Thermography와 유한요소분석법을 이용한 하악 제2유구치의 열확산도 평가 (EVALUATION OF THERMAL DIFFUSION IN LOWER End PRIMARY MOLAR WITH THERMOGRAPHY AND FINITE ELEMENT ANALYSIS)

  • 임상에서 설비의 노후나 고장 등으로 인해 고속 핸드피스의 물 분사에 의한 냉각이 종종 불완전하게 일어날 수 있다. 또한 성인에 비해 치료에 관한 공포가 심하여 비협조적인 소아환자의 경우에는 물의 분사나 고속 흡인기의 사용을 거부하거나 지극히 혐오하는 경우도 드물지 않다. 이처럼 냉각이 충분히 이뤄지지 않은 상황에서 시행되는 다양한 술식은 치수조직에 치명적인 손상을 가할 수 있다. 본 연구의 목적은 임상 수복 술식을 시행하는 동안 각 치아표면에서 발생되는 열이 치수 내부로 전달되는 양상을 조사하기 위함이었다. 석고 블록에 식립된 90개의 하악 제2유구치를 대상으로 수종의 수복 술식을 시행하였고, 해당 치아표면에서 발생되는 열의 온도를 적외선 카메라와 thermography를 이용하여 측정하고, 이를 토대로 유한요소 분석을 통한 치질내 열전도에 관한 평가를 시행하였다. 본 연구에서 시행된 임상 술식의 주수 하 최고 표면 온도는 $30.8^{\circ}C{\sim}43.6^{\circ}C$, 비 주수 하에서는 $51.2^{\circ}C{\sim}103.4^{\circ}C$의 범위를 보였다. 술식 중에서는 치관 형성과정이 가장 높은 온도를, 아말감 제거 과정이 가장 낮은 표면온도를 보였다. 동일 술식 내에서의 주수와 비 주수 조건간에는 모든 술식이 유의한 차를 나타냈다(p<0.05). 유한요소 분석에서 나타난 치수각의 온도변화는 모든 비 주수 조건에서는 큰 폭으로 상승한 반면, 주수 하에서는 전반적으로 미약한 변화를 보였다. 이상의 결과는 고속 절삭기를 사용한 수복 과정에 주수에 의한 충분한 냉각이 치수 생활력의 유지를 위해서 필수적임을 시사해준다. 향후 보다 다양한 술식과 조건을 부여한 유사 분야의 연구가 추천된다.

미맹출 영구치 폭경예측방법의 정확도에 관한 비교연구 (A COMPARATIVE STUDY ON THE ACCURACY OF PREDICTION METHODS OF UNERUPTED CANINE AND PREMOLARS IN THE MIXED DENTITION)

  • The purpose of this study was to determine the accuracy of the prediction methods which are frequently used in the clinic, such as Moyers', Tanaka and Johnston's, Song's Hixon and Oldfather's, Kaplan's, Smith's. These prediction methods were compared to the data, which were obtained from 100 Kyung-Hee University students (50 males and 50 females) with normal occlusion of permanent dentition, using a retrospective statistical analysis. The standard periapical radiopgraphs for the lower premolars using 16-inch long cone technique and the yellow stone dental casts were taken from each samples. The mesiodistal widths of 4 incisors, canine, premolars were measured on the dental casts, and those of premolars were measured on the radiographs. The correlation coefficients, paired samples t-tests, regression coefficients between the actual measuredments of the samples and the predicted measurements of each methods were analyzed. The results obtained were as follows: 1. The prediction methods using periapical radiographs tended to show a stronger correlation than the methods using dental casts; the correlation coefficients between the actual widths of the lower canine and premolars and the predicted widths of the tested methods were higher in Smith, Kaplan, Hixon & Oldfater, and Tanaka & Johnston (p<0.001). 2. The Moyers' probability chart at 65 % showed a tendency toward the least difference between the actual measurements and the predicted widths of the lower canine and the premolars, and the methods of Smith, Kaplan, Tanaka & Johnston also showed a less difference. 3. By means of evaluation in correlation coefficients and paired smaples t-tests, the Smith and Tanaka & Johnston in male, and the Moyers' 65 % and Song in female showed a high tendency toward accurate methods. 4. The regression equations by models that are derived from the study were as follows: Male(Lt.) :Y=10.799+0.475X Femle(Lt.) :Y= 7.427+0.609X (Rt.) :Y=11.043+0.462X (Rt.) :Y= 8.475+0.556X 5. Integrating 3D facial scanning in a digital workflow to CAD/CAM design and fabricate complete dentures for immediate total mouth rehabilitation

