  • Bruxism is extensively defined as a diurnal or nocturnal parafunctional habit of tooth clenching or grinding. The etiology of bruxism may be categorized as central factors or peripheral factors and according to previous research results, central factors are assumed to be the main cause. Bruxism may cause tooth attrition, cervical abfraction, masseter hypertrophy, masseter or temporalis muscle pain, temporomandibular joint arthralgia, trismus, tooth or restoration fracture, pulpitis, trauma from occlusion and clenching in particularly may cause linea alba, buccal mucosa or tongue ridging. An oral appliance, electromyogram or polysomnogram is used as a tool for diagnosis and the American Sleep Disorders Association has proposed a clinical criteria. However the exact etiology of bruxism is yet controversial and the selection of treatment should be done with caution. When the rate of bruxism is moderate or greater and is accompanied with clinical symptoms and signs, treatment such as control of dangerous factors, use of an oral appliance, botulinum toxin injection, pharmacologic therapy and biofeedback therapy may be considered. So far, oral appliance treatment is known to be the most rational choice for bruxism treatment. For patients in need of esthetic correction of hypertrophic masseters, as well as bruxism treatment, botulinum toxin injection may be a choice.

Lateral Cephalometric Measurements of Class I Malocclusion Patients with Uncertainty (불확도를 고려한 Class I 부정교합 환자의 측방두부방사선영상 계측값)

  • The aim of this study was to obtain the traceability of the software used to analyze lateral cephalometry and to calculate the uncertainty of the measurements. Furthermore, this study aimed to provide a basis for obtaining standard references for measurement values for orthodontic treatment in children. Cephalometric data were collected from 100 children diagnosed with class I malocclusion between the ages 6 to 13 years who visited the pediatric dentist at Seoul National University Dental Hospital. To ensure traceability, a phantom device was created. Correction values were calculated by measuring the length and angle of the phantom device using the software. Type A uncertainty was calculated by obtaining the standard deviation of cephalometric measurements of 100 persons and the standard error of repeated measurements. Determination of the type B uncertainty was induced by minimum resolution and the position of the head. Using these, the combined standard uncertainty was obtained and the expanded uncertainty was calculated. The results of this study confirm that the currently used software has high accuracy and reliability. Furthermore, the uncertainty of orthodontic measurements in Korean children aged 6 to 13 years was calculated, and distribution range for class I malocclusion with 95% confidence interval was suggested.

Analysis on the development necessity of a safety health knowledge test for young children (유아 안전보건지식 측정도구 개발 필요성에 대한 분석)

  • The purpose of this study is to emphasize the importance of the safety and health ability to safely protect body in a risky situation in infancy, but due to the lack of a valid test tool for measuring infant safety knowledge, measure the safety and health knowledge of infants. In order to achieve the purpose of the study, opinions of education experts and early childhood education experts were collected, and suitable factors for measuring safety and health knowledge of children aged 3-5 years were extracted. The measurement questions developed based on this were surveyed on 186 children aged 3-5 years old to verify the relevance of the question composition and the reliability of the contents. In the distribution of the item response, values of 0.8 or less were deleted, and items with a correlation of r<.40 or less were also deleted, and the reliability test showed more than .70. After exploratory factor analysis, it was developed into 23 questions in 3 areas. The total score of safety and health knowledge for the final question and the degree of correlation were above .80. The tool for measuring child safety and health knowledge developed in this study was confirmed to be appropriate for item discrimination, item reliability, and content validity, and thus will be used as basic data for measuring child safety and health knowledge. After the early childhood safety and health education program is implemented, Further research should verify the effectiveness of the early childhood safety and health program.

