Effectiveness of endodontic retreatment using WaveOne Primary files in reciprocating and rotary motions

  • Objectives: This study evaluated the efficiency of WaveOne Primary files (Dentsply Sirona) for removing root canal fillings with 2 types of movement: reciprocating (RCP) and continuous counterclockwise rotation (CCR). Materials and Methods: Twenty mandibular incisors were prepared with a RCP instrument (25.08) and filled using the Tagger hybrid obturation technique. The teeth were retreated with a WaveOne Primary file and randomly allocated to 2 experimental retreatment groups (n = 10) according to movement type: RCP and CCR. The root canals were emptied of filling material in the first 3 steps of insertion, until reaching the working length. The timing of retreatment and procedure errors were recorded for all samples. The specimens were scanned before and after the retreatment procedure with micro-computed tomography to calculate the percentage and volume (mm3) of the residual filling material. The results were statistically evaluated using paired and independent t-tests, with a significance level set at 5%. Results: No significant difference was found in the timing of filling removal between the groups, with a mean of 322 seconds (RCP) and 327 seconds (CCR) (p < 0.05). There were 6 instrument fractures: 1 in a RCP motion file and 5 in continuous rotation files. The volumes of residual filling material were similar (9.94% for RCP and 15.94% for CCR; p > 0.05). Conclusions: The WaveOne Primary files used in retreatment performed similarly in both RCP and CCR movements. Neither movement type completely removed the obturation material, but the RCP movement provided greater safety.

Buckling resistance, torque, and force generation during retreatment with D-RaCe, HyFlex Remover, and Mtwo retreatment files

  • Objectives: This study compared the buckling resistance of 3 nickel-titanium (NiTi) retreatment file systems and the torque/force generated during retreatment. Materials and Methods: The buckling resistance was compared among the D-RaCe (DR2), HyFlex Remover, and Mtwo R25/05 retreatment systems. J-shaped canals within resin blocks were prepared with ProTaper NEXT X3 and obturated by the single-cone technique with AH Plus. After 4 weeks, 4 mm of gutta-percha in the coronal aspect was removed with Gates-Glidden drills. Retreatment was then performed using DR1 (size 30, 10% taper) followed by DR2 (size 25, 4% taper), HyFlex Remover (size 30, 7% taper), or Mtrwo R25/05 (size 25, 5% taper) (15 specimens in each group). Further apical preparation was performed with WaveOne Gold Primary. The clockwise torque and upward force generated during retreatment were recorded. After retreatment, resin blocks were examined using stereomicroscopy, and the percentage of residual filling material in the canal area was calculated. Data were analyzed using 1-way analysis of variance with the Tukey test. Results: The HyFlex Remover files exhibited the greatest buckling resistance (p < 0.05), followed by the Mtwo R25/05. The HyFlex Remover and Mtwo R25/05 files generated the highest maximum clockwise torque and upward force, respectively (p < 0.05). The DR1 and DR2 files generated the least upward force and torque (p < 0.05). The percentage of residual filling material after retreatment was not significantly different between file systems (p > 0.05). Conclusions: NiTi retreatment instruments with higher buckling resistance generated greater clockwise torque and upward force.

Composition and Diversity of Salivary Microbiome Affected by Sample Collection Method

  • Purpose: The purpose of this study was to investigate whether various saliva collection methods affect the observed salivary microbiome and whether microbiomes of stimulated and unstimulated saliva and plaque differ in richness and diversity. Methods: Seven sampling methods for unstimulated saliva, stimulated saliva, and plaque samples were applied to six orally and systemically healthy participants. Bacterial 16S ribosomal RNA genes of 10 major oral bacterial species, namely, Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Fusobacterium nucleatum, Prevotella intermedia, Prevotella nigrescens, Streptococcus mitis, Streptococcus sobrinus, and Lactobacillus casei, were analyzed by real-time polymerase chain reaction. We comprehensively examined the dependence of the amount of bacterial ribosomal DNA (rDNA), bacterial-community composition, and relative abundance of each species on sample collection methods. Results: There were significant differences in the bacterial rDNA copy number depending on the collection method in three species: F. nucleatum, P. nigrescens, and S. mitis. The species with the highest richness was S. mitis, with the range from 89.31% to 100.00%, followed by F. nucleatum, P. nigrescens, T. denticola, T. forsythia, and P. intermedia, and the sum of the proportions of the remaining five species was less than 1%. The species with the lowest observed richness was P. gingivalis (<0.1%). The Shannon diversity index was the highest in unstimulated saliva collected with a funnel (4.449). The Shannon diversity index was higher in plaque samples (3.623) than in unstimulated (3.171) and stimulated (3.129) saliva and in mouthwash saliva samples (2.061). Conclusions: The oral microbial profile of saliva samples can be affected by sample collection methods, and saliva differs from plaque in the microbiome. An easy and rapid technique for saliva collection is desirable; however, observed microbial-community composition may more accurately reflect the actual microbiome when unstimulated saliva is assayed.

