Multiple implant therapy with multiple inductions of general anesthesia in non-compliant patients with schizophrenia: A case report

  • The effectiveness of dental implants in patients with disability, who are non-compliant during treatment, is controversial because of their poor oral health. Thus, oral health-care and management in such patients is concerning. Moreover, limited information is available on prognosis after implant placement. Herein, we describe a patient with schizophrenia who underwent dental implantation under multiple inductions of general anesthesia (5 times) and required conservative treatment and tooth extraction for multiple dental caries and retained roots because of inadequate oral health-care. Postoperatively, fracture of the prosthodontics and progression of dental caries were observed, and with 3 additional inductions of general anesthesia, conservative treatment, implant surgery, and prosthesis implantation were conducted. Postoperative 12-month follow-up since the last prosthesis implantation showed successful results. For patients with schizophrenia, multiple implantation can reduce horizontal bone loss and achieve aesthetic results compared to treatment with removable prosthodontics and could serve as an alternative treatment modality.

  • 방사선치료를 받은 상악에 임플란트 지지 고정성 보철물을 이용하여 수복하는 것은 골치유능력이 낮기 때문에 상세한 치료계획이 필요하다. All-on-4 개념에 의한 임플란트 식립은 골이식을 피하면서 임플란트를 매식할 수 있어 유리하다. 일반적으로 경사된 임플란트에는 기성 경사형 지대주를 사용해왔다. 본 임상증례에서는 computer-aided design and computer-aided manufacturing (CAD/CAM)으로 제작된 지대주를 사용하였다. 본 증례는 all-on-4 개념에 의해 임플란트를 매식하고 CAD/CAM titanium 지대주를 제작한 다음 CAD/CAM zirconia 고정성 보철물로 수복하여 좋은 결과를 얻었기에 보고하고자 한다.

  • 디지털 자료를 이용한 진단 및 즉시 임플란트 식립, 그리고 보철 수복 방법은 전통적인 방법에 비하여 비교적 간단한 자료 채득을 통해 치료 과정을 간소화하고 정확도를 개선하며 데이터의 다양한 활용이 가능하다는 장점이 있다. 임플란트 즉시 식립 시 더 나은 심미적 결과를 얻기 위해서는 발치와의 치유 중에 임플란트 주변 연조직을 개선해야 한다. 이를 위하여 임시 보철물의 즉시 수복을 통해 출현윤곽의 형태를 적절하게 유도할 수 있으며, 향후 최종 수복시에 좋은 임상결과를 얻을 수 있어 환자 만족도가 높다. 본 증례는 상악 좌우 중절치에 외상 후 수평 치근파절이 발생함에 따라 심미성과 치조골 상태를 고려한 발치 후 임플란트 즉시 식립 및 부하를 계획하였다. 디지털 구강 스캐너를 이용한 인상 채득을 통해 외상 치아에 외력을 가하지 않으면서 보다 심미적인 보철물의 제작을 위한 디지털 진단 왁스업을 시행하였으며, 이를 토대로 이상적인 식립 위치를 결정하고 3D 프린팅 수술용 가이드를 이용한 임플란트 즉시 식립을 시행하였다. 식립 5일 후 임시 보철물을 수복하였으며, 3개월 동안 임시 보철물의 형태를 이용한 연조직 개선 및 형태 유도 과정을 거쳐 최종 지르코니아 보철물 수복하였다.

Single-tooth implant restoration with alveolar bone augmentation in the maxillary anterior tooth region: a case report

  • In case of gingival recession and alveolar bone defects due to tooth loss for a long period of time in a single tooth in the maxillary anterior region, it is not easy to obtain aesthetic results with a single implant prosthesis. For aesthetic restoration, it is important to preserve hard and soft tissues through alveolar bone augmentation as well as restore harmony with adjacent teeth and soft tissues by placing the implant in an ideal location. In this case, an implant was placed using guided bone regeneration and a connective tissue graft simultaneously with immediate implantation after extraction from the maxillary anterior region where only residual root was left for a long period of time.

