire consisted of 14 items, The results were as follows .1. Compared to normal subjects, SCI patients had significantly higher BDI scores.2. When the cut-off point of HDI scores was assumed 21, 26.1% of normal subjeets and 75.5% of SCI patients appeared to be depressed. Depression in SCT patients were not related with age, sex, marital status, injury evel, severity, duration, pain, functional independence and medical charge. Depression in SCI patients were related with education level, economic level, age at injury, motivation. Depression in normal subjects were related with age, education level, economic level, but not related with sex, marital status. There were significant differences at 30 items of BDI between normal subjects and SCI patients. The items were as fellows . 1 sadness, 2 pessimism, 3 failure, 4 dissat-isfaction, 5 guilt, 6 punishment. 7 self-dislike 9 suicidal, 10 crying, 11 irritability, 12withdrawl, 13 indecisive, 14 self-image, IS work inhibition, 16 insomnia, 17 fatigue,18 anorexia, 19 weight loss, 20 hypochondria, 21 libido loss.

The Relationship between Eating Disorders, Self-esteem and Depression among in College Women (여대생의 섭식장애행동과 자아존중감 및 우울간의 관계)

  • Purpose: The purpose of this study was to investigate the relationship between eating disorder, self-esteem and depression in college women. Method: Participants were 282 college women. Data were collected between Nov. 1st and 30th, 2003, and provide basic data on eating disorder levels in college women and basic data for health control programs. The instrument was a questionnaire consisting of 8 items on general characteristics and weight control, 24 on eating disorders, 10 on self-esteem and 10 on depression. Analysis of the data was done using numbers, percentages, means and standard deviations, t-test and Pearson correlation coefficients. SPSS WIN 10.0 was used in data analysis. Result: The greatest difference for eating disorders was between the group with normal body weight and the group with low body weight (t=-6.94, p=.000). There was a high positive correlation between Body Mass Index and eating disorder (r=.383, p<.01), between eating disorder and depression (r=.161, p<.01). There was a high negative correlation between eating disorder and self-esteem (r=-.196, p<.01), and between self-esteem and depression (r=-.537, p<.01). Conclusion: These results indicate that college women need more education and counseling on dietary concerns. Also, systematic efforts to establish a more health conscious social standard for beauty should be taken. Further empirical and experimental studies are required to investigate factors influencing attitudes towards eating held by college women and to determine variables which affect various specific dimensions of these attitudes.

Investigation on Factors Affecting Adolescent Depression: Using Korean Children and Youth Panel Survey (청소년 우울증 영향요인에 관한 연구 : 아동·청소년 패널데이터 이용)

  • Objectives: This study aimed to analyze the association between adolescent depression and personal factors such as interpersonal relationship and personal characteristics. Methods: The data used for this study was taken from the 2014 Korean Children and Youth Panel Survey. Of those 2,351 subjects in a data set, data of 1,938 subjects were analyzed after excluding 413 subjects with missing information. Hierarchical regression analysis was conducted for multi-variate analysis. In addition, controlling effects of ego-resiliency was analyzed. SPSS 22.0. was utilized for statistical analyses. Results: The study found that the depressive index was higher in women than men, and lower in adolescent who has a good relationship with parents and friends. This study also found that adolescents with high ego-resiliency are more vulnerable to depression. Conclusions: It is recommended to understand and to utilize ego-resiliency of adolescents, in order to reduce adolescents depression. Promoting good relationship with parents and friends will also positively impact to lower adolescents depression.

Dietary Habits and Factors Associated with Depression in Yangpyeong-gun Elderly (양평군 노인에서 우울 지수, 영양불량과 식생활 실천 요인)

  • This study was conducted to analyze relationships between depression indices, mini nutritional assessment scores, and nutritional quotients among 80 elderly in Yangpyeong-gun and to identify factors that help prevent depression and malnutrition. Nutrition assessment scores were low in the high-risk group (PHQ-9 score ≥10), and nutritional quotient scores were lower in the high-risk group than in the normal group (PHQ-9 score ≤4). Interestingly, the consumption frequencies of fruits, eggs, and nuts were low in the high-risk group, and subjective health awareness, dental condition, and sleep were poorer. The total PHQ-9 score was correlated with malnutrition, body mass index, calf circumference, weight change, independent daily living, reduced meal amount, water intake, and the need for help when eating. Analysis of correlations between items of the PHQ-9 and nutritional status evaluation indices showed that a self-perceived feeling of depression, low energy, difficulty controlling sleep or appetite, negative thoughts (e.g., failure, disappointment), and difficulty concentrating were negatively correlated with total nutritional status scores. These results show that attention is required when food or water intake decreases and that deviation from normal sleep and appetite cycles flags the need to prepare guidelines to prevent depression.