Retrospective clinical and radiographic evaluation of restored endodontically treated teeth

  • Objectives: The aim of this study was to perform a clinical and radiographic analysis of endodontically treated teeth (ETT) restored with cast metal posts (CMPs) or prefabricated glass fiber posts (GFPs) and crowns. Materials and Methods: Fifty ETT were restored with 25 CMPs and 25 GFPs at a private dental clinic between 2001 and 2016. The restorations consisted of 12 all-ceramic crowns, 31 metal-ceramic crowns, and 7 composite resin crowns. Demographic data, type of teeth, type of post-and-core system, time of placement, crown restorations, the number of proximal contacts, the type of antagonist, and reports of any complications after post-and-core placement were recorded for each patient. Assessments were performed at baseline (radiographic) and follow-up (radiographic and clinical). Data were analyzed by the McNemar test, the Pearson χ2 test, and Kaplan-Meier survival curves (α = 0.05). The mean follow-up was 67.6 months. Results: No significant difference was observed for any of the radiographic parameters when the baseline and final radiographs were compared. In the clinical evaluation, anatomical form (p = 0.009) and occlusion (p = 0.001) showed significant differences according to the type of crown restoration; specifically, metal-ceramic and all-ceramic crowns outperformed composite resin crowns. Conclusions: CMPs and GFPs showed favorable results for restoring ETT after 6 years of follow-up. All-ceramic and metal-ceramic crowns showed higher survival rates and better clinical outcomes.

Correlation between different methodologies used to evaluate the marginal adaptation of proximal dentin gingival margins elevated using a glass hybrid

  • Objectives: This study aimed to evaluate the effect of aging on the marginal quality of glass hybrid (GH) material used to elevate dentin gingival margins, and to analyze the consistency of the results obtained by 3 in vitro methods. Materials and Methods: Ten teeth received compound class II cavities with subgingival margins. The dentin gingival margins were elevated with GH, followed by resin composite. The GH/gingival dentin interfaces were examined through digital microscopy, scanning electron microscopy (SEM) using resin replicas, and according to the World Dental Federation (FDI) criteria. After initial evaluations, all teeth were subjected to 10,000 thermal cycles, followed by repeating the same marginal evaluations and energy dispersive spectroscopy (EDS) analysis for the interfacial zone of 2 specimens. Marginal quality was expressed as the percentage of continuous margin at ×200 for microscopic techniques and as the frequency of each score for FDI ranking. Data were analyzed using the paired sample t-test, Wilcoxon signed-rank test, and Pearson and Spearmen correlation coefficients. Results: None of the testing techniques proved the significance of the aging factor. Moderate and strong significant correlations were found between the testing techniques. The EDS results suggested the presence of an ion-exchange layer along the GH/gingival dentin interface of aged specimens. Conclusions: The marginal quality of the GH/dentin gingival interface defied aging by thermocycling. The replica SEM and FDI ranking results had stronger correlations with each other than either showed with the digital microscopy results.