  • PURPOSE. To integrate extra-oral facial scanning information with CAD/CAM complete dentures to immediately rehabilitate terminal dentition. MATERIALS AND METHODS. Ten patients with terminal dentition scheduled for total extraction and immediate denture placement were recruited for this study. The patients were submitted to a facial scanning procedure using the in-office PritiMirror scanner with bite registration records in-situ. Definitive stone cast models and bite records were subsequently submitted to a lab scanning procedure using the lab scanner (iSeries DWOS; Dental Wings). The scanned models were used to create a virtual teeth setup of a complete denture. Using the intra-oral bite records as a reference, the virtual setup was incorporated in the facial scan thereby facilitating a virtual clinical evaluation (teeth try-in) phase. After applying necessary adjustments, the virtual setup was submitted to a CAM procedure where a 5-axis industrial milling machine (M7 CNC; Darton AG General) was used to fabricate a full-milled PMMA immediate provisional prosthesis. RESULTS. Total extractions were performed, the dentures were immediately inserted, and subjective clinical fit was evaluated. The immediate provisional prostheses were inserted and clinical fit, occlusion/articulation, and esthetics were subjectively assessed; the results were deemed satisfactory. All provisional prostheses remained three months in function with no notable technical complications. CONCLUSION. Ten patients with terminal dentition were treated using a complete digital approach to fabricate complete dentures using CAD/CAM technology. The proposed technique has the potential to accelerate the rehabilitation procedure starting from immediate denture to final implant-supported prosthesis leading to more predictable functional and aesthetics outcomes.

여러 가지 Die spacing material의 물리적 성질에 대한 연구 (A STUDY ON THE PHYSICAL CHARACTERISTICS OF THE THREE COMMONLY USED DIE SPACING MATERIALS)

  • As an optimal quality of the restorations, there should be a least amount of seating discrepancy between the casting and abutment teeth. However, high viscosity of the cementing medium and its resulting thickness may prevent complete seating of the restoration. The use of die spacing material provides adequate internal relief for the cementing medium. The purpose of this study is to compare the thickness of three commonly used die spacing materials. Materials and Methods: Stone plates were fabricated and divided into 12 sections to be painted with die spacers. Tru-Fit, Whip-Mix and Belle do St. Claire die spacer which are commonly used in dental practice were tested in this study. Each die spacers were painted layer by layer according to the manufacturer's recommendation. The average thickness of each die spacers were measured with light microscope(${\times}100$) and compared between them. Results and Conclusions. A silver-colored Tru-Fit die spacer has the lowest value of thickness without statistical significance comparing with a gold-colored Tru-Fit die spacer and a gray layer of Whip-Mix die spacer has the highest value of thickness without any statistical significance comparing with Belle de St. Claire die spacer. Three and four layers of Tru-Fit die spacer and two layers of Whip-Mix and Belle de St. Claire die spacers seem to be in the acceptable range of thickness of 25 to $45{\mu}m$ for optimal seating of the restorations. The standard experimental design and method should be fur thor evaluated for more consistent and objective results.

A comparative study of cone-beam computed tomography and digital panoramic radiography for detecting pulp stones

