  • The design process of the dental handpiece is described. The parameters of the high speed air turbine are estimated. The effect of supply hole on the stiffuess and damping of the air bearing for handpiece is studied numerically. The Reynolds equation is solved by using the divergence formulation and the perturbation method. The test rig is built and the test procedure is developed for the turbine rotational speed measurement by using Fourier transform of noise generated by the turbine during steady operation.

  • The most valuable site of laser treatment is analgesic effect and fast healing process. If we understand an use this advantage correctly, it will helpful in relationship with patients and hospital management. So this study reported clinical cases about laser used in esthetic treatment with literature review. Periodontal treatment using laser was taken for patient who complain about gingival swelling during orthodontic treatment. And, esthetic plastic treatment using laser for alveoar reduction was taken for patient who complained about esthetic problem of maxillary gingiva. The treatment using laser can shorten the inconvenience and complicated treatment process and minimize patient's fear and discomfort during treatment procedure. So patients and clinicians can expect competent result in clinical cases. The laser for esthetic treatment was valuable.

  • Oral appliance therapy is a simple, reversible way for improving snoring and/or obstructive sleep apnea. It may be indicated for the patients who are unable to tolerate continuous positive airway pressure (CPAP) or who have potential risks for surgical intervention. Oral appliance therapy increases airway space by the providing stable anterior positioning of the mandible, pulling out tongue, lifting up soft palate, or changing the muscle activity of the genioglossus. Currently, more than 80 different types of oral appliances have been introduced for snoring and/or obstructive sleep apnea. They are classified by their characteristics such as mode of action, adjustability and material used. This article provides a detailed clinical protocol and treatment procedure for oral appliance therapy.

  • The clinical investigation and operation procedure were described on the gunshot wound which involved on soft, hard palate and nasal cavity. The patient, 19 years old, female, admitted in Han Yang Medical Center with clinical diagnosis of maxillofacial injuries on Nov. 1973. No Significant signs include of airway obstruction, Oro-nasal bleeding were revealed only exception of rupture and perforation on the soft, hard palate. For closure and reduction of destructed palatal wound, operation was done in out patient dental clinic under local anesthesia by means of Langenbeck method. And to control of post-operative inflammation and reactive swelling, administration of accurate antibiotics and physical therapy were performed for 5 days after operation. On the 10th day after administration, patient was discharged with satisfactory result of operation.

  • Therapeutic injections into the craniofacial region can be a complex procedure because of the nature of its anatomical structure. This technical note demonstrates a process for creating an extra-oral template to inject therapeutic substances into the temporomandibular joint and the lateral pterygoid muscle. The described process involves merging cone-beam computed tomography data and extra-oral facial scans obtained using a mobile device to establish a correlated data set for virtual planning. Virtual injection points were simulated using existing dental implant planning software to assist clinicians in precisely targeting specific anatomical structures. A template was designed and then 3D printed. The printed template showed adequate surface fit. This innovative process demonstrates a potential new clinical technique. However, further validation and in vivo trials are necessary to assess its full potential.

  • The need for repairing removable dentures has grown as the population had aged. The direct methods allow existing dentures to be repaired without interrupting their use. However, if patient compliance is low, direct methods may be challenging. Moreover, attaching an artificial tooth to a metal base is a complicated procedure because it necessitates casting a retentive element and soldering it to a metal base. This clinical report describes how to add an artificial tooth to a metal base, reline denture bases, and reestablish occlusion on relined removable dentures using indirect methods. Existing removable dentures were successfully repaired and their service life was efficiently extended using the methods described.

  • FRC/ceromer system provides the clinician with a durable, flexible, and esthetic alternative to conventional porcelain fused to metal crowns. FRC is the matrix which is silica-coated and embedded in a resin matrix. The ceromer material which is a second generation indirect composite resin contains silanized, microhybrid inorganic fillers embedded in a light-curing organic matrix. FRC/ceromer restoration has a several advantages: better shock absorption, less wear of occluding teeth, translucency, color stability, bonding ability to dental hard tissues, and resiliency. It has versatility of use including inlay, onlay, single crown, and esthetic veneers. With adhesive technique, it can be used for single tooth replacement in forms of inlay adhesion bridge. In single tooth missing case, conventional PFM bridge has been used for esthetic restoration. However, this restoration has several disadvantages such as high cost, potential framework distortion during fabrication, and difficulty in repairing fractures. Inlay adhesion bridge with FRC/ceromer would be a good alternative treatment plan. This article describes a cases restored with Targis/Vectris inlay adhesion bridge. Tooth preparation guide, fabrication procedure, and cementation procedure of this system will be dealt. The strength/weakness of this restoration will be mentioned, also. If it has been used appropriately in carefully selected case, it can satisfy not only dentist's demand of sparing dental hard tissue but also patient's desire of seeking a esthetic restorations with a natural appearance.

  • Space maintainers needs following extraction of primary molars, due to severe dental caries under general anesthesia. Delivery of conventional space maintainers has three of the disadvantages of the method, there being a impression taking and time consuming due to laboratory procedure, and delivery problem due to relaxed tongue. For this problem improved, modified space maintainer is prefabricated before procedure, and adjust, delivery in operation room, favorable results were obtained. The following results were obtained. 1. Delivery time is very reduced than conventional space maintainers. 2. After space maintainer delivery, follow check-up, without impingement of soft tissue, distortion of wire. After space maintainer delivery, continuous research is required so that space maintainer delivery is applied to usual dental treatment, and it is necessary of long term research.

  • Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.

Pseudo-Aneurysm in Internal Maxillary Artery Caused by Radiofrequency Ablation: Literature Review with a Case Report

  • The case of pseudo-aneurysm of internal maxillary artery (IMA) in oral and maxillofacial region is known to be very rare. The etiology of this case was regarded as IMA injury by radiofrequency ablation (RFA) and such incidence was not reported previously. One case of false aneurysm in the IMA was referred from local dental clinic to our department. Left facial swelling was observed with severe trismus immediately after radiofrequency procedure for masseteric nerve block in local dental clinic. Despite of medication and surgical intervention, the swelling did not subside and there was massive bleeding and pulsation on one of the follow ups. The traumatic vascular disorder was suspected and finally diagnosed with angiography and treated by embolization procedure. RFA targeting masseteric nerve or trigeminal ganglion may cause traumatic injury to adjacent anatomic structures such as IMA, resulting in pseudo-aneurysm. Clinicians must be aware of potential damages of RFA. Angiography enables the solid diagnosis for pseudo-aneurysm, and selective embolization can be optimum treatment method.