임플란트 보철 기능후 고정체 주변 콘빔CT 골밀도 평가 (Bone density around the fixture after function of implant molar prosthesis using CBCT)

  • Purpose : The purpose of this study was to examine the significance of increased bone density according to whether bone grafts were applied using demographic data with Cone Beam Computed Tomography (CBCT) and to compare the bone densities between before and after implant prosthesis using the Hounsfield index. Materials and Methods : Thirty-six randomly selected computed tomography (CT) scans were used for the analysis. The same sites were evaluated digitally using the Hounsfield scale with V-Implant $2.0^{TM}$, and the results were compared with maxillary posterior bone graft. Statistical data analysis was carried out to determine the correlation between the recorded Hounsfield unit (HU) of the bone graft and implant prosthesis using a Mann-Whitney U test and Wilcoxon Matched-pairs test. Results : The bone grafted maxillary posterior teeth showed an increase in the mean values from-157 HU to 387 HU, whereas non-grafted maxillary posterior teeth showed an increase from 62 HU to 342 HU. After implantation, the grafted and non-grafted groups showed significantly higher bone density than before implantation. However, the grafted group showed significantly more changes than the non-grafted group. Conclusion : Bone density measurements using CBCT might provide an objective assessment of the bone quality as well as the correlation between bone density (Hounsfield scale) and bone grafts in the maxillary molar area.

  • Kole's Anterior segmental osteotomy of the mandible is commonly used to close an anterior open bite, to depress an elevated anterior dentoalveolar segment, or to retrude or advance a dentoalveolar segment. The procedure is often combined with an anterior maxillary segmental osteotomy to correct bimaxillary protrusion. We report 53-year-old woman who the extruded state of mandibular anterior alveolar segment was corrected using an mandibular anterior alveolar segmental osteotomy and dental implantation of the anterior maxilla. We planned to remove the old prosthesis, and then perform an anterior mandibular segmental osteotomy and implant restoration of the anterior maxilla. We suggest that anterior segmental osteotomy is very useful for rehabilitating edentulous patients with malaligned alveolar segment.

발치와 치조제 보존술 후 식립한 임플란트 주위 골 변화: 후향적 방사선학적 분석 (Periimplant bone change after alveolar ridge preservation: radiographic retrospective study)

  • 목적: 구치부 임플란트 식립을 위하여 발치와 치조제 보존술을 시행한 환자를 대상으로 임플란트 식립 직 후, 최종 보철물 장착 후 및 유지관리 기간에 따른 치조제 높이 변화량을 방사선학적으로 측정하여 임플란트 주위 골 변화를 평가하고자 하였다. 대상 및 방법: 치조제 보존술을 시행한 후 식립한 총 36개의 임플란트를 대상으로 각 시기에 촬영한 파노라마 방사선 사진을 이용하여 근·원심측의 치조제 높이를 각각 수직적 수치로 측정하였다. 환자의 연령, 성별, 전신질환, 임플란트 식립 위치, 골 이식재 유형, 차폐막 적용 방법, 술자의 숙련도, 측정 시기간의 치조제 높이 변화량은 평균, 표준편차를 구하였고, 치조제 높이 변화량과 각각의 요소들 간의 연관성은 독립표본 t검정, 기간별 측정한 치조제 높이 간의 연관성은 대응표본 t검정을 시행하였다(P < .05). 결과: 각 시기별 치조제 높이 변화량은 각각의 시기 사이에 모두 통계적으로 유의한 차이를 보였고(P < .05), 각 시기 간의 치조제 높이 변화량과 평가된 요소들은 통계적으로 유의한 연관성을 보이지 않았다. 결론: 발치와 치조제 보존술 후 식립된 임플란트 주위 골조직은 환자 및 수술적 요소에 영향을 받지 않고 유지기간동안 안정적으로 유지되었으며, 발치와 치조제 보존술은 임상적으로 사용가능한 술식이라고 사료된다.