  • Purpose: The aim of this study was to compare cone-beam computed tomography (CBCT) and digital panoramic radiography (DPR) for the detection of pulp stones. Materials and Methods: DPR and CBCT images of 202 patients were randomly selected from the database of our department. All teeth were evaluated in sagittal, axial, and coronal sections in CBCT images. The systemic condition of patients, the presence of pulp stones, the location of the tooth, the group of teeth, and the presence and depth of caries and restorations were recorded. The presence of pulp stones in molar teeth was compared between DPR and CBCT images. Results: Pulp stones were identified in 105 (52.0%) of the 202 subjects and in 434 (7.7%) of the 5,656 teeth examined. The prevalence of pulp stones was similar between the sexes and across various tooth locations and groups of teeth (P>.05). A positive correlation was observed between age and the number of pulp stones(${\rho}=0.277$, P<.01). Pulp stones were found significantly more often in restored or carious teeth (P<.001). CBCT and DPR showed a significant difference in the detection of pulp stones(P<.001), which were seen more often on DPR than on CBCT. Conclusion: DPR, as a 2D imaging system, has inherent limitations leading to the misinterpretation of pulp stones. Restored and carious teeth should be carefully examined for the presence of pulp stones. CBCT imaging is recommended for a definitive assessment in cases where there is a suspicion of a pulp stone on DPR.

Comparison of accuracy between panoramic radiography, cone-beam computed tomography, and ultrasonography in detection of foreign bodies in the maxillofacial region: an in vitro study

  • Objectives: Foreign bodies (FBs) account for 3.8% of all pathologies of the head and neck region, and approximately one third of them are missed on initial examination. Thus, FBs represent diagnostic challenges to maxillofacial surgeons, rendering it necessary to employ an appropriate imaging modality in suspected cases. Materials and Methods: In this cross-sectional study, five different materials, including wood, metal, glass, tooth and stone, were prepared in three sizes (0.5, 1, and 2 mm) and placed in three locations (soft tissue, air-filled space and bone surface) within a sheep's head (one day after death) and scanned by panoramic radiography, cone-beam computed tomography (CBCT), and ultrasonography (US) devices. The images were reviewed, and accuracy of the detection modalities was recorded. The data were analyzed statistically using the Kruskal-Wallis, Mann-Whitney U-test, Friedman, Wilcoxon signed-rank and kappa tests (P<0.05). Results: CBCT was more accurate in detection of FBs than panoramic radiography and US (P<0.001). Metal was the most visible FB in all of modalities. US was the most accurate technique for detecting wooden materials, and CBCT was the best modality for detecting all other materials, regardless of size or location (P<0.05). The detection accuracy of US was greater in soft tissue, while both CBCT and panoramic radiography had minimal accuracy in detection of FBs in soft tissue. Conclusion: CBCT was the most accurate detection modality for all the sizes, locations and compositions of FBs, except for the wooden materials. Therefore, we recommend CBCT as the gold standard of imaging for detecting FBs in the maxillofacial region.

의치 연성이장재에 대한 Candida albicans의 부착과 침투연구 (IN VITRO STUDY ON THE ADHERENCE AND PENETRATION OF CANDIDA ALBICANS INTO DENTURE SOFT LINING MATERIALS)

  • Purpose : Colonization of denture soft lining materials by Candida albicans can result in clinical problem, and deterioration of the materials. This study aimed to compare the retention and penetration of C. albicans into four denture soft lining materials commonly used. Materials and methods : Four denture soft lining materials (Coe-comfort$^{(R)}$, Coe-soft$^{(R)}$, GC soft liner$^{(R)}$, and Tissue conditioner$^{(R)}$) discs were prepared to glass slide and dental stone. Adherence of yeast to surfaces was monitored after one hour incubation of standardized washed cell suspension with test disc surfaces. Adherent cells stained with acridine orange were counted fluorescence microscopy. Penetration of yeast into materials bonded with acrylic resin after 1, 2, 3,4, 5, 6 and 7 days incubation was observed through sections stained using acridine orange and estimated to quantitative analysis using radioisotope. Results : There was statistical significance in cell numbers between smooth and rough surfaces(p<0.05). Higher numbers of cells were observed on rough surfaces. There was statistical significance in adherent cell numbers into smooth and rough surfaces individually(p<0.05). According to the increase of incubation periods, the cells penetrated into denture soft lining materials were shown to increase. The differences among all kinds of soft liner were statistically significant(p<0.05),and the largest number of cells penetrated into soft liners was observed in the Coe-soft$^{(R)}$. Conclusion : Initial adherence and penetration of yeast into denture soft lining materials has been influenced by surface roughness and chemical composition of them. The selection of appropriate materials and their fabrication may promote clinical performance.