Effects of Pre-Sintering Surface Treatment and Liner Application on the Shear Bond Strength of Zirconia and Pressable Ceramic (소결 전 지르코니아 표면처리와 라이너 사용에 따른 지르코니아와 열가압성형도재의 전단결합강도)

  • Purpose: This study was intended to investigate the effect of applying liner for chemical bonding and physical surface roughness created on zirconia by using a sandpaper before sintering on the bond strength between the two materials. Methods: Zirconia blocks were cut using a low-speed cutter. Plate-shaped specimen($6mm{\times}6mm{\times}3mm$) was fabricated by sintering after giving surface roughness according to four kinds of sandpapers. Depending on whether or not to use liner, 60 specimens were divided into two groups ZN(non-liner), ZL(liner), and the two groups were subdivided into four groups respectively in accordance with sandpaper used, totaling eight groups (n=10). The surface roughness (Ra) values and shapes before sintering were observed, and shear bond strength after pressing ceramic plasticity was measured with a universal testing machine. For a test of the significance, a one-way ANOVA was performed, and Tukey's multiple comparison test was conducted. Results: The observation of the surface roughness was SB04($2.22{\pm}1.16{\mu}m$), SB08($2.98{\pm}0.33{\mu}m$), SB12($2.44{\pm}1.32{\mu}m$), SB20($2.34{\pm}0.59{\mu}m$) and SA04($2.34{\pm}0.67{\mu}m$), SA08($1.28{\pm}0.90{\mu}m$), SA12($2.03{\pm}1.60{\mu}m$), SA20($2.19{\pm}1.73{\mu}m$). In the case of ZN Group, the shear bond strength was ZN04($23.26{\pm}3.83MPa$), ZN08($21.76{\pm}2.33MPa$), ZN12($20.49{\pm}3.01MPa$), ZN20($24.98{\pm}4.22MPa$)(p<0.05). As for ZL Group, the shear bond strength was ZL04($25.09{\pm}5.67MPa$), ZL08($22.98{\pm}2.26MPa$), ZL12($21.54{\pm}5.70MPa$), ZL20($23.98{\pm}3.23MPa$)(p<0.05). Conclusion: The research results showed that the bond strength of Zirconia core and Pressing ceramic was further improved by physical surface treatment before sintering, rather than by chemical bonding through liner surface treatment.

Reproducibility of Lateral Cephalometric Landmarks According to Radiographic Image Enhancement (방사선상 enhancement 정도에 따른 측모두부방사선규격사진 계측점 설정의 재현도)

  • The purpose of this study was to evaluate the reproducibility of lateral cephalometric landmarks according to radiographic image enhancement, and to contribute to the identification of cephalometric landmarks. Lateral cephalograms of ten individuals were taken and stored into computer. The images were then enhanced up to four grades by Quick Ceph Image Pro$^{TM}$ on condition that the gray-scale equalization number was 50 and the detail enhancement number was 50. After thirty two landmarks were identified on monitor images by five observers, the deviations from the mean, the distances estimated between identified points and the mean point of five identified points, were evaluated for each landmark at each enhancement grade. Through the statistical analysis, following results were obtained. 1. In case of unenhanced radiographic images, the inter-observer reproducibility of the landmarks showed a large variation. 2. The comparison of deviation from the mean according to the degree of radiographic image enhancement for each landmark showed that the inter-observer reproducibility was significantly different at 5 landmarks. 3. The landmark of pterygomaxillary fissure showed higher reproducibility at enhancement grade 1 and 2 images than at unenhanced images. So did the landmark of posterior nasal spine at enhancement grade 1 images, and the landmark of menton at enhancement grade 2, 3 and 4 images respectively. The above results suggest that the reproducibility of some landmarks can be increased by radiographic image enhancement during the identification of the lateral cephalometric landmarks on the monitor.

