A case of full mouth rehabilitation with orthodontic treatment in patient with extensive tooth erosion and wear using monolithic zirconia prostheses (광범위한 치아부식 및 마모가 있는 환자에서 교정치료와 단일구조 지르코니아 보철물을 이용하여 완전 구강 회복술을 시행한 증례)

  • Extensive dental erosion and wear will cause serious loss of function and aesthetics in the mouth. In order to recover this condition, careful analysis of the patient's bite relationship is required. In particular, a treatment plan should be established considering the possibility of reproduction of the vertical dimension and centric relation, and appropriateness of the occlusal plane and anterior guidance. Also, the choice of prosthetic materials is an important consideration in patients with severe wear. In this case, patients with overall wear and erosion on tooth was established anterior guidance by orthodontic treatment and fully restored with monolithic zirconia, without increasing vertical dimension.

Erosive Effect and Color of Mouthrinsing Solutions on Enamel Surface (시판중인 구강양치용액의 법랑질 표면의 착색과 침식에 미치는 영향)

  • The aims of this study were to evaluate the effect of native acidity upon dental hard tissues by measuring pH in mouth-rinse, which is on the market, and to evaluate even influence of dental stain caused by mouse-rinse given using for a long time. The experimental group was used Garglin, Nextcare, Listerine, and Hexamedin. The control group was used sterile distilled water. As a result, pH was indicated to be low in Nextcare(4.70) and Listerine(4.16). In a change of color tone, $b^*$ value of indicating Yellow-blue showed the lower value than the early color tone in all groups. There was statistical significance in Nextcare and Listerine. ${\Delta}L^*$ value of indicating light -dark gets darker after using Nextcare, Listerine and Hexamedine. A change(VHN) in surface hardness of enamel showed reduction in surface hardness after 14 days in Nexcare and Listerine. As a result of confirming a change in enamel surface by using Scanning Electron Microscopy, the erosion aspect on the enamel surface could be confirmed in groups of Nexcare and Listerine. Ra value in surface roughness of enamel could be observed the enamel surface, which got rougher compared to other experimental groups.

SCANNING ELECTRON MICROSCOPIC STUDY OF THE EFFECT OF ACIDIC DRINK ON ENAMEL EROSION : A CASE REPORT (탄산음료에 의해 부식된 법랑질 표면변화에 대한 증례발표)

  • It is widely believed that the increase in the prevalence of dental erosion is due to soft drinks. The purpose of this study was to assess the influence of soft drinks and etchant on the erosion of tooth enamel and the effect of tooth brushing on the abrasion of eroded enamel, and the effect of milk, saliva and Tooth $Mousse^{(R)}$ on the remineralization. The changes in tooth enamel surface were observed by scanning electron microscophy. The results were as follows; 1. The pH of Coca-Cola and Chilsung-Cider were higher than the pH of FineEtch 37 and $Tyrian^{TM}SPE$. 2. The SEM photos revealed that the pattern of the surface eroded by Coca-Cola and Chilsung-Cider were more obvious than that eroded by etchants. 3. The results of remineralization by saliva, milk, Tooth $Mousse^{(R)}$ were not different from each other. 4. The remineralization by saliva decreased tooth brushing abrasion.

Incidental occurrence of an unusually large mastoid foramen on cone-beam computed tomography and review of the literature

  • The incidental finding of an enlarged mastoid foramen on the right posterior mastoid region of temporal bone is reported, together with a discussion of its clinical significance. A 67-year-old female underwent the pre-implant assessment of a maxillary left edentulous region. A cone-beam computed tomographic (CBCT) image was acquired and referred for consultation. Axial CBCT slices revealed a unilateral, well-defined, noncorticated, low-attenuation, transosseous defect posterior to the mastoid air cells in the right temporal bone. The borders of the osseous defect were smooth and continuous. No other radiographic signs suggestive of erosion or sclerosis were noted in the vicinity. The density within the defect was homogenous and consistent with a foramen and/or soft tissue. The patient's history and physical examination revealed no significant medical issues, and she was referred to a neuroradiologist for a second opinion. The diagnosis of an enlarged mastoid foramen was made and the patient was reassured.

STUDY ON THE PRIMARY TOOTH ENAMEL EROSION CAUSED BY CHILDREN BEVERAGE (어린이 음료수에 의한 유치 법랑질 침식에 대한 연구)

  • The purpose of this study is to examine the acidity of beverages that are sold for children in the market and to estimate by experiment the degree of the enamel erosion of deciduous teeth that is attributed to the beverages. The pH value of children beverages among top sale list in Seoul and Gyeonggi province were measured. And 7 beverages with the pH value of 3.5 or lower were selected. The enamel erosion of deciduous teeth by the children beverages for 1, 5, 10, and 30 minutes respectively was examined. The results were as follows: 1. The acidity of most of the children beverages examined in this study was the pH level of 5.5 or lower that can cause the tooth erosion. 2. The microhardness value of the enamel of deciduous teeth was lower as exposing it to children beverages. The reduction of surface microhardness value was significant from 1 minute after the exposure, and the reduction was continuously observed until 30 minutes has passed. 3. The higher the acidity of beverages, the larger the reduction in microhardness value caused after the exposure of 30 minutes to the beverages. There was no significant relation between the acidity of beverages and the reduction rate of surface microhardness value. The other factors such as calcium, phosphorus, and fluorine or buffering of the beverages may affect the enamel erosion.

