An Epidemiologic study on the Orthodontic Patients Who Visited Department of Orthodontics, Chosun University Dental Hospital Last 10 Years(1990${\sim}$1999) (최근 10년간 조선대학교 부속치과병원 교정과에 내원한 부정교합 환자에 관한 역학적 연구(1990${\sim}$1999))

  • With socioeconomic development and change of esthetic recognition, the demand for orthodontic treatment and number of orthodontic patients has been increasing so rapidly. And frequency of malocclusion was changed. So this study was done in an attempt to provide an epidemiologic study so that we can accomodate their orthodontic needs adequately and to obtain the reliable quantitative information regarding the characteristics of orthodontic patients. Distribution and trends were examined in 3,070 malocclusion patients who had been examined and diagnosed at Department of Orthodontics, Dental Hospital, Chosun University over 10 year-period from 1990 to 1999. The results were as follows : 1. The number of patients per year was increasing trend and higher visiting rate in female(56.5%) than in male(43.5%). 2. Age distribution had shown 7${\sim}$ 12 year-old group being the largest(37.9%) and each percentage of 13${\sim}$18, 19${\sim}$24, above-19, 0${\sim}$6 year${\sim}$old group was 32.0%, 19.6%, 7.1%, 3.4%. 3. Hellman dental age IVa which is completion of the permanent dentition showed the highest percentage in male and female. 4. Geographic distribution showed a majority of patients in Kwang Ju(71.0%). Group within the distance 10km from Chosun Dental Hospital was 56.3% and group within 20km was 14.7%. 5. Anterior cross bite showed the highest percentage in chief complaints and percentage of Mn. prognathism and protrusion of Mx. teeth was 12.6%, 12.2%. 6. Distribution in the types of malocclusion according to the Angle's classification had shown; 38.9% for Class I, 20.7% for Class II division 1, 2.0% for Class III division 2, 38.4% for Class III. 7. In the dental vertical dysplasia according to the Angle's classification, deep bite was the most frequent in Class II div.1 and div. 2(24.3%, 56.7%) and open bite in Class III(21.4%). 8. In the skeletal sagittal dysplasia, 39.3% of skeletal Class II was due to the undergrowth of the mandible and 46.3% of skeletal Class III was due to the overgrowth of the mandible. 9. Distribution in orthodontic treatment acceding to the extraction and nonextraction had shown 66.9% for nonextraction case, 33.1% for extraction case, and four first bicuspids have been extracted in the highest percentage(38.6%). 10. Patients who had orthognathic surgery comprised 7.9%, with an increasing trend.

  • In esthetic treatment of bimaxillary protrusion, it is important to move backward the anterior teeth segment. For the backward movement of the anterior teeth segment, orthodontic force and segmental osteotomy have been applied on the clinical treatment until recently. These methods caused long treatment time, anchorage loss, the possibility of root resorption and the complication followed by segmental osteotomy. Therefore, corticotomy has become a major concern lately. This research has been conducted to study the efficiency of corticotomy in the treatment of bimaxillary protrusion comparing the profile change, canine retraction velocity and space closing time. The research compared and analyzed space closing time, canine retraction velocity and profile change in two groups of patients. Both groups were formed out of patients over 18 years old who visited the department of dentistry in Hanyang University for treatments. The experimental group who was treated by corticotomy and Skeletal Anchorage System(SAS). The control group who received orthodontic treatment using SAS. The following results are produced after analyzing both groups. The significant statistic difference in space closing time has been observed in the experimental group as compared with the control group(p<0.05). In the experimental group, the significant statistic increase in canine retraction velocity was also observed(p<0.05). There was no significant difference in profile change between the control group and the experimental group(p<0.05). As a result, orthodontic treatment using corticotomy has a reasonable efficiency in space closing time as compared with the existing orthodontic treatment. Therefore, it is concluded that corticotomy with orthodontic treatment can be reasonably applied to dentofacial abnormality.

