Analysis of dental utilization and expenditure of patients with chronic diseases

  • Objectives: Using the annual data from the 2016 Korean Health Panel, this study aims to identify the factors that affect the dental utilization and expenditure of patients with chronic diseases, and to provide basic data to explain the inequality gap in dental utilization. Methods: The dental utilization and expenditure of 3,557 patients with chronic diseases were analyzedfor frequency using the SPSS Windows version 23.0 (SPSS Inc. IL, USA). Analysis of the factors that affect dental utilization and expenditure were performed using a multiple regression analysis. The level of statistical significance was 0.05. Results: The frequency of dental utilization in patients with chronic diseases was high for subjects who were younger than 65 years and those whose education level was below high school. The frequency of dental utilization was relatively lower for subjects who did not have disabilities and those with healthier subjective health status. The dental expenditure of patients with chronic diseases was higher in subjects who were younger than 65 years and those with greater household income. Conclusions: The above findings suggest that a plan is needed to control dental utilization by efficiently managing chronic diseases, and that a policy-based plan is needed to devise ways to supplement the uninsured medical expenses of dental care.

치주질환 유병자의 생애 치과의료비 추정 (Estimation of lifetime dental expenditures for periodontitis)

  • Objectives: The objective of this paper was to estimate a South Korean's lifetime dental expenditures for periodontitis. Methods: For our study, we collected data from the Korea Health Panel Survey (KHPS), from 2010 to 2016. The outpatient dental care data of 1,919,608 samples of periodontitis were extracted using R version 3.0 and estimations of lifetime dental expenditures for them were generated using Excel. Results: Over 50% of the lifetime dental expenditure of South Koreans was spent for periodontitis, and incurred after the age of 40. The results showed that an estimate of average per capita lifetime dental expenditure for men (approximately 13 million won) was greater than that for women (approximately 8.8 million won) for periodontitis. Conclusions: Efficient methods for the prevention and management of periodontitis are necessary, and a new paradigm of health care system is required to reduce dental expenditure through its prevention.

우리나라 치과 외래의료비 지출규모와 치과 외래의료비 지출에 미치는 요인 (Expenditure in ambulatory dental care and factors related to its spending)

  • This study estimates the total health expenditure of ambulatory dental care and explores the factors related to disbursements. The study used two waves of a 2008 Korea Health Panel (KHP) survey, of which each wave is composed of 7866 households and 24,659 persons. The KHP includes missing expanses of reimbursement data of the National Health Insurance (NHI), such as out-of-pocket, drugs, and private health insurance. The study estimates total monthly ambulatory dental expenditure and the sub-special categories of dental care. For influential factors analyses, the study exploits log-linear model with age, gender, education, job, equivalence income, the status of chronic diseases, means-tested benefit recipients, private insurance, and the composite deprivation index as independent variables. The total monthly outpatient health spending is estimated to be 102,468 won per household, and for dental, each household spends 31,115 won per month. Older age, means-test recipients, non-regular workers are more likely to spend less money on dental care, whereas private insurers, high income, and those who live in less deprived areas are more likely to spend more money for dental services. From the study we found that the KHP data are more suitable to estimate the total amount of health care markets, especially when the NHI coverage is low, such as for dental care in Korea.

치과위생사와 치위생과 학생들의 4차 산업혁명에 대한 인식도와 교육요구도에 관한 연구 (A study on the dental hygiene's and dental hygiene students's perception and the educational demands in the 4th industrial revolution of the health care professionals)

  • Objectives: This study investigated the perception and educational needs of the health care professionals in wake of the fourth industrial revolution. Methods: This study was conducted among 115 students from the Department of Dental Hygiene and 122 dental hygienists from Gwangju and Jeonnam, from June 25 to September 20, 2020. The questionnaire consisted of one question on the perception of the concept of the fourth industrial revolution and two questions on the influence of the fourth industrial revolution. It contained a total of eight questions on the desired education hours and education expenditure. Results: The degree of perception of the concept and influence of the fourth industrial revolution was 3.20 for both dental hygiene students and dental hygienists; the average of the two groups was the same. Regarding the educational experience, 'no experience' was the most marked response among both dental hygiene students and dental hygienists and no statistically significant difference was observed (p>0.05). Conclusions: As demanded by the changing times, it is necessary to identify the differences in educational demands of the two groups, develop educational programs according to the fourth industrial revolution, as demanded by the changing times, and apply educational programs that fit the educational needs of each group.