Correction of Upper Lip Depression Using Conchal Cartilage Graft in Unilateral Cleft Lip Deformity (일측구순열변형에서 이갑개연골이식술을 이용한 상구순 함몰의 교정)

  • Purpose: To correct the upper lip depression after the correction of unilateral cleft lip, autologous grafts such as bone, dermal, fascial grafts and fat injections or alloplastic implants are used. Transplanted bones, dermis and fascia have a tendency to be absorbed and have donor morbidity. Fat injections are absorbed inconsistently and alloplastic implants have problems such as foreign body reactions, protrusions and infections. Authors corrected the upper lip depression using conchal cartilage graft in unilateral cleft lip deformity and the results was analysed with photos. Methods: 26-unilateral cleft lip and 2-microform cleft lip cases, totally 28 cases were performed. Their mean age was 21.89 years. The male and female cases were 12 and 16, respectively. Under anesthesia (general: 18 cases and local: 10 cases), cavum conchae (n=8), cymba conchae (n=16) and whole conchae (n=4) were harvested. Transversely cut the margin of the obtained cartilage, we cut out the most bent portion and put a partial-thickness incision on concave surface in cases of excessive convexity. Then, we performed the onlay graft of the conchal cartilage via scar revision site in unilateral cleft lip and via the reconstruction site of the cupid bow in microform cleft lip. The augmentation of the upper lip was evaluated with photos. Adapting the baseline connecting between the both cheilions as a horizontal standard line, we measured the highest point among the tangents between the upper lip and nose (point a), the lowest point (point c), the middle point between a and c (point b) and the vertical line from the alare (point d) to the horizontal standard line. To assess the postoperative symmetry, we compared cleft side upper lip contour index (%) A,B,C,D=(a,b,c,d)-ch ${\times}$ 100/(ch-ch) and non-cleft side upper lip contour index (%) A',B',C',D'= (a',b',c',d')-ch ${\times}$ 100 / (ch-ch).h) Results: After the surgery, no complication was found except in one case which double layers graft performed in the cleft lip deformity, the lateral portion was protruded. The upper lip contour index, the difference of A and A' were-0.83%, and thus the mild depression was persisted. Difference of B and B', C and C', D and D' were 0.83%, 1.07%, 0.90%. There were statistically significant difference, and thus the depression of upper lip were improved generally. Conclusion: Authors performed the onlay graft of the conchal cartilage in unilateral cleft lip deformity and found that the depression of the upper lip was well corrected except the uppermost part when photogrammetrically analyzed.

According to Depression Index, Difference of Response to Mental Stress and Effect to Aromatherapy (우울 지수에 따른 일과성 스트레스 및 향기요법 반응에 대한 HRV 분석)

  • Objectives : The purpose of this study is to show the differences of mental stress measured with heart rate variability (HRV) and to evaluate the effects of Aromatherapy under mental stress, according to the depression index. Methods : The volunteers were allocated into 2 groups by BDI scores. Each group is divided into 2 subgroups again. After instrumentation and 10-minutes rest period, mental stress by operation was provided for 15-minutes. HRV was recorded before and after the mental stress. Then the control groups rested for 15-minutes. The experimental groups were treated with Aromatherapy. And then HRV was recorded. Results : 1. There were no significant differences in the mean HRT and SDNN between each group. 2. LF, HF, and LF norm showed different changes in each group. 3. LF/HF ratio showed significant changes. Conclusions : These results suggest that psychic factor makes an important rule under the mental stress and the effect of Aromatherapy, and Aromatherapy is effective to recover mental stress.

Dietary Characteristics of the Elderly Living Alone from Low Income Status in Bucheon City

  • This study was performed to investigate the dietary characteristics and related factors of the elderly female living alone. The subjects were 70 single living elderly female residing in Bucheon city and receiving benefits from the government based social welfare programs. Seventy nine percent of the subjects had self reported monthly income less than 200,000 (won) and forty one percent of the subjects were paying more than 100,000 (won) for food expense. The number of side dishes for a meal was two or three (35.71 %) or one or two (31.40%); fifty seven percent of the subjects ate reheated left-over for lunch and 70.0% for dinner. The proportion of the single living elderly at high nutrition risk ($\geq$ 6 as evaluated with Nutrition Risk Index Score) was 77.1 %. Cereals and their products contributed most to the macronutrients like energy, protein and carbohydrates and vitamin B. In contrast vegetables and their products contributed most to the fiber, minerals (Ca and Fe) and vitamins (vitamin A, vitamin B and vitamin C). The mean total score of depression was 8.59${\pm}$2.69 (full score: 12) meaning that the extent of depression was serious.

