Unique case of a geminated supernumerary tooth with trifid crown

  • Gemination, a relatively uncommon dental anomaly, is characterized by its peculiar representation as a tooth with a bifid crown and a common root and root canal. It usually occurs in primary dentition. To come across gemination in a supernumerary tooth is a rare phenomenon. The purpose of this paper is to present a unique case of hyperdontia wherein gemination in an impacted supernumerary tooth resulted in a trifid crown unlike the usual bifid crown. The role of conventional radiographs as well as computed tomography, to accurately determine the morphology and spatial location, and to arrive at a diagnosis, is also emphasized in this paper.

  • Double tooth is a dental anomaly consequent to fusion of two or more teeth or gemination of a single tooth. This report describes a unique case of double tooth in relation to a mandibular lateral incisor exhibiting the presence of four root canals. The role of conventional radiography and advanced three-dimensional imaging techniques in the better assessment of complex root canal systems and their aid in endodontic management has also been highlighted.

Necrosis of intact premolar caused by an adjacent apical infection: a case report

  • Although periapical inflammatory lesions are usually resulted by infection in the root canal system, this rare case showed that a periapical lesion related to an infected tooth may cause pulpal necrosis in adjacent intact tooth, with no history or clinical signs of caries, disease, trauma or developmental anomaly. This case also suggests that the periapical lesion can be treated conservatively, without surgical intervention. Furthermore, this case highlights the importance of prompt treatment of apical periodontitis before the lesion becomes extensive as well as follows up of large lesions.

치아 이상의 발생 빈도와 양상에 관한 연구 (A STUDY ON PREVALENCE AND PATTERN OF DENTAL ANOMALIES)

  • An objective definition of the anomaly is not avaliable and most investigators define the term differently or fail to describe their criterion. Because dental anomaly may lead to many complications, early detection and diagnosis of dental anomalies are essential steps in the evaluation of the child patient and in treatment planning. These procedures require detailed medical and dental histories, through clinical examination and the use of radiographs. So, this study was designed to find out the prevalence of dental anomalies. The clinical and roentgenographic examination was undertaken for 8,054 children at age from 0 to 15 years and statistical analysis was done. The results were as follows : 1. Among the examined subjects, 2,134 subjects(26.5%) showed dental anomalies. The prevalence of individual dental anomalies were as follows : supernumerary teeth 15.6%, congenitally missing teeth 6.6%, fusion 2.2%, odontoma 0.35%, microdontia 1.2%, macrodontia 0.05%, gemination 0.22%, talon cusp 0.36%, dens evaginatus 0.24%, dens invaginatus 0.15%, dilaceration 0.27%, taurodontism 0.09%, abnormal tooth position 1.7%, natal & neonatal teeth 0.92%, amelogenesis imperfecta 0.01%. 2. Significant correlations between the groups with individual dental anomalies were as follows : between group I and other groups, there was negative correlation, especially group I and group II. And the correlation coefficient between male and female showed differences. 3. For the supernumerary teeth group, the prevalence of male was higher than female(p<0.001). While for the congenitally missing teeth, macrodontia, microdontia, abnormal tooth position group, the prevalence of female was greater than male(p<0.001). 4. For the congenitally missing teeth group, the mandibular primary lateral incisor showed the highest incidence in primary dentiton, while mandibular lateral incisor in permanent dentition. In the mandible(72.5%), this site showed higher prevalence than in maxilla. In the case with deciduous congenitally missing teeth, the prevalence of successive permanent congenitally missing teeth was about 33.9%, the incidence was highest in mandibular lateral incisors. 5. Most supernumerary teeth existed on middle area and showed inverted position and unerupted state. In addition, supernumerary teeth showed higher incidence on maxilla (99.3%). 6. In the case with deciduous fused teeth, the prevalence of successive permanent congenitally missing teeth was 39.9%, while that of successive permanent fused teeth was 2.7%. And the highest rate of the prevalence (40.3%) appeared in fusion of mandibular deciduous lateral incisor and canine. 7. In the case of odontoma, the prevalence was higher in maxilla(78.6%) and anterior region(82.7%) than mandible and posterior region.

