  • Background: To prepare for possible emergency situations during dental treatment, it is helpful to know how often and what kinds of emergencies may arise. This study set out to evaluate the incidences, causes, treatments, and outcomes of emergency situations in the outpatient clinic of a dental teaching hospital in Korea. Methods: We retrospectively reviewed the records of patients who had experienced an emergency situation and emergency response team activated in a selected outpatient clinic between November 2004 and November 2013. Specific information about the emergency cases was collected, including the patient characteristics and the frequency, types, treatments, and outcomes of the emergency situations. Results: We identified 35 instances of emergency situations in 2,890,424 patients (incidence = 0.012 per 10,000 outpatients). The number of cases was as follows: 10 (28.6%) in the Department of Periodontics, 10 (28.6%) in the Department of Oral and Maxillofacial Surgery, 6 (17.1%) in the Department of Oral and Maxillofacial Radiology, 4 (11.4%) in the Department of Prosthodontics, 2 (5.7%) in the Department of Conservative Dentistry, 2 (5.7%) in the Department of Pediatric Dentistry, and 1 (2.9%) in the Department of Orthodontics. Three (8.6%) of the emergency situations arose before treatment, 22 (62.9%) during treatment, 7 (20.0%) after treatment, and 2 (5.7%) in a patient's guardian. Conclusions: In accordance with the growing elderly population and more aggressive dental procedures, the number of emergency situations may increase in the future. We recommend that clinicians keep in mind airway management and the active control of emergency situations.

  • Objectives: The purpose of this study is to investigate the factors affecting medical knowledge and practice of dental treatment for systemic disease among dental health care workers. Methods: A self-reported questionnaire was filled out by 222 dental health care workers working in Seoul, Daejeon, Busan, Gyeonggi province, Chungcheong province, and Jeolla province within the period between May 1 - June 30, 2016. Knowledge and medical knowledge about the clinical treatment of patients suffering from systemic disease and their practice were composed of items that were corrected, supplemented, and developed by themselves based on previous research. Results: Factors affecting knowledge about clinical treatment of patients suffering from systemic disease were place of employment, treatment about systemic disease, and practice of dental treatment for systemic diseases. Predictive power was 38.5%. Factors affecting practice of clinical treatment of patients suffering from systemic disease were sex, place of employment, treatment about systemic disease, the basic equipment and drugs needed for emergency care, and knowledge of dental treatment for systemic diseases. Predictive power was 39.1%. Conclusions: Dental health care workers' knowledge and practice of dental treatment of patients suffering from systemic diseases were important factors influencing each other.

  • Deep sedation is considered for the dental treatment of pediatric or disabled patients who have severe anxiety or involuntary movement. Deep sedation using sevoflurane inhalation in emergency dental practice, therefore, is also preferred for fast induction and recovery. This survey consists of 121 people with pediatric or disabled patients who underwent dental treatment under deep sedation using sevoflurane inhalation from January 2013 to October 2013. Patients who were scheduled for deep sedation were classified into a non-emergency sevoflurane sedation group, whereas patients who underwent emergency sedation due to trauma and patients with disabled characteristics itself were classified into an emergency sevoflurane sedation group. Of 121 patients studied, 95 patients received dental care under non-emergency sedation, 26 patients received dental care under emergency sevoflurane sedation. The two groups were analyzed according to: gender; age; primary reason for sedation; duration of sedation; treatment time; induction methods; treatment information; and departments. Non-emergency sevoflurane sedation in pediatric or disabled patients was safe and effective for controlling the behavior. Emergency sevoflurane sedation was a useful method for younger pediatric patients with traumatic injury who need simple, short time emergency treatment. Deep sedation using sevoflurane inhalation not only will reduce the use of general anesthesia gradually but also will be a useful method to emergency treatment for pediatric or disabled patients.

Methods : Among 375 middle school students, a total of 340 filled out the self-administered questionnaire (response rate of 90.7%). The data were analyzed statistically using chi-square analysis. Results : 32.9% of respondents received emergency care education of dental trauma. Knowledge by general information about dental trauma was not so different from the other questions. Tooth storage medium comprised 25.0% and 48.5% in milk and saline, respectively. Conclusions : In order to get the knowledge about emergency treatment of dental trauma in middle school students, it is necessary to develop the professional education program for middle school students.

Trauma was the most common cause in dental emergency patients, followed in order by toothache, odontogenic infection, temporomandibular joint (TMJ) disorder and oral hemorrhage. Soft tissue injury was most prevalent in the trauma group, followed by tooth injury and facial bone fractures. In the tooth injury group, tooth fracture (56.7%) showed the highest incidence followed in order by tooth subluxation (18.2%), tooth concussion (16.9%), tooth avulsion (11.5%) and alveolar bone fractures (3.7%). In the facial bone fracture group, mandibular fractures (81.8%) showed the highest incidence followed in order by maxilla fractures (15.7%), nasal bone fractures (9.0%), zygomaticomaxillary complex fractures (5.4%), orbital bone fractures (2.5%). In mandibular bone fractures, the most common location was the symphysis (70.1%), followed in order by the mandibular angle (33.0%), mandibular condyle (22.8%) and mandibular body (13.6%). In the infection group, a submandibular space abscess (46.2%) was most common followed in order by a buccal space abscess (17.4%), canine space abscess (16.9%) and submental space abscess (12.3%). TMJ dislocation (89.3%) showed the highest incidence in the TMJ disorder group, followed by TMJ derangement (10.7%). In the other group, a range of specific symptoms due to post operation complications, trigeminal neuralgia, chemical burns and foreign body aspiration were reported. Conclusion: For the rapid and appropriate care of the dental emergency patients, well-organized system should be presented in oral and maxillofacial surgery. And it is possible under analysis of pattern and the variation of the dental emergency patients.

  • This study aims to analyze dental care at the 25th World Scout Jamboree (WSJ) using a predesigned standardized dental chart to provide a reference for the operation of dental emergency clinics at future large-scale youth events. The dental charts of all patients treated at the 25th WSJ emergency dental clinic were examined. Fisher's exact tests were used to analyze the differences between youth scouts and adult leaders. Eighty visits of 71 patients were included in the study. The largest number of patients was from Asia-Pacific (49.3%), followed by patients from Europe (40.8%). In terms of chief complaints, oral ulcers (19.7%), lesions of the lips (18.3%), and orthodontic discomfort (15.5%) were the most prevalent. The analysis showed a significant difference (p = 0.0023) between youth scouts and adult leaders in the distribution of patients in the first half (July 30th - August 3rd) and second half (August 4th - August 8th) of the event. There was a higher incidence of dental conditions associated with hot and humid environments, unlike typical emergency dental settings. Therefore, despite adequate preparation, unexpected dental visits may occur, and it is recommended that operations be conducted in community-based dental hospitals to allow for immediate and efficient responses in international camps.
