Background : Nausea and vomiting associated with chemotherapy are common side effects which remain difficult to control. Acute phase nausea and vomiting (0-24 hours after induction of chemotherapy) parallels plasma serotonin release, which explains the effectiveness of $5-HT_3$ receptor antagonists. Serotonin released from gastrointestinal enterochromaffin cells may mediate chemotherapy-induced emesis. In this study, we analyzed urinary excretion of 5-HIAA, the main metabolite of serotonin. Methods : Eight men and four women were studied in their cisplatin chemotherapy cycle. Urinary 5-hydroxyindoleaoetic aicd (HIAA) levels were determined before and during a 24-hour period under ondansetron prophylaxis. Results : Urinary 5-HIAA excretion for a 24-hour period was increased in all patients after induction of cisplatin (P=0.002). Conclusion : Cisplatin chemotherapy is associated with serotonin release in the acute phase. Our finding may provide evidence for a relationship between emesis and serotonin following cisplatin chemotherapy.

Streptococcus parasanguinis is a Viridans group bacteria that is most often discovered in the oral cavity and causes dental plaque and endocarditis in a rat model. It has low virulence but an unknown relationship to human respiratory infections. We report on a 61-year-old woman who developed hemoptysis followed by pleuritic chest pain after conscious sedation during a gastroscopic polypectomy and was diagnosed with pneumonia and parapneumonic effusion from Streptococcus parasanguinis isolated in pleural fluid. Microaspiration during the procedure was presumed to play a role in the pathogenesis.

기관지폐쇄에 의한 폐농양 혹은 공동성 병변에 발생한 암성 농양은 임상 양상 및 방사선학적 소견이 흡인성 폐농양과 흡사하여 감별이 어려워서 폐암을 조기 진단할 수 있는 기회를 놓치기 쉽다. 따라서 항생제에 대한 반응이 뚜렷하지 않거나 선행 요인이 없고 비전형적인 위치에 발생한 폐농양의 경우 악성 병변의 동반 가능성을 염두에 두고 적극적인 검사가 필요하다. 저자들은 발열, 기침, 혈담, 호흡곤란을 주소로 내원하여 폐농양 의심하에 항생제 치료에도 불구하고 병변이 악화된 67세 남자 환자에서 간세침흡인생검을 통해 다발 전이성 농양을 일으킨 편평상피세포암성 농양으로 진단된 예가 있어 문헌 고찰과 함께 보고하는 바이다.

Endobronchial tuberculosis is defined as a tuberculous infection of the tracheobronchial tree and has a prevalence of up to 50% in active pulmonary tuberculosis cases. The most common complication of endobronchial tuberculosis is bronchial stenosis; benign fistula formation by endobronchial tuberculosis is rare, especially inter-bronchial fistula formation. We reported a rare case of a 73-year-old woman with a fistula between the right upper bronchus and bronchus intermedius. A diagnosis of inter-bronchial fistula caused by endobronchial tuberculosis was based on the results of chest computed tomography scans, bronchoscopy, and microbiological and pathological tests. The patient was treated with anti-tuberculous medication, and her symptoms gradually improved.

The following case studies illustrate the application of oriental medical treatment of patients diagnosed with neurogenical bladder. The outstanding symptoms of two cases were general weakening and voiding difficulty, urinary retention and lack of urine sense. In one case, neurogenical bladder was caused by an operation on the femur, and in the other case, by diabetes and anemia. Western medical treatment didnot have any effect in these cases, but after oriental medical treatment, self-voiding was possible. These resu1t suggest that oriental treatment should be considered for patients with neurogenical bladder.

A Case of dizziness treated with Singi-hwan is reported. Singi-hwan is an herbal formula historically used for Jing(精) deficiency in Traditional Korean Medicine and dizziness is a common symptom presented in patients with Jing(精) deficiency. An elderly patient diagnosed with senile dizziness was treated using Singi-hwan for ten consecutive days in Dunsan Oriental Hospital, and has shown positive outcome.
