Investigation of anodized titanium implants coated with triterpenoids extracted from black cohosh: an animal study

  • PURPOSE. The purpose of this study was to evaluate bone response to anodized titanium implants coated with the extract of black cohosh, Asarum Sieboldii, and pharbitis semen. MATERIALS AND METHODS. Forty anodized titanium implants were prepared as follows: group 1 was for control; group 2 were implants soaked in a solution containing triterpenoids extracted from black cohosh for 24 hours; group 3 were implants soaked in a solution containing extracts of black cohosh and Asarum Sieboldii for 24 hours; group 4 were implants soaked in a solution containing extracts of pharbitis semen for 24 hours. The implants from these groups were randomly and surgically implanted into the tibiae of ten rabbits. After 1, 2, and 4 weeks of healing, the nondecalcified ground sections were subjected to histological observation, and the percentage of bone-to-implant contact (BIC%) was calculated. RESULTS. All groups exhibited good bone healing with the bone tissue in direct contact with the surface of the implant. Group 2 ($52.44{\pm}10.98$, $25.54{\pm}5.56$) showed a significantly greater BIC% compared to that of group 3 ($45.34{\pm}5.00$, $22.24{\pm}2.20$) with respect to the four consecutive threads and total length, respectively. The BIC% of group 1 ($25.22{\pm}6.00$) was significantly greater than that of group 3 ($22.24{\pm}2.20$) only for total length. CONCLUSION. This study did not show any remarkable effects of the extract of black coshosh and the other natural products on osseointegration of anodized titanium implants as coating agents. Further studies about the application method of the natural products on to the surface of implants are required.

Comparison of fit accuracy and torque maintenance of zirconia and titanium abutments for internal tri-channel and external-hex implant connections

  • PURPOSE. This in vitro study aimed to evaluate the effect of implant connection design (external vs. internal) on the fit discrepancy and torque loss of zirconia and titanium abutments. MATERIALS AND METHODS. Two regular platform dental implants, one with external connection ($Br{\aa}nemark$, Nobel Biocare AB) and the other with internal connection (Noble Replace, Nobel Biocare AB), were selected. Seven titanium and seven customized zirconia abutments were used for each connection design. Measurements of geometry, marginal discrepancy, and rotational freedom were done using video measuring machine. To measure the torque loss, each abutment was torqued to 35 Ncm and then opened by means of a digital torque wrench. Data were analyzed with two-way ANOVA and t-test at ${\alpha}=0.05$ of significance. RESULTS. There were significant differences in the geometrical measurements and rotational freedom between abutments of two connection groups (P<.001). Also, the results showed significant differences between titanium abutments of internal and external connection implants in terms of rotational freedom (P<.001). Not only customized internal abutments but also customized external abutments did not have the exact geometry of prefabricated abutments (P<.001). However, neither connection type (P=.15) nor abutment material (P=.38) affected torque loss. CONCLUSION. Abutments with internal connection showed less rotational freedom. However, better marginal fit was observed in externally connected abutments. Also, customized abutments with either connection could not duplicate the exact geometry of their corresponding prefabricated abutment. However, neither abutment connection nor material affected torque loss values.

Bone response around immediately placed titanium implant in the extraction socket of diabetic and insulin-treated rat maxilla (인슐린으로 조절되는 당뇨쥐 상악에서 발치 후 즉시 임플란트 주변에서 골형성)

  • Introduction: Dental implants are used routinely with high success rates in generally healthy individuals. By contrast, their use in patients with diabetes mellitus is controversial because altered bone healing around implants has been reported. This study examined the bone healing response around titanium implants placed immediately in rats with controlled and uncontrolled diabetes. Materials and Methods: Twenty rats were divided into the control, insulin-treated and diabetic groups. The rats received streptozotocin (60 mg/kg) to induce diabetes; animals in the insulin-treated group also received three units of subcutaneous slow-release insulin. A titanium implant ($1.2{\times}3\;mm$) was placed in the extraction socket of the maxillary first molar and bone block was harvested at 1, 2 and 4 weeks. Results: Bone formation around the implants was consistently (from 1 to 4 week post-implantation) slower for the diabetic group than the control and insulin-treated group. Bone morphogenesis in the diabetic rats was characterized by fragmented bone tissues and extensive soft tissue intervention. Conclusion: The immediate placement of titanium implants in the maxilla of diabetic rats led to an unwanted bone healing response. These results suggest that immediate implant insertion in patients with poorly controlled diabetes might be contraindicated.

Selection of Nickel-Titanium Files according to the Clinical Procedure and Factors of File Fracture: A Narrative Review

  • In this article, the contemporary root canal treatment procedure using nickel-titanium (NiTi) instruments was reviewed to understand the correlations between the properties of files and safety of the clinical usage. Literatures were reviewed according to the process of clinical procedure of the root canal preparation, mainly for shaping during orifice flaring, glide-path preparation, and main canal instrumentation. Considering the reasons for NiTi file fracture, clinically implacable issues and ideas were discussed to reduce the fracture risk and increase clinical efficiency of the NiTi file systems. Various kinds of NiTi file systems have their own characteristics and properties given from their geometries and heat treatments and so on. Proper selection and careful usage of the NiTi file systems may reduce the risk of file fracture and increase the efficiency of NiTi file systems. Understanding of the clinical implications from the mechanical properties and characteristics of the engine driven NiTi instruments may decrease the risk of NiTi file fractures and increase the success rate in root canal treatment.