Full mouth rehabilitation of the patient with severe tooth erosion using collarless porcelain fused to gold restorations: a case report (Collarless 금속 도재 보철물을 이용한 심한 erosion 환자의 전악수복 증례)

  • Excessive tooth wear causes loss of tooth structure, disharmony of occlusal plane, functional and esthetic problems. Although the decrease of occlusal vertical dimension may be compensated by growth of alveolar bone, if the length of tooth is not enough for the retention of restoration, minimum increase of occlusal vertical dimension is required without discomfort of the patient. In this case, 33-year-old woman drinks more than 1 liter of soft drinks a day and has bruxism in night time, visited in Seoul National University Dental Hospital with chief complaint of generalized tooth wear and related esthetic and functional problems. It was considered as a loss of occlusal vertical dimension based on the accelerated tooth wear caused by erosion and bruxism and facial appearance, phonetic, esthetic, functional evaluations. It was planned to raise occlusal vertical dimension by provisional restoration two times for patient's adaptation, 3 mm and 2 mm each, total 5 mm. Confirming no discomfort and clinical symptom during total 16 weeks after restoration with provisional fixed restoration, it was restored with porcelain fused to gold crown and bridge. Because the patient was young woman, anterior teeth were restored with collarless porcelain fused to gold crown. This case presents that satisfactory esthetic and functional result by full mouth rehabilitation with increase of occlusal vertical dimension.

Erosive potential of several fruit-flavored liquors in Korea (국내 시판 중인 일부 과일 리큐어의 치아 부식능 평가)

  • Purpose: The aim of this study was to evaluate the erosive potential of several fruit-flavored liquors marketed in Korea by measuring pH, titratable acidity, and variations in pH utilizing an ISO method. Materials and Methods: Six fruit-flavored liquors were investigated. An orange juice and three kinds of soju were selected as the control beverage. The pH and titratable acidity of the beverages were determined by 1 M NaOH until the pH 5.5 and 7.0 (recorded as $TA_{5.5}$ and $TA_{7.0}$, respectively). All measurements were performed in triplicate. The process of ISO 28888 was used for measurement of the pH variation (${\Delta}pH$) using an under-saturated hydroxyapatite solution (screening solution) and determining the difference between the initial and final pH of the screening solution. The average ${\Delta}pH$ was determined based on 4 measurements. Results: All the fruit-flavored liquors tested in this study showed a pH lower than the critical pH for dental erosion (4.5). The average pH of the fruit-flavored liquors was 2.77 and was also lower than the pH of the soju and an orange juice (control drinks). $TA_{5.5}$ and $TA_{7.0}$ values of the fruit-flavored liquors were 4.92ml and 7.13ml, respectively. The fruit-flavored liquors showed an erosive potential capable of damaging the enamel surfaces, whereas the orange juice had the highest titratable acidity. The changes in pH determined using the screening solution confirmed that the fruit-flavored liquors had erosive potential, with ${\Delta}pH$ value of 1.53. Conclusions: The fruit-flavored liquors tested in this study might have a strong potential to erode dental hard tissues.

  • The purpose of this study is to report a clinical case of a patient whose vertical dimension of occlusion was lost. There are many reasons for the loss of vertical dimension and one of those can be a tooth wear problem. Thus in this article we will review shortly types of tooth wear: attrition, abrasion, and erosior. And layour the basic philosophy and principles in regaining the esthetics and function of a patient's oral condition with a history of psychological wear, through full mouth restorations.

A Study of Burkitt's Lymphoma Occurred in the Mandible (하악골에 발생한 Burkitt 임파종에 관한 연구)

  • The authors observed a case of Burkitt's lymphoma, occurred in the mandible, of 6-year-old female patient who admitted to the Department of Oral Radiology, Kyung Hee University Medical Center. The serial radiograms, clinical findings, and microscopid findings had been taken and obtained following results: 1. In serial radiograms, invasive and infiltrative bone destruction in the both mandibular body region was observed. Perforation and erosion of cortical plate of the mandibular angle area and loss of alveolar lamina dura in involved teeth were also noticed. 2. In microscopic findings, a monotous overgrowth of undifferentiated monomorphic lymphoreticular cells found. Macrophages with and abundant clear cytoplasm are usually found scattered uniformly throughout the tumor, producing the very characteristic 'starry-sky' appearance.

Delayed diagnosis of a primary intraosseous squamous cell carcinoma: A case report

  • Primary intraosseous squamous cell carcinoma is a rare malignant central jaw tumor derived from odontogenic epithelial remnants. Predominantly, it affects mandible, although both jaw bones may be involved. This report describes a 60-year-old man who was initially misdiagnosed with a periapical infection related to the right lower wisdom tooth. After four months, the patient presented to a private dental clinic with a massive swelling at the right side of the mandible. Panoramic radiographs and advanced imaging revealed a lesion with complete erosion of the right ramus, which extended to the orbital floor. A biopsy from the mandibular angle revealed large pleomorphic atypical squamous cells, which is the primary microscopic feature of a poorly differentiated squamous cell carcinoma.