지대치삭제의정확도가레이져신터링기술로제작된Co-Cr 코핑의변연적합도에미치는영향 (Influence of the accuracy of abutment tooth preparation on the marginal adaptation of Co-Cr alloy copings fabricated with a selective laser sintering technology)

  • 목적: 본 연구의 목적은 지대치 삭제의 정확도와 metal coping의 변연적합도와의 상관관계를 알아보는 데 있다. 이를 통해, 세 가지 다른 제작 방식 (주조, milling, 기법)이 이 상관관계에 미치는 영향을 비교해 보고자 한다. 재료 및 방법: 두 개의 master medel을 서로 다른 방식으로 제작하였다; 첫 번째 모델은 치과의사에 의해 deep chamfer margin을 가지도록 지대치 삭제된 것이고, 두 번째 모델은 3-D designing software program을 이용하여 동일한 삭제원칙에 따라 제작되었다. 각각의 모델에 대하여 세 가지 제작 방식으로 코발트-크롬 코핑을 12개씩 제작하여, 총 72개의 시편을 얻었다. 각 시편을 master model상에 적합시키고 공초점 레이저 주사 현미경으로 변연적합도를 측정하였다. 결과: SLS system (P=.0231)과 주조방식(P<.0001)에서는 computer designed model이 hand prepared model에 비하여 유의하게 우수한 변연적합도를 보였다. 그러나 milling group에서는 두 모델 간에 유의한 차이를 나타내지 않았다(P=.9962). 결론: 본 연구에 한하여, 지대치 삭제의 정확도가 금속 코핑의 변연적합도에 미치는 영향은 그 제작 방식에 따라 달랐다. 제작 방식에 따른 변연적 합도는 SLS system이 가장 우수하였고 지대치 삭제의 정확도에 의해 영향을 받았다. 한편, milling 방식은 세 가지 방식 중 가장 큰 변연 격차를 나타내었으며 지대치 삭제의 정확도에 영향을 받지 않았다.

  • Purpose: Rehabilitation of the edentulous posterior maxilla with dental implants often poses difficulty because of insufficient bone volume caused by pneumatization of the maxillary sinus and by crestal bone resorption. Sinus grafting technique was developed to increase the vertical height to overcome this problem. The present study was designed to evaluate the sinus floor augmentation with anorganic bovine bone (Bio-$cera^{TM}$) using histomorphometric and clinical measures. Patients and methods: Thirteen patients were involved in this study and underwent total 14 sinus lift procedures. Residual bone height was ${\geq}2mm$ and ${\leq}6mm$. Lateral window approach was used, with grafting using Bio-$cera^{TM}$ only(n=1) or mixed with autogenous bone from ramus and/or maxillary tuberosity(n=13). After 6 months of healing, implant sites were created with 3mm diameter trephine and biopsies taken for histomorphometric analysis. The parameters assessed were area fraction of new bone, graft material and connective tissue. Immediate and 6 months after grafting surgery, and 6 months after implantation, computed tomography (CT) was taken and the sinus graft was evaluated morphometric analysis. After implant installation at the grafted area, the clinical outcome was checked. Results: Histomorphometry was done in ten patients.Bio-$cera^{TM}$ particles were surrounded by newly formed bone. The graft particles and newly formed bone were surrounded by connective tissue including small capillaries in some fields. Imaging processing revealed $24.86{\pm}7.59%$ of new bone, $38.20{\pm}13.19%$ connective tissue, and $36.92{\pm}14.51%$ of remaining Bio-$cera^{TM}$ particles. All grafted sites received an implant, and in all cases sufficient bone height was achieved to install implants. The increase in ridge height was about $15.9{\pm}1.8mm$ immediately after operation (from 13mm to 19mm). After 6 months operation, ridge height was reduced about $11.5{\pm}13.5%$. After implant installation, average marginal bone loss after 6 months was $0.3{\pm}0.15mm$. Conclusion: Bio-$cera^{TM}$ showed new bone formation similar with Bio-$Oss^{(R)}$ histomorphometrically and appeared to be an effective bone substitute in maxillary sinus augmentation procedure with the residual bone height from 2 to 6mm.