  • Supravalvular aortic stenosis is relatively uncommon form of congenital heart disease and the most important lesion of this anomaly is various narrowing of the aortic lumen just above the sinus of Valsalva. We experienced a case of hourglass type of supravalvular aortic stenosis involving lcm from length from lcm above the sinus of Valsalva. The patient was associated with mental retardation, peculiar facies and dental anomaly. The diagnosis was confirmed preoperatively by retrograde left heart catheterization and left ventriculography. An incision was made in the ascending aorta and into the right coronary and noncornary sinus. Care was taken to protect the right coronary artery. A Y-shaped patch of Dacron was made to enlarge the stenotic portion of aorta. Postoperative pressure gradient between the aorta and left ventricle markedly reduced 36 mmHg in comparison with preoperative pressure gradient 150mmHg. The boy was discharged without any event.

Difficult intubation caused by an immature upper airway in a patient with cri-du-chat syndrome: a case report

  • Cri-du-chat syndrome (CdCS) is caused by the deletion of the short arm of chromosome 5. Most patients with CdCS develop intellectual disabilities. Therefore, they have poor oral hygiene and a high caries index. However, treating such patients is not an easy task, because of the difficulty in communication. General anesthesia may be a useful option in adult patients with CdCS and intellectual disability. General anesthesia should be administered very carefully, owing to the presence of comorbid diseases, which may need airway management. Infants with CdCS need general anesthesia if they have a concomitant cardiac anomaly. Intubation is reportedly difficult for such patients was, owing to the structural and functional abnormalities in the larynx and vocal cords. The purpose of this study was to report a case of difficult intubation while inducing general anesthesia in a patient with CdCS during dental treatment, due to a narrow larynx and trachea.

  • Transposition is defined as a dental anomaly manifested by a positional interchange of 2 adjacent teeth within the same quadrant of the dental arch. Maxillary canine-first premolar [Mx4-3] transposition is the most frequent tooth transposition reported in the literature. In this case report, an orthodontic correction of a transposition of the maxillary left canine and first premolar with the help of palatally located mini-implant anchorage is described. Esthetic and occlusal evaluations suggested alignment of the transposed teeth to their correct anatomic positions in the dental arch. The clinical result at the end of the treatment was satisfactory. Alignment was obtained, and intercuspation was adequate. Nevertheless, the maxillary canine showed facial recession, probably because it was initially positioned buccally. Supporting tissue was examined after treatment and no alveolar bone damage was observed.

상악 우측 유전치부에 발생한 삼중치 : 증례 보고 (Triplication of upper right primary anterior teeth : a case report)

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

Dental Management of First Permanent Molars in Molar-incisor Malformation Patients: A Case Report

  • Molar incisor malformation (MIM) has been introduced as a new type of dental anomaly. Currently, the morphological and histological characteristics of MIM are known; however, its etiology has not been clearly identified. To date, the long-term prognosis of first permanent molars (FPM) affected by MIM has rarely been reported, and few treatment guidelines have been established. The purpose of this case report was to present guidelines for the extraction of FPM affected by MIM, depending on the presence of the third molar. In patients with a third molar, spontaneous mesial shift of the posterior molars might be induced by extracting the FPM at an appropriate time, that is, when the second permanent molar is at an early furcation stage of the tooth. However, it is recommended that FPM be preserved for as long as possible if a third molar does not exist. When an FPM needs to be extracted, it is suggested to consider space maintenance.

  • Paramolar tubercle은 드문 발생학적 치아 기형으로 구치의 협면 또는 설면에서 나타나는 추가적인 교두로 정의된다. 영구치에 융합된 Paramolar tubercle의 존재는 근관치료를 어렵게 하는 원인이 될 수 있다. 그러므로 이러한 변이에 대한 적절한 이해가 성공적인 근관치료를 위해 중요하다. 콘빔 전산화 단층 촬영(CBCT)은 복잡한 증례에서 해부학적 구조를 이해하는 데 도움을 줄 수 있다. 따라서 본 증례는 Paramolar tubercle이 융합된 상악 제2소구치의 비외과적 근관치료에 대한 증례에 대해 보고하고자 한다.
