올세라믹 심미 수복재료의 최신 동향과 적합한 시멘트 선택 및 사용 (Recent Trend of Esthetic All-Ceramic Materials and Guidelines for Correct Cementation)

  • Recently, there are much improvement in optical and mechanical properties of dental ceramic materials coupled with improved fabrication techniques, which have caused a considerable shift in the preference of the dentists to ceramic restorations. Because the chemical composition and microstructure of all-ceramic materials are different by the type, correct choice of cement type and surface treatment procedure, and cementation strategy is essential for the success of ceramic restorations with adequate retention and decreased incidence of complications. This manuscript reviews on the most often prescribed and some newly developed ceramic materials, and the selection criteria and usage guidelines of cement materials that are used in conjunction with various ceramic materials. This manuscript emphasizes that continuous updating the information of newly developed ceramic and cement materials and application techniques by the dentists and dental staffs are demanding in response to the constantly improving ceramic and cement materials and corresponding application protocol changes.

Dental Plaque Streptococci가 생산하는 세포외 다당류에 관한 연구 (STUDIES ON THE EXTRACELLULAR POLYSACCHARIDES PRODUCED BY ISOLATED DENTAL PLAQUE STREPTOCOCCI)

  • The present investigation describes the chemical nature of purified extracellular polysaccharides synthesized by tow resembling streptococci, Streptococcus salivarius strain SD-1 and Streptococcus mitis strain SD-9, which was isolated from human dental plaque, with the following results. 1. Extracellular polysaccharide by Streptoccus salivarius strain SD-1 is recovered mostly in Fraction I-and II-45% of the presne purification procedure, leaving trace amount in Fraction I-and II-70%. 2. The dextran is the major polysaccharides in Fraction I-and II-45% of both strains with minor amounts of levan. 3. The Fraction I-and II-45% of both strains contain glucose and fructose, which its 70% of the same fractions glucose only. 4. It appears that the glycerol was the major end product of the Smith degradation of the Fraction I-45% of both strains.

비출혈 환자에서 굴곡성 기관지를 이용한 맹목적 기관내 삽관 (Blind Intubation Using Fiberoptic Bronchoscope in Epistaxis)

  • Nasotracheal intubation is an essential procedure during general anesthesia for dental treatment. Fiberoptic intubation is best accomplished by those who perform it as part of their daily practice. But nasal approach of fiberoptic intubation has some complications such as epistaxis and laryngeal injury. Especially, epistaxis is common and it make fiberoptic intubation because of limited view. When the epistaxis obstruct the field of vision we have to withdraw the fiberoptic bronchoscope and consider the other method for securing the airway. We succeeded in securing the airway of patient who had epistaxis during the fiberoptic nasotracheal intubation without withdrawing the fiberoptic bronchoscope. We used blind intubation guided by light source placed the tip of fiberoptic bronchscope applied to lightwand intubation.

치수절제술 시 고려해야 할 유구치 치근과 근관 (Consideration of primary molar's root and root canal for pulpectomy treatment)

  • The purpose of the pulp treatment is to make the function of the primary molar in the oral cavity possible for as long as possible until the exfoliation of the tooth or the development of the occlusion is as long as possible. The pulpectomy is a relatively common procedure for the pediatric dentist in the clinic with dental care of the children nowadays. Primary molar is morphologically more susceptible to dental caries than permanent tooth, and the dental pulp treatment is frequently performed. Furthermore, unlike permanent teeth, the root canal has a large degree of curvature and morphological diversity and complexity, careful consideration is needed. Therefore, it is very important to comprehensively understand the morphological characteristics and diversity of the root and root canals for the successful pulp treatment of the primary molars.

Foreign body aspiration during dental treatment under general anesthesia: A case report

  • Foreign body aspiration in dental clinics is the most common cause of respiratory emergencies. There are no reports on foreign body aspiration during dental treatment under stable general anesthesia because the patient neither has voluntary movements nor reflex actions. This is a case report on the fall of a prosthesis in the larynx, which occurs rarely under general anesthesia. During the try-in procedure, the prosthesis slid from the surgeon's hand and entered the retromylohyoid space, and while searching for it, it passed down the larynx to the endotracheal tube balloon, leading to a dangerous situation. The prosthesis was promptly removed using video-assisted laryngoscope and forceps, and the patient was discharged without any complications.

Anesthetic management for dental surgery in a child with glycogen storage disease type IIIa: a case report

  • Glycogen storage disease (GSD) is a group of inherited disorders, which result in the deficiency of enzymes involved in glycogen metabolism, leading to an accumulation of glycogen in various organs. Deficiency of amylo-1-6-glicosidase (debranching enzyme) causes glycogen storage disease type III (GSD III). The main problems that anesthesiologists face in patients with GSD III include hypoglycemia, muscle weakness, delayed awakening due to abnormal liver function, possible difficulty in airway, and cardiomyopathy. In the face of these difficulties, airway preparation and appropriate glucose monitoring and support during the fasting period are important. The doses of the drugs to be used should be calculated considering the increased volume of distribution and decreased metabolic activity of the liver. We present the case of a child with GSD IIIa who underwent dental prosedation under general anesthesia. She was also being prepared for liver transplantation. This case was additionally complicated by the patient's serious allergic reaction to eggs and milk.