Insertion and removal torques according to orthodontic mini-screw design (교정용 미니스크류의 디자인에 따른 식립 및 제거 토오크)

  • Objective: This study was designed to analyze the primary and secondary stability characteristics of orthodontic mini-screws of tapered design when compared with the cylinder mini-screw. Methods: A total of 48 mini-screws were placed into the buccal alveolar bone of the mandible in 6 male beagle dogs. Comparison was made between tapered and cylinder type mini-screws (Biomaterials Korea, Seoul, Korea). Maximum insertion torque (MIT) was measured using a torque sensor (Mark-10, MGT 50, USA) during installation, and maximum removal torque (MRT) was recorded after 3 and 12 weeks of loading. Results: Taper mini-screws showed a higher MIT value of 22.3 Ncm compared with cylinder mini-screw showing 13.6 Ncm (p < 0.001). The MRT of the taper mini-screw showed a significantly higher value of 9.1 Ncm than those of cylinder mini-screw of 5.7 Ncm at 3-weeks after installation (p < 0.05). However, there was no difference in the MRT value between the taper and cylinder mini-screws at 12 weeks of loading. Conclusions: These results showed that the high insertion torque of the taper mini-screw design increases initial stability until 3 weeks of loading, but does not have any effect on the secondary stability at 12 weeks of loading.

A STUDY ON THE STRESS DISTRIBUTION OF POSTERIOR FIXED PARTIAL DENTURE WITH VARIOUS ALL-CERAMIC SYSTEMS (구치부 고정성 국소의치에서 전부도재 시스템에 따른 응력분포에 관한 연구)

  • Purpose. The purpose of this study was to analyze stress distribution of all ceramic posterior fixed partial denture using a three dimensional finite element method. Material and method. A three dimensional finite element model was created to demonstrate all-ceramic posterior fixed partial denture and then this computer model measured the stress distribution of the all ceramic bridges which has a ceramic core materials such as Zirconia, IPS Empress. 2. In-Ceram zirconia, Metal-Ceramic. Also the stress distribution was examined according to loading sites when force was applied to sites such as the central area of second premolar the mesial connector of pontic, the central fossa of pontic, the distal connector of pontic, and the central fossa of second molar. Results. 1. In all the materials of the core in this study, von Mises stress indicated that the stress increased as force was applied to loaded sites, just at those points, on the connector, and the margin in the area adjacent to the connectors. 2. The maximum principal stress was much higher in the lower part of the connectors than in any other region. 3. As the load was applied to the different locations, the research showed a consistent increase of stress in the lower connectors. The maximum value of the von Mises stress was two or three times greater when the load was applied directly to the connectors rather than indirectly through another stressed region. 4. In the case of In-Ceram zirconia, the stress in lower connectors was the highest of all the reference points, the stress showed 75% of all the maximum stress. Ziconia showed 72%, Metal Ceramic 67% and IPS Empress 2 50%. 5. In the case of Ziconia, the stress was well dispersed in each reference point that the stress differences were smaller when compared to In-Cream ziconia.

Maxillary sinus septum;panoramic radiographic and dental computed tomographic analyses in the planning of implant surgery (상악동 중격;임플란트 수술 계획시 파노라마와 치과용 전산화 단충촬영 분석)

  • Surgical intervention in the posterior maxillary region requires detailed knowledge of maxillary sinus anatomy and the possible anatomical variations. This study evaluated the incidence, location of maxillary sinus septa by using radiographic (panoramic radiography and computed tomography) findings and comparison of panoramic radography with CT in antral anatomical variation. This study was based on data from 70 sinuses in partial dentate maxilla. The sample consisted of 61 patients(25 women and 36 men, with ages ranging between 19 and 77 years and a mean age of $49.4{\pm}11.3$ years) who were being treatment-planned to receive implant-supported restorations. First, the panoramic images were examined for the presence of antral septa by radiologist and examiner who don't know about CT findings. And incidence of antral septa was evaluated using an axial plane of CT image. The incidence of septa was compared between panoramic radiography and CT. The accuracy of the incidence was compared between radiologists and dentists. A total of 20 septa were found in 70 sinuses on CT image and the prevalence of one or more septa per sinus was found to be 28.6%. The assumed incidence of septa on panoramic radiography was $27.6%{\pm}2.2%$ in radiologist and $31.9%{\pm}5.8%$ in dentists. Erroneous diagnosis rate was 11.42% in radiologist and 15.96% in dentists. 40% of antral septa were located in the anterior(premolar) region, 30% of septa were located in the middle(first molar) and posterior(second molar) region separately. Prior to implant placement, it seems appropriate to consider panoramic radiography as a standard radiographic examination and periapical radiographs may be used to complete the findings in regions not sharply depicted in the panoramic radiograph. And cross-sectional imaging should be used in sites with severe bone loss and close proximity of the maxillary sinus.

