Molar-Incisor Malformation: Three Cases of a Newly Identified Dental Anomaly (Molar-Incisor Malformation: 최근 확인된 치아 이상의 증례 보고)

  • Molar-incisor malformation (MIM) is a recently described dental anomaly characterized by root malformation in permanent first molars accompanied sometimes by abnormal root forms in primary second molars or enamel defects in maxillary central incisors. This report presents three cases of MIM along with a review of previous studies. Three patients exhibited abnormal root forms in the permanent first molars, with varying degrees of deformation. Two of the patients experienced medical events at birth. One of the patients was a monozygotic twin, whose twin sister exhibited normal dentition without any significant abnormalities. The present report also reviews recently reported cases of MIM in literature. In the management of MIM-associated clinical issues, consideration of microscopic features and accompanying characteristics might facilitate early diagnosis and comprehensive treatment planning.

Successful Treatment Using Wire-reinforced Interdental Splint for a Puppy with Rostral Mandibular Fractures

  • A five-month-old toy poodle was referred for treatment of bilateral mandibular fractures. Preoperative skull and dental radiographs showed rostral mandibular fractures and permanent teeth that had not yet erupted. In addition to providing stability for the healing of fractures, it is very important to focus on restoring normal occlusion. The mandibular fractures were repaired using interdental wiring in a dentate area where the deciduous canines and fourth premolars were stable and could be used to anchor the interdental fixation. In the absence of an erupted permanent mandibular first molar, the modified Risdon wiring was performed using the deciduous mandibular fourth premolars as anchor teeth. An intraoral splint was placed on the mandibular teeth using self-curing composite resin. Five months after surgery, the patient had experienced no complications, and the permanent teeth had erupted normally. In this case, which involves a small-breed puppy with deciduous dentition, treatment with a wire-reinforced interdental splint using intact deciduous fourth premolar teeth as anchor teeth can be considered as a suitable method for rostral mandibular fracture restoration.

  • When a delayed eruption of tooth is diagnosed, the causative factor should be detected before an establishment of treatment plan, if possible. Although a panoramic radiograph is enough to evaluate the position of tooth and the stage of tooth development, a 3-D Dental CT would be a powerful tool to reveal a spatial relationships between objects. The reported case showed a delayed eruption of lower left permanent canine and a mini-plate with screws adjacent to the impacted canine. Although the screws adjacent to the root of impacted tooth showed a close proximity, it was not presumed that these screws would interfere the eruption of the tooth. The impacted canine did not show any spontaneous eruption during observation. After the mini-plate and screws were removed, an orthodontic traction using elastic power chain was performed. The position of mini-plates and screws should be carefully designed to avoid damaging the tooth follicles or tooth roots in the jaw. Also the screws should be removed before an orthodontic traction to prevent damaging the root surface of impacted tooth.

  • Odontomas generally appear as small, solitary, or multiple radio-opaque lesions found on routine radiographic examinations. It is a comparatively common odontogenic tumor, and may lead to interfere with the eruption of its associated tooth. In general, odontomas occur more often in permanent dentition and are very rarely associated with primary teeth. This report deals with five rare cases of primary teeth impaction associated with odontomas, with spontaneous eruption occurring in all five cases after simple surgical removal of the odontoma. Impacted primary teeth may be associated with defects in development and eruption of their permanent successors, and thus long-term observation is necessary until the permanent successors erupt.

  • Numerous challenges in treating permanent dentition are encountered, especially in treating immature teeth with pulp necrosis. Historically, calcium hydroxide application during long periods of time was used to induce the formation of a calcific barrier across the open apex. In 2004, a new treatment modality for the management of the open apex was introduced. This treatment was named as 'revascularization' and gained acceptance among dentists. The protocol was different from the traditional apexification techniques in that the canal was irrigated and disinfected with a combination of three antibiotics (ciprofloxacin, metronidazole and minocycline). At the next appointment, bleeding was induced and the canal was sealed with MTA. Successful regenerative endodontic treatment of necrotic immature permanent teeth can provide continued root development, increased thickness in the dentinal walls and apical closure. These developments of a functional pulp-dentin complex have a promising impact on retaining the natural teeth, the goal of the dental health care.

Pressure Root Resorption of the Second Molar Caused by Third Molar Impaction: A Case Report of Severely Resorbed Root with Vital Pulp

  • Pressure root resorption can be observed during the eruption of permanent dentition, especially of the maxillary canines (affecting lateral incisors) and mandibular third molars (affecting mandibular second molars). Since the cause of root resorption of the adjacent affected teeth is evident, treatment simply involves extraction of the impacted tooth. However, there have been few reports on the prognosis of the remaining resorbed tooth, as dentists often choose to extract them when damage due to root resorption is observed. We report a case involving a tooth that was severely resorbed due to pressure from an adjacent impacted tooth. After extraction of the impacted tooth, the remaining tooth retained vital pulp and survived as a functional tooth.

  • The author have observed a case of supernumerary tooth occured between the maxillary right primary central Incisor and lateral incisor. Supernumerary permanent tooth is a common clinical finding, but supernumerary primary tooth is very rare. Supernumerary tooth result from aberrations in the initiation or proliferation period of the life cycle of the tooth.

  • Very few reports are available on the occurrence of abnormality of both enamel and dentin. This case has some characteristiced of both amelogenesis imperfecta and dentinogenesis imperfecta. Clinically, the enamel of primary dentition was completely absent and when the permanent teeth came to the pediatric dept. of Dankook University Dental Hopital for treatment. Fixed-removable type resin plate was delivered to increase vertical dimension and to solve esthetic and functional defects. Hypoplastic teeth were restored either stainless steel crown or composite resin restoration or both. The periodic recall check and oral hygiene education are recommended.

  • Horizontal root fractures are defined as those that involve cementum, dentin and pulp, comprising from 0.5 to 7% of injuries in permanent dentition on dental trauma. Diagnosis is made through clinical findings and radiographic exams, the latter frequently being limited by the position of the fracture line. Treatment varies according to the displacement of crown portion and vitality of the fragments. Authors describe clinical cases of maxillary right central incisor with horizontal root fractures in dental clinic, Jeju National University Hospital.

  • 1,620 patients who visited our department from 1966 to 1977.7.15, were surveyed on the yearly tendency of patient distribution and the state of Angle's clssification. 1. There was increased visiting rate of patint a year. Patient distribution who visited in our department in the year 1976 was five 샤든 than that in the year 1966. 2. 10-15 age group was 41% in total visiting patient and 20 age over group was 20.4%. 3. Permanent dentition group was 66.2% among three dentitions but it was showedthe tendency that age group is lower year by year. 4. Class I malocclusion was 51.1% in total visiting patient, moreclass III maloulsion was prevalent than class II malocclusion.
