Assessing the Influence of Anteroposterior Lip Position Based on Esthetic Line on the Perceived Attractiveness

  • Purpose: The purpose of this study was to assess the effects of lip anteroposterior position based on esthetic line on the perceived attractiveness. Materials and Methods: We selected a 20s female standard lateral photograph which was within average range of cephalometric analysis, modified lips anteroposterior position based on esthetic line into 5 pictures. This study investigated and compared the preference of facial profile among the groups; male : female and dental relevance: non-dental relevance. Total 255 judges (male : female=138 : 117, relevant : non-relevant=159 : 96) who were 20s to 30s were asked to rate these photographs based in lip attractiveness using visual analogue scale (VAS). Result: All groups had similarity the average of VAS of moved backward lips 2 mm were highest and moved forward lips 4 mm were lowest. Comparing between male group and female group, there were significant differences in all pictures except for original which was not modified. In the dental groups, moved forward lips 2 mm had significant difference and the average in dental relevant group were lower than non-relevant group in lip protrusion. Conclusion: The preference about lip protrusion was similar irrespective of dental knowledge or gender. All groups preferred retrusion of lips to protrusion of lips. In female group, they had higher the average of VAS. In relevant group, they disliked protrusion rather than retrusion of lips significantly.

관골궁 골절의 정복시 수술 중 C-Arm 사용의 유용성: 증례보고 (The Avalibility of C-Arm in Reduction of Zygomatic Arch Fracture Intraoperatively: Case Report)

  • The zygoma is second most commonly vulnerable facial bone in fracture, in number only by nasal fractures. It is difficult to evaluate reduction state intraoperatively, because almost surgeons reduce the fractured zygoma by blind method. We suggest the use of orthopedic C-arm intraoperatively. We use plain radiography, CT to evaluate preoperative state. Gilles approach or intraoral approach were used to reduce the fractured zygomatic arch. The C-arm was positioned at chin area, used to evaluate reduction intraoperatively. We got postoperative image by CT or submento-vertex view. There are variable methods to evaluate reduction intraoperatively: palpation, ultrasonography, CT, plain films. C-arm is considered superior diagnostic tool to other methods. The use of intraoperative C-arm was very efficient, it could bring better results.

  • A malignant sarcoma of soft tissue that arises near but not in a joint, most often in an adolescent or young adult, is typically slowgrowing, and may escapes notice until it causes pain. The diagnosis of this tumor can be suspected by X-ray or imaging, made by biopsy, and confirmed by chromosome studies. Cytogenic studies reveal a translocation between the X chromosome and chromosome 18 in the tumor cells. The most common treatment for the tumor is surgery to remove the entire tumor, nearby muscle, and lymph nodes. Synovial sarcoma tends to recur locally and to involve regional lymph nodes. Distant metastasis occurs in half cases. The well-documented and adequately followed cases are currently lacking and this report described an instance of Synovial sarcoma good result after treatment and review of literature.

  • A reconstruction following a resection of malignant oral cavity tumors is one of the most difficult problems in recent oral oncology. For a better understanding of oral and maxillofacial reconstructive procedures, basic and advanced microvascular anastomosis techniques must be learned and memorized. The aim of this article was to clarify and define the microvascular anastomosis methods, such as primary closure after an arteriotomy, end to side anastomosis, end to end anastomosis, and side to side anastomosis with an artery and vein. This review article discusses the basic skills regarding microvascular anastomoses with brief schematic diagrams in the Korean language. This article is expected to be helpful, particularly to young doctors in the course of the Korean national board curriculum periods for oral and maxillofacial surgery.

Surgical excision of osteochondroma on mandibular condyle via preauricular approach with zygomatic arch osteotomy

  • Background: Osteochondroma is a benign tumor that tends to develop in mandibular condyle and coronoid process in the craniofacial region. If tumor mass has grown from condyle into the infratemporal space with zygomatic arch obstructing the access, there are risks associated with surgical exposure and local resection of these masses. Case presentation: This study reports on a case of osteochondroma on mandibular condylar head where we treated with surgical excision via preauricular approach with 3D analysis. After the local resection, there were no surgical and post-operative complications until 8-month follow-up period. Conclusions: In local excision of osteochondroma, our method is a minimally invasive method. It is a good example of osteochondroma treatment.