임플란트 수술용 가이드를 이용하여 임플란트를 식립한 환자의 만족도 (Satisfaction of patients with implants placed using the implant surgical guide)

  • Background: This study was aimed at improving the satisfaction of patients with implants placed using an implant surgical guide by analyzing patient satisfaction with the implant procedure carried out using the guide . Methods: We extracted convenient samples from patients with implants who had visited dental hospitals and clinics in Mokpo from March 2 to June 30, 2022. We used 228 copies of the self-report survey for analyses. Results: All items of implant satisfaction were positively correlated with the use of an implant surgical guide. Conclusions:The results of this study showed that implant satisfaction can be increased with the use of implant surgical guides.

장애인 환자의 치과치료를 위한 전신마취 시 협조도와 마취 유도 방법에 대한 통계적 고찰 (Clinico-statistical Analysis of Cooperation and Anesthetic Induction Method of Dental Patients with Special Needs)

  • Background: This study aimed to evaluate the cooperative levels of dental patients requiring general anesthesia during dental treatments. Anesthetic induction methods for patients were also recorded and analyzed using descriptive statistics. Methods: Total 566 patients who visited Seoul National University Dental Hospital Clinic for Persons with Disabilities were reviewed on pre-anesthetic review and anesthesia records. The cooperative levels of patients were graded by 4 levels and induction methods used for the patients during general anesthesia application were analyzed. Results: More than half of patients(55.8%) were willing to receive the anesthetic induction(cooperative level 1), 18.6% were minimally cooperative(level 2), 20.8% needed physical restraint prior to induction(level 3), and 4.8% was poorly cooperative and induction procedure was performed under an unconscious condition after ketamine intramuscular injection(level 4). There was no gender difference in cooperative levels(P=0.11). Patients over 30 years revealed better cooperation levels compared to other age groups(P<0.05). For patients of level 1, 53.5% were anesthetized in a way of intravenous induction, while 77.1% out of patients of level 3 were anesthetically induced through inhalation method. Conclusion: Many dental patients with special needs were not cooperative to receive anesthetic induction. Additional behavioral support may be applied to poorly cooperative patients for the safe and successful clinical outcome.

Comparative evaluation of the effectiveness of two innovative methods in the management of anxiety in a dental office: a randomized controlled trial

  • Background: The first dental experience is vital in molding a child's attitude towards dentistry and dental outcomes. The cooperation of a child during dental treatment is essential to render successful and high-quality treatment. Dental anxiety is common in children undergoing dental treatment. The success of pediatric dental treatments and patient comfort depends on controlling the levels of patient anxiety in clinical settings. This study aimed to compare the effectiveness of the recorded maternal voice and virtual cognitive tool (Roogies application) in the management of pediatric dental patients. Methods: The study was carried out with children aged of 4-7 years [n = 80, (40 male and 40 female)], without any past dental history, and were randomly allocated into two groups. After informed consent was obtained, the entire procedure was explained to the parents. Anxiety was assessed pre-, during, and post-treatment by measuring pulse rate, and recording Venham Picture Test (VPT) scores. Group A [n = 40; 20 boys and 20 girls)] was provided with a headphone that played a recorded maternal voice. Group B [n = 40; 20 boys and 20 girls)] was administered the virtual cognitive tool. After conditioning the children, oral prophylaxis was performed for both groups. A comparative evaluation was conducted for each treatment session. Results: The intra-group comparison of VPT scores and heart rate for patients assigned to the recorded maternal voice showed a statistically significant difference in dental anxiety (P-value ≤0.001). Conclusion: This study demonstrated that a reduction in dental anxiety with the help of recorded maternal voice forms an important component of non-pharmacological behavior management. Alternatively, the use of a virtual cognitive tool as an anxiety-reducing technique can also be advocated.

Fabricating a Ceramic-Pressed-to-Metal Restoration with Computer-Aided Design, Computer-Aided Manufacturing and Selective Laser Sintering: A Case Report

  • Even though a conventional metal ceramic restoration is widely in use, its laboratory procedure is still technique-sensitive, complex, and time-consuming. A ceramic-pressed-to-metal restoration (PTM) can be a reliable alternative. However, simplified laboratory procedure for a PTM is still necessary. The article is to propose a technique that reduces time and effort to fabricate a PTM with the aid of computer-aided design, computer-aided manufacturing and selective laser sintering technologies.