  • Purpose : Anterior region is crucial area for esthetic implant restoration. However, the alveolar process undergoes atrophy after removal of teeth and creates unfavorable situation for implant installation. The knowledge of the thickness of alveolar bone is required to estimate and expect the bone resorption after extraction. The aim of this study is to measure facial, palatal and faciopalatal bone thickness on maxillary anterior teeth. Methods : Facial, palatal, and faciopalatal bone thickness were measured on the computed tomography (CT) images from 57 patients, using an image analyzer program (Ondemand$3D^{(R)}$, Cybermed, Seoul, Korea). Results : The thickness of facial bone in incisors, lateral incisors and canines were less than 1 mm. The thickness of facial bone increased from anterior to posterior region and the thickness of palatal bone increased from posterior to anterior region. Conclusion : The measurement can be used for planning implant surgery before extraction. CT has are clinically useful in the evaluation of thickness of alveolar bone.

  • This article describes a clinical protocol for the conventional rehabilitation of patient diagnosed with partial anodontia. A combined dental therapy approach was used and included endodontic therapy and root capping on the maxillary central incisors, fabrication of a maxillary overdenture, and fabrication of mandibular konus overdenture supported by 3 konus abutments. Within this protocol, tooth-supported overdenture prostheses are used for 2 purposes: first, to obtain the most rigid retention and function at an established maxillary-mandibular relationship; and second, to continuously maintain function and esthetic appearance applying immediate dentures after teeth extraction. The idea behind this protocol and its associated clinical procedures is presented along with a discussion compared with implant therapy. In the case introduced, and after 7 years of observation, the therapy can be seen as a success. We increased the occlusal vertical height in this case, but it would be more appropriate to see this as recovering the occlusal vertical height that was lost. The process of increasing the occlusal vertical height, that is restoration of the face, modification of the extrinsic occlusion of the incisors, and retraction of the mandible is very difficult and important. Ultimately, class III malocclusion is fixed, adequate occlusal vertical height is gained, and the retracted posterior anodontial portion is restored by prosthodontic dentures based on the rigid support theory. The result of the therapy done on the later-achieved malocclusion with partial anodontia on the posterior portion must consider the following in order to maintain the safety of the esthetics of the tooth and face for a period of time: 1) occlusal restoration with an ideal occlusal vertical height, 2) allowance of the final occlusion induced by the functional relationship of the upper and lower jaw, 3)final occlusion functionally induced by the lip competence limit.

Effect of coloring agent dryness on zirconia color (치과용 지르코니아 착색제의 건조정도가 지르코니아 색조에 미치는 영향)

  • One of the most important factor of esthetic prosthesis is the color of the teeth. Zirconia is produced in the process of design, milling, coloring, drying, and sintering. The effect of the drying degree of zirconia colorant on the color tone is examined. For the experiment, a total of 45 zirconia specimens(15 for each) were fabricated by using cad/cam system. The zirconia specimens(L block(LAZOR), Z block(Zircos-E block posterior), A block(AlphaZ)) were then subjected to zirconia sintering in accordance with the manufacturer's instructions, using a chemical coloring agent to Non-drying(0sec), intermediate (10sec), and completely dried (10min) temperature holding times. The color tone was measured using a spectrophotometer. The results were analyzed with a One-way analysis of variance and the Tukey post-hoc test (${\alpha}=.05$) The $L^*$ values of L, A specimens, $a^*$ values of A specimens, and $b^*$ values of Z specimens were not statistically different according to the degree of drying (p>0.05), While the $L^*$ value of Z specimen, $a^*$ value of L, Z specimen, $b^*$ values of L, A specimens were statistically different according to the degree of drying(p <0.05). In conclusion, the drying of zirconia colourants improves zirconia brightness and increases the optical properties of the enamel.