요오드 소독제에 침전된 고무인상재의 체적안정성 (Dimensional stability of rubber impression materials immersed in iodophor disinfectant)

  • 목적: 요오드계 소독제의 고무인상체 안정성에 미치는 영향을 조사하고자 하였다. 연구 재료 및 방법: 폴리설파이드, 실리콘, 폴리이써 고무인상재를 이용하여 72개의 인상체를 채득하였다. 이를 요오드계 소독제인 iodophor 수용액에 각각 10분, 30분간 침적시키고 모형을 제작하여 대조군과 광학현미경을 이용하여 체적 안정성을 평가하였다. 결과: 모든 고무인상체는 0.5% 미만의 체적변화를 보였으나 30분 침적은 10분 침적에 비해 통계적으로 유의하게 변화량이 증가하였다(P < 0.05). 결론: 요오드계 소독제에 고무인상재를 침적시 10분정도의 시간경과가 고무인상재의 체적변화에 영향이 적었다.

3차원 컴퓨터 그래픽 기술을 이용한 KONUS 내관의 설계와 제작 (DESIGN AND FABRICATION OF INNER KONUS CROWN USING THREE DIMENSIONAL COMPUTER GRAPHICS)

  • A fabrication method of inner and outer crown using CAD/CAM is presented. The information of abutment teeth is transferred to a computer through a 3-dimensional scanner. A Konus inner and outer crown is designed on a computer and a real crown is machined based on this design using CAM. This method can save laboratory time and reduce inaccuracies compare to conventional casting procedure. A stone model with six prepared abutment teeth from a patient was used in this study. Three dimensional information from the model was transferred to a computer using a contact type 3-dimensional scanner with a $25{\mu}m$ accuracy. All margins were identified on a computer image where there is a change in surface taper of a model. To provide a cement space, the image of a inner sur face of a Konus inner crown was duplicated $25{\mu}m$ apart from the surface of a prepared abutment teeth image. The cement space was $20{\mu}m$ at the cervical margin. All Konus crowns were machined with a $10{\mu}m$ accuracy. It was concluded that this method can reduce working-time for the laboratory process and increase accuracy. A further research is required to make a simplified process for a more complex prosthesis.

상악 제1소구치에서 전부도재관의 finish line 형태에 따른 파절강도와 응력 분포에 관한 연구 (Fracture Resistance and Stress Distribution of All Ceramic Crowns with Two Types of Finish Line on Maxillary First Premolar)

  • he purpose of this study was to compare the fracture resistance of the IPS Empress ceramic crown with 1.0mm width rounded shoulder, which is usually recommended in all ceramic crown, and 0.5mm width chamfer finish lines on the maxillary first premolar. 30 sound maxillary first premolars were selected and then storaged in 5% NaOCl and saline. 15 teeth were performed preparation for each group(1.0mm rounded shoulder, 0.5mm chamfer). After 30 stone dies were made for each group, the IPS Empress ceramic crowns were fabricated and cemented with resin cement(Bistite resin cement, Tokuyama Soda Co. LTD., Japan) on the natural teeth. The cemented crowns were mounted on the positioning jig and the universal testing machine(Zwick Z020, Zwick Co., Germany)was used to measure the fracture strength, with stress loading on the occlusal surface between buccal and lingual cusp. And also, three-dimensional finite element model was used to measure the stress distribution with two types of the finish lines(1.0mm rounded shoulder, 0.5mm chamfer) and two loading conditions(both buccal and lingual cusp inclination, lingual cusp inclination only). The result of the this study were as follows. In the fracture resistance experiment according to the finish line, the mean fracture strength of rounded shoulder(842N) showed higher value than that of the chamfer(590N) (p<0.05). In the three dimensional finite element analysis of all ceramic crown, metal die and natural teeth model did not show any differences in stress distribution between finish lines. Generally, when force was loaded on the occlusal inclination of buccal and lingual cusp, the stress was concentrated on the loading point and the central groove of occlusal surface. When force was loaded only on the occlusal inclination of lingual cusp, the stress was concentrated on the lingual finish line and loading point.
