일부 지역사회 노인들의 구강보건행동에 따른 구강건강상태 (A Survey on the Oral Health Conditions according to Dental Health Behaviors of Elderly People in Community)

  • 본 연구는 지역사회 노인들의 구강보건행동과 구강건강상태를 알아보고 그에 대한 관련성을 검토하고자 하였다. 조사대상은 충청남도 공주시 6개면에 거주하고 있는 65세 이상 노인들 중 2005년 1월 1일부터 2월 28일까지 2개월 동안 보건소 및 보건지소에서 치과검진을 받은 206명(남자 100명, 여자 106명)으로 하였다. 조사는 구강보건행동과 잇몸출혈 자각증상에 대해 설문조사를 실시하였고, 치과의사에 의해 구강검사를 실시하였으며, 구강보건행동에 따른 구강건강상태에 대한 지표(우식치, 처치치, 상실치, DMFT 및 CPITN)를 검토하였다. 주요 결과는 다음과 같다. 1. 전체 조사대상자들의 구강보건행동을 보면 "년 1회 이상 치과진료를 받는다"는 군이 24.3%, "주 1회 이상 구강 내 관찰을 한다"는 군이 20.4%, "1일 3회 이상 잇솔질을 한다"는 군이 15.5%, "3분 이상 잇솔질을 한다"는 군이 24.3%, "주당 2~3회 이상 보조용품을 사용한다"는 군이 7.8%, 비흡연군이 68.9%, 감미음료 및 감미식품을 "섭취하지 않는다"는 군이 23.3% 이었다. 2. 전체 조사대상자들의 구강건강상태를 보면 잇몸출혈의 경우 "항상 있다" 8.7%, "가끔 있다" 28.2%, "거의 없다" 32.0%, "없다" 31.1%의 비율이었다. 우식치는 평균 1.2개 이었으며, 처치치는 9.9개, 상실치는 7.4개, DMFT는 18.4개, CPITN은 2.4이었다. 3. 구강보건행동별 구강건강상태를 보면, 년 1회 이상의 치과진료를 받지 않은 군은 받은 군보다 상실치(p=0.002), DMFT(p=0.002) 및 CPITN(p=0.018)이 유의하게 많았고, 주 1회 이상 구강 내 관찰을 하지 않는 군은 하는 군에 비해 처치치는 유의하게 적은 반면(p=0.002), 상실치(p=0.000)와 CPITN(p=0.000)은 유의하게 많았다. 잇솔질 횟수별로는 "1일 3회 미만 군"이 "1일 3회 이상 군" 보다 우식치(p=0.000), 상실치(p=0.000), DMFT(p=0.000) 및 CPITN(p=0.000)이 유의하게 많았고, 잇솔질 시간별로는 "3분 미만군"이 "3분 이상 군" 보다 상실치(p=0.002)와 DMFT(p=0.041)가 유의하게 많은 반면, 처치치는 유의하게 적었다(p=0.036). 보조용품 사용여부별로는 "사용하는 군"보다 "사용하지 않는 군"에서 DMFT(p=0.041) 및 CPITN(p=0.018)이 유의하게 많았고, 흡연습관 별로는 흡연군이 비흡연군보다 우식치(p=0.035) 와 CPITN (p=0.001)이 유의하게 많았다. 4. 다변량 회귀분석 결과 우식치수에 영향을 미치는 요인으로는 잇솔질 횟수, 성별 및 구강 내 관찰이 유의한 변수로 선정되었고(설명력 14.2%), 처치치수에는 성별, 구강 내 관찰, 보조용품사용, 잇솔질 횟수, 주관적 건강상태 및 감미음료섭취 여부(설명력 18.2%)가, 상실치수에는 잇솔질 횟수와 연령(설명력 13.9%)이, DMFT에는 잇솔질횟수, 성별, 치과진료여부, 연령(설명력 13.5%)이, CPITN에는 흡연여부, 치과진료여부, 보조용품사용여부(설명력 10.8%)가 유의한 변수로 선정되었다. 이상의 결과를 볼 때 노인들의 구강보건행동은 매우 저조한 상태로 보이며, 따라서 구강건강상태도 양호하지 못한 것임을 시사한다. 또한 구강건강상태는 여러 구강보건행동 요인에 따라 영향을 받게 됨으로 구강보건행동에 대한 홍보 및 교육이 필요하다고 본다.