한국 소아치과의 현재와 전망 - 치아우식증관리 분야를 중심으로 - (PRESENT SITUATION AND PROSPECT OF PEDIATRIC DENTISTRY IN KOREA - FOCUSED ON MANAGEMENT OF DENTAL CARIES -)

  • General status of pediatric dentistry in Korea is to conduct vigorous academic activities and specialized medical care centering the Korean Association of Pediatric Dentistry (KAPD) that has about 1,000 pediatric dentists as members, pediatric dentistry departments of 11 Colleges of Dentistry, numbers of pediatric dentistry training institutions and private clinics specialized in children. From 1996, the accredited pediatric dentists were produced by the KAPD and from 2008, the state began to produce the accredited pediatric dentists. Since then, doctors with expertise in pediatric care had opened private clinics in addition to the university hospitals, it became the basis of a momentum to deepen the specialty of pediatric dentistry. The Dentistry community of Korea is going through rapid and profound changes recently, and the underlying reasons for such changes can be classified largely into a few categories: (1) Decreasing population and structural changes in population (2) Increase in numbers of dentists, (3) Changes in the pattern of dental diseases and (4) Changes in medical environment. In Korea, the children population in the age range of 0 ~ 14 years old had been decreased by 2 million in 2010 compared to that of 2000 due to reduction of birth rate. The current population of children in the age range of 0 ~ 4 years old in 2010 takes up 16.2% of the total population, but it is estimated that such percentage would decrease to 8.0% by 2050. Such percentage is largely behind the estimated mean global population of 19.6% by 2050. On the other hand, the number of dentists had been largely increased from 18,000 in 2000 to 25,000 in 2010. And it is estimated that the number will be increased to 41,000 by 2030. In addition, the specialized personnel of Pediatric dentistry had been shown as increased by 2.5 times during past 10 years. For the changes in the pattern of dental diseases, including dental caries, each df rate of 5 years old children and 12 years old children had been decreased by 21.9% and 16.7% respectively in 2010 compared to 2000. Each df Index also had been decreased by 2.5 teeth and 1.2 teeth respectively. The medical expenditure of Korea is less than that of OECD and more specifically, the expenditure from the National Health Plan is less than OECD but the expenditure covered by households is larger than OECD. These facts indicate that it is considered as requiring the coverage of the national health plan to be reinforced more in the future and as such reinforcement needs continuous promotion. In medical examination pattern of Pediatric dentistry, the preventive and corrective treatment were increased whereas the restorative treatment was decreased. It is considered that such change is caused from decrease of dental caries from activation of the prevention project at national level. For the restorative treatment, the restorations in use of dental amalgam, pre-existing gold crowning and endodontic treatment had been decreased in their proportion while the restorative treatment in use of composite resin had been increased. It is considered that such changes is caused by the change of demands from patients and family or guardians as they desired more aesthetic improvement along with socio-economic growth of Korean society. Due to such changes in dentistry, the pediatric dentistry in Korea also attempts to have changes in the patterns of medical examination as follows; It tends to implement early stage treatment through early diagnosis utilizing various diagnostic tools such as FOTI or QLF. The early stage dental caries so called white spot had been included in the subjects for dental care or management and in order to do so, the medical care guidelines essentially accompanied with remineralization treatment as well as minimally invasive treatment is being generalized gradually. Also, centering the Pediatric dentists, the importance of caries risk assessment is being recognized, in addition that the management of dental caries is being changed from surgical approach to internal medicinal approach. Recently, efforts began to emerge in order to increase the target patients to be managed by dentists and to expand the application scope of Pediatric dentistry along with through such changes. The interest and activities of Pediatric dentists which had been limited to the medical examination room so far, is now being expanded externally, as they put efforts for participating in the preventive policy making process of the community or the state, and to support the political theories. And also opinions are being collected into the direction that the future- oriented strategic political tasks shall be selected and researches as well as presentations on the theoretical rationale of such tasks at the association level.

  • Objectives: This study aimed to verify the impact of dental health care insurance coverage policy by analyzing the changes in dental care utilization and expenditures over 5 years from 2012 to 2016, when the dental health insurance coverage expansion policy was implemented. From the national cohort data collected by the Korea Health Panel Survey, a retrospective study was conducted for all household members using dental services. Methods: This study statistically verified the difference in the dependent variables by frequency analysis, chi-square test, t-test, and one-way analysis of variance (SPSS version 22, IBM Co. USA, p<0.05). Results: The annual utilization rate steadily increased from 23.4% to 26.1% between 2012 and 2016. Although there were differences in utilization rates by gender, age, and income level, patients kept using the dental services in 2016 regardless of the type of health insurance. The average annual copayment for patient expenditures (out-of-pocket amount) increased from ₩463,844 to ₩537,401 in 2012 and 2016, respectively. Of the dental care expenditures over 5 years, the ratio of uninsured expenses by the elderly decreased from 38.5% to 25.9%, and the national health insurance service coverage increased from 40.3% to 49.1%. Conclusions: Although this policy did not reduce overall patient expenditures, it has been found that there was a positive effect on the elderly and low-income groups; it increased the utilization and access to dental services.

