Rhinolith: An incidental radiographic finding

  • Rhinoliths are foreign bodies composed of mineralised deposits in the nasal cavity that are rarely encountered in routine dental practice. The current report presents a case of a 77-year-old female patient who visited a dental treatment centre for a routine dental check-up and prophylaxis. On the clinical examination, the patient did not report any breathing difficulty, nasal obstruction, or any form of discharge. Panoramic radiography showed a horizontally magnified radiopaque structure in the right nasal fossa region. Cone-beam computed tomography (CBCT) revealed a hyperdense entity with a laminated presentation in the right nasal fossa. The hyperdense entity was diagnosed as a rhinolith based on the radiographic findings. Although the patient was asymptomatic, a referral was made to an ear, nose, and throat surgeon for a further evaluation. This report highlights the importance of CBCT imaging in the diagnosis of soft-tissue calcifications.

Diagnostic reference levels in intraoral dental radiography in Korea

  • Purpose: The objectives of this study were to survey the radiographic exposure parameters, to measure the patient doses for intraoral dental radiography nationwide, and thus to establish the diagnostic reference levels (DRLs) in intraoral dental X-ray examination in Korea. Materials and Methods: One hundred two intraoral dental radiographic machines from all regions of South Korea were selected for this study. Radiographic exposure parameters, size of hospital, type of image receptor system, installation duration of machine, and type of dental X-ray machine were documented. Patient entrance doses (PED) and dose-area products (DAP) were measured three times at the end of the exit cone of the X-ray unit with a DAP meter (DIAMENTOR M4-KDK, PTW, Freiburg, Germany) for adult mandibular molar intraoral dental radiography, and corrections were made for room temperature and pressure. Measured PED and DAP were averaged and compared according to the size of hospital, type of image receptor system, installation duration, and type of dental X-ray machine. Results: The mean exposure parameters were 62.6 kVp, 7.9 mA, and 0.5 second for adult mandibular molar intraoral dental radiography. The mean patient dose was 2.11 mGy (PED) and 59.4 $mGycm^2$ (DAP) and the third quartile one 3.07 mGy (PED) and 87.4 $mGycm^2$ (DAP). Doses at university dental hospitals were lower than those at dental clinics (p<0.05). Doses of digital radiography (DR) type were lower than those of film-based type (p<0.05). Conclusion: We recommend 3.1 mGy (PED), 87.4 $mGycm^2$ (DAP) as the DRLs in adult mandibular molar intraoral dental radiography in Korea.

Splint 및 비외과적 치주치료를 통하여 치주질환에 이환된 치아의 안정화 증례 보고 (Stability of periodontally compromised teeth after splint and non-surgical therapy: two cases followed-up for 1 to 3 years)

  • 본 증례는 치주적으로 이환된 하악 전치부를 치은연상 치석제거술과 치근활택술을 포함한 비외과적 치주치료, 교합 조정 및 치아 간 치아고정술을 통해 회복한 경우이다 임상적, 방사선학적 평가를 술 후 1 - 3년간 시행하였다. 두 증례 모두 임상적 계측치에서 향상된 결과를 보였고 방사선학적 평가에서 현저한 치조골 및 치조백선의 재생이 관찰되었다. 이번 증례는 발치가 고려되는 심도의 치주질환에 이환된 치아에서 비외과적 치주치료와 치아고정술을 통해 치아를 보존할 수 있는 가능성을 보여준 사례로 보고하는 바이다.