Prescription of antibiotics after tooth extraction in adults: a nationwide study in Korea

  • Objectives: This study aimed to understand the nationwide patterns of antibiotic prescription after tooth extraction in adult patients. Materials and Methods: This study analyzed dental records from the National Health Insurance Service-National Sample Cohort (NHIS-NSC) database on 503,725 tooth extractions performed in adults (≥19 years) during 2011-2015. Patient sex, age, household income, systemic disease (diabetes mellitus and hypertension), type of dental institution, region of dental institution, year of prescription, and type of tooth extraction procedure were considered. The antibiotic prescription rate and broad-spectrum antibiotic prescription frequency were analyzed using chi-squared tests. Factors affecting the prescription of broad-spectrum antibiotics were evaluated using multivariate logistic regression analysis. Results: The rate of antibiotic prescription after tooth extraction was 81.85%. Penicillin was most commonly prescribed (45.25%), followed by penicillin with beta-lactamase inhibitors (18.76%), metronidazole (12.29%), and second- to fourth-generation cephalosporins (11.52%). The proportion of broad-spectrum antibiotics used among all prescribed antibiotics was 45.88%. Conclusion: The findings of this study demonstrate that the rate of antibiotic prescription after tooth extraction is higher in Korea than in other countries. Furthermore, broad-spectrum antibiotics are used more frequently, which may indicate unnecessary drug prescription, an important contributor to antibiotic resistance.

산재환자의 주관적 구강건강상태와 보건태도가 구강건강 영향지수(OHIP)에 미치는 영향 (The effect of subjective perception and attitude of oral health on the oral health impact profile in industrial accident injury patients)

  • Objectives : The purpose of this study was to investigate the oral health impact profile (OHIP) in the industrial accident injury patients, and the effect of subjective perception and attitude of oral health on the oral health-related quality of life in industrial injury patients. Methods : A self-reported questionnaire was filled out by 496 industrial accident injury patients from October 29 to November 30, 2013. Data were analyzed using SPSS ver 20.0 program for chi-square test, t-test, ANOVA, Scheffe test, and Pearson's correlation coefficient. Results : The OHIP-14 was higher oral health-related quality of life in young people, women, highly educated persons, and those having high income and frequent scaling service. The subjective perception and attitude of oral health was closely correlated to the OHIP-14. The factors influencing on the oral health-related quality of life were the subjective perception and attitude toward oral health. Conclusions : It is necessary to emphasize the oral health promotion program for industrial injury patients. The government should pay more attention to the policy for the improvement of the oral health of industrial accident injury patients in the future.

Definition and management of odontogenic maxillary sinusitis

  • Background: Maxillary sinusitis of odontogenic origin, also known as maxillary sinusitis of dental origin or odontogenic maxillary sinusitis (OMS), is a common disease in dental, otorhinolaryngologic, allergic, general, and maxillofacial contexts. Despite being a well-known disease entity, many cases are referred to otorhinolaryngologists by both doctors and dentists. Thus, early detection and initial diagnosis often fail to detect its odontogenic origin. Main body: We searched recent databases including MEDLINE (PubMed), Embase, and the Cochrane Library using keyword combinations of "odontogenic," "odontogenic infection," "dental origin," "tooth origin," "sinusitis," "maxillary sinus," "maxillary sinusitis," "odontogenic maxillary sinusitis," "Caldwell Luc Procedure (CLP)," "rhinosinusitis," "functional endoscopic sinus surgery (FESS)," "modified endoscopy-assisted maxillary sinus surgery (MESS)," and "paranasal sinus." Aside from the PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) trial, there have been very few randomized controlled trials examining OMS. We summarized the resulting data based on our diverse clinical experiences. Conclusion: To promote the most efficient and accurate management of OMS, this article summarizes the clinical features of rhinosinusitis compared with OMS and the pathogenesis, microbiology, diagnosis, and results of prompt consolidated management of OMS that prevent anticipated complications. The true origin of odontogenic infections is also reviewed.