Single-insertion technique for anesthetizing the inferior alveolar nerve, lingual nerve, and long buccal nerve for extraction of mandibular first and second molars: a prospective study

  • Objectives: Appropriate and accurate local anesthetic (LA) techniques are indispensable in the field of oral and maxillofacial surgery to obtain a satisfactory outcome for both the operating surgeon and the patient. When used alone, the inferior alveolar nerve block (IANB) technique requires supplemental injections like long buccal nerve block for extraction of mandibular molars leading to multiple traumatic experiences for the patient. The aim of this study was to anesthetize the inferior alveolar, lingual, and long buccal nerves with single-needle penetration requiring a minimal skillset such as administering a conventional IANB through introduction of the Benny Joseph technique for extraction of mandibular molars. Materials and Methods: This was a prospective study conducted in the Department of Oral and Maxillofacial Surgery, Kunhitharuvai Memorial Charitable Trust (KMCT) Dental College, Calicut, India. The duration of the study was 6 months, from June to November 2017, with a maximum sample size of 616 cases. The LA solution was 2% lignocaine with 1:100,000 adrenaline. The patients were selected from a population in the range of 20 to 40 years of age who reported to the outpatient department for routine dental extraction of normally positioned mandibular right or left first or second molars. Results: Of the 616 patients, 42 patients (6.8%) required re-anesthetization, a success rate of 93.2%. There were no complications such as hematoma formation, trismus, positive aspiration, and nerve injuries. None of the cases required re-anesthetization in the perioperative period. Conclusion: The Benny Joseph technique can be employed and is effective compared with conventional IANB techniques by reducing trauma to the patient and also requires less technique sensitivity.

Sinus augmentation with poly(ε) caprolactone-β tricalcium phosphate scaffolds, mesenchymal stem cells and platelet rich plasma for one-stage dental implantation in minipigs

  • Purpose: This study evaluated the efficacy of a tube-shaped poly(ε) caprolactone - β tricalcium phosphate (PCL-TCP) scaffold with the incorporation of human umbilical cord-derived mesenchymal stem cells (hUCMSCs) and platelet-rich plasma (PRP) for bone regeneration in the procedure of single-stage sinus augmentation and dental implantation in minipigs. Methods: Implants were placed in the bilateral sides of the maxillary sinuses of 5 minipigs and allocated to a PCL-TCP+hUCMSCs+PRP group (n=5), a PCL-TCP+PRP group (n=5), and a PCL-TCP-only group (n=6). After 12 weeks, bone regeneration was evaluated with soft X-rays, micro-computed tomography, fluorescence microscopy, and histomorphometric analysis. Results: Four implants failed (2 each in the PCL-TCP+hUCMSCs+PRP and PCLTCP+hUCMSC groups). An analysis of the grayscale levels and bone-implant contact ratio showed significantly higher mean values in the PCL-TCP+hUCMSCs+PRP than in the PCL-TCP group (P=0.045 and P=0.016, respectively). In fluoromicroscopic images, new bone formation around the outer surfaces of the scaffolds was observed in the PCLTCP+hUCMSCs+PRP group, suggesting a tenting effect of the specially designed scaffolds. Bone regeneration at the scaffold-implant interfaces was observed in all 3 groups. Conclusions: Using a tube-shaped, honeycombed PCL-TCP scaffold with hUCMSCs and PRP may serve to enhance bone formation and dental implants' osseointegration in the procedure of simultaneous sinus lifting and dental implantation.

치과 임플란트 수술 환자의 수면 질과 공포감의 관련성 (Correlation between quality of sleep and dental fear in implant surgery patients)

  • Objectives : The purpose of this study is to investigate the correlation between quality of sleep and dental fear in implant surgery patients. Methods : A self-reported questionnaire was filled out by 153 implant patients from December 2012 to February 2013 in dental clinics and hospitals in Busan and Changwon. Data were analyzed by descriptive analysis, t-test, ANOVA and multiple regression analysis using SAS version 9.20. Results : The quality of sleep in the implant patients was $41.0{\pm}6.0$. The systemic diseases influenced on the quality of sleep and dental fear also affected the quality of sleep. Conclusions : The results of the analysis of a relationship between the fear perception of dental implant surgery patients and the quality of their sleep showed that the patients with systemic diseases, the patients with a fear by the physical stimulation relating to implant treatment showed the low quality of sleep.

치아발거 문제에 관하여 (THE STUDIES FOR CAUSES OF THE TOOTH MORTALITY)

  • The authores had made the studies for causes of the tooth mortality. Sampling of studies was 5711 patients coming in department of oral surgery, infirmary of dental college, S.N.U. between year from 1965 to 1969. The results were obtained as follow. 1) The most freguency of tooth extraction due to inflammation and dental caries. 2) Male was heigher frequency than female due to fracture by accidental trouble.