Subjective Sleep Quality in Depressed and Non-Depressed Mothers During the Late Postpartum Period (산욕후기 모성의 산후우울 정도에 따른 주관적 수면의 질에 관한 연구)

  • Purpose: This study aimed to explore the subjective sleep quality of depressed and non-depressed mothers in the late postpartum period and to determine the relationship with their health promoting behaviors, family functioning, parenting stress. Method: A non-probability sample of 128 mothers completed a self-administered questionnaires at 4-6weeks postpartum. The Edinburgh postnatal Depression Scale (EPDS) and Pittsburgh Sleep Quality Index were used to measure mother's experiences of depression symptoms and sleep. Related factors of sleep quality were measured by the Korean Family Functioning Scale, Health Promoting Lifestyle Profile, and Parenting Stress Index. The data was analysed using t-test, one-way ANOVA, and the Pearson's correlation coefficients. Result: The results indicated that the depressed mothers (EPDS$\geq$ 10) had poorer sleep quality than the non-depressed mothers(EPDS < 10), reported shortened sleep duration, and experienced more daytime dysfunctions. Depressed mothers who had no job, did not drink coffee, and were primipara tended to report poorer sleep quality. There were significant correlation between poorer sleep quality and lower health promoting behaviors, higher family intimacy and lower family communication, and higher parenting stress among depressed mothers. Conclusion: Our findings support the view that depressed mothers' experiences of poor sleep are much higher than non-depressed mothers and multi-faced. Nurse professionals should screen for sleep problems in the depressed mothers with a different biopsychosocial and behavioral aspect from the non-depressed mothers in the late postpartum period.

Cytoprotective action of Rubi Fructus by modulation of Reactive Oxygen Species, peroxynitrite and $Ca^{2+}$ (복분자(覆盆子)의 세포내 ROS, $ONOO^-$ 생성 및 $Ca^{2+}$ 증가 억제에 의한 혈관내피세포 보호작용)

  • Objectives : Poststroke depression is a frequent and specific entity that impaires the rehabilliation and functional recovery of patients with hemiplegia. The author evaluated the effect of Banhahubak-tang(Banxiahoupotang) in patients with poststroke depression. Methods : 38 patients suffering from poststroke depression(determined by Diagnostic and Statistical Manual of Mental Disorders, revised. 3rd edition. and Beck Depression Inventory[BDI] cutoff $point{\geqq}10$) in Kyunghee Oriental hospital were randomized into two groups; treatment group(n=19) and control group(n=19). The treatment group was prescribed with Banhahubak-tang(Banxiahoupotang) three times a day fur a week. Control troop was prescribed with other herbal medicines used for stroke Patients three times a day for a week. Patients were evaluated by use of BDI scale, Modified Barthel Index, Depression of Ki score, Yin syndrome score, and Yang syndrome score. Among 38 patients, 24 patients got BDI scores above 21, which is the cut-off score for depression in Korean. The same procedures and assessments described above were applied. Results : Treatment group did not significantly improve compared with control group. Results yielded only slight significance (P=0.086). Especially. patients with poststroke depression as yin syndrome improved more significantly on BDI than those classified as yang syndrome. When BDI cutoff point for depression was defined as being ${\geq}\;21$, treatment group did not significantly improve compared with control group(P=0.114). However, patients with poststroke depression classified as yin syndrome were also significantly improved on BDI than those classified as yang syndrome. Conclusions : This study suggests that Banhahubak-tang(Banxiahoupotang) is significantly effective in patients with poststroke depression classified as yin syndrome.

Study on Stress, Depression, Binge Eating, and Food Behavior of High School Girls Based on Their BMI (여고생의 BMI에 따른 스트레스, 우울, 폭식 및 식행동에 관한 연구)

  • The purpose of this study was to investigate the relationships of stress, depression, binge eating and food behavior of high school girls with their degree of BMI. One hundred twenty high school girls from the Seoul metropolitan area participated in this study. Self-administrated questionnaires to search for degree of stress, depression, binge eating, and food behavior were distributed. Anthropometric assessment was carried out to calculate the body mass index(BMI). The results are as follows: The mean of age and BMI of the subjects were $17.6{\pm}0.6$ years and $21.0{\pm}2.9kg/m^2$(ranged from 15.4 to $31.7kg/m^2$), respectively. 16.7%, 65.8% and 17.5% of the subjects were classified as under weight, normal weight and overweight, respectively. The average scores of stress, depression and binge eating were $124.9{\pm}31.4$, $15.2{\pm}9.8$, $29.5{\pm}7.8$ points, respectively. The food behavior score was $11.5{\pm}2.4$ points. Their degree of stress, depression, binge eating was significantly higher in overweight high school girls compared to underweight or normal weight high school girls. There were significant correlations of BMI with stress(p<0.05), depression(p<0.001), and binge eating(p<0.01). Also there were significant correlations of food behavior with stress(p<0.05), depression(p<0.05) and binge eating(p<0.001). These results indicate that the high school girls who have higher BMI seem to have more stress, depression, and binge eating habits. Also the high school girls who have more stress, depression and binge eating habits have poor food behaviors. Therefore proper control of emotions is required to maintain desirable food behaviors in high school girls.