Evaluation of subjective satisfaction of dental implant patients

  • Objectives: The goal of this study was to estimate the overall satisfaction level of dental implant patients and further evaluate factors influencing satisfaction. Materials and Methods: Self-administered questionnaires were mailed to patients who received dental implant therapy at Seoul National University Bundang Hospital (Seongnam, Korea) from October 2003 to April 2005. The main portion of the questionnaire was shared to evaluate the level of satisfaction with implant therapy. The questionnaires contained evaluations of influencing factors, which were classified as pain-related, service-related, and complication-related. Results: The responses from 93 patients (41 males, 52 females) with a total of 325 implants were included in the analysis, and the mean score for overall satisfaction level with implant therapy was 8.26. Female patients showed higher visual analogue scale (VAS) scores for both pain during (P=0.000) and after implant surgery (P=0.016). Male patients showed more 'negative' values for the reasonability of treatment cost (P=0.008) and the adequacy of the treatment period (P=0.022). Conclusion: The subjective satisfaction of patients was influenced by various factors, especially complication-related factors.

  • Objectives : The study explored perceptional gap of dentists and their employees between expectation and satisfaction. Methods : For analysis, the study surveyed 407 employees and 91 dentists on their expectation and satisfaction depending on expectation. Results : When the satisfaction on a question was lower than expectation; the rate of sufficiency was below 100%, it is necessary to improve on the question. Contrarily when the satisfaction on a question was higher than expectation; the rate of sufficiency was upper 100%, it is necessary to continuously support the question. The most of satisfaction of employees on their dentists and their organization were lower than employees's expectation of those. The most of satisfaction of dentists on their own and employees were lower than dentists's expectation of those as similar as employees cases, It means that all of questions need to improve continuously. Conclusions : The issues for prior decision to change these phenomena are 1) improvement of communication each other, 2) recovery of credence each other, and 3) leadership style change from authoritarian to consideration.

Anti-inflammatory effect of new calcium hydroxide paste containing silicon-substituted hydroxyapatite in lipopolysaccharide-stimulated macrophages

  • Objectives: Calcium hydroxide, a root canal temporary sealer, has long been used and it has anti-bacterial and anti-inflammatory activity. To investigate the properties of a newly developed calcium hydroxide paste comprising silicon-substituted hydroxyapatite (Si-HA), we examined the anti-inflammatory activity of the new calcium hydroxide paste in RAW 264.7 macrophages stimulated by lipopolysaccharide (LPS), which causes infection of the root canal. Methods: The test materials, including Calcipex II as control group and the newly developed TRC paste, were extracted from cell culture media and then diluted for experiment. In LPS-stimulated RAW 264.7 cells, the cytotoxicity and nitric oxide (NO) production of test materials were measured by MTT assay and Griess reagents, respectively. Also, the expression of the inducible nitric oxide synthase (iNOS) was assessed by western blotting. Results: The IC50 values of Calcipex II and TRC paste were $17.6mg/m{\ell}$ and $13.5mg/m{\ell}$, respectively. The level of NO, increased by LPS, was dose-dependently inhibited more by TRC paste than Calcipex II treatment. In addition, iNOS expression was decreased by 71% and 92% at concentrations of $2mg/m{\ell}$ and $20mg/m{\ell}$ of TRC paste, respectively. Conclusions: We demonstrated that the Si-HA calcium hydroxide paste has a slightly improved anti-inflammatory property and further studies are needed before clinical recommendations are proposed.