The preliminary study for three-dimensional alveolar bone morphologic characteristics for alveolar bone restoration

  • Background: The concept of the ideal morphology for the alveolar bone form is an important element to reconstruct or restore the in maximizing esthetic profile and functional alveolar bone restoration. The purpose of this preliminary study is to evaluate the normal alveolar bone structure to provide the standard reference and guide template for use in diagnosing for implant placement, determining the correct amount of bone augmentation in actual clinical practice and producing prostheses based on three-dimensional imaging assessment of alveolar bone. Methods: This study was included 11 men and 11 women (average age, 22.6 and 24.5 years, respectively) selected from among 127 patients. The horizontal widths of alveolar bone of maxilla and mandible were measured at the crestal, mid-root, and root apex level on MDCT (multi-detector computed tomography) images reconstructed by medical imaging software. In addition, tooth dimensions of the central incisors, canines, second premolars, and first molars of maxilla and mandible, including the horizontal width of the interdental alveolar bone crest, were also measured and statistically analyzed. Results: The horizontal alveolar bone width of the palatal side of maxilla showed a distinct increment from the alveolar bone crest to the apical region in both anterior and posterior areas. The average widths of the maxillary alveolar ridge were as follows: central incisor, 7.43 mm; canine, 8.91 mm; second premolar, 9.57 mm; and first molar, 12.38 mm. The average widths of the mandibular alveolar ridge were as follows: central incisor, 6.21 mm; canine, 8.55 mm; second premolar, 8.45 mm; and first molar, 10.02 mm. In the buccal side, the alveolar bone width was not increased from the crest to the apical region. The horizontal alveolar bone width of an apical and mandibular border region was thinner than at the mid-root level. Conclusions: The results of the preliminary study are useful as a clinical guideline when determining dental implant diameter and position. And also, these measurements can also be useful during the production of prefabricated membranes and customized alveolar bone scaffolds.

The Effects of Calcium Phosphate Glass on Mouse Calvarial Cell (Calcium Phosphate Glass가 마우스 두개골 세포에 미치는 영향)

  • The goal of periodontal treatment is not only to arrest the progression of the disease but also to promote the functional, esthetic regeneration of the periodontium. Flap operation, bone graft, guided tissue regeneration, growth factors and bone morphogenetic protein have been used for this purpose. Among these techniques of regeneration, alloplastic graft, especially calcium phosphate is getting more attention recently. The purpose of this study was to evaluate the effects of calcium phosphate glass on mouse calvarial cell in vitro. The toxicity of calcium phosphate glass was measured using MTT assay, the synthesis of collagen was measured using collagen assay, and ALP activity was measured. The experimental groups were cultured with calcium phosphate glass(both AQ-, and HT-CPG) in concentration of 0.01, 0.02, 0.1, 0.2g/ml. The results are as follows 1. In concentrations not exceeding 0.02g/ml, both the groups(AQ-CPG, HT-CPG) didn't show any toxicity on mouse calvarial cell(p<0.05). 2. In both the experimental groups are the concentration of 0.02g/ml, collagen expressions were significantly up-regulated (p<0.05). 3. In both the experimental groups are the concentration of 0.02g/ml, ALP activity was not significantly up-regulated, but ALP activity in both experimental groups were greater than control group(p<0.05). The results suggested that the use of calcium phosphate glass may promotes periodontal regeneration. Ongoing studies are necessary in order to determine their regeneration effects.

Cellular viability and genetic expression of human gingival fibroblasts to zirconia with enamel matrix derivative ($Emdogain^{(R)}$)