Kodak Insight 치과필름의 특성에 관한 연구 (Study on the characteristics of Insight dental x-ray film)

  • Purpose: To investigate the characteristics of the newly marketed, Insight dental X-ray film. Materials and Methods: Kodak Ultraspeed (DF-58), E-speed, Agfa Dentus M2, and Kodak Insight (IP-21) films were radiographed using a Trophy intra-oral radiographic machine. 10 step exposure times were prepared and each step exposure was monitored using a FH 40G (ESM Eberline Instruments) dosimeter for each of the 4 types of intra-oral film. All films were manually processed and the radiographic densities at 6 sites of each processed film were measured, and the characteristic curves of each of the 4 types intra-oral films were created utilizing these dosimetric data and radiographic densities, based on ISO 5779. The film contrast, speed, and base plus fog density of Insight film were compared with those of the 3 other films examined in this experiment. Results : E-speed film showed greatest average gradients followed by Insight film. E-speed and Ultraspeed film showed great average gradients at low density levels. Insight film showed the fastest speed followed by E-speed, Dentus M2 and Ultraspeed film. Dentus M2 film showed greatest base plus fog density level followed by Insight film. Conclusion : Kodak Insight film showed fastest film speed with comparable film contrast on characteristic curve.

Squamous cell carcinoma arising within a maxillary odontogenic keratocyst: A rare occurrence

  • Squamous cell carcinoma (SCC) arising within the lining of an odontogenic keratocyst (OKC) is a rare occurrence. Although potentially locally destructive, OKC is a benign odontogenic process that typically presents with clinical and radiographic features characteristic of a benign intraosseous neoplasm. We present the clinical and radiographic features of a maxillary mass that demonstrated SCC arising from the lining of an OKC. Although the initial clinical and radiographic presentation suggested an infection or malignant neoplasm, biopsies revealed an infiltrative well-differentiated SCC contiguous with and arising from the focus of a pre-existing OKC. The patient subsequently underwent a type II hemi-maxillectomy with neoadjuvant chemoradiation. This report discusses the clinical and radiographic features associated with intraosseous malignancies, especially those arising from an otherwise benign odontogenic lesion. While the majority of OKCs are benign, the current report illustrates the potential for carcinomatous transformation within the lining of an OKC.

Evaluation of canalis basilaris medianus using cone-beam computed tomography

  • The aim of this report is to present two cases of canalis basilaris medianus as identified on cone-beam computed tomography (CBCT) in the base of the skull. The CBCT data sets were sent for radiographic consultation. In both cases, multi-planar views revealed an osseous defect in the base of the skull in the clivus region, the sagittal view showed a unilateral, well-defined, non-corticated, track-like low-attenuation osseous defect in the clivus. The appearance of the defect was highly reminiscent of a fracture of the clivus. The borders of osseous defect were smooth, and no other radiographic signs suggestive of osteolytic destructive processes were noted. Based on the overall radiographic examination, a radiographic impression of canalis basilaris medianus was made. Canalis basilaris medianus is a rare anatomical variant and is generally observed on the clivus. Due to its potential association with meningitis, it should be recognized and reported to avoid potential complications.

  • Recently the patient exposure by medical and dental x-ray examination has grown rapidly and diagnostic radiology represents the largest source of man-made radiation. For the patient protection, the principle of justification and optimization should be followed. All the radiographic examinations have to show a potential benefit to the patient weighing against the potential risk. After they are justified, the radiographic exposure should be kept as low as reasonably achievable, taking into account economic and social factors. For the safe use of radiation in dentistry, the radiation safety management in accordance with the legislation is important. The present status and the future of radiation safety management in dental radiology in Korea and other country will be discussed.

Deep Learning in Dental Radiographic Imaging

  • Deep learning algorithms are becoming more prevalent in dental research because they are utilized in everyday activities. However, dental researchers and clinicians find it challenging to interpret deep learning studies. This review aimed to provide an overview of the general concept of deep learning and current deep learning research in dental radiographic image analysis. In addition, the process of implementing deep learning research is described. Deep-learning-based algorithmic models perform well in classification, object detection, and segmentation tasks, making it possible to automatically diagnose oral lesions and anatomical structures. The deep learning model can enhance the decision-making process for researchers and clinicians. This review may be useful to dental researchers who are currently evaluating and assessing deep learning studies in the field of dentistry.