A within-subject comparison of short implants in the posterior region: retrospective study of up to 10 years

  • Purpose. This intra-patient retrospective study of up to 10 years evaluated the clinical success and risk factors of 6- and 8-mm long implants and their respective prostheses. Materials and Methods. The sample consisted of patients treated at a Military Polyclinic dental service, who received both 6- and 8-mm long tissue level implants in the posterior region of the same arch. Data were collected from the dental charts, clinical and radiographic exams, self-report of sleep bruxism, measurement of maximum occlusal force, and clinical crown-to-implant (C/I) ratio. Data were analyzed by descriptive and inferential statistics with univariate and hierarchical multivariate models, at the 0.05 significance level. Results. The 30 patients (27 women) had 85 implants and 83 prostheses. Two implants were lost before prosthesis installation (implant survival: 97.6%). Ten events of prosthetic complication (screw tightening loss) occurred in five patients (success rate: 87.9%) in a single moment. Only the variable C/I ratio had a significant effect for repairable prosthesis complication (P<.05). Conclusion. The results suggest that 6- and 8-mm long implants have similar long-term clinical success for implants and prostheses.

언어 네트워크 분석을 통한 노인 구강 건강 연구 동향 탐구 (Exploring the research trends of elderly oral health through language network analysis)

  • Objectives: The purpose of this study is to explore the research trends of elderly oral health through a language network analysis. Methods: A total of 354 published studies with 668 keywords were collected from the Research Information Sharing Service (RISS) between 2000 and 2022. Language network analysis was performed using Textom 6.0, Ucinet 6.774, and NetDraw 2.183. Results: The most frequent keywords were 'elderly', 'oral health', 'quality of life', and 'OHIP-14'. The result of frequency-inverse document frequent keywords showed similar results to the most frequent keywords. The N-gram of keywords shows that 'elderly', 'oral health' (18 times) and 'elderly', 'depression' (7 times). As a results of the analysis of degree centrality and between centrality, 'elderly', 'oral health', and 'quality of life' were found to be high. The CONCOR analysis identified the main clusters of 'quality of life', 'oral health behavior', 'health', and 'oral function disorder'. Conclusions: The results of the current study could be available to know research trends in elderly oral health and it is necessary to improve more comprehensive study in follow-up study.

가족발달단계에 따른 간호요구영역에 관한 연구 (Study of The Area of Nursing Need by the Family Developmental Stage)