  • PURPOSE. The objective of this study was to investigate the biologic effects of enamel matrix derivative (EMD) with different concentrations on cell viability and the genetic expression of human gingival fibroblasts (HGF) to zirconia surfaces. MATERIALS AND METHODS. Immortalized human gingival fibroblasts (HGF) were cultured (1) without EMD, (2) with EMD $25{\mu}g/mL$, and (3) with EMD $100{\mu}g/mL$ on zirconia discs. MTT assay was performed to evaluate the cell proliferation activity and SEM was carried out to examine the cellular morphology and attachment. The mRNA expression of collagen type I, osteopontin, fibronectin, and TGF-${\beta}1$ was evaluated with the real-time polymerase chain reaction (RT-PCR). RESULTS. From MTT assay, HGF showed more proliferation in EMD $25{\mu}g/mL$ group than control and EMD $100{\mu}g/mL$ group (P<.05). HGFs showed more flattened cellular morphology on the experimental groups than on the control group after 4h culture and more cellular attachments were observed on EMD $25{\mu}g/mL$ group and EMD $100{\mu}g/mL$ group after 24h culture. After 48h of culture, cellular attachment was similar in all groups. The mRNA expression of type I collagen increased in a concentration dependent manner. The genetic expression of osteopontin, fibronectin, and TGF-${\beta}1$ was increased at EMD $100{\mu}g/mL$. However, the mRNA expression of proteins associated with cellular attachment was decreased at EMD $25{\mu}g/mL$. CONCLUSION. Through this short term culture of HGF on zirconium discs, we conclude that EMD affects the proliferation, attachment, and cell morphology of HGF cells. Also, EMD stimulates production of extracellular matrix collagen, osteopontin, and TGF-${\beta}1$ in high concentration levels. CLINICAL RELEVANCE. With the use of EMD, protective barrier between attached gingiva and transmucosal zirconia abutment may be enhanced leading to final esthetic results with implants.

Mechanical strength of Zirconia Abutment in Implant Restoration (지르코니아 임플란트 지대주의 기계적 강도에 관한 연구)

  • Purpose: As the esthetic demands of dental implant patients are increased, the demands of zirconia as implant abutment material are also increased. It has non-metalic color, good biocompatibility, high strength and high toughness. Even thought the advatage of zirconia abutment, there are a few studies about mechanical properties of zirconia abutment. This study evaluated the mecanical strength with compressive bending strength and endurance limit of implant-zirconia abutment assembly. Materials and Methods: Static and cyclic loading of implant-Zirconia abutment assembly were simulated under worst case condition according to ISO. Test groups were implants of external butt joint with straight regular diameter and angled regular diameter zirconia abutment, implant of external butt joint with narrow straight diameter zirconia abutment and implant of internal conical joint with straight narrow diameter zirconia abutment. All test group were evaluated the mecanical strength with compressive bending strength and endurance limit. After fatique testing, fracture surface were examined by SEM. Results: The compressive bending strengths exceed 927N. Regular diameter zirconia abutment were stronger than narrow diameter zirconia abutment(P<.05). The endurance limits ranged from 503N to 868N. Conclusion: Within the limitation of this study, zirconia implant abutment exceeded the estabilished values for maximum incisal biting forces reported in the literature.

The Esthetic Impact of Extraction and Nonextraction Treatments on Korean People (발치, 비발치를 동반한 교정치료 전후의 안모의 변화에 관한 인지도)

  • The concept of extraction in orthodontic treatment has been changed many times. Even today, criteria of extraction or nonextraction is still changing. In this study, changes depending on the evaluator's perception of treatment outcomes were compared in both extraction and nonextraction cases. In this study, premolar extracted 59 patients and nonextracted 60 patients, totally 119 patients who finished orthodontic treatment in Dankook University Dental Hospital orthodontic clinic were enrolled. Evaluation sections made up of specialists and laypersons assessed soft tissue traced from lateral cephalometric radiographs with visual analogue scale before and after the treatment. And the results were statistically analyzed. Thus, the conclusions drawn are as follows: 1. Average score is 5.76 in extraction, which is larger than 5.28 of nonextraction case. Improvement of facial profile was more favorably accepted in extraction case. 2. 5.875 in the group of specialists were higher evaluation than 5.165 in the group of layperson. 3. Specialists gave significantly higher ratings in the extraction than nonextraction. 4. A higher rating in extraction case of the layperson group has no significant difference with nonextraction case. 5. Nonextraction patients were given higher ratings from specialist group. 6. A higher rating of specialist group in extraction case has no significant difference with layperson group.