타이타늄의 표면개질에 따른 도재 결합 특성 (THE BOND CHARACTERISTICS OF PORCELAIN FUSED BY TITANIUM SURFACE MODIFICATION)

  • Statement of problem: Titanium is well known as a proper metal for the dental restorations, because it has an excellent biocompatibility, resistance to corrosion, and mechanical property. However, adhesion between titanium and dental porcelains is related to the diffusion of oxygen to the reaction layers formed on cast-titanium surfaces during porcelain firing and those oxidized layers make the adhesion difficult to be formed. Many studies using mechanical, chemical and physical methods to enhance the titanium-ceramic adhesion have been actively performed. Purpose: This study meant to comparatively analyse the adhesion characteristics depending on different titanium surface coatings after coating the casts and wrought titanium surfaces with Au and TiN. Material and method: In this study, the titanium specimens (CP-Ti, Grade 2, Kobe still Co. Japan) were categorized into cast and wrought titanium. The wrought titanium was cast by using the MgO-based investment(Selevest CB, Selec). The cast and wrought titanium were treated with Au coating($ParaOne^{(R)}$., Gold Ion Sputter, Model PS-1200) and TiN coating(ATEC system, Korea) and the ultra low fusing dental porcelain was fused and fired onto the samples. Biaxial flection test was done on the fired samples and the porcelain was separated. The adhesion characteristics of porcelain and titanium after firing and the specimen surfaces before and after the porcelain fracture test were observed with SEM. The atomic percent of Si on all sample surfaces was comparatively analysed by EDS. In addition, the constituents of specimen surface layers after the porcelain fracture and the formed compound were evaluated by X-ray diffraction diagnosis. Result: The results of this study were obtained as follows : 1. The surface characteristics of cast and wrought titanium after surface treatment(Au, TiN, $Al_2O_3$ sandblasting) were similar and each cast and wrought titanium showed similar bonding characteristics. 2. Before and after the biaxial flection test, the highest atomic weight change of Si component was found in $Al_2O_3$ sandblasted wrought titanium(28.6at.% $\rightarrow$ 8.3at.%). On the other hand, the least change was seen in Au-Pd-In alloy(24.5at.% $\rightarrow$ 9.1at.%). 3. Much amount of Si components was uniformly distributed in Au and TiN coated titanium, but less amount of Si's was unevenly dispersed on Al2O3 sandblasting surfaces. 4. In X-ray diffraction diagnosis after porcelain debonding, we could see $Au_2Ti$ compound and TiN coating layers on Au and TiN coated surfaces and $TiO_2$, typical oxide of titanium, on all titanium surfaces. 5. Debonding of porcelain on cast and wrought titanium surface after the biaxial flection is considered as a result of adhesion deterioration between coating layers and titanium surfaces. We found that there are both adhesive failure and cohesive failure at the same time. Conclusion: These results showed that the titanium-ceramic adhesion could be improved by coating cast and wrought titanium surfaces with Au and TiN when making porcelain fused to metal crowns. In order to use porcelain fused to titanium clinically, it is considered that coating technique to enhance the bonding strength between coating kKlayers and titanium surfaces should be developed first.

티타늄에 대한 레진과 도재의 결합 강도에 관한 연구 (The study on the shear bond strength of resin and porcelain to Titanium)