  • The Community Health Service considers the family as a service unit and places the emphasis of its service on the health problems and the nursing needs of the family rather than the individual. From the conceptual point of view that tile community health service is both health maintenance and health promotion of the family, the community health nurse should have a knowledge of the growth and development of the family and be responsible for the comprehensive support of normal family development. The community health nurse often is in a position to make a real contribution to normal family development. In order to investigate the relationship between the areas of nursing need and family development, the following objectives were established 1. To discover the general characteristics of the study population by the stage of family development. 2. To discover specific nursing needs in relation to the family developmental stage, and to determine the intensity of the nursing needs and the ability of the family to cope with these needs. 3. To discover overall family health nursing problems in relation to the family developmental stage and determine the intensity of the nursing need and the problem solving ability of family. Definitions : The family developmental stages as classified by Dually were used stage 1. Married couples(without children) stage 2. Childbearing Families (oldest child birth to 30 months of age) stage 3. Families with preschool children (oldest child 2½-to 6 years) stage 4. Families with schoolchildren (oldest child 6 to 13 years). stage 5. Families with teenagers (oldest child 13 to 20 years) stage 6. Families as launching centers (first child gone to last child′s leaving home). stage 7. Middle- aged parents (empty nest to retirement) stage 8. Aging family member (retirement to death of both spouses) The areas of nursing need were defined as those used in the study, "A Comprehensive Study about Health and Nursing Need and a Social Diagram of the Community", by tile Nursing research Institute and Center for population. and Family Planning, July 1974. The study population defiled and selected were 260 nuclear families ill two myron of Kang Hwa Island. Percent, mean value and F- test were utilized in tile statistical analysis of the study result. Findings : 1. General characteristics of the study population by tile family developmental stage ; 1)The study population was distributed by the family developmental stage as follows : stage 1 : 3 families stage 2 : 13 families stage 3 : 24 families stage 4 : 41 families stage 5 : 50 families stage 6 : 106 families stage 7 : 13 families stage 8 : 10 families 2) Most families had 4 or 5 members except for those in stage, 1, 7, and 8. 3) The parents′ present age was older in the higher developmental stage and their age at marriage was also younger in the higher developmental stages. 4) The educational level of parents was primarily less than elementary school irrespective of the developmental stage. 5) More than half of parents′ occupations were listed as laborers irrespective of the developmental stage, 6) More than half of the parents were atheists irrespective of the developmental stage. 7) The higher the developmental stage(from stage 2 to stage 6 ), the wider the distribution of children′s ages. 8) More than half of the families were of middle or lower socio-economic level. 2. Problems in specific areas of nursing need by family developmental stage, the intensity of nursing need and the problem solving ability of the family : 1) As a whole, many problems, irrespective of the developmental stage, occurred in tile areas of Housing and Sanitation, Eating Patterns, Housekeeping, Preventive Measures and Dental care. Problems occurring ill particular stages included the following ; stage 1 : Prevention of Accident stage 2 : Preventive Vaccination, Family Planning. stage 3 : Preventive Vaccination, Maternal Health, Family Planning, Health of Infant and Preschooler. stage 4, 5 : Preventive Vaccination, Family Planning, Health of School Children. stage 6 : Preventive Vaccination, Health of School Children. 2) The intensity of the nursing need in the area of Acute and Chronic Diseases was generally of moderate degree or above irrespective of the developmental stages except for stage 1. Other areas of need listed as moderate or above were found in the following stages: stage 1 : Maternal Health stage 3 . Horsing and Sanitation, Prevention of Accident. stage 4 . Housing and Sanitation. stage 5 : Housing and Sanitation, Diagnostic and Medical Care. stage 6 : Diagnostic and Medical care stage 7 : Diagnostic and Medical Care, Housekeeping. stage 8 : Housing and Sanitation, Prevention of Accident, Diagnostic and Medical Care, Dental Care, Eating Patterns, Housekeeping. 3) Areas of need with moderate problem solving ability or less were as follows : stage 1 : Diagnostic and Medical Care, Maternal Health. stage 2 : Prevention of Accident, Acute and Chronic Disease, Dental Care. stage 3 : Housing and Sanitation, Acute and Chronic Disease, Diagnostic and Medical Care, Preventive Measure, Dental Care, Maternal Health, Health of Infant and preschooler, Eating Patterns. stage 4 : Housing and Sanitation, Prevention of Accident, Diagnostic and Medical Care, Preventive Measure, Dental Care, Maternal Health, Health of New Born, Health of Infant and Preschooler, Health of school Children, Eating Patterns, Housekeeping. stage 5 . Problem occurrence, the degree of nursing need and the degree of problem solving ability 1 nursing need areas for the family as a whole were as follows : 1) The higher the stages(except stage 1 ), the lower the rate of problem occurrence. 2) The higher the stage becomes, the lower the intensity of the nursing need becomes. 3) The higher the stages (except stages 7 and 8), the higher. the problem solving ability. Conclusions ; 1) When the nursing care plan for the family is drawn up, depending upon the stage of family development, higher priority should be give to nursing need areas ① at which problems were shown to occur ② where the nursing need is shown to be above moderate degree and ③ where the problem solving ability was shown to be of moderate degree. 2) The priority of the nursing service should be Placed ① not on those families in the high developmental stage but on those families in the low developmental stage ② and on those areas of need shown in stages 7 and 8 where the degree nursing need was high and the ability to cope low.