Electrochemical hydrothermal treatment on Pure Titanium by the method of Cathodic reduction (음극환원법에 의한 Pure Ti의 전기화학적 열수처리)

  • The purpose of this study was to examine the optimum condition of impulse during the anodic spark oxidation applying pulse current as well as to find the excellent condition for HA precipitation the after electrochemical hydrothermal treatment by cathode reduction method. After anodic spark oxidation, the anodized specimen and the Pt plate connected cathode and anode, respectively. Hydrothermal treatment performed at 90, 120, $150^{\circ}C$ for 2 hours in the electrolyte containing $K_2HPO_4$, $CaCl_2{\cdot}2H_2O$, Tris(Hydroxymethyl)-$(CH_2OH)_3\;CNH_2$(Aminomethane), and NaCl. The optimum impulse voltage for anodic spark oxidation was 350V. The optimum pulse cycle measured at 10 mS. The HA crystals precipitated excellently by cathode reduction at $150^{\circ}C$ for 2 hours. The phases of anatase, rutile, and HA coating on the surface of modified titanium surface immersed in Hanks' solution for 3weeks were detected by XRD measurement and the intensity of HA crystal phase has increased by temperature and time of hydrothermal treatment. According to the our experiments, we found that Pure Ti will be good materials of bioactivity and biocompatibility.

The Biocompatibility of HA Film Deposition on Anodized Titanium Alloy

  • A thin film hydroxyapatite (HA) films was deposited on anodized titanium by RF sputtering method. The anodized titanium enhanced the biocompatibility of the Ti and the bioactivity was improved further by the HA deposited on the anodized Ti. $TiO_2$ layer with $0.2{\sim}0.5{\mu}$ diameter pore size was formed on the Ti surface by anodization. Anodized $TiO_2$ layer analysis HA film deposited, oxide pore size and number decreased compared with non-HA deposited surface. The corrosion resistance of HA deposited/anodized Ti was higher than that of the non-treatment Ti alloy in Hank's solution, indicating better protective effect. From the results of cell culture using MTT assays, the best cell proliferation showed in HA deposited surface after anodization of Ti surfaces compared with another surface treatment.

Loosening torque of Universal Abutment screws after cyclic loading: influence of tightening technique and screw coating

  • PURPOSE. The purpose of this study was to evaluate the influence of tightening technique and the screw coating on the loosening torque of screws used for Universal Abutment fixation after cyclic loading. MATERIALS AND METHODS. Forty implants (Titamax Ti Cortical, HE, Neodent) (n=10) were submerged in acrylic resin and four tightening techniques for Universal Abutment fixation were evaluated: A - torque with 32 Ncm (control); B - torque with 32 Ncm holding the torque meter for 20 seconds; C - torque with 32 Ncm and retorque after 10 minutes; D - torque (32 Ncm) holding the torque meter for 20 seconds and retorque after 10 minutes as initially. Samples were divided into subgroups according to the screw used: conventional titanium screw or diamond like carbon-coated (DLC) screw. Metallic crowns were fabricated for each abutment. Samples were submitted to cyclic loading at $10^6$ cycles and 130 N of force. Data were analyzed by two-way ANOVA and Tukey's test (5%). RESULTS. The tightening technique did not show significant influence on the loosening torque of screws (P=.509). Conventional titanium screws showed significant higher loosening torque values than DLC (P=.000). CONCLUSION. The use of conventional titanium screw is more important than the tightening techniques employed in this study to provide long-term stability to Universal Abutment screws.

Influence of Anodic Oxidation Film Formed on Titanium onto Cell Attachment and Proliferation (양극 산화에 의해 티타늄 표면에 형성된 산화 피막이 세포 부착 및 성장에 미치는 영향)

  • This study was purposed to evaluate the influence of anodically oxidized film on titanium (Ti) onto MG-63 osteoblast-like cell attachment and activity. Only scratch lines created by polishing were seen in ASR and ANO-1 groups. About $1.5{\mu}m$-thick homogeneous oxide film which has pores of about $0.5{\mu}m$ diameter were formed in ANO-12. The crystalline structure of the oxide films formed by anodization in phosphoric acid electrolyte was $TiP_2O_7$. The total protein amounts of ANO-1 and ANO-12 groups showed higher values of maximum protein amount than that of AS-R group. At 3 days of incubation, total protein amount showed higher value in ANO-2 when comparing to that of AS-R (p<0.05). Based on the results of ALPase activity test, the degree of MG-63 cell differentiation for initial mineralization matrix formation was similar. For all the test groups after 1 day of incubation, MG-63 cells grew healthily in mono-layer with dendritic extensions. After incubation for 3 days, the specimen surfaces were covered more densely by cells, and numerous micro filaments were extruding to the extracellular matrix.