  • 연구목적: 최근 임플란트 상부보철물의 주재료로서 티타늄의 수요가 증가하고 있고, 급속도로 발전하고 있는 CAD/CAM (computer - aided design/computer-aided manufacturing) 기술이 접목되어 티타늄을 절삭하여 제작하는 방법이 주목을 받고 있으며 치과 임상에서 점점 그 영역이 넓어지고 있다. 다만, 하나의 티타늄괴를 절삭하여 만드는 방법의 특성상 기계적 유지력을 얻을 수 있는 비드 등을 형성할 수 없고, 통상적인 재료인 금 합금이나 도재용 합금 주조체에 비해 도재와의 결합력도 떨어지는 것이 보완해야 할 점으로 지적되고 있다. 이에 본 연구는 절삭형 티타늄을 이용한 보철물 제작에 많이 사용되고 있는 열중합 의치상 레진, 간접 복합 레진, 도재와 Grade II 순수 티타늄 사이의 결합 강도를 비교 평가해 보고자 하였다. 연구 재료 및 방법: 지름 9 mm, 높이 10 mm의 Grade II 순수 티타늄 원통형 시편 37개를 3군으로 나누어 각각 직경 7 mm, 높이 1 mm의 열중합 의치상 레진 (Lucitone 199, DENTSPLY Trubyte, York, USA), 간접 복합 레진 (Sinfony, 3M ESPE, Seefeld, Germany), 도재 (Triceram, Dentaurum, Ispringen, Germany)와 결합시켰다. 시편은 $5-55^{\circ}C$에서 1000회 열순환 처리 후, 범용 시험기 (Instron, Universal Testing Machine, Model 4465, USA)를 이용하여 1 mm/min의 속도로 하중을 가하여 전단결합강도를 측정하였다. 파절된 단면의 양상을 관찰하고 각 군별 파절양상을 조사하였다. 측정값은 one-way ANOVA와 Scheffe's multiple range test (${\alpha}=0.05$)로 분석하였다. 결과: 열중합 의치상 레진인 Lucitone 199 ($17.82{\pm}5.13\;MPa$)의 결합 강도가 가장 높았으며, 도재인 Triceram ($12.97{\pm}2.11\;MPa$), 복합레진인 Sinfony ($6.00{\pm}1.31\;MPa$) 순으로 감소하였다. Lucitone 199와 Sinfony 군의 파절 양상은 대부분이 부착성 파절인 데에 반해 Triceram 군에서는 복합성 파절이 많았다. 결론: CAD/CAM을 이용한 절삭형 티타늄 구조물 상방에 전장용 심미 재료로는 열중합형 의치상 레진이 가장 강한 결합 강도를 보인다. 기존의 주조체의 유지구 등에서 얻는 강도에 비해 약하고, 부착성 파절이 많은 점 등은 향후 이들 재료와 티타늄간의 결합력을 높이기 위한 보다 많은 연구가 이루어져야 할 것을 시사한다.

치과 교정용 자가중합형 Resin의 중합 온도 변화에 따른 굽힘 특성 (Flexural Properties according to Change of Polymerization Temperature of Autopolymerized Resin for Orthodontic)

  • 치과 교정용 자가중합형 레진의 중합 온도가 기계적 특성에 미치는 영향을 알아보기 위하여 폴리머와 모노머의 부피비는 2.5:1로 하고, 중합 온도를 $25^{\circ}C$, $40^{\circ}C$, $70^{\circ}C$의 세 group으로 나누었다. 실험을 위해 각 group 당 10개씩의 시편을 중합 시간 15분, 중합 압력 3 bar로 통일하여 제작하였다. 그 후 시편을 실험 전까지 $37^{\circ}C$의 증류수에 48시간 이상 보관한 다음, Intron (3344)을 이용하여 각 시편의 FS 및 EM을 측정하였다. 이렇게 하여 얻어진 값들은 SPSS ver. 16.0을 사용하여 일원 배치 분산 분석을 하였으며 Scheffe의 사후 검정을 실시하여 다음과 같은 결과를 얻었다. 치과 교정용 자가중합형 레진의 FS 평균값을 비교해 보았을 때 $25^{\circ}C$ group은 $71.500{\pm}5.546MPa$, $40^{\circ}C$ group은 $74.920{\pm}4.237MPa$, $70^{\circ}C$ group은 $76.880{\pm}4.304MPa$의 범위였으며, $25^{\circ}C$ group<$40^{\circ}C$ group<$70^{\circ}C$ group 순으로 차이가 있었으나 통계적으로 유의한 차이는 없었다(p=0.052). 치과 교정용 자가중합형 레진의 EM 평균값은 $25^{\circ}C$ group이 $2.164{\pm}0.073MPa$, $40^{\circ}C$ group은 $2.258{\pm}0.069MPa$, $70^{\circ}C$ group은 $2.282{\pm}0.147MPa$의 범위였으며, $25^{\circ}C$ group<$40^{\circ}C$ group<$70^{\circ}C$ group 순으로 중합 온도가 높을수록 유의한 차이를 보였다(p<0.039). 이상의 실험 결과로 미루어 볼 때 EM 값은 $25^{\circ}C$ group<$40^{\circ}C$ group<$^70{\circ}C$ group 순으로 통계적 유의성이 인정되는 수준의 상승이 있었던 반면, FS 값은 통계적 유의성은 보이고 있지 않으나 강도의 증가에 영향을 미치고 있으므로, 치과 교정 장치 제작 시 일정 수준의 중합 온도 상승은 장치의 기계적 물성을 증가시킬 것이라고 판단된다.