Maxillary Sinus Floor Augmentation Using Autogenous Tooth Bone Graft in Combination with Platelet-Rich Plasma for Dental Implants: Case Series

  • Purpose: To determine the benefits of autogenous tooth bone (ATB) graft in combination with platelet-rich plasma (PRP) in the rates of success and survival of dental implants placed simultaneously with maxillary sinus floor augmentation (MSFA). Materials and Methods: Patients who visited the Department of Oral and Maxillofacial Surgery at Ulsan University Hospital from 2012 to 2014 and underwent simultaneous placement of implants with MSFA using ATB plus PRP were included in the study. Success and survival rates of the implants were evaluated based on the parameters of age and sex of the patient, site, follow-up period, residual bone height before surgery, diameter, and length of implant, sinus mucosa impairment, and postoperative complications. Result: A total of 23 patients and 67 implants were included in this study. The average age of the patients was $53.78{\pm}10.00$ years. The average follow-up period after installation of the prosthesis was $53{\pm}5$ months. The success and survival rates of the implants after placement of prosthesis were 95.52% and 97.01%, respectively. Conclusion: Combination of ATB and PRP showed high overall success rate, and it can be concluded that this combination is a predictable bone graft procedure for MSFA.

Evaluation of failed implants and reimplantation at sites of previous dental implant failure: survival rates and risk factors

  • Purpose: The purpose of this study was to evaluate failed implants and reimplantation survival and to identify the relative risk factors for implant re-failure. Methods: Ninety-one dental implants were extracted between 2006 and 2020 at the National Health Insurance Service Ilsan Hospital, including 56 implants in the maxilla and 35 implants in the mandible that were removed from 77 patients. Patient information (e.g., age, sex, and systemic diseases) and surgical information (e.g., the date of surgery and location of the implants and bone grafts) were recorded. If an implant prosthesis was used, prosthesis information was also recorded. Results: In total, 91 first-time failed dental implants in 77 patients were analyzed. Of them, 69 implants in 61 patients received reimplantation after failure. Sixteen patients (22 implants) refused reimplantation or received reimplantation at a different site. Eight of the 69 reimplants failed again. The 1-year survival rate of the 69 reimplants was 89.4%. Age at reimplantation and smoking significantly increased the risk of reimplantation failure. However, a history of taking anti-thrombotic agents showed a statistically significant negative association with reimplantation failure. Of the failed implants, 66% showed early failure and 34% showed late failure of the initial implantation. All 8 re-failed implants showed early failure. Only 3 of these 8 failed reimplants were re-tried and the second reimplants all survived. Conclusions: The total survival rate of implants, which included reimplants and second reimplants was 99.2%, although the survival rate of the initial implantations was 96.3%. Previous failure did not affect the success of the next trial. Reimplantation failure was more strongly affected by patient factors than by implant factors. Therefore, each patient's specific factors need to be meticulously controlled to achieve successful reimplantation.

상악 전치부 발치 즉시 식립시 골유도재생술과 혈관개재 골막-결합조직 판막술(VIP-CT graft)의 활용 (The Effective Utilization of GBR and VIP-CT(Vascularized Interpositional Periosteal Connective Tissue) graft in the Anterior Maxillary Immediate Implantation : A Clinical Case Report)

  • 상악전치부에서 치아 임플란트의 심미적 회복은 경조직, 연조직, 심미적 보철물이 조화를 이루어야 하며 임상의에게 도전적인 과제이다. VIP-CT graft와 같은 새로운 기법이 많은 볼륨의 연조직 이식을 위해서 소개되어졌다. VIP-CT graft의 장점은 많은 연조직의 결손부위를 회복할 수 있고 술후 수축이 적다는 것이다. 게다가 그것은 부가적인 혈행공급을 통해 경조직의 증대도 촉진실킬 수 있다. 이 기법은 환자분의 불편감을 줄여주고 치료시간을 단축시킨다. 이 논문에서는 순면 결손부위가 있는 상악전치부에서 즉시 임플란트 식립을 할때 골이식과 VIP-CT graft 술식을 하는 과정에 대해 설명드리고자 한다.