THE EFFECT OF MONOMER TO POWDER RATIO ON POLYMERIZATION SHRINKAGE-STRAIN KINETICS OF POLYMER-BASED PROVISIONAL CROWN AND FIXED PARTIAL DENTURE MATERIALS

  • Statement of problem. Although a number of previous investigations have been carried out on the polymerization shrinkage-strain kinetics of provisional crown and fixed partial denture (FPD) materials, the effect of the changes of liquid monomer to powder ratio on its polymerization shrinkage-strain kinetics has not been reported. Purpose. The purpose of this study was to investigate the influence of liquid monomer to powder ratio of polymer-based provisional crown and FPD materials on the polymerization shrinkage-strain kinetics. Material and methods. Chemically activated acrylic provisional materials (Alike, Jet, Snap) were investigated. Each material was mixed with different liquid monomer to powder ratios by volume (1.0:3.0, 1.0:2.5, 1.0:2.0, 1.0:1.5, 1.0:1.0). Time dependent polymerization shrinkage- strain kinetics of all materials was measured by the bonded-disk method as a function of time at $23^{\circ}C$. Five recordings were taken for each ratio. The results were statistically analyzed using one-way ANOVA and the multiple comparison Scheffe test at the significance level of 0.05. Trends were also examined by linear regression. Results. At 5 minutes after mixing, the polymerization shrinkage-strains of all materials ranged from only 0.01% to 0.49%. At 10 minutes, the shrinkage-strain of Alike was the highest, 3.45% (liquid monomer to powder ratio=1.0:3.0). Jet and Snap were 2.69% (1.0:2.0) and 1.58% (1.0:3.0), respectively (P>0.05). Most shrinkage (94.3%-96.5%) occurred at 30 minutes after mixing for liquid monomer to powder ratio, ranging from 1.0:3.0 to 1.0:1.0. The highest polymerization shrinkage-strain values were observed for the liquid monomer to powder ratio of 1.0:3.0. At 120 minutes after mixing, the shrinkage-strain values were 4.67%, 4.18%, and 3.07% for Jet, Alike, and Snap, respectively. As the liquid monomer to powder ratio increased, the shrinkage-strain values tend to be decreased linearly (r=-0.769 for Alike, -0.717 for Jet, -0.435 for Snap, $r^2=0.592$ for Alike, 0.515 for Jet, 0.189 for Snap; P<0.05). Conclusion. The increase of the liquid monomer to powder ratio from 1.0:3.0 to 1.0:1.0 had a significant effect on the shrinkage-strain kinetics of polymer-based crown and FPD materials investigated. This increased the working time and decreased the shrinkage-strain during polymerization.

PREVALENCE AND RISK FACTORS OF SEVERE EARLY CHILDHOOD CARIES IN PRESCHOOL CHILDREN IN IKSAN CITY (익산시 취학전 어린이의 중증 유아기 우식증 유병률과 위험요인)

  • Preschool children in Iksan city, 3-5 years old, were examined for their prevalence of severe early childhood caries. The case definition was 1 or more cavitated, filled, or missing (due to caries) smooth surfaces in primary maxillary anterior teeth. Feeding behavior and oral hygiene behavior were investigated by a questionnaire. The prevalence was 20.8% for 36-47 months, 23.6% for 48-59 months, and 30.8% for 60-71 months. Going to sleep with bottle beyond 1 year (P<0.05), immediate removal of bottle after child fell on sleep (P<0.01), beginning time of toothbrushing (P<0.01) had significant relationship with the prevalence of severe early childhood caries. Toothbrushing frequency of primary care provider and use of oral cleaning aids by primary care provider had a tendency to be related with the prevalence(P<0.10).