계량경제모델에 의한 치과의사 이력수급에 관한 연구 (Demand and Supply of Dental Manpower by the Econometrical Model)

  • The demand and supply of dentists was studied by the econometrical model. It is based on the data of socio-economic-cultural statistic from 1967 to 1977; GNP, average monthly consumption expenditure per household, consumption of milk, populat ion, consumption of energy, water supply per person a day and entrance rate of senior high school. 1.The curved regression equation and multiple correlation coefficient (R)between the number of dentists(Y) and year (x)were $Y=4,122(1.06)^x$,R=0.995. 2. From 1982 to 1985, expected demand and expected supply will be approximately balanced. But dentists will be oversupplied conspicuously from the year of 1986. Such a oversupply will be remarkably incresed to the amount of 1860 in the year of 2000. 3. It is seemed that balanced number of graduates of dental colleges will be about 350 to the year of 1985, from the year of 1986, will be about 450, from the year of 1981, will be about 600 and from the year of 1996, will be about 700. 4. In 2000, persons served by each dentist will be 3550 by the expected supply and 4120 by the expected demand.

소득계층별 보건의료이용의 양적.질적 차이 분석 -광주.전남 지역주민을 대상으로- (Quantitative and Qualitative Difference in the Utilization of Health Care - Based on the Survey of Gwangju-Jeonnam Residents)

  • The purpose of this study is to analyze the equity of health care utilization by income groups in terms of both quantity and quality of care, which is measured by expenditure, type of care, and type of health care institutions. Equity in health care utilization is measured by HIwv index, based on the survey of 1,480 Gwangju-Jeonnam residents. Health care utilization in terms of the probability and quantity of outpatient and inpatient care show equitable or pro-poor inequitable distribution, whereas the distribution of health care expenditure, which can account for the quality of care, is pro-rich inequitable, implying that the better off tend to use more expensive medical care. In terms of the types of care, simple visits for basic care show equitable distribution, whereas the distribution of the utilization of traditional tonic medicine, comprehensive health examination, CT, MRI, and ultrasound is pro-rich inequitable. Utilization of general hospitals and traditional health institutions show pro-rich inequitable distribution, hospitals and dental care institutions equitable, and physician clinics and public health centers pro-poor inequitable.

COVID-19로 인한 비대면 교육의 만족도와 구강보건교육의 방향성 (Satisfaction and direction of oral health education for non-face-to-face education due to COVID-19)

  • Owing to the Corona virus (COVID-19) crisis, virtual education has been expanded. Accordingly, this study was conducted to determine the direction of oral health education by examining participants' satisfaction with virtual education and educational media preferences. This study collected data from a Naver Form online survey targeting 290 university students across the country, from May 10 to 31, 2021. The collected data were analyzed using IBM SPSS 20.0. According to the data, satisfaction with virtual classes was 3.36 points in 5-point Likert scale, satisfaction factors were reduced commuting time and money expenditure, and the highest dissatisfaction factor was a decrease in lecture concentration. The media platform that most interested students pursing oral health education was YouTube. The oral health education that participants wished to receive through virtual education included how to prevent tooth decay, how to prevent gum disease, and how to brush teeth. In conclusion, it is necessary to develop various media like Zoom, YouTube, and virtual reality programs so that students feel motivated to utilize oral health education and improve oral health.

서울시 거주 청소년과 성인의 수돗물 불소농도조정사업 의식에 관한 조사연구 (Knowledge of adolescents and adults about water fluoridation among the residents of Seoul)

  • Objectives : The purpose of this study was to understand and compare the knowledge of the adolescents and adults among the residents of Seoul metropolitan city about water fluoridation program. Methods : A total of 613 adolescents and adults between the age of 15 and 31-49 was surveyed by the questionnaire. Among them, the source of drinking water, the source of cooking water, awareness of caries prevention effect of water fluoridation program, awareness of cost benefit of health insurance of water fluoridation program, approval of water fluoridation program, awareness of health safety of water fluoridation program were surveyed. Results : 85.5% of adolescents and 88.9% of adults were surveyed to use public water supply for drinking water, and 95.3% of adolescents and 96.8% of adults were surveyed to use public water supply for cooking water. But only 14.0% of adolescents, comparing to the 42.1% of adults, were surveyed to be aware of the caries prevention effect of water fluoridation, and only 6.0% of adolescents, comparing to the 24.6% of adults, were surveyed to be aware of the caries prevention effect of water fluoridation to reduce to health insurance expenditure. About 82.6% of adolescents, comparing to the 92.1% of adults, were surveyed to vote for the fluoridation program, and only 6.8% of adolescents, comparing to the 28.6% of adults, were surveyed to be aware of health safety of water fluoridation program. Conclusions : Extensive oral health education program to the adolescents are necessary about using water fluoridation to prevent the dental caries.
